Bupivacaine pleural effusion mimicking a hemothorax after a thoracoscopic microdiscectomy with epidural anesthesia.

PURPOSE: Post-operative pain after video-assisted thoracoscopic surgery is often treated using thoracic epidural analgesics or thoracic paravertebral analgesics. This article describes a case where a thoracic disc herniation is treated with a thoracoscopic microdiscectomy with post-operative thoracic epidural analgesics. The patient developed a bupivacaine pleural effusion which mimicked a hemothorax on computed tomography (CT). METHODS: The presence of bupivacaine in the pleural effusion was confirmed using a high performance liquid chromatography method. RESULTS: The patient underwent a re

Thoracic cavity irrigation prevents retained hemothorax and decreases surgical intervention in trauma patients.

INTRODUCTION: Retained hemothorax (HTX) is a common complication following thoracic trauma. Small studies demonstrate the benefit of thoracic cavity irrigation at the time of tube thoracostomy (TT) for the prevention of retained HTX. We sought to assess the effectiveness of chest irrigation in preventing retained HTX leading to a secondary surgical intervention. METHODS: We performed a single-center retrospective study from 2017 to 2021 at a Level I trauma center, comparing bedside thoracic cavity irrigation via TT versus no irrigation. Using the trauma registry, patients with traumatic HTX w

Does preinjury anticoagulation worsen outcomes among traumatic hemothorax patients? A nationwide retrospective analysis.

INTRODUCTION: Up to a quarter of all traumatic deaths are due to thoracic injuries. Current guidelines recommend consideration of evacuation of all hemothoraces with tube thoracostomy. The aim of our study was to determine the impact of pre-injury anticoagulation on outcomes of traumatic hemothorax patients. MATERIALS AND METHODS: We performed a 4-year (2017 - 2020) analysis of the ACS-TQIP database. We included all adult trauma patients (age ≥18 years) presenting with hemothorax and no other severe injuries (other body regions <3). Patients with a history of bleeding disorders, chronic liver

Hemothorax caused by costal exostosis injuring diaphragm: a case report and literature review.

BACKGROUND: Osteochondromas, also known as exostoses, are the most common benign tumors of bone and can be classified into isolated and multiple osteochondromas. A great majority of osteochondromas is asymptomatic, painless, slow-growing mass, and incidentally found. However, osteochondromas occurring in adolescence or in adult patients can grow in size and become symptomatic as a result of mechanical irritation of the surrounding soft tissues or peripheral nerves, spinal cord compression, or vascular injury. CASE PRESENTATION: We present a case of a 13-year-old girl with spontaneous hemothor

Hemothorax From an Anomalous Bronchial Artery Bleed in an Infected Intralobar Pulmonary Sequestration.

Pulmonary sequestration is a rare congenital malformation. It represents 0.15-6.4% of all congenital pulmonary malformations. It is characterized by non-functional, dysplastic mass of lung tissue that is not in communication with the normal tracheobronchial tree and is associated with a systemic arterial supply. We report a young gentleman in his mid-thirties who presented with community-acquired pneumonia from an infected intralobar pulmonary sequestration which subsequently developed a hemothorax from an anomalous bronchial artery bleed.

A 43-Year-Old Man With Rapidly Respiratory Failure and Spontaneous Massive Hemothorax.

A 43-year-old man urgently was referred to the hospital complaining of rapidly worsening dyspnea and right-side chest wall discomfort for 1 hour. Two hours later, he experienced acute respiratory failure that subsequently required intubation and invasive mechanical ventilation, thus he was transferred to ICU. He had no fever, weight loss, or bleeding tendency. He was previously healthy with no history of trauma and was not currently on any medication.

Diagnosis of neurofibromatosis type 1 after rupture of aneurysm and consequent fatal hemothorax.

Patients with neurofibromatosis type 1 (NF1) can develop both benign and malignant tumors throughout their lives. A 49-year-old man was transferred to the emergency department with complaints of sudden right dorsal pain and respiratory discomfort. He was in shock on arrival. On finding significantly decreased permeability of the left lung field in chest X-ray, drainage was immediately performed. Subsequent computed tomography (CT; Lammert et al., 2005) angiography revealed the extravasation of contrast media from the deep carotid artery, a branch of subclavian artery. It suggested rupture of a

Pulmonary sequestration presented as massive left hemothorax and associated with primary lung sarcoma.

