Relation between QT interval dispersion and heart rate.

Because the relation between QT dispersion (QTd) and heart rate (HR) are different from that between QT interval and HR, QTd could be overadjusted at a high HR and be underadjusted at a slow HR if we use Bazett's formula to adjust QTd. HR adjustment of QTd is not needed to evaluate repolarization dispersion.

Heart rate variability preceding onset of atrial fibrillation.

The purpose of this article is to discuss the relationship between sympathovagal dysfunction and the occurrence of paroxysmal atrial fibrillation. Onset of atrial fibrillation is subject to circadian variation; shorter episodes occur more frequently during the day, whereas longer episodes occur less frequently at night. Vagally mediated atrial fibrillation typically is preceded by bradycardia, is not triggered by stress, occurs more often at night, is more common in men, and occurs at a younger age. Sympathetically mediated atrial fibrillation is less frequent, typically occurs during the day,

Effect of heart rate on T wave alternans.

INTRODUCTION: T wave alternans (TWA) is a promising technique for detecting arrhythmia vulnerability. Previous studies in animals demonstrated that the magnitude of TWA is dependent on heart rate. However, the effects of heart rate on TWA in humans and the clinical relevance of this effect remain controversial. METHODS AND RESULTS: This was a prospective evaluation of pacing rate and monitoring lead configuration on TWA in subjects undergoing electrophysiologic study. Measurements of TWA were performed on 45 patients in the absence of antiarrhythmic drugs. Recordings were made in normal sinus

[Effects of vestibular stimulus on heart rate variability].

Possible effects of vestibular stimulation autonomic nerve function in pilots were studied by observing changes of Heart Rate Variability (HRV). Time domain and frequency domain analyses demonstrated that the primarily averages, standard deviation of the HRV averages(SD), least or greatest value, deviation of greatest minus least value, interrelated averages, square root of the mean of the sum of squares of differences between adjacent R-R intervals (RMSSD), total power(TP), low frequency(LF), high frequency(HF), LF/TP, HF/TP, and LF/HF all had significant increase or decrease (P <0.05 - <0.01

Heart rate-dependence of arterial distensibility in vivo.

OBJECTIVES: Viscous and inertial components contribute to arterial distensibility and compliance in vitro. The purpose of our study was to determine whether this phenomenon is of relevance in vivo, namely, whether arterial compliance is altered by an increase in heart rate. DESIGN: Arterial diameter was assessed by an echo-Doppler device in a common carotid and femoral artery, namely, in a large elastic and a muscle artery. The studies were performed in 12-week-old pentobarbitone-anaesthetized Wistar-Kyoto rats subjected to atrial pacing via a transjugular unipolar catheter at five different

Botulinum toxin: influence on respiratory heart rate variation.

Remote adverse effects of local intramuscular botulinum toxin were investigated in a prospective follow-up study. Twenty-six patients with spasmodic torticollis were examined (18 women, eight men, 45 +/- 13 years). Respiratory heart rate variation (HRV) was investigated by a computerized method. Different parameters were recorded (beats per minute, coefficient of variation, root mean square successive difference (RMSSD), spectral analysis, difference and quotient between maximum and minimum RR intervals, mean circular resultant). After one intramuscular injection of 12.5 ng botulinum toxin (Po

[Heart rate disorders in potassium and magnesium deficiency].

Potassium and magnesium deficiencies are common in patients with heart disease. These are often coexistent and pathophysiologically related. Potassium deficiency cannot be treated without correction of concomitant magnesium deficiency. Correlations between serum levels and body stores are very poor for both ions. Therefore diagnosis and treatment of these conditions based on serum levels alone are erroneous. There is some evidence that it is primarily the intracellular depletion of these ions which is arrhythmogenic. Magnesium infusion has been proved effective in treatment of torsade de point

Heart rate variability in patients with major depression.

We have previously reported decreased heart rate variability upon standing in panic disorder patients compared with controls. In this study, we extend our report to include patients with major depression (n = 19). Compared to normal controls (n = 20) and panic disorder patients (n = 30), there was no significant difference in the immediate changes in heart rate upon standing in the depressed group. The standing heart rate variability (R-R variability) was significantly lower in panic disorder patients compared to both normal controls and depressed patients as indicated by the corrected standar

Apnoeic heart rate responses in humans. A review.

