Emerging viral threats in Gabon: health capacities and response to the risk of emerging zoonotic diseases in Central Africa.

Emerging infectious diseases (EID) are currently the major threat to public health worldwide and most EID events have involved zoonotic infectious agents. Central Africa in general and Gabon in particular are privileged areas for the emergence of zoonotic EIDs. Indeed, human incursions in Gabonese forests for exploitation purposes lead to intensified contacts between humans and wildlife thus generating an increased risk of emergence of zoonotic diseases. In Gabon, 51 endemic or potential endemic viral infectious diseases have been reported. Among them, 22 are of zoonotic origin and involve 12

Evaluation of the diagnostic performance of iChroma serological test in high SARS-CoV-2 seroprevalence setting in Gabon.

Coronavirus disease 2019 (COVID-19) has been a major global health concern over the past few years. The African region has remained less affected in terms of mortality, however, estimating the true cumulative incidence in this region is challenging due to several limitations. Serological tests can help evaluate the SARS-CoV-2 exposure at both individual and population levels. Several serological rapid tests have been developed, but their performance at population level is not always known. This current study aimed to evaluate the diagnostic performance of the iChroma test for the rapid detecti

Total IgE levels in HIV-negative and HIV-positive individuals infected with parasites in rural settings of Gabon.

BACKGROUND: The immune response of people living with HIV (PLHIV) coinfected with intestinal and/or blood helminths is still understudied in Gabon. The aim of this study was to compare IgE levels between HIV-positive and HIV-negative people depending on parasite carriage. METHODS: Samples from PLHIV and HIV-negative individuals, carriers or not of intestinal parasites and/or microfilariae and/or Plasmodium falciparum, were used as the study materials. The total IgE assay was performed using the VIDAS PC. RESULTS: Samples from 152 participants were included in the study, 91 HIV-positive and 6

Antimicrobial Resistance and Phylo-Groups of Escherichia coli at the Human-Primate Interface in Gabon: A One Health Study.

BACKGROUND/OBJECTIVES: Antimicrobial resistance (AMR) is a global threat to human, animal, and environmental health. Among bacteria, E. coli is frequently used as a key indicator of AMR. Despite their genetic proximity to humans, studies on AMR in Non-Human Primates (NHPs) remain limited, particularly in semi-anthropized environments. This study aims to characterize the antibiotic resistance profiles and phylo-groups of E. coli isolated from NHPs and humans at a primatology center. METHODS: A total of 143 stool samples were collected, including 125 from NHPs and 18 from humans. Isolates were

Malaria Prevalence in Asymptomatic and Symptomatic Children Living in Rural, Semi-Urban and Urban Areas in Eastern Gabon.

BACKGROUND: Malaria remains a major public health issue in the world despite a decline in the disease burden. However, though symptomatic malaria is diagnosed and treated, asymptomatic infections remain poorly known and support transmission. This study assessed the prevalence of symptomatic and asymptomatic Plasmodium spp. infections in three areas in Gabon to monitor and evaluate the impact of malaria. METHODS AND RESULTS: A cross-sectional study was conducted in three areas of Gabon. Febrile and afebrile children aged 6 months to 15 years were included in this study. Malaria prevalence was

Validation of artificial intelligence-based digital microscopy for automated detection of Schistosoma haematobium eggs in urine in Gabon.

INTRODUCTION: Schistosomiasis is a significant public health concern, especially in Sub-Saharan Africa. Conventional microscopy is the standard diagnostic method in resource-limited settings, but with limitations, such as the need for expert microscopists. An automated digital microscope with artificial intelligence (Schistoscope), offers a potential solution. This field study aimed to validate the diagnostic performance of the Schistoscope for detecting and quantifying Schistosoma haematobium eggs in urine compared to conventional microscopy and to a composite reference standard (CRS) consist

A long-term follow-up study on otoacoustic emissions testing in paediatric patients with severe malaria in Gabon.

BACKGROUND: In a previous study, severe and cerebral malaria have been connected with acute cochlear malfunction in children, demonstrated by a decrease of transitory evoked otoacoustic emissions (TEOAEs) reproducibility. This study aims to determine whether cochlear malfunction persists for 4 years after recovery from severe malaria in a subset of the previous study's collective. Follow-up TEOAEs were performed on site (CERMEL, Hôpital Albert Schweitzer, Lambaréné, Gabon) or at the participants' homes; 33 out of 90 participants included in the initial investigation by Schmutzhard et al. could

Preliminary Evidence for the Absence of Cystic Echinococcosis in Gabon: A Cross-Sectional Pilot Survey in Humans and Definitive Hosts.

