Effects of ankle joint degree of freedom of knee-ankle-foot orthoses on loading patterns and triceps surae muscle activity on the paretic side in individuals with subacute severe hemiplegia: a retrospective study.

BACKGROUND: Individuals with subacute severe hemiplegia often undergo alternate gait training to overcome challenges in achieving walking independence. However, the ankle joint setting in a knee-ankle-foot orthosis (KAFO) depends on trunk function or paralysis stage for alternate gait training with a KAFO. The optimal degree of ankle joint freedom in a KAFO and the specific ankle joint conditions for effective rehabilitation remain unclear. Therefore, this study aimed to investigate the effects of different degrees of freedom of the ankle joint on center-of-pressure (CoP) parameters and muscle

Frequent discordance between clinical and musculoskeletal ultrasound examinations of foot disease in juvenile idiopathic arthritis.

OBJECTIVE: To evaluate the levels of agreement from independent clinical examination (CE) by a pediatric rheumatologist and podiatrist and an ultrasound (US) examination of articular and periarticular foot disease in juvenile idiopathic arthritis (JIA). METHODS: Thirty patients with JIA and a history of foot disease underwent CE and US examination of 24 foot joints, 10 tendons, and 6 periarticular soft tissues. Each site was examined independently by a rheumatologist and a podiatrist for synovitis and tenderness/swelling. The same sites were examined independently by a sonographer for effusio

Segmentation of foot and ankle complex based on kinematic criteria.

Although various foot models were proposed for kinematics assessment using skin makers, no objective justification exists for the foot segmentations. This study proposed objective kinematic criteria to define which foot joints are relevant (dominant) in skin markers assessments. Among the studied joints, shank-hindfoot, hindfoot-midfoot and medial-lateral forefoot joints were found to have larger mobility than flexibility of their neighbour bonesets. The amplitude and pattern consistency of these joint angles confirmed their dominancy. Nevertheless, the consistency of the medial-lateral forefo

Long-term outcome of rheumatoid arthritis has improved.

X-rays of 20 finger joints, 10 foot joints and 2 wrists of 50 female patients with seropositive rheumatoid arthritis of 10-15 years' duration from 1962, 1972, 1982 and 1992 were evaluated blindly for erosions. A highly significant decline was observed in erosions in finger joints. Erosions had also declined in wrists and in foot joints, though less than in fingers. This may indicate improvement in the long-term outcome of rheumatoid arthritis.

Arthritis and related diseases of the foot and ankle: rehabilitation and biomechanical considerations.

Arthritis and other rheumatic conditions are among the most common disabilities in the United States. Although arthritic conditions affect the knee and hip joints more frequently than the joints of the foot, disorders of the foot are particularly disabling. This article outlines the types of arthritis conditions affecting the foot and discusses strategies for management and rehabilitation.

Intra-articular corticosteroid injections in the foot and ankle: a prospective 1-year follow-up investigation.

UNLABELLED: Most evidence for the efficacy of intra-articular corticosteroids is confined to the knee, with few studies considering the joints of the foot and ankle. The aim of this study was to identify the long-term efficacy of corticosteroid injection in foot and ankle joints. All patients undergoing intra-articular corticosteroid injections into foot and ankle joints over a 10-month period were recruited into the study. Patients were asked to complete a foot-related quality of life questionnaire, namely the Foot and Ankle Outcome Score, immediately before intra-articular injection and at s

Ankle and Pantalar Arthrodesis: End-Stage Salvage in Cavus Foot.

Bony alignment is the primary goal in foot and ankle reconstruction of the cavovarus foot. This condition presents as a malalignment causing a medial overload of the ankle articular surface and lateral overload of the hindfoot, midfoot, and forefoot. A painful gait associated with articular degeneration of the numerous joints can lead to a chronic and rigid arthrosis of joints, warranting arthrodesis of the affected joints accordingly.

Compensatory Movements of the Midfoot Joints Influence Gait Pattern After Arthroscopic Ankle Arthrodesis.

BACKGROUND: Arthroscopic ankle arthrodesis (AAA) is a standard procedure for end-stage osteoarthritis of the ankle. One of the main concerns after AAA remains the development of secondary osteoarthritis in the subtalar and tarsal joints in the long term. This development is thought to be due to a compensatory increased mobility and therefore increased load on the adjacent joints. Therefore, the aim of the study was to analyze the residual motion of the tarsal joints, the load distribution under the foot, and the influence of tarsal joint motion on load distribution and gait pattern after AAA.

[Retrograde intramedullary nail fixation for Charcot neuroarthropathy].