Pulmonary sequestration is an uncommon disease, accounting for only approximately 1.5% of all congenital pulmonary malformations. In most cases, the diagnosis is a result of accidental radiological findings; it is rarely accompanied by clinical symptoms, and is more commonly associated with other congenital malformations. Herein, we reported a case of pulmonary sequestration presented as massive left hemothorax and associated with primary lung sarcoma. A pneumonectomy via thoracotomy was attended with complete resection of sequestration and of sarcoma. The postoperative course was unremarkable

Delayed hemothorax superimposed on extrapleural hematoma after blunt chest injury: a case report.

We report a case showing delayed hemothorax superimposed on extrapleural hematoma after blunt chest injury. The patient was a 56-year-old man with a medical history of alcoholic hepatopathy who presented with a chief complaint of dyspnea on exertion. One month prior to presentation, he had sustained a fall that had resulted in left simple rib fracture. A chest radiograph taken at the time of injury showed notable reduction in the permeability of the left lung field along with high pulmonary collapse as well as rightward deviation of the shadow of the inferior mediastinum. Chest CT images showe

Management of medicolegal natural deaths from hemopericardium or hemothorax using postmortem CT scanning.

At the Victorian Institute of Forensic Medicine the forensic pathologist provides expert medical advice to the Coroner. Cases of natural death from hemopericardium and hemothorax are reasonably common in the forensic setting and are readily identifiable on postmortem CT (PMCT) scanning. The management of these cases raises interesting and challenging issues for the forensic pathologist. We present three cases in which PMCT angiography was useful in the management of each individual case and illustrate the potential of the technique in medico-legal death investigation.

Hydatid cyst, an unusual cause of spontaneous hemothorax and diagnostic thoracoscopy: case report.

Hydatid disease is a common parasitic disease in areas where sheep and cattle are raised and is currently endemic in the eastern and southwestern parts of Turkey. Patients with hydatid cysts typically present with cough, chest pain, dyspnea, hemoptysis, or allergic reactions. When ruptured, these cysts may cause hemoptysis, dyspnea, and hydatid thorax. Previously published series of cyst hydatid have reported cyst hydatid rupture and hemothorax secondary to trauma, but nontraumatic hemothorax due to spontaneous rupture of hydatid cyst has not been defined. We discuss the clinical features of a

Life-threatening hemothorax resulting from lung hernia after minimally invasive mitral valve surgery.

Lung hernia following minimally invasive mitral valve surgery is an uncommen entity. We report the case of a male patient who developed a lung hernia as a sequela to limited access mitral valve surgery. Two months after discharge, the patient presented with a bulge in the region of the lateral thoracotomy related to respiration which could be provoked by a Valsalva maneuver. In the night following admission the patient had acute cardiovascular decompensation with worsening dyspnea, pallor and hypotension. The patient was quickly transferred to the ICU, where a chest X-ray revealed the presence

An Uncommon Cause of Massive Hemothorax: Pulmonary Arteriovenous Malformation in the Right Middle Lobe.

Pulmonary arteriovenous malformations (PAVMs) are rare vascular anomalies that can lead to severe complications when ruptured. We report the case of a 65-year-old active smoker who presented to the emergency department with acute pleuritic chest pain, severe dyspnea, and hypoxemia. Chest CT revealed a large PAVM in the right middle lobe, associated with massive hemothorax and a small pneumothorax. The patient had no history of hereditary hemorrhagic telangiectasia. Initial management included pleural drainage and endovascular embolization with a 16 mm Amplatzer™ device. Due to persistent respi

True Hemothorax as the Initial Presentation of Prostate Adenocarcinoma with Pleural Metastasis: A Case Report.

True hemothorax as an initial manifestation of prostate cancer has not yet been reported. An adult male presented with acute right-sided pleural effusion. Diagnostic thoracentesis yielded hemorrhagic pleural fluid, with a pleural fluid hematocrit ≥ 50% of the peripheral blood hematocrit, confirming hemothorax. Contrast-enhanced computed tomography revealed prostate enlargement, pulmonary nodules, and pleural masses. Transbronchial biopsies of the pulmonary nodule confirmed metastatic adenocarcinoma positive for prostate-specific antigen and negative for thyroid transcription factor-1, thus est

Time to resolution of radiologically detected hemothorax in trauma patients: A retrospective observational study.

BACKGROUND: Traumatic hemothorax is a common complication of chest trauma; however, the timeline for its resolution, even with chest tube thoracostomy, remains unclear. AIM: To determine the time to resolution of the hemothorax to ensure safe discharge based on chest radiography (CXR) findings. METHODS: A retrospective observational study was conducted at Hamad General Hospital, Qatar, from June 2014 to October 2019, including all patients with hemothorax diagnosed via computed tomography (CT) following chest trauma. Based on the initial imaging study, the hemothorax was divided into right,
