A physiological body-cooling unit for treatment of heat stroke.

The high incidence of heat stroke during the Mecca pilgrimage in hot weather prompted the development of a new method of rapidly cooling hyperpyrexial patients. A cooling bed was devised which achieved a high rate of evaporative cooling by spraying patients with water at 15 degrees C and warm air, which reached the body surface at 30-35 degrees C. The cooling bed also made possible the administration of ancillary treatment both efficiently and hygienically.

Heat stroke during a 'run for fun': A case report.

An athlete taking part in a mini-marathon race on a cold mid-winter morning collapsed with heat stroke after completing 19,5 km. To stop himself coughing during the run, he had taken a proprietary cough mixture containing 18 mg of the phenothiazine antihistaminic, promethazine, before the start. A possible explanation for the development of heat stroke is that the promethazine interfered with the body's ability to dissipate the enormous endogenous heat load which is generated during the course of a marathon race.

[Walking in the sun: heat stroke and heat exhaustion during the Four-Day March in Nijmegen, in 2006].

The annual Four-Day March in Nijmegen, The Netherlands, in July 2006 was cancelled after the first day because two participants had died, men aged 65 and 57 years, and many had become unwell while walking in unusually high ambient temperatures. However, the cause of death of the two who died turned out to be cardiovascular and not heat-related. The case of two of the people that became unwell, men aged 58 and 59 years, respectively, shows that heat stroke and heat exhaustion were important causative conditions. Heat-related illnesses are relatively uncommon in the Netherlands due to its temper

Core temperature responses during passive heat exposure differentiate mice previously exposed to exertional heat stroke and endotoxemia.

Exertional heat stroke (EHS) induces thermoregulatory and inflammatory disturbances, yet whether core temperature (Tcore) responses are altered during subsequent passive heat exposure (PHE) remains unknown. Our purpose was to determine whether prior exposure to EHS or lipopolysaccharide (LPS)-induced systemic inflammation alters Tcore response during a subsequent PHE in mice. Receiver operating characteristic (ROC) curve assessment was performed as a secondary analysis to establish the ability of the Tcore response to discriminate animals previously exposed to EHS or LPS. Thirty-one female mic

Heat stroke management: clinical phenotypes and implications for cooling strategies.

BACKGROUND: Heat stroke is a life-threatening condition demanding rapid core temperature reduction. However, optimizing cooling strategies is challenging due to pronounced patient heterogeneity. This study aimed to identify distinct clinical phenotypes of heat stroke to enable personalized cooling management. METHODS: This two-stage study analyzed heat stroke patients from seven tertiary hospitals in China. The prospective cohort was derived from an ongoing, prospectively registered clinical trial (NCT05923931). Latent class analysis was applied to this prospective cohort to derive a phenotyp

Altered static brain activity and functional connectivity after heat stroke.

This study aimed to investigate the alteration of brain function based on resting-state functional MRI in patients after heat stroke. This study included 10 cases of patients after heat stroke and 10 cases of healthy controls. Abnormal brain function was calculated using amplitude of low-frequency fluctuations (ALFF) and degree centrality analysis, as well as functional connectivity analysis based on regions of interest (ROI). Correlation analyses were performed to evaluate the association between brain function changes and clinical scales. Combining ALFF and degree centrality results, the dec

Acute liver failure and hepatic encephalopathy in exertional heat stroke.

A 31-year-old man was brought to Accident & Emergency after collapsing during a race. On presentation, the patient had a temperature of 41.7°C (rectal). External cooling was started immediately. The patient was intubated in view of a Glasgow Coma Scale of 7 and was transferred to theintensive therapy unit. Laboratory results revealed an acute kidney injury, rhabdomyolysis, disseminated intravascular coagulopathy and acute liver failure. The patient was encephalopathic, jaundiced and difficult to sedate. His liver function continued to deteriorate with alanine aminotransferase (ALT) levels reac

Exertional rhabdomyolysis and heat stroke: Beware of volatile anesthetic sedation.

