Cholinesterase inhibitors and Pisa syndrome: a pharmacovigilance study.

STUDY OBJECTIVES: Case reports suggest a relationship between cholinesterase inhibitors (ChEIs) and Pisa syndrome (PS), also known as pleurothotonus, a form of dystonia, but this relationship has not been systematically examined. Our objective was to estimate the adjusted reporting ratios of PS with donepezil, rivastigmine, and galantamine in the United States Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS) database. DESIGN: Retrospective analysis of adverse event reports in the FAERS database. PATIENTS: Patients with drug-related adverse events in the FAERS databas

Cholinesterase inhibitors and beyond.

Cholinesterase inhibitors (ChEIs) were introduced in the therapy of Alzheimer Disease (AD) in the nineteen nineties with great expectations. The hopes and large interest raised by these drugs are well demonstrated by 12,000 references listed by PubMed under 'ChEI' for 1995-2007. The list is reduced to 2500 if we confine ourselves to 'ChEIs and dementia'. Of them, about 500 were published in the last two years. Whereas an increase in brain acetylcholine and an improvement of cognitive deficits have been consistently demonstrated in animal models of AD, from aging rats to transgenic mice, the cl

Bipolar disorder and health-related quality of life : review of burden of disease and clinical trials.

BACKGROUND AND OBJECTIVE: Studies suggest that some acetylcholinesterase inhibitors (AChEIs) increase rapid eye movement (REM) sleep and nightmares in patients with Alzheimer's disease (AD) but few have studied their effect on other sleep parameters. The objective of this study was to examine differences in sleep architecture in AD patients taking different AChEIs. METHODS: 76 participants (51 men, 25 women) [mean age = 78.2 years; SD = 7.7] with mild to moderate AD underwent medication history screening as well as polysomnography to determine the percentage of each sleep stage. Participants

Interaction of cholinesterase inhibitors with snake venom acetylcholinesterase from snake species of Southern Africa.

Acetylcholinesterase (AChE) is present in the venom of Elapid snakes, except the Mambas. Although the physiological role of AChE in snake venom is uncertain, it may contribute to acute neuromuscular paralysis following envenomation. With the aim of repurposing clinically used drugs for snakebite envenoming, this study investigated the inhibition of AChE from snake species of Southern Africa. Hemachatus haemachatus and Naja nivea venom presented with high AChE activity, which was inhibited by tacrine, galantamine and donepezil with IC50 values ranging from 0.49 to 3.73 μM.

Cholinesterase inhibitors for patients with dementia: Patterns of prescribing and disparities in treatment.

BackgroundCholinesterase inhibitors (ChEIs) are cornerstones of the symptomatic treatment of Alzheimer's disease (AD) and dementia with Lewy bodies (DLB) and are also prescribed for vascular dementia (VaD). Despite their widespread use, patterns of ChEI prescribing are unclear.ObjectiveOur objective was to examine the prevalence, timing, and types of ChEI prescriptions before and after dementia diagnosis.MethodsWe analyzed electronic health record and claims data for patients diagnosed with AD, DLB, or VaD between October 2015 and August 2022 from a large U.S. healthcare system. Metabolomic Based Insight Reveals a few Metabolites at Various Germination Stages of Black Rice, Demonstrating Potential Efficacy Against Dementia and Other Neurodegenerative Conditions.

Dementia is a brain disorder that impairs the cognitive abilities like memory, thinking, reasoning, and judgement, thereby restricts an individual's capacity to carry out daily activities. Alzheimer's disease (AD) is a prominent example of one such condition, representing approximately 60-70% of dementia cases and is characterized as an irreversible multifaceted neurodegenerative disorder. The enzyme Acetylcholinesterase (AChE) is a significant contributor to dementia and other neurodegenerative disorder, where this enzyme hydrolyses acetylcholine, a crucial neurotransmitter, thereby disruptin

Different TMS patterns of intracortical inhibition in early onset Alzheimer dementia and frontotemporal dementia.

OBJECTIVE: To investigate putative changes in cortical excitability of patients affected by early-onset mild dementia by means of transcranial magnetic stimulation (TMS) and to verify whether a peculiar neurophysiological profile may contribute to characterise Alzheimer's disease (AD) vs frontotemporal dementia (FTD). METHODS: Motor threshold and intracortical inhibition (ICI) and facilitation (ICF) after paired-pulse TMS (inter-stimulus intervals from 1 to 20 ms) were studied in two groups of early-onset demented patients with a neuropsychological profile suggestive of AD (n = 12) and FTD (n

A Systematic Review of Effects of Cholinesterase Inhibitors and Memantine on Cognitive Domains in Alzheimer's Disease.

