Osteomyelitis in the head and neck.

CONCLUSION: All bones of the facial skeleton and spine are susceptible to osteomyelitis due to various predisposing conditions. Current radiological tools are sufficient to provide adequate diagnosis. Treatment can be conservative resection of the diseased bone with adequate clearance in all cases except in cases of osteomyelitis due to osteoradionecrosis (ORN) where resection has to be more radical. OBJECTIVE: In today's antibiotic era, osteomyelitis in the head and neck is a rare occurrence. Dealing with osteomyelitis in head and neck bones is not the same as in other bones of the body due

[Mycoses of the head and neck].

In recent years, mycoses have emerged as important infections in clinical practice. This phenomenon is explained by the ever growing number of immunocompromised patients and the increasing number of people travelling in areas where fungal diseases are endemic. Head and neck infections are common in disseminated mycoses and may simulate carcinoma or cause upper airway obstruction. The most frequent causative yeasts or yeast-like organisms include Candida albicans, Cryptococcus neoformans, Histoplasma capsulatum var capsulatum, Blastomyces dermatitidis, Paracoccidioides brasiliensis and Coccidio

[Fractures of the humeral head].

Fractures of the humeral head are very common in elderly people, with 70% of all such fractures being seen at an age of more than 60 years. For the radiological examination of the fracture, x-rays from two levels are mandatory. The number and position of the fragments, assessment of intact or ruptured periosteum between the fragments and muscle forces acting on the fragments have to be determined from the x-rays. A 3-D CT scan can be very useful for better understanding of the character of the fracture. The remaining displacement between fragments after reduction have to be evaluated according

[Osteonecrosis of the femoral head].

The femoral head is the main location of avascular osteonecrosis. The lesion remains asymptomatic for several months or years before causing non specific hip pain. Risk factors have been identified, mainly femoral neck fractures, corticosteroid therapy and related conditions (lupus erythematosus, organ transplantations), alcohol abuse, dyslipidemia, sickle cell disease, HIV infection, caisson workers, Gaucher's disease, male sex. When typical radiological signs are lacking, MRI is the best investigation. Progression toward hip joint damage highly depends on the necrotic volume assessed at MRI.

Melioidosis of the head and neck.

Melioidosis is a potentially deadly infection that can affect any organ system. Reports of melioidosis of the ENT/head and neck region are relatively uncommon. Four cases are presented: (i) parotid abscess evolving into necrotising fasciitis, (ii) acute sinusitis and parapharyngeal cellulitis resulting in upper airway obstruction, (iii) acute suppurative lymphadenitis (iv) and chronic suppurative otitis media causing meningoencephalitis. Three of the four cases are believed to be unique, as a literature review of melioidosis in ENT/head and neck is also presented. Some practical issues of mana

[Carcinoma of the pancreatic head].

Focusing mainly on invasive ductal carcinoma of the pancreas, the history of radical surgery for this type of cancer is reviewed, including pancreatoduodenectomy, total pancreatectomy, extended radical surgery with main vascular resection and extended lymphadenectomy/retroperitoneal soft-tissue clearance, pylorus-preserving pancreaticoduodenectomy. In addition, duodenum-preserving pancreatic head resection, which has recently attracted attention in this field, as an operative technique for less-malignant, noninvasive intraductal papillary adenocarcinoma is also reviewed.

Histoplasmosis in the head and neck.

Histoplasmosis is a fungal infection which when disseminated can be potentially fatal, especially in the immunocompromised patient. Disseminated histoplasmosis can present with orolaryngeal manifestations which often mimic carcinoma or tuberculosis. A high index of suspicion and diagnosis with biopsy is needed. Two recent cases are presented and the literature reviewed. As the number of immunocompromised patients increases, more cases of histoplasmosis in the head and neck will be encountered. Early treatment can reduce morbidity and mortality.

Angiosarcoma of the head and neck.