Research examining the heart rate response of humans to apnoea and apnoeic immersion in water has tended to produce equivocal results. Controversy exists in relation to the existence and onset, course and ultimate extent of apnoeic bradycardia, either at rest or during exercise. The reduction in heart rate that has been observed in response to apnoea and apnoeic immersion is considered by some to be a component of an oxygen conserving mechanism termed the mammalian dive reflex. Doubt has been expressed, however, as to whether humans do possess the complex cardiovascular response evident in div

Heart rate variability and sudden infant death syndrome.

The sudden infant death syndrome (SIDS) is the most common cause of death in infancy. The pathophysiological mechanism leading to SIDS is still obscure. In the QT hypothesis, the mechanism must be an arrhythmogenic sympathetic imbalance: the infants die suddenly of cardiac arrhythmia. Recently, it has been suggested that analysis of heart rate variability (HRV), expressed as standard deviation or variance analysis, can provide adequate information on sympathovagal interaction. We studied 150 newborns enrolled in a previous prospective electrocardiographic study to evaluate the predictive value

Fetal heart rate changes associated with general anesthesia.

Decreased fetal heart rate variability was noted 90 seconds after the induction of general anesthesia with sodium thiopentone and fentanyl in a patient undergoing basket extraction of a renal calculus at 30 weeks' gestation. The fetal sleep pattern lasted for 105 minutes after the anesthetic was discontinued, 45 minutes after the mother was fully awake.

Computerized analysis of resting fetal heart rate tracings.

Previous attempts to computerize the analysis of fetal heart rate (FHR) using mainframe or minicomputers were not accepted in routine clinical practice because of their complexity, cost, and lack of validation. We describe the first practical, inexpensive system integrated into a clinical protocol that analyzes and documents FHR baseline, long-term variability, and episodic events using a standard microcomputer. A comparison of computerized and manual analyses of 284 nonstress tests (NSTs) of high-risk patients revealed no significant differences in determining the number of acceleration and d

Electronic fetal heart rate monitoring: why the dilemma?

As medical technology becomes more and more complex, both the benefits and risks increase. Perhaps we have misled ourselves and, unwittingly, our patients into expecting the perfect outcome by complication-free care. As we learn more about the pathophysiology of disease it becomes apparent that our expectations are unrealistic. In addition, as clinicians we have become more sophisticated about the utility of clinical testing. We have learned that the predictive value of a test is related to its sensitivity and specificity, and to the prevalence of the disease in the population being tested. We

Sinusoidal fetal heart rate pattern associated with gastroschisis.

A case of true sinusoidal fetal heart rate associated with fetal gastroschisis is presented. No other known etiologic factors associated with a sinusoidal pattern were present. The pathophysiologic mechanism in this case is unclear, but fetal anomalies may be associated with elevated arginine vasopressin levels leading to the distinctive sinusoidal fetal heart rate response.

The heart rate of Romano-Ward syndrome patients.

The Romano-Ward prolonged QT syndrome is associated with an abnormality of ventricular recovery, rendering the patients susceptible to life-threatening ventricular arrhythmias. The pathophysiology has not been clearly defined. It has been proposed that the syndrome involves disparity of right and left cardiac sympathetic activity, with the left side dominant. This could be the result of left-sided overactivity or right underactivity. The right-sided nerves contain the chronotropic fibers, which affect heart rate. A deficiency of the effect of the right-sided nerves might, therefore, be manifes

Significant fetal bradycardia during antepartum heart rate testing.

Reviewed were 800 patients who underwent antepartum fetal heart rate testing. Significant fetal bradycardia (less than 90 beats per minute for longer than 50 seconds or a decrease of 40 beats per minute below baseline for longer than 50 seconds) was evaluated relative to its relationship to fetal outcome. Twelve fetuses exhibited significant bradycardia during monitoring. All of these babies, when delivered within a short period of time after the tracing, showed manifestations of ongoing intrauterine jeopardy. The finding of significant fetal bradycardia during antepartum testing should prompt

Treatment of sinus tachycardia with heart-rate feedback.

A 23-year-old male with a history of sinus tachycardia was treated with heart-rate feedback in eight sessions. From a pretreatment average of 103 beats per minute (BPM) as recorded in two separate baseline sessions, the heart rate dropped to 66 BPM in the last treatment session. At follow-up the heart rate seemed stabilized within the normal range, averaging 68 BPM after 1 month and 73 BPM after 12 months. Simultaneous EMG recordings of m. frontalis suggest a generalized effect of this treatment. Results are discussed in terms of somatic and respiratory mediation.