BackgroundAlzheimer's Disease (AD) is characterised by progressive cognitive decline. Cholinesterase inhibitors (ChEI) (donepezil, rivastigmine and galantamine) and memantine have been the mainstay treatment and have showed their effectiveness on total cognitive scores, but their effects on individual cognitive domains remain unclear. This systematic review examined their impact on individual cognitive domains.MethodPubMed, Cochrane, MEDLINE, Web of Science and PsycINFO were searched (1st January 1999 - 31st March 2025) for studies evaluating the effects of ChEI and memantine on cognitive doma

Cholinesterase inhibitors and sexual disinhibition in dementia: a FAERS disproportionality study.

Sexual disinhibition under cholinesterase inhibitors (CHEIs; donepezil, rivastigmine, galantamine) appears only as scattered case reports and is largely absent from product labels. We examined whether it is disproportionately reported for CHEIs relative to memantine, a same-indication dementia drug of distinct mechanism, using the publicly available US FDA Adverse Event Reporting System (FAERS). We analysed 47 quarters (2014Q3-2026Q1; 15,901,334 de-duplicated reports) in a within-class active-comparator design, computing reporting odds ratios (ROR, 95% CI) with Benjamini-Hochberg FDR control (

Coclaurine N-methyltransferase-like enzymes drive the final biosynthetic reaction of the anti-Alzheimer's drug galanthamine in Amaryllidaceae.

Galanthamine, an isoquinoline alkaloid used to treat symptoms of Alzheimer's disease, is predominantly extracted from Amaryllidaceae plants, yet its supply remains limited. In this study, we identified, isolated, and characterized N-methyltransferases (NMTs) from three galanthamine-producing species: Leucojum aestivum, Lycoris radiata, and Hippeastrum papilio. The transcriptomic analysis identified five unique NMT isoforms, among which LaLrHpNMT1, an isoform highly conserved across all three species, exhibited the highest catalytic activity. Phylogenetic and structural analyses revealed that t

Evaluation of four Narcissus cultivars as potential sources for galanthamine production.

Galanthamine, an alkaloid present in the Amaryllidaceae is currently undergoing clinical trials for the treatment of Alzheimer's. Common daffodils, Narcissus spp., contain galanthamine and other alkaloids. Four commercial Narcissus cultivars were evaluated as potential sources of galanthamine. Planting depths, planting densities, bulb size or flower bud removal did not affect galanthamine content.

New sulfonamide derivatives based on 1,2,3-triazoles: synthesis, in vitro biological activities and in silico studies.

Eight new hybrid constructs containing a series of sulfonamide and 1,2,3-triazole units were designed and synthesized. Anticancer, antioxidant and cholinesterase activities of these hybrid structures were investigated. In our design, the Cu(I)-catalyzed click reaction between N,4-dimethyl-N-(prop-2-yn-1-yl)benzenesulfonamide (6) and aryl azides 8a-h was used. Antioxidant activity values of 9f (IC50: 229.46 ± 0.001 μg/mL) and 9h (IC50: 254.32 ± 0.002 μg/mL) hybrid structures were higher than BHT (IC50: 286.04 ± 0.003 μg/mL) and lower than Ascorbic acid (IC50: 63.53 ± 0.001 μg/mL) and α-Tocopher

Prescribing trends in cognition enhancing drugs in Australia.

BACKGROUND: This study sought to examine the trends in the prescribing of subsidized and unsubsidized cognition enhancing drugs (CEDs) in Australia over five years from 2002 to 2007. Subsidized cholinesterase inhibitor medication could be prescribed to people with mild to moderate Alzheimer's disease (AD) once a specialist physician had confirmed this diagnosis. Memantine was available for use in moderately severe AD but not subsidized. METHODS: We analyzed the Medicare Australia and Drug Utilisation Sub-Committee databases for CED prescription data, 2002-2007, by gender, age and prescriber c

Neuropsychiatric Symptoms in Dementia: Considering a Clinical Role for Electroencephalography.

OBJECTIVE: Degenerative dementia is characterized by progressive cognitive decline and neuropsychiatric symptoms. People with Alzheimer's disease (AD), the most common cause of dementia, show synaptic loss and disruption of functional brain networks along with neuritic plaques and neurofibrillary tangles. Electroencephalography (EEG) directly reflects synaptic activity, and among patients with AD it is associated with slowing of background activity. The purpose of this study was to identify associations between neuropsychiatric symptoms and EEG in patients with dementia and to determine whethe

Does kynurenic acid act on nicotinic receptors? An assessment of the evidence.

As a major metabolite of kynurenine in the oxidative metabolism of tryptophan, kynurenic acid is of considerable biological and clinical importance as an endogenous antagonist of glutamate in the central nervous system. It is most active as an antagonist at receptors sensitive to N-methyl-D-aspartate (NMDA) which regulate neuronal excitability and plasticity, brain development and behaviour. It is also thought to play a causative role in hypo-glutamatergic conditions such as schizophrenia, and a protective role in several neurodegenerative disorders, notably Huntington's disease.
