Macroeconomic shock-response functions of health system resilience and equity: a global simulation study.

To quantify how macroeconomic shocks propagate into health system resilience and equity, we develop a globally comparable stress-testing framework isolating within-country responses to standardised shocks. Using a panel of 205 countries (2000-2022; 3,009 observations) with harmonised World Bank data, we construct three indices: Economic Shock, Health System Resilience and Health Equity. Symmetric ±5%, ±10% and ±20% shocks generate counterfactual scenarios, with responses estimated via simulation and validated using linear elasticity, Random Forest and XGBoost. Macroeconomic shocks produce pred

Health equity for young Black sexually minoritized men through their eyes: a qualitative study.

BACKGROUND: Young Black sexually minoritized men (YBSMM) are a key population for Ending the HIV Epidemic though their care engagement is limited due to inequities shaped by social determinants of health. This study explores clinic experiences and preferences among YBSMM in Chicago to inform implementation of equitable care delivery interventions for this key population. METHODS: In-depth-interviews were conducted with N = 24 cisgender YBSMM aged 18–29, across HIV serostatus, who were part of the Neighborhoods & Networks (N2) cohort in Chicago, Illinois, USA. The interview protocol examined ba

The hidden health equity crisis: readability assessment of online information regarding periodontitis-diabetes relationship.

BACKGROUND: Despite strong evidence linking periodontitis as a risk factor for diabetes, this association remains significantly underrepresented in online health information. Given that online resources serve as a primary source of health education, this study examines the content coverage and readability of diabetes-related websites from government and health organizations. METHODS: A total of 154 health and diabetes-related websites were screened, with only 28 containing any reference to periodontitis-diabetes interrelationship. Readability analysis was specifically conducted on this subset

Telepsychiatry, access, and equity: accelerating mental health care for rural and low-income youth.

For rural youth, seeking mental health care often carries high stakes: in tight-knit communities where 'everyone knows everyone,' privacy is limited, and delays in mental health support can allow manageable symptoms to escalate into crises. Telepsychiatry provides confidential, flexible, and timely access, enabling youth to seek help early and receive support without delay. This perspective synthesizes current evidence on telepsychiatry's benefits and challenges and highlights opportunities for growth through hybrid care models, policy reforms pertaining to payment parity and credentialing by

Accelerating the fight towards malaria elimination: Bridging gaps to achieve health equity in India.

Malaria continues to remain a serious threat to public health, especially in regions with socio-economic and healthcare disparities. The work attempts to contextualize the current scenario of malaria transmission, the advancement made towards its elimination and the multi-dimensional strategies that may be required to overcome cultural and regional barriers; with a focus on India's goal to eliminate malaria by 2030. The epidemiological spread of malaria across the country is nuanced; wherein an overall reduction of malaria cases recorded over the years (signifying success) could be juxtaposed

Health equity in COVID-19 testing among patients of a large national pharmacy chain.

BACKGROUND: Several social determinants of health and other structural factors drive racial and ethnic disparities in COVID-19 risk, morbidity, and mortality. Public-private collaborations with community pharmacies have been successful in expanding access to COVID-19 testing and reaching historically underserved communities. The objectives of this study were to describe individuals who sought testing for COVID-19 at a national community pharmacy chain and to understand potential racial and ethnic inequities in testing access, positivity, and infection with emerging variants of concern. Enhanced glucose processing in gestational diabetes diagnosis: Effects on health equity and clinical outcomes.

OBJECTIVES: Gestational diabetes is diagnosed using an oral glucose tolerance test (OGTT), which has limited accuracy, reproducibility and practicality. We assessed the effect of enhanced pre-analytical glucose processing upon glucose concentrations, gestational diabetes diagnosis, health equity and pregnancy outcomes, and if HbA1c was a suitable alternative. METHODS: We recruited pregnant women with ≥1 risk factor to a prospective observational cohort study of pregnancy hyperglycaemia, endocrine causes, lipids, insulin and autoimmunity (OPHELIA), from nine UK centres. HIV and Other Sexually Transmitted Infections in Rhode Island: Trends, Disparities, and Health Equity.