Angiosarcoma of the head and neck is a rare tumour of vascular origin that affects the elderly. A 74-year-old man who presented with bruise-like macules of the scalp and face is reported. He was treated for a few months with different antibiotics and anti-allergic medication by his own family doctor, and referred for specialist opinion when he failed to derive any benefits from the medications. A biopsy was obtained from the lesion and proved to be an angiosarcoma. A review of the literature indicates that the most important prognostic factor in this particular disease is the size of the lesio

[Plasmacytomas of the head and neck].

Neoplastic proliferation of plasma cells results in a population of immunologically homogeneous cells that can produce diffuse (multiple myeloma) or localized (extramedullary plasmacytomas and solitary plasmacytoma of bone) disease. In otorhinolaryngologic literature these neoplasms are rarely described and their nosological arrangement is often confused. The presence of a plasma cell neoplasm can be a surprise and sometimes a diagnostic challenge to the head and neck surgeon. Proper management of such lesions needs to be individualized according to their expected biologic behaviour. The recen

[Head injury in the elderly].

We report 281 consecutive admissions of head-injured patients over 65 years of age. 2/3 were males, as in younger age groups, despite the decreased activity and increased proportion of females among the elderly. The main causes of injury were falls (71.8%; mostly at home) and pedestrian accidents (20.6%). There was a high incidence of cardiovascular disease among the men. The unique set of causes and types of head injuries in this age group (1/3 chronic subdural hematomas, 1/3 cerebral contusions and 1/5 acute subdural hematomas), as well as the grim outcome, justify regarding head injury in t

Teratomas of the head and neck.

Teratomas of the head and neck are uncommon. They are composed of tissues from all three germ layers with varying degrees of differentiation. They arise from pluripotent stem cells and ectopic embryonic nongerm cells. Head and neck teratomas most commonly emerge during the neonatal period and are associated with airway obstruction and high rates of mortality in untreated patients. The tumors are usually benign, but when found during adulthood, they have a high incidence of malignancy. The most common site of occurrence is the cervical region and the nasopharynx. We presented a rare case of ter

Hot-headed or impulsive?

A sample of men in prison was interviewed and rated for the presence of six behaviours commonly thought to be impulsive: alcohol abuse (CAGE positive), sedative dependence, other drug abuse, pathological gambling, repeated aggression and self-harm. They were also asked to complete the EPQ and 15 questionnaires. Out of 61 men on a 'special protection, wing, 51 agreed to take part. We found that the score on the Impulsiveness scale of the 15 correlated with the number of disorders present in individuals, even when corrected for age. Pathological gambling and self-harm did not correlate with the

Plasmacytoma of the pancreatic head.

Plasmacytomas of the pancreas are extremely rare. This is a report of a patient with plasmacytoma of the head of the pancreas who also had an extramedullary plasmacytoma of the epipharynx 19 months earlier. Computed tomographic (CT) scans revealed a multilobular homogeneous mass in the pancreas. Endoscopic biopsy of adjacent polypoid lesions in the duodenum allowed confirmation of the diagnosis.

Mucormycosis of the head and neck.

Rhinocerebral mucormycosis is an uncommon but devastating infection of the head and neck. The fungus most commonly affects diabetics who are in ketoacidosis, but can affect other immunosuppressed individuals such as leukemics, burn patients, and patients with chronic renal failure. The pathogenicity of the organism is characterized by a ketone reductase system and its ability to invade arterial blood vessels. Treatment is directed towards correction of the underlying disorder, surgical debridement, intravenous antifungal agents, and possibly a new adjuvant, hyperbaric oxygen. A detailed review

Paragangliomas of the head and neck.

The medical records of 64 patients with paraganglioma of the head and neck seen between 1954 and 1984 at the University of Texas M.D. Anderson Hospital, Houston, were reviewed. Lesions were found in the carotid body (n = 24), jugulotympanic area (n = 37), vagal nerve (n = 4), and larynx (n = 1). A hereditary pattern was identified in six patients (9%). Metastases developed and caused the death of two patients (8%). No functional secreting tumors were identified. Paragangliomas are often misdiagnosed, both clinically and pathologically. Angiography is the definitive clinical study for diagnosis
