Decision-making in Hallux Valgus Correction.

Hallux valgus deformity is the most common surgical indication in the foot. The achievable result is highly dependent on preoperative planning and the choice of surgical procedure. Consideration of anatomic specifications, angulation at the MTP and intermetatarsal 1/2 (IMT), and joint status at the metatarsophalangeal and TMT joints provide the basis for this. Minor, moderate, and severe deformities can be assigned to different osteotomy heights and, within these, a surgical procedure. Each method, if applied appropriately, is suitable and its correct use allows simple operations with maximum

Avoiding Hallux Sesamoidectomy: A Narrative Review.

Hallux sesamoid pain arises from various pathological conditions, such as fracture nonunion, painful plantar keratosis, sesamoiditis, and avascular necrosis. Traditionally, sesamoidectomy is the primary surgical approach for these conditions, but its outcomes are inconsistent. In recent years, extracorporeal shock wave therapy (ESWT) and platelet-rich plasma (PRP) have gained popularity as effective conservative treatments in orthopedic practice. This review explored treatment approaches that avoid sesamoidectomy. First, we examined studies on ESWT and PRP for hallux sesamoid pathologies. We a

Hallux Valgus: Prevalence and Treatment Options.

BACKGROUND: Hallux valgus is a common deformity. Surgical treatment strategies have evolved markedly in recent years. We report on the administrative prevalence of this condition and the available treatments for it. METHODS: The administrative prevalence of hallux valgus and the treatments provided for it in Germany were determined from data supplied by BARMER, a statutory health insurance carrier. The classification and treatment of hallux valgus are outlined in a narrative overview, with particular attention to a meta-analysis. RESULTS: The administrative prevalence of hallux valgus in Ger

Outcomes After Hallux Interphalangeal Joint Arthrodesis.

BACKGROUND: Hallux interphalangeal joint arthrodesis (HIPJA) is indicated for a variety of pathologies. Despite high nonunion rates, techniques remain unchanged. The aim of this study is to examine nonunion and complication rates and describe risk factors for treatment failure. METHODS: A query of an institutional database was performed to identify all patients undergoing HIPJA procedure over a 10-year period. Records were reviewed to the procedure, evaluate patient factors, indications, and radiographic/clinical arthrodesis. Radiographic union was defined as 2 cortical continuations or bridg

Arthroscopic Sesamoidectomy for Hallux Sesamoid Disorders.

In this retrospective case series, we aimed to study arthroscopic sesamoidectomy, including surgical methods, clinical outcomes, and complications. We retrospectively reviewed the medical records of patients with hallux sesamoid disorders who underwent arthroscopic sesamoidectomy from July 2015 to July 2017. The visual analog scale for rating pain, Japanese Society for Surgery of the Foot scale scores, number of days taken to return to normal daily living, number of months taken to return to playing sports, and complications were analyzed. The mean follow-up duration was 3.2 years. Among 14 pa

Does hallux valgus impair physical function?

BACKGROUND: The relationships between radiographic hallux valgus (HV) and various physical functions independent of knee osteoarthritis (KOA) were examined among residents of a mountain village in Japan. METHODS: Study participants were recruited from mountain village residents aged ≥50 years. Participants' height, weight, and body mass index (BMI) were measured, and baseline data, including age, sex, and foot pain, were obtained using interviews and questionnaires. Radiography of the feet and knees was performed to assess the presence of HV (HV angle ≥20°) and KOA (Kellgren-Lawrence grade ≥

Percutaneous Surgery for Severe Hallux Valgus.

Severe hallux valgus is a challenge to treat. If the basal osteotomy is a well known surgery for severe deformity, the chevron osteotomy is usually used in mild to moderate deformity. With a accurent fixation the chevron can also be used in severe deformity. Both techniques can be performed percutaneously and offer reliable techniques.

First metatarsophalangeal arthrodesis for hallux valgus.

