The authors report about 2 cases of Budd-Chiari syndrome occurring during the immediate postoperative period of extensive hepatectomy, after exeresis of a large biliary cyst (20 cm in diameter) through 6/7 sectoriectomy and after exeresis of a large sarcomatous tumor preserving only sectors 2 and 3 of the liver using an "ex-vivo/in-situ" technique. These 2 cases lead to discuss the diagnosis, treatment and, most importantly, the prevention of acute suprahepatic obstruction after extensive right hepatectomy. Similar cases are reported in the literature after partial liver transplantation. Preve

In three patients who underwent hepatectomy for solitary hepatocellular carcinoma (HCC), adrenal metastases, on the right sides of two patients and the left side of the third were subsequently detected by ultrasonography (US) and/or computed tomography (CT), and successfully resected after an average interval of 16 months from hepatectomy. There were no metastatic lesions in the lung, lymph node or bone. Two patients, however, who were found to have metastasis in the right adrenal also had multiple small recurrent foci in the residual liver. The latter were controlled by arterial embolization

(2'-5')Oligoadenylate synthetase [(2'-5')A synthetase], which synthesizes a series of oligoadenylates ppp-(A2'p)n5'A [collectively referred to as (2'-5')A], has been described previously in rat liver cells, where its concentration varied with the growth status of this organ--i.e., it decreased during the early phase of rat liver regeneration after partial hepatectomy. Because double-stranded RNA, the only known activator of this enzyme, has been detected in rat liver nuclei, (2'-5')A synthesis could occur in this tissue in vivo. INTRODUCTION: As our population ages, older patients are increasingly presenting for surgical management of advanced malignancies. Hepatectomy for colorectal liver metastasis (CRLM) prolongs survival. The goal of this study was to understand postoperative morbidity and the rate of failure to rescue in older patients undergoing major hepatic resection for CRLM.
METHODS: We reviewed the American College of Surgeons (ACS) National Surgical Quality Improvement (NSQIP) targeted data set for hepatectomy from 2014 to 2024 to identify patients who underwent major hepatectomy for CRLM. Univariate and

INTRODUCTION: Perivascular epithelioid cell tumors (PEComas) are rare mesenchymal tumors composed of cells exhibiting an epithelioid morphology. These cells typically arrange around small blood vessels (perivascular spaces) and display dual differentiation characteristics of smooth muscle cells and melanocytes. Diagnosis is challenging due to the absence of specific symptoms or tumor markers. This case features a young male patient with a large hepatic PEComa, whose imaging findings resemble those of hepatocellular carcinoma. We have detailed the entire process from diagnosis to treatment to a

Living donor hepatectomy requires maximum safety while aiming to minimize surgical invasiveness. Single-port (SP) robotic platforms may offer the potential to reduce abdominal wall trauma and improve cosmesis compared with multiport robotic approaches; however, clinical experience in living liver donation is currently lacking. This report describes the first 3 consecutive living donor left lateral sectionectomies performed using a SP robotic system at a high-volume robotic donor hepatectomy center in 2026. All procedures were completed robotically without conversion, the operation duration dec

BACKGROUND: Postoperative pain control is often suboptimal, with many patients experiencing inadequate relief or side effects. Methadone, a long-acting μ-opioid agonist and NMDA antagonist, provides stable analgesia and reduces postoperative opioid requirements when administered as a single intraoperative dose. However, its use has not been studied in living donor hepatectomy patients.
METHODS: We conducted a retrospective review of a quality improvement initiative involving 50 living donor hepatectomy patients treated from 2023 to 2025. 32 received usual opioid care and 18 received methadone

BACKGROUND: Anatomic liver resection (ALR) requires precise transection along the true intersegmental/sectional plane while preserving vascular integrity of the remnant liver. Conventional approaches often rely on ischemic demarcation or venous landmarks, which may not perfectly match anatomical boundaries. Robotic platforms and indocyanine green (ICG) fluorescence have enabled refined visualization and dexterity, supporting the concept of bona fide ALR-defined as anatomical resection that exactly follows the Glissonean territory while preserving remnant inflow and outflow integrity. The "deta

Budd-Chiari syndrome (BCS), caused by venous outflow obstruction, results in hepatic congestion and portal hypertension. BCS is also associated with a relatively high incidence of hepatocellular carcinoma (HCC). Selecting a minimally invasive approach based on hemodynamic assessment for the management of HCC arising from BCS is essential. A hepatic tumor located in liver segment 8 region was identified in an 88-year-old female patient with BCS. Following a detailed preoperative hemodynamic evaluation using angiography, a thoracoscopic transthoracic hepatectomy (TTH) was successfully performed.

INTRODUCTION: Following major liver resection, posthepatectomy liver failure (PHLF) is associated with a high mortality rate. As there is no therapy for PHLF available, avoidance remains the main goal. A sufficient future liver remnant (FLR) is one of the most important factors to reduce the risk for PHLF; however, it is not known which patients benefit of volumetric assessment prior to major surgery.
METHODS: A retrospective, bi-institutional cohort study was conducted including all patients who underwent major hepatectomy (extended right hepatectomy, right hepatectomy, extended left hepatec

In the recent years, the number of liver resections has seen an impressive growth. Usually, hepatic resections remain the treatment of various liver diseases, such as malignant tumors, benign tumors, hydatid disease, and abscesses. Despite technical advancements and tremendous experience in the field of liver resection of specialized centers, there are moderately high rates of postoperative morbidity and mortality, especially in high-risk and older patient populations. Although ultrasonography is usually the first-line imaging examination for postoperative complications, Computed Tomography (C

BACKGROUND: Surgical resection is the only treatment modality that ensures complete tumor removal in patients with liver tumors involving a major hepatic vein. Central hepatectomy is a challenging procedure that often result in large defect at the right hepatic vein, which is not amenable to suturing or end-to-end anastomosis. Meanwhile, good outflow reconstruction is essential for early postoperative recovery and long-term survival.
METHODS: We describe a simple technique for reconstructing the right hepatic vein. The technique is an effective method for reconstructing large venous defects a

BACKGROUND: Despite the successful oncological results of liver transplantation, patients with hepatocellular carcinoma (HCC) can develop tumor recurrence. When technically feasible, liver resection represents the preferred treatment for recurrent HCC, even in the setting of transplanted patients. Recent progresses in minimally invasive liver resections have pushed the surgical community to attempt more challenging cases. We report a full laparoscopic left hepatectomy for HCC recurrence on transplanted liver.

PURPOSE: Portal vein thrombosis (PVT) following hepatectomy is potentially life-threatening. We aimed to evaluate the incidence of PVT after hepatectomy for hepatocellular carcinoma and identify coagulation and fibrinolytic factors that could predict early-stage postoperative PVT.
METHODS: A retrospective analysis was conducted on 65 hepatocellular carcinoma patients who underwent radical hepatectomy. The risk factors for postoperative PVT were identified based on univariate and multivariate analyses, and the levels of coagulation and fibrinolytic factors were measured during the perioperativ

BACKGROUND: LT for infants less than 5 kg remains a challenge with high technical complication rates, which is further compounded by large-for-size grafts requiring hyper-reduction. The benefits of MIDH especially for standard left lateral segment (LLS) resection have been unequivocally demonstrated. However, given the fine margins of error, the highly challenging technical aspects of anatomical graft reduction test the limits of safety and may not be routinely feasible with the conventional laparoscopic approach.
