A variety of cold exposure injuries were discussed, including frostnip, chilblains, trench foot, frostbite, and hypothermia. The usual precipitating factors for each were discussed along with the clinical course, the pathophysiology, and management from the perspective of one being in the field or backcountry. Prevention is the key to all cold-related problems as adequate treatment is often not possible out of the hospital setting. Detailed recommendations were made for all conditions, relative to treatment and prevention, with special emphasis on hypothermia.

Experience with 22 consecutive thoracic sympathectomies is presented. Diagnoses included minor causalgia (12), mixed vasospastic and occlusive disease (4), Raynaud's disease (4), and frostbite with ulceration (2). The operation was a transaxillary resection of ganglia T2, T3, and T4. Both objective and subjective results indicate that this type of sympathectomy is as good as more extensive resections without the risk of a Horner's syndrome. Morbidity was minor but frequent and included dysesthesia, pneumothorax, and compensatory sweating. The success rate of 77%.

Different types of dermoplasty aimed at the closure of reactive wounds were applied in the treatment of 26 cases with protracted non-healing wounds (up to 30 years) following burns, frostbites, open fractures, outcomes of acute suppurative diseases with sepsis phenomena. The treatment failed only in one patient. The transplantation of free stamp-like skin grafts proved to be the most effective one. Biostimulating effect of the single grafts which fuse has been noted.

Thermal injuries are one of the most common injuries in both civilian and combat scenarios. The importance of clinical determination of burn and frostbite injuries and treatment involves understanding the pathophysiology and mechanisms of these injuries while continually reviewing literature and studying new treatment modalities. This present review examines the (1) epidemiology, (2) etiology, (3) pathophysiology and classification, and (4) treatment of thermal injuries occurring to the foot. In addition to the paucity of new literature and studies on thermal injury, this is the first review,

Cold panniculitis has been described in children and young women following cold exposure. Histopathologically, cold panniculitis shows a mostly lobular panniculitis, which consists of an infiltrate of lymphocytes and histiocytes in the fat lobules. Usually, the dermis shows a superficial and deep perivascular infiltrate mostly composed of lymphocytes, with no vasculitis. Inflammation is most intense at the dermal-subcutaneous junction. Differential diagnosis of cold panniculitis should be established with subcutaneous fat necrosis of the newborn, sclerema neonatorum, poststeroid panniculitis,

Increased participation in outdoor activities and the epidemic of homelessness have caused the incidence of cold injuries in the civilian population to rise dramatically over the last 20 years. Knowledge of the treatment is crucial for emergency physicians in rural and urban areas. Recent developments have significantly advanced the understanding of the pathophysiology of hypothermic and frostbite injuries. Together with improved rewarming techniques and use of radiological assessment of tissue viability, future advancements should allow for a more aggressive and active approach to the managem

In April-May 1996 a Norwegian expedition climbed Mount Everest from the Tibetan side, Eight out of the ten members reached the summit. The expedition did not experience any serious incidents but took part in the rescue and treatment of climbers from other expeditions. Eleven cases are reported. Four of these were fatal. Cerebral oedema and frostbite were the most common problems in addition to hypothermia and exhaustion. The author also discusses acute mountain sickness, including clinical features, treatment and prophylaxis.

This paper describes a Cold Weather Ensemble Decision Aid (CoWEDA) that provides guidance for cold weather injury prevention, mission planning, and clothing selection. CoWEDA incorporates current science from the disciplines of physiology, meteorology, clothing, and computer modeling. The thermal performance of a cold weather ensemble is defined by endurance times, which are the time intervals from initial exposure until the safety limits are reached. These safety limits correspond to conservative temperature thresholds that provide a warning of the approaching onset of frostbite and/or hypoth

An elderly man, without heat in his home for two days, presented with cold, painful, swollen feet. Second-degree frostbite was present. After having moist packs applied to his feet for five days, the patient became febrile and hypotensive, and wet gangrene necessitated left below-knee and right above-knee amputations. Aeromonas hydrophila was isolated from the patient's blood and cultures of the pedal wounds. An etiologic role of the moist pack therapy is postulated.

Hypothermia is classically defined as a reduction in the body's core temperature below 95.0°F (35.0 °C). Most reported cases of hypothermia are due to environmental exposure to low ambient temperatures (accidental hypothermia). Other causes of hypothermia include sepsis, severe hypothyroidism (myxedema coma), acute spinal cord injury, diabetic ketoacidosis, multisystem trauma, and prolonged cardiac arrest. Frostbite, chillblain and trench foot all have specific therapies to prevent limb loss. There have been significant advances in these therapeutic options over the past decade which are detai

BACKGROUND: The transradial approach in percutaneous coronary procedures has become the primary vascular access worldwide. Cryotherapy is frequently used to promote analgesia and vasoconstriction. This study aimed to evaluate the safety of forearm puncture site cooling using the novel hemostatic Cryoband protocol.
METHODS: The proposed protocol modifies pneumatic wristband devices by incorporating a cooling plate of frozen gel on the forearm, situated beneath the air cuff. This is a pilot, prospective, single-arm study involving 100 patients. The primary endpoints were device success and time

The spectrum of cold injuries ranges from frostnip, chilblains to severe frostbite. Cold injuries occur upon prolonged exposure to freezing temperature and are pathologically a combination of ice crystal formation in the tissue resulting in inflammation, thrombosis and ischemia in the extremities, often necessitating limb amputation in extreme cases due to tissue necrosis. Severe forms of frostbite are a cause of major concern to patients as well as the treating physician. Due to the lack of effective treatment modalities and paucity of research on prophylaxis and therapeutics of cold injuries

Milia en plaque (MEP) is a relatively rare benign epithelial lesion, with its etiology and standard treatment course yet to be established. A 10-year-old female presented with bilateral white auricular papules and plaques persisting for over 7 months. Following a history of frostbite on both auricles, white and yellow millimeter-sized papules developed on erythematous patches. Clinical and histopathological examinations confirmed a diagnosis of MEP. The patient was treated using radiofrequency ablation in combination with cyst content extraction via a comedone extractor. No recurrence was obse

With the increase in outdoor events, there is an inevitable rise in climate-related environmental emergencies. Heat exposure can place athletes at risk for life-threatening heatstroke which requires emergent diagnosis and rapid in-field management. Cold exposure can lead to hypothermia, frostbite, and other nonfreezing injuries that require prompt evaluation and management to minimize morbidity and mortality. Altitude exposure can lead to acute mountain sickness or other serious neurologic or pulmonary emergencies. Finally, harsh climate exposure can be life-threatening and require appropriate

The use of single photon emission computer tomography (SPECT/CT) in acute vascular injury is not well documented. SPECT/CT combines the anatomic detail of computer tomography with the functional vascular perfusion of photon emission to determine the viability of osseous structures and surrounding soft tissue. The superimposed imaging provides the practitioner with a reliable anatomic image of viability of a specific anatomic area following insult or injury. We present two cases, bilateral lower extremity frostbite, and symmetric peripheral gangrene in which this imaging modality provided guida

As arctic warfare becomes a center focus within Special Operations, cold weather injury looms as both a medical and operational threat. While cold weather injury can range from pernio to hemodynamically unstable systemic hypothermia, the more minor injuries are far more common. However, these present a challenge in austere medical care and can drastically impact mission capability. We present a case of a Special Operations crew chief with cold weather digital injury while at the Arctic Isolation Course in Alaska and his subsequent clinical course. Prevention remains the key for mitigating thes