Over the past 10 years, sexually transmitted infections (STIs) have seen a significant resurgence in the United States despite the availability of effective treatments and reliable prevention methods. Rhode Island has experienced a similar uptick in the incidence of syphilis, chlamydia and gonorrhea, with many cases occurring among gay, bisexual, men who have sex with men (GBMSM), which coincides with a recent concerning rise in congenital syphilis cases. We reviewed the most recent STI trends for the state of Rhode Island in 2022. During this year, 6,883 cases of STIs were reported to the Dep

Organisation of preventive child health services: Key to socio-economic equity in vaccine uptake?

Background: Measles has made a comeback in Western Europe, with more cases being reported each year. One factor behind this development is low vaccination coverage in socially disadvantaged segments of the population in many countries. This study investigates whether socioeconomic patterns of uptake of the measles, mumps and rubella (MMR) vaccine in the Nordic countries differ by national organisation of preventive health services for children. Methods: MMR vaccine uptake before the age of two years was analysed in register data from Denmark, Finland, Iceland and Sweden, linked to family indic

Community Coalitions Implementing and Sustaining Policy, Systems and Environment Strategies for Rural Health Equity.

Rural communities have assets that can be leveraged to address health disparities, particularly through complex policy, systems and environment (PSE) change approaches. Eleven rural community coalitions across Georgia participated in The Two Georgias Initiative, a 5-year health equity project from 2017 to 2022 designed to address rural-urban health disparities. The coalitions addressed community health concerns through implementing county-specific PSE and programmatic strategies tracked through a Community Change Tracking Tool. This mixed-methods analysis assessed the variation in strategies i

Healthy Ageing without Health Equity? Capability, Justice, and Ageing Governance in the UN Decade.

The U.N. Decade of Healthy Ageing frames later life through functional ability, environments, and well-being rather than through demographic burden alone. This article argues that healthy ageing becomes ethically unstable when it is pursued without health equity. It first distinguishes the historically influential vocabulary of active ageing from the later healthy-ageing framework centred on functional ability. It then develops a bioethical account of capability-enabling ageing governance: assistance and long-term care are ethically necessary, but they become insufficient when downstream servi

Wildfire and Smoke Risk Communication: A Systematic Literature Review from a Health Equity Focus.

Effective wildfire smoke risk and evacuation communication is urgently needed to avert unnecessary deaths as wildfires increase in frequency and intensity. Human exposure to wildfires has doubled in the last two decades. Low-income, marginalized communities are the most disadvantaged in their ability to respond. A systematic literature review of wildfire and wildfire smoke risk communication research between 2014 and 2024 was conducted. Web of Science and Scopus databases were searched using the keywords "wildfire", "communication", "wildfire smoke", "risk", and "public health", resulting in 2

Mitigating child health inequalities through equity, diversity, inclusion, and accessibility school practices in Canada.

Unhealthy lifestyle behaviours and mental disorders disproportionately affect children living in deprived neighbourhoods. In Canada, schools are encouraged to adopt equity, diversity, inclusion, and accessibility (EDIA) practices to tackle these inequalities. We examined whether integrating EDIA school practices into curriculum and programming mitigates the impact of neighbourhood deprivation on children's lifestyle behaviours and mental health. In 2023/2024, we surveyed 1970 students in grades 4-6 (aged 9-12) from 28 elementary schools in Alberta. Students self-reported diet, physical activit

PURPOSE OF REVIEW: This review examines why substantial disparities persist in the management of cardiogenic shock (CS) despite advances in therapy and the existence of clinical guidelines. We synthesize structural, clinical, and ethical mechanisms that shape access to timely and advanced care and highlight equity-oriented solutions. RECENT FINDINGS: Across contemporary cohorts, women, older adults, racial/ethnic minorities, and socioeconomically disadvantaged patients remain less likely to receive early coronary angiography, revascularization, or mechanical circulatory support (MCS) and expe