UNLABELLED: Charcot arthropathy is defined as an articular degenerative, chronic, progressive disease that affects one or more peripheral joints, develops as a result of a failure in the normal sensory perception (pain and proprioception) in the innervation of joints located in the foot and ankle, is characterized by destructive bone lesions without the presence of infection. Neuropathy is directly associated with diabetes mellitus type 1 and 2. Surgery is indicated when there is severe involvement of soft tissue, foot joints are unstable, at the presence of chronic or recurrent ulcers or when

Quantitative assessment of foot structure in rheumatoid arthritis by a foot digitizer: detection of deformities even in the absence of erosions.

OBJECTIVE: Foot involvement is a major feature in rheumatoid arthritis (RA), leading to structural deformities. Methods to allow a 3-dimensional (3-D) evaluation of foot structure in RA to be applicable in daily clinical practice have not been evaluated. This study assessed the use of a foot digitizer, a noninvasive 3-D scanner collecting objective quantitative data of the feet, to evaluate the presence of foot structure abnormalities in an RA outpatient cohort. METHODS: Foot digitizer data of RA patients were compared with healthy controls. Subanalyses were performed to find relationships wi

Complex reconstruction for the treatment of dorsolateral peritalar subluxation of the foot. Early results after distraction arthrodesis of the calcaneocuboid joint in conjunction with stabilization of, and transfer of the flexor digitorum longus tendon to, the midfoot to treat acquired pes planovalgus in adults.

BACKGROUND: The successful correction of flatfoot in children through lengthening of the lateral column, osteotomy of the medial cuneiform, and advancement of the posterior tibial tendon led to the introduction of similar procedures to treat acquired pes planovalgus secondary to attrition or rupture of the posterior tibial tendon in adults. However, to our knowledge, no study has been published documenting whether these procedures are effective treatment for acquired flatfoot in adults. METHODS: The functional and radiographic results of complex reconstruction of a painful, flexible flatfoot

Deformity or dysfunction? Osteopathic manipulation of the idiopathic cavus foot: A clinical suggestion.

Observed gait abnormalities are often related to a variety of foot deformities such as the cavus foot, also known as pes cavus, cavovarus, uncompensated varus, and the high arched foot. When gait abnormalities related to cavus foot deformities produce symptoms or contribute to dysfunctional movement of the lower extremity, foot orthotics are commonly used to accommodate the deformity and optimize the function of the lower extremity. In more severe cases, surgical intervention is common. Hypomobility of the many joints of the foot and ankle may be mistaken as an idiopathic cavus foot deformity.

Age-related differences in foot structure and function.

The aim of this study was to compare foot characteristics and plantar force and pressure patterns in young and older people. Fifty young (mean age 20.9+/-2.6 years) and 50 older (mean age 80.2+/-5.7 years) people without foot problems underwent tests of foot posture, range of motion, strength, sensation and deformity. Plantar force and pressure distribution during gait were evaluated using a floor-mounted resistive sensor mat system. Older participants exhibited flatter/more pronated feet, reduced range of motion of the ankle and 1st metatarsophalangeal joints, a higher prevalence of hallux va

Effect of Foot Anthropometric Measurements on Pain and Functional Disability of Pregnant and Nonpregnant Women.

BACKGROUND: Weight gain during pregnancy, loosening of the foot ligaments with relaxin hormone release, and anthropometric and biomechanical changes can be seen in the foot with the growing fetus. We sought to determine whether these pregnancy-related changes cause painful musculoskeletal problems by altering the proximal structures of the lower extremities and the structure of the spine. METHODS: Both feet of 210 women (half in the third trimester of pregnancy) aged 18 to 40 years with similar demographic characteristics were evaluated by measuring navicular drop, hallux valgus angle, foot w

Foot kinematics and kinetics data for different static foot posture collected using a multi-segment foot model.

This dataset presents human foot joints kinematics and kinetics data during walking, classified by static foot posture, filling a gap in existing lower limb databases that lack data on foot joints beyond the ankle or on static posture data, despite its link to foot and lower limb pathologies. Kinematics were recorded using a three-dimensional mocap system, and kinetics through a pressure platform, employing a multi-segment foot model including the ankle, midtarsal and first metatarsophalangeal joint. The dataset contains 350 recordings of right foot joint angles and moments and contact pressur

Fusion in posttraumatic foot and ankle reconstruction.

Despite appropriate acute treatment, many foot and ankle injuries result in posttraumatic arthritis. Arthrodesis remains the mainstay of treatment of end-stage arthritis of the foot and ankle. An understanding of the biomechanics of the foot and ankle, particularly which joints are most responsible for optimal function of the foot, can help guide reconstructive efforts. A careful history and physical examination, appropriate radiographs, and, when necessary, differential selective anesthetic blocks help limit fusion to only those joints that are causing pain. Compression fixation, when possibl