In view of the enormous popularity of mass sporting events such as half-marathons, the number of patients with exertional rhabdomyolysis or exercise-induced heat stroke admitted to intensive care units (ICUs) has increased over the last decade. Because these patients have been reported to be at risk for malignant hyperthermia during general anesthesia, the intensive care community should bear in mind that the same risk of life-threatening rhabdomyolysis is present when these patients are admitted to an ICU, and volatile anesthetic sedation is chosen as the sedative technique. As illustrated by

Protective mechanism of Xuebijing injection against heat stroke in rats.

This study aimed to investigate the protective mechanism of Xuebijing injection (XBJ) against heat stroke (HS) in rats. Adult male Sprague Dawley rats were randomly divided into normal control (NC), normal saline-treated HS (NS-HS) and XBJ-treated HS (XBJ-HS) groups. At 47 and 57 min from the initiation of heat stress (42.5-43.5°C), the plasma levels of certain cytokines [interleukin (IL)-1β, IL-6 and tumor necrosis factor-α], biochemical indicators (creatinine, blood urea nitrogen, aspartate aminotransferase, alanine aminotransferase and alkaline phosphatase) and coagulation indicators (activ

Sudden death during jungle trekking: a case of heat stroke.

Heat stroke, which is also known as "sun stroke," is a medical emergency, and fatalities can occur unless it is diagnosed early and treated efficiently. Heat stroke may manifest quite suddenly, giving little time to differentiate it from extreme physical exhaustion in collapsed subjects. It is also known to lead to serious disseminated intravascular coagulation. Sudden death in a young female is presented who collapsed after trekking in a hilly, jungle area in Malaysia on a warm, humid day. She had joined a weight reduction programme a few weeks earlier. She was found collapsed and in a semico

Energy metabolism in exertional heat stroke with acute renal failure.

BACKGROUND: Heat stroke is the clinical syndrome produced when the body overheats. It can develop in the army and in healthy civilian populations who physically exert themselves in a hot and humid environment during the summer, and may result in a significant number of heat-related deaths. Since strenuous exercise is one of the major exacerbating and precipitating factors, the incidence of exertional heat stroke (ExHS) is high among military personnel undergoing military training. Furthermore, acute renal failure (ARF) may occur in 25% of patients with ExHS and it can cause metabolic alteratio

Fatal classic and exertional heat stroke--report of four cases.

Heat stroke is the outcome of impaired heat dissipation which is aggravated by hot and humid environmental conditions. The very young and debilitated on the one hand and healthy individuals under considerable physical stress on the other are vulnerable to heat stroke. Post-mortem findings will depend on the time lapse between the stroke event and death. We report on the deaths resulting from heat stroke in a 12-month-old baby and three 19-year-old soldiers. Reconstruction of the environmental conditions enables elucidation of the circumstances that precipitated exogenous hyperpyrexia. The Disc

[Heat stroke and disseminated intravascular coagulation. Apropos of 2 cases].

Two cases of fatal heat stroke, concerning a 20 year-old soldier and a 44 year-old psychiatric patient, treated with neuroleptics, are reported. The clinical picture, starting suddenly with coma and hyperthermia, was quite identical for both. Secondarily, while hyperthermia decreases and the conscience improved partially, an hemorrhagic syndrome similar to a consumption coagulopathy, acute renal insufficiency and acute hepatic failure appear. Death occurred after aggravated neurological disorders and respiratory distress. The anatomical lesions spread on all the viscera include tubular nephrit

Evaluation of a Novel Classification of Heat-Related Illnesses: A Multicentre Observational Study (Heat Stroke STUDY 2012).

The Japanese Association for Acute Medicine Committee recently proposed a novel classification system for the severity of heat-related illnesses. The illnesses are simply classified into three stages based on symptoms and management or treatment. Stages I, II, and III broadly correspond to heat cramp and syncope, heat exhaustion, and heat stroke, respectively. Our objective was to examine whether this novel severity classification is useful in the diagnosis by healthcare professionals of patients with severe heat-related illness and organ failure. A nationwide surveillance study of heat-relate
