Sensor-derived heel pressure metrics capture reversible gait dysfunction beyond conventional gait measures in normal pressure hydrocephalus.

BACKGROUND: The response to the cerebrospinal fluid (CSF) tap test helps determine the surgical eligibility of individuals with idiopathic normal pressure hydrocephalus (iNPH); however, conventional gait assessments have limited specificity and low negative predictive value. We investigated whether commercially available, low-cost sensor-embedded insoles could offer sensitive gait biomarkers for detecting changes in gait following the CSF tap test. METHODS: Seventy-seven individuals with iNPH were prospectively enrolled. Participants performed conventional gait assessments, including the 10-m

Beta burst-driven adaptive deep brain stimulation for gait impairment and freezing of gait in Parkinson's disease.

Freezing of gait is a debilitating symptom of Parkinson's disease that is often refractory to medication. Prolonged beta bursts within the subthalamic nucleus are associated with worse impairment and freezing, which are improved with deep brain stimulation. The goal of the study was to investigate the feasibility, safety and tolerability of beta burst-driven adaptive deep brain stimulation for gait impairment and freezing of gait in Parkinson's disease. Seven individuals with Parkinson's disease were implanted with the investigational Summit™ RC + S deep brain stimulation system (Medtronic, PL

DUO-GAIT: A gait dataset for walking under dual-task and fatigue conditions with inertial measurement units.

In recent years, there has been a growing interest in developing and evaluating gait analysis algorithms based on inertial measurement unit (IMU) data, which has important implications, including sports, assessment of diseases, and rehabilitation. Multi-tasking and physical fatigue are two relevant aspects of daily life gait monitoring, but there is a lack of publicly available datasets to support the development and testing of methods using a mobile IMU setup. We present a dataset consisting of 6-minute walks under single- (only walking) and dual-task (walking while performing a cognitive tas

Gait pattern in myotonic dystrophy (Steinert disease): a kinematic, kinetic and EMG evaluation using 3D gait analysis.

We investigated the gait pattern of 10 patients with myotonic dystrophy (Steinert disease; 4 females, 6 males; age: 41.5+7.6 years), compared to 20 healthy controls, through manual muscle test and gait analysis, in terms of kinematic, kinetic and EMG data. In most of patients (80%) distal muscle groups were weaker than proximal ones. Weakness at lower limbs was in general moderate to severe and MRC values evidenced a significant correlation between tibialis anterior and gastrocnemius medialis (R=0.91). An overall observation of gait pattern in patients when compared to controls showed that mos

Accelerometer versus footswitch evaluation of gait unsteadiness and temporal characteristics of gait in two elderly patient groups.

We examined anterior-posterior and medio-lateral head and trunk movements during gait using accelerometers, and a footswitch evaluation of temporal gait parameters of elderly fallers with a primary diagnosis of Orthostatic Hypotension, elderly fallers without Orthostatic Hypotension, and a control group of healthy elderly non-fallers. We wanted to evaluate whether both sets of measures can be used to differentiate between patient groups. We were able to significantly differentiate between the three elderly groups to the same extent using both sets of measures.

The common and specific osteoarthritis gait characteristics: A quantitative grading system for OA associated gait changes in mice.

OBJECTIVE: Osteoarthritis (OA) severity in animal models has been quantified primarily through grading systems reflecting the changes of joint morphology. There is a lack of a uniform grading system to quantify the changes in motion. Here, we develop a quantitative, easy-to-use grading system for OA-associated gait changes in mice. DESIGN: We characterized the mouse gait changes in a surgery induced post-traumatic OA (PTOA) model and aging-associated OA (aging-OA) models using DigiGait. This treadmill system captured running motions for video analysis. We developed and validated the OA Gait g

Gait abnormalities and ability to alter gait in children with idiopathic toe walking with a mild/moderate calf contracture.

BACKGROUND: Idiopathic toe walking is a common pediatric condition with those who continue to walk on their toes beyond childhood described as having persistent idiopathic toe walking (ITWp). Children with ITWp and mild/moderate contractures might be able to alter their gait pattern to a more typical gait. The purpose of this study was to compare self-selected toe walking to a "best-walk" pattern for children with ITWp and mild/moderate gastrocnemius/soleus contractures. METHODS: This prospective multi-site study recruited children with ITWp, aged 6-18 years, and physical exam dorsiflexion wi

Validity of Linear and Nonlinear Measures of Gait Variability to Characterize Aging Gait with a Single Lower Back Accelerometer.

The attractor complexity index (ACI) is a recently developed gait analysis tool based on nonlinear dynamics. This study assesses ACI's sensitivity to attentional demands in gait control and its potential for characterizing age-related changes in gait patterns. Furthermore, we compare ACI with classical gait metrics to determine its efficacy relative to established methods. A 4 × 200 m indoor walking test with a triaxial accelerometer attached to the lower back was used to compare gait patterns of younger (N = 42) and older adults (N = 60) during normal and metronome walking. The other linear a

Frontal plane knee moment in clinical gait analysis: A systematic review on the effect of kinematic gait changes.

INTRODUCTION: The frontal plane knee moment (KAM1 and KAM2) derived from non-invasive three-dimensional gait analysis is a surrogate measure for knee joint load and of great interest in clinical and research settings. Many aspects can influence this measure either unintentionally or purposely in order to reduce the knee joint load to relieve symptoms and pain. All these aspects must be known when conducting a study or interpreting gait data for clinical decision-making. METHODS: This systematic review was registered with PROSPERO (CRD42020187038). Evaluation of overall gait quality in Perthes disease. Are global gait indices of value for a local musculoskeletal disorder?

BACKGROUND: Until now follow-up- and outcome evaluations in Perthes disease are analysing predominantly subjective results and clinical/radiological parameters. To enlarge the assessment by quantitative, functional aspects a summary measure for overall gait quality would be helpful. Therefore, the objective of this study is to evaluate whether commonly used global gait indices are sensitive to detect gait deviations during the early stages of the Perthes disease. METHODS: 3D gait data of 70 patients scheduled for containment improving surgery with the diagnosis of Perthes were included. A gro
