Chronic Kidney Disease of Non-traditional Etiology in a Young Man From Central America: Geography, Poverty, and Uncertain Pathophysiology Create a Formidable Medical Challenge.

A man in his early 20s with kidney biopsy-confirmed focal segmental glomerulosclerosis (FSGS) was admitted with one month of nausea and vomiting, intermittent episodes of confusion, shortness of breath, and dysuria. He reported that many people from his native village in Central America, where he harvested sugarcane as a child, have died from kidney disease, including his father and cousin. He believed the source of disease to be agrochemicals found in the village's water supply. Although FSGS would be a rare manifestation, the patient's risk factors strongly suggested chronic kidney disease o

STI phase and the geography of sexual partnerships: prevalence of long-distance sexual contacts among chlamydia, gonorrhea, and coinfected STI cases in Manitoba, Canada.

We utilized public health data and a geographic information system (ArcGIS) to study long-distance sexual partnerships (opposite and same sex partnerships) among chlamydia, gonorrhea, and coinfected STI (sexually transmitted infection) cases. The top 10% of relationships among chlamydia and gonorrhea cases and their contacts equaled or exceeded 198 km and 237 km respectively; the top 15% of partnerships among coinfected cases equaled or exceeded 207 km. This research also detected proportionately more long-distance partnerships among gonorrhea cases than among chlamydia cases. Wasserheit and A

Rising threats of ceftolozane-tazobactam resistance in P. aeruginosa and A. baumannii: Species-, phenotype-, and geography-specific insights from a systematic review and meta-analysis.

BACKGROUND: Ceftolozane-tazobactam (C/T) is increasingly used to treat multidrug-resistant Gram-negative infections, but rising resistance threatens its effectiveness. Understanding global patterns is essential for guiding therapy and stewardship. METHODS: We conducted a systematic review and meta-analysis of studies reporting C/T resistance in Pseudomonas aeruginosa and Acinetobacter baumannii. Pooled resistance proportions were estimated using random-effects models, with subgroup analyses by species, resistance phenotype, infection source, geography, and study period. Heterogeneity, tempora

Associations of four indexes of social determinants of health and two community typologies with new onset type 2 diabetes across a diverse geography in Pennsylvania.

Evaluation of geographic disparities in type 2 diabetes (T2D) onset requires multidimensional approaches at a relevant spatial scale to characterize community types and features that could influence this health outcome. Using Geisinger electronic health records (2008-2016), we conducted a nested case-control study of new onset T2D in a 37-county area of Pennsylvania. The study included 15,888 incident T2D cases and 79,435 controls without diabetes, frequency-matched 1:5 on age, sex, and year of diagnosis or encounter. We characterized patients' residential census tracts by four dimensions of s

Nuclear and mitochondrial genome sequencing of North-African Leishmania infantum isolates from cured and relapsed visceral leishmaniasis patients reveals variations correlating with geography and phenotype.

Although several studies have investigated genetic diversity of Leishmania infantum in North Africa, genome-wide analyses are lacking. Here, we conducted comparative analyses of nuclear and mitochondrial genomes of seven L. infantum isolates from Tunisia with the aim to gain insight into factors that drive genomic and phenotypic adaptation. Isolates were from cured (n=4) and recurrent (n=3) visceral leishmaniasis (VL) cases, originating from northern (n=2) and central (n=5) Tunisia, where respectively stable and emerging VL foci are observed. All isolates from relapsed patients were from Kairo

A Census Tract-Level Examination of Differences in Social Determinants of Health Among People With HIV, by Race/Ethnicity and Geography, United States and Puerto Rico, 2017.

OBJECTIVE: Social and structural factors, referred to as social determinants of health (SDH), create pathways or barriers to equitable sexual health, and information on these factors can provide critical insight into rates of diseases such as HIV. Our objectives were to describe and identify differences, by race/ethnicity and geography, in SDH among adults with HIV. METHODS: We conducted an ecological study to explore SDH among people with HIV diagnosed in 2017, by race/ethnicity and geography, at the census-tract level in the United States and Puerto Rico. We defined the least favorable SDH

'I care for my grandma because she's one of my best friends': Intergenerational caregiving and the everyday geographies of aging in Palm Beach County, Florida.

In this article, we examine the intricate dynamics of caregiving arrangements where individuals under the age of 18 and older adults care for each other and share caregiving responsibilities for other household members experiencing chronic illness, disability, or medical needs. Drawing on a subset of geospatial maps, sketch mapping, and interview data collected from a two-year research project with caregiving youth, the article explores how caregiving shapes the lived experiences of both young people and older adults in peri-urban Florida, and the spaces and places available to them in their e

Hypothesis: Cystic fibrosis carrier geography reflects interactions of tuberculosis and hypertension with vitamin D deficiency, altitude and temperature. Vitamin D deficiency effects and CF carrier advantage.

Interactions between selective factors (hypertension and tuberculosis) and environmental effects (vitamin D deficiency [VDD], temperature, and altitude) largely explain cystic fibrosis (CF) carrier geography. For VDD sequelae such as hypertension and tuberculosis vulnerability, clinical evidence of carrier protection is supported by indications that decreased CF arylsulfatase B activity suppresses tuberculosis, and that excess CF salt loss decreases blood pressure. A need for salt retention in the tropics selected against CF carriers despite possible advantages against cholera, typhoid, and ot

Integrative phylogenetics and geography unveil Quaternary diversification and mitochondrial-nuclear discordance in Odorrana schmackeri sensu lato (Anura, Ranidae): Roles of Qinghai-Tibetan uplift, climate oscillations, and asymmetric gene flow.

Odorrana schmackeri sensu lato, a group of montane stream-dwelling amphibians in southern China, provides an ideal model for deciphering how Cenozoic geological and climatic dynamics drive cryptic diversification. By integrating mitochondrial 12S/16S rRNA sequences from 1,398 samples spanning 107 populations and genome-wide SNP data from 193 individuals across 18 populations, we reconstruct the geographic structure and speciation history of O. schmackeri sensu lato. Our analyses resolved six evolutionarily distinct lineages in O. schmackeri sensu lato, i.e., Odorrana hejiangensis, Odorrana ich

Towards a "trauma-informed spaces of care" model: The example of services for homeless substance users.

While clinical practitioners have long recognized the importance of trauma-informed models of care, geographies of care scholars have been slow to engage with and address trauma in its methodologies for better understanding environments that support, or hinder, care for people. Marrying the conceptual contributions of geographies of care, trauma geographies, and geographies of addiction, this paper aims to advance the inquiry of trauma-informed spaces of care. Drawing on the example of the homeless substance user, we present a novel theoretical imperative for considering trauma on both an indi