Cystic echinococcosis (CE) is a globally endemic zoonosis caused by the larval stage of the Echinococcus granulosus sensu lato (s.l.) complex. Although the disease is known to be highly prevalent in certain parts of North and East Africa, data on CE, both in humans and definitive hosts, are extremely scarce for Central Africa. The present study assessed the epidemiology of CE in humans and dogs in rural Gabon. An ultrasound and serologic survey was conducted in volunteers from rural villages in Gabon. A two-step approach was used for serological testing with an indirect hemagglutination assay

Molecular Detection of Fastidious and Common Bacteria as Well as Plasmodium spp. in Febrile and Afebrile Children in Franceville, Gabon.

Malaria was considered as the main cause of fever in Africa. However, with the roll back malaria initiative, the causes of fever in Africa may change. This study aimed to evaluate the prevalence of bacteria and Plasmodium spp. in febrile and afebrile (controls) children from Franceville, Gabon. About 793 blood samples from febrile children and 100 from controls were analyzed using polymerase chain reaction (PCR) coupled with sequencing. Plasmodium spp. was the microorganism most detected in febrile (74.5%, 591/793) and controls (13%, 13/100), P < 0.0001. Its coinfection with bacteria was found

Impact of Anti-Retroviral Treatment and Cotrimoxazole Prophylaxis on Helminth Infections in HIV-Infected Patients in Lambaréné, Gabon.

BACKGROUND: Foci of the HIV epidemic and helminthic infections largely overlap geographically. Treatment options for helminth infections are limited, and there is a paucity of drug-development research in this area. Limited evidence suggests that antiretroviral therapy (ART) reduces prevalence of helminth infections in HIV-infected individuals. We investigated whether ART exposure and cotrimoxazole preventive therapy (CTX-P) is associated with a reduced prevalence of helminth infections. METHODOLOGY AND PRINCIPAL FINDINGS: This cross-sectional study was conducted at a primary HIV-clinic in La

Prevalence of hepatitis B virus infection and associated factors among students living on campus in South-eastern Gabon.

BACKGROUND: The Hepatitis B-virus is the etiological agent of an infection that affects liver functions and potentially causes disorders. Approximately, 300 million people are living with acute or chronic HBV-infections worldwide, and only a fraction of them have access to medical care. The sub-Saharan African (SSA) regions house around 80 million of reported infection cases, and certain SSA-groups like pregnant women and drug-addict are among the most vulnerable. In as much as there is insufficient national data on young adult groups, this study aims to determine HBV-prevalence and associated

Ebola and Marburg viruses IgG detections in small ruminants and dogs from villages within outbreak areas in Gabon.

The Ebola virus (EBOV) and Marburg virus (MARV) have been in circulation in Africa for several decades and are the cause of numerous outbreaks. There has been very little research on the role of domestic animals in their transmission to humans, but studies have only been conducted in dogs and pigs where relatively high levels of IgG was detected. These levels suggest that ruminants, which have not been studied, should also be investigated. This study aims at evaluating the circulation of MARV and EBOV in dogs, sheep and goats and to assess their exposure to these two viruses. Between November

Recurring Transient Tooth Pain as Newly Described Symptom of Migratory Loiasis: A Mixed-Methods Study in Rural Gabon.

Loiasis, a filarial pathogen exclusively endemic in central and western Africa, causes a wide spectrum of symptoms. Understanding the breadth of its clinical manifestations is of importance for adequate patient care and to understand its disease burden. Recurring transient pain in the oral cavity was reported as a self-perceived symptom of loiasis in in-depth interviews of patients in a high transmission region in Gabon. Pain was described as stabbing in character and transient for a few days in its temporal course. A quantitative epidemiological survey indicated that transient tooth pain was

Enhanced pro-inflammatory cytokine responses following Toll-like-receptor ligation in Schistosoma haematobium-infected schoolchildren from rural Gabon.

BACKGROUND: Schistosoma infection is thought to lead to down-regulation of the host's immune response. This has been shown for adaptive immune responses, but the effect on innate immunity, that initiates and shapes the adaptive response, has not been extensively studied. In a first study to characterize these responses, we investigated the effect of Schistosoma haematobium infection on cytokine responses of Gabonese schoolchildren to a number of Toll-like receptor (TLR) ligands. METHODOLOGY: Peripheral blood mononuclear cells (PBMCs) were collected from S. haematobium-infected and uninfected
