Neurological and vestibular findings in three cases of Multiple Sclerosis.

Multiple sclerosis (MS) is a chronic and inflammatory autoimmune disease that affects the central nervous system (CNS). Dysfunction of body balance is also a common symptom and may be related to neurological injuries resulting from this disease. The aim of this study was to characterize the neurological and vestibular findings of three clinical cases diagnosed with MS. Data on the neurological evaluation and the magnetic resonance imaging of the skull were collected from the medical records. The patients responded to an initial interview and underwent clinical assessment of body balance and Vi

The map of dizziness in vestibular schwannoma.

INTRODUCTION: Dizziness is a frequent complaint in patients with vestibular schwannoma (VS). An abnormal vestibulo-ocular reflex (VOR) can explain this dizziness in patients with VS. The video Head impulse test (vHIT) offers a chance to describe specifically the VOR findings in such patients. STUDY DESIGN: Retrospective cases series study in a tertiary referral hospital. METHODS: Fifty consecutive patients with VS were classified in accordance with the morphology of the VOR; gain, covert saccade, and overt saccade were analyzed both in the affected side and in the healthy side. Posterior Circulation Stroke Misdiagnosed as Benign Paroxysmal Positional Vertigo: A Case Report.

Posterior circulation strokes are frequently underrecognized in emergency departments, especially when dizziness is the presenting complaint without overt motor deficits. Diagnostic delays often occur when providers anchor on benign causes such as benign paroxysmal positional vertigo. Early computed tomography and magnetic resonance imaging may miss acute infarcts in the posterior fossa, whereas the bedside head-impulse, nystagmus, and test of skew plus hearing examination has demonstrated greater sensitivity for detecting stroke in acute vestibular syndrome. Long-term follow-up of vestibular neuritis.

The long-term outcome of vestibular neuritis was evaluated in 35 patients using 4 bedside tests (head impulse, head heave, head shake, vibration), the caloric test and two surveys that explored the degree of disability (Dizziness Handicap Inventory [DHI]) and the anxiety and depression levels (Hospital Anxiety and Depression Scale [HADS]). Sixty-six percent of patients showed a positive bedside clinical test or calorics abnormalities. DHI scores were significantly higher in the patients group, while HADS scores were comparable between patients and controls. Disability from vestibular neuritis

The vestibular implant: Opinion statement on implantation criteria for research.

This opinion statement proposes a set of candidacy criteria for vestibular implantation of adult patients with bilateral vestibulopathy (BVP) in a research setting. The criteria include disabling chronic symptoms like postural imbalance, unsteadiness of gait and/or head movement-induced oscillopsia, combined with objective signs of reduced or absent vestibular function in both ears. These signs include abnormal test results recorded during head impulses (video head impulse test or scleral coil technique), bithermal caloric testing and rotatory chair testing (sinusoidal stimulation of 0.1 Hz).

Ictal downbeat nystagmus in Ménière disease: A cross-sectional study.

OBJECTIVE: To determine the mechanism of ictal downbeat nystagmus in Ménière disease (MD), we compared the head impulse gain of the vestibulo-ocular reflex (VOR) for each semicircular canal between patients with (n = 7) and without (n = 70) downbeat nystagmus during attacks of MD. METHODS: We retrospectively analyzed the results of video-oculography, video head-impulse tests, and cervical vestibular-evoked myogenic potentials (VEMPs) in 77 patients with definite MD who were evaluated during an attack. RESULTS: Pure or predominant downbeat nystagmus was observed in 7 patients (9%) with unilat

Vestibular neuritis: is there any evidence of an asymmetric distribution?

Statistics in the literature showed that neuro-otological diseases (i.e. sudden hearing loss or tinnitus) occur predominantly in the left ear. In a seven-study meta-analysis of patients suffering from vestibular neuritis, Reiß found no clear dominance of one side (50.8% on the right side, 48.4% on the left side and 0.8% on both sides). The purpose of this study is to investigate the laterality of vestibular neuritis in a distinct population of patients. Lateralization of vestibular neuritis was studied in 160 patients treated at Elblandklinikum Radebeul from January 2004 to December 2009. Ther

Bilateral Idiopathic Sequential Neuritis: Case Report and Literature Review.