Arthrodesis of the first metatarsophalangeal joint is a reliable operation in the treatment of selected cases of hallux valgus. It corrects deformity of hallux valgus and metatarsus primus varus, leading to good functional results with a low complication rate. It is a technique well suited to patients with hallux valgus associated with degenerative changes or severe deformity, and those for whom primary hallux valgus surgery has failed.

Post-traumatic dynamic hallux varus instability.

Acquired hallux varus secondary to traumatic disruption of the lateral joint structures of the 1st MTPJ is uncommon and has only been reported in the literature once previously.(4) We present a case of traumatic hallux varus that is unique since the deformity is dynamic in nature. In our patient the hallux remained reduced on standing weight bearing films, and luxated only during fluoroscopic stress testing. We also describe our surgical correction where a soft tissue anchor alone was utilized to stabilize the joint through repair of the lateral capsule and collateral ligament. One year follow

[Treatment algorithm proposed for hallux rigidus].

Hallux rigidus is the most frequent arthrosis of the foot and ankle and the second most important condition involving the first metatarsophalangeal joint after hallux valgus. If left untreated, it may cause important alterations in gait, mobility and activities of daily living. The purpose of this review article is to describe a complete treatment algorithm for all grades of this condition, after a thorough bibliographic review.

Modified Youngswick procedure for hallux limitus.

Multiple surgical procedures have been described for the correction of hallux limitus deformity. We describe a new modification of the Youngswick procedure for the surgical treatment of hallux limitus. Other procedures for hallux limitus correction are also discussed. This modified Youngswick procedure will provide a new approach to treating hallux limitus secondary to metatarsus primus elevatus when shortening of the first metatarsal is not indicated.

Hallux proximal phalanx Akin-Scarf osteotomy.

Numerous hallux proximal phalanx osteotomies have been described, but the Akin-type varisation or adduction osteotomies are currently the most commonly used by foot and ankle surgeons. This article describes the hallux proximal phalanx Akin-Scarf osteotomy. This osteotomy combines the inherent stability of the Scarf-type osteotomy with the versatility of the Akin-type osteotomies to correct pathologic hallux abductus interphalangeus, hallux equinus, or an abnormal digital length pattern, and it is an invaluable tool during the global surgical approach used for hallux valgus repair.

Venous thrombosis after hallux valgus surgery.

BACKGROUND: Although surgery for the treatment of hallux valgus is frequently performed, the exact rate of deep vein thrombosis following this procedure is unknown. We performed a single-center, prospective, phlebographically controlled study to quantify the rate of venous thrombosis following operative correction of hallux valgus. METHODS: Consecutive patients undergoing chevron bunionectomy for correction of hallux valgus deformity were enrolled in the study. Patients with clinical or hematological risk factors for venous thrombosis were excluded. One hundred patients with a mean age of 48.

Mitchell osteotomy for adolescent hallux valgus.

Sixty-nine Mitchell osteotomies augmented with smooth-pin fixation and a trapezoidal step-off osteotomy to maintain ray length were performed on 46 adolescent patients with a painful hallux valgus deformity. Average follow-up time was 6 years. By using radiographic, clinical, and subjective patient data, we found an overall excellent or good result in 91% of cases. Nine feet were found to be cosmetically unsatisfactory, 11 required special shoewear, 10 had mild residual pain with high heels or strenuous running, and three had pain severe enough to restrict activities. Range of motion was norma

[Hallux valgus--the Dega surgical technic].

The author gives an account of one possible approach to the surgical treatment of valgus deformities of the toe. He corrects by the described subcapital wedge-shaped osteotomy of the first metatarsal bone the valgus deviation of the toe in the first metatarsophalangeal joint and at the same time he remodels the metatarsal arch of the foot. The surgical procedure is described and the results of operations, incl. complications, are evaluated. During check-up examinations some 85% of the patients found the results of the operation satisfactory. 13.5% of the patients reported pain when burdening t