Bilateral sequential vestibular neuritis is a rare condition reported in literature involving the vestibular nerve sequentially. Nowadays, the video head impulse test (vHIT) and vestibular-evoked myogenic potentials provide specific patterns of reduced vestibulo-ocular reflex (VOR) that allow differentiation between complete and partial nerve involvement. These tests are essential in cases of bilateral vestibular loss to monitor vestibular compensation. The case of a 56-year-old man who presented to a tertiary vestibular center with acute left-sided vestibular loss, following a documented cont

Quantifying the vestibulo-ocular reflex with video-oculography: nature and frequency of artifacts.

Video-oculography devices are now used to quantify the vestibulo-ocular reflex (VOR) at the bedside using the head impulse test (HIT). Little is known about the impact of disruptive phenomena (e.g. corrective saccades, nystagmus, fixation losses, eye-blink artifacts) on quantitative VOR assessment in acute vertigo. This study systematically characterized the frequency, nature, and impact of artifacts on HIT VOR measures. From a prospective study of 26 patients with acute vestibular syndrome (16 vestibular neuritis, 10 stroke), we classified findings using a structured coding manual. Of 1,358 i

Bedside evaluation of dizzy patients.

In recent decades there has been marked progress in the imaging and laboratory evaluation of dizzy patients. However, detailed history taking and comprehensive bedside neurotological evaluation remain crucial for a diagnosis of dizziness. Bedside neurotological evaluation should include examinations for ocular alignment, spontaneous and gaze-evoked nystagmus, the vestibulo-ocular reflex, saccades, smooth pursuit, and balance. In patients with acute spontaneous vertigo, negative head impulse test, direction-changing nystagmus, and skew deviation mostly indicate central vestibular disorders. In

[Frequency-specific analysis of the hVOR Prior of Cochlear Implant Operation].

Dizziness is one of the most common postoperative complications after a cochlear-implant (CI) surgery. With our prospective, matched-paired controlled study, we could demonstrate that patients with distinctive sensorineural hearing loss--even without any complaints of dizziness--already have a reduced horizontal vestibular-ocular-reflex (hVOR). Compared to controls, CI patients presented with a significantly reduced gain. 9 out of 17 CI patients showed physiological results in rotatory testing and video head thrust testing. One patient presented with pathological results in both tests. Remarka

[The characteristics of semicircular canal injury frequency in vestibular neuritis patients].

Objective:To evaluate the frequency characteristics of semicircular canal injury in patients with vestibular neuritis. Method:Fifty-three patients with vestibular neuritis were respectively evaluated by caloric test(CT),head shaking test(HST),video head impulse test (vHIT),which reflects semicircular canal function in the low, middle, high frequency region. The asymmetric value of CT unilateral semicircular canal reaction (UW), head shaking nystagmus induced by HST, gain value of VHIT (VHIT-G) and presence or absence of saccade (VHIT-S) were used as observation indicators. Enhanced Eye Velocity With Backup Saccades in vHIT Tests of a Menière Disease Patient: A Case Report.

Reduced eye velocity and overt or covert compensatory saccades during horizontal head impulse testing are the signs of reduced vestibular function. However, here we report the unusual case of a patient who had enhanced eye velocity during horizontal head impulses followed by a corrective saccade. We term this saccade a "backup saccade" because it acts to compensate for the gaze position error caused by the enhanced velocity (and enhanced VOR gain) and acts to return gaze directly to the fixation target as shown by eye position records. We distinguish backup saccades from overt or covert compen

An investigation of the NOCSAE linear impactor test method based on in vivo measures of head impact acceleration in American football.

The performance characteristics of football helmets are currently evaluated by simulating head impacts in the laboratory using a linear drop test method. To encourage development of helmets designed to protect against concussion, the National Operating Committee for Standards in Athletic Equipment recently proposed a new headgear testing methodology with the goal of more closely simulating in vivo head impacts. This proposed test methodology involves an impactor striking a helmeted headform, which is attached to a nonrigid neck. The purpose of the present study was to compare headform accelera

Intratympanic gentamicin injections for Meniere disease: vestibular hair cell impairment and regeneration.

The authors treated 22 patients with intratympanic gentamicin. Vestibular function was measured using caloric and head impulse tests and vestibular evoked myogenic potentials induced by high amplitude sounds and short duration galvanic currents. Roughly one-third of the patients, after initially losing their caloric responses and displaying refixation saccades to head impulse tests, recovered within 2 years of the lesion. Vertigo did not recur in patients in whom the galvanic response was abolished.
