Juvenile hyalin fibromatosis.

A boy had multiple large tumors on the scalp, whitish nodules on the nape and both sides of the neck, hypertrophic gingiva, and severe flexural contractures of hip and knee joints. The histopathologic structure of the tumor was characteristic of juvenile hyalin fibromatosis. The tumor cells were embedded in an amorphous eosinophillic ground substance. X-ray films revealed numerous osteolytic and osteociastic lesions that are important findings in the study of this disease.

Clinical Outcomes of Arthroscopic Annular Release Around the Greater Trochanter in Severe Gluteal Muscle Contracture Cases: A Technical Note and Case Series.

OBJECTIVE: Severe gluteal muscle contracture often requires surgical release, yet conventional open procedures are associated with extensive soft tissue trauma, significant postoperative pain, and prolonged rehabilitation. This study aimed to introduce an arthroscopic annular release technique for severe gluteal muscle contracture and evaluate its clinical outcomes. METHODS: A retrospective review was conducted using data from patients with severe gluteal muscle contracture who underwent arthroscopic treatment between September 2019 and October 2022. Inclusion criteria were patients presented

A computer model of slipped capital femoral epiphysis. Causes of fixation error and chondrolysis.

Penetration of the joint by metal pins can occur when treating slipped capital femoral epiphysis. Predisposing factors are present in the usual surgical methods. These include x-ray detail loss secondary to the pelvic projection and failure to obtain specific x-ray orientation to the axis of the capital epiphysis. One operative strategy to reduce these predispositions is presented. A probable mechanism of how chondrolysis can occur without joint penetration following extra capsular osteotomy is described with a theoretical remedy. Computer disease modeling and treatment simulation are the basi

Giant Cell Tumor of the Femoral Head: A Case Report and Review of the Literature.

INTRODUCTION: Primary total hip replacement in hypo plastic proximal femur is difficult due to the presence of a small canal, soft- tissue contracture, fragility of bone, and poor femoral cement mantle when used. Intraoperatively, there could be occurrence of fracture of the femur, inadequate fit, and fill with cement less femoral component. It is found unilaterally in cases of osteonecrosis of femoral head, post- traumatic, and in sequelae of childhood septic arthritis. CASE REPORT: A 45-year- old male patient presented to the OPD with complaints of pain in the left hip and difficulty in walk

Evaluating the effectiveness of interventions targeting only the acetabular side in the surgical treatment of Legg-Calvé-Perthes disease.

OBJECTIVE: This study aims to show the effectiveness of only acetabular-side surgeries for hips affected by Legg-Calvé-Perthes disease. PATIENTS AND METHODS: Twelve patients who underwent two different acetabular osteotomies -pembersal osteotomy and triple osteotomy - were evaluated retrospectively. Clinically, patients were examined for hip range of motion, flexion contracture, and hip extensor and abductor strength; the results were evaluated using the Harris hip score. Radiologically, the hips were assessed according to three different radiological parameters: lateral center edge angle, ac

Muscle involvement and motor function in amyoplasia.

The most common form of arthrogryposis multiplex congenita (AMC) is amyoplasia. Diagnostic criteria are highly specific, with decreased muscle mass, typical contractures and limb positioning at birth. Intellectual development is normal. The aims of this study were to investigate how muscle strength and contractures affect motor function in amyoplasia, and to relate current functional status to joint positions at birth. Medical records were reviewed and a clinical examination was performed, with investigation of muscle strength, range of motion, and motor function. Thirty-five patients with amy

A rare case of arthrogryposis multiplex congenita in a 2-year-old boy case report.

Arthrogryposis multiplex congenita (AMC) is an uncommon condition present from birth that is marked by a combination of weakened muscles and multiple joint contractures. We present a case of a 2-year-old boy with AMC, who was born to consanguineous parents in Saudi Arabia. He presented with musculoskeletal abnormalities of all four limbs, including symmetric contractures in multiple joints of the body, bilateral developmental dysplasia of the hip, and vertical talus. Dysmorphic features included low-set ears, chin recession, triangular face, and nevus flammeus on the face. The child also had l

[Ehlers-Danlos syndrome--clinico-pathology and orthopedic treatment].

Eight cases of Ehlers-Danlos syndrome (EDS) were surveyed for clinical manifestations and orthopaedic treatment. Cases of EDS were clinically subdivided into three groups. The first group was characterized by multiple congenital contractures in the extremities, so that it was very difficult to distinguish it from arthrogryposis multiplex congenita in early infancy. Since laxity of the soft tissue coexists with contracture in this group, over-correction of deformity caused much more disability than under-correction, especially in clubfoot. Care should therefore be taken to avoid this. The third

Applications in hip pathology.

Everyday clinical practice frequently leads us to suspect a close relationship between the lumbar spine and the hip-joints. Sagittal balance fundamentally expresses a postural strategy mobilizing the dynamic structure of the lumbar-pelvic-femoral complex in an authentic balance by which obligatory coupled movements transmit stresses in a single structure, the spine, to the two-part structure of the lower limbs, and vice-versa. Flexion contracture is a frequent hip pathology, but congenital dislocation and ankylosis of the hip have the greatest impact on the spine, due to excessive mechanical s

Total Hip Arthroplasty without Subtrochanteric Shortening Osteotomy in a Neglected Posterior Hip Dislocation: A Case Report.

INTRODUCTION: Neglected posterior hip dislocation is uncommon and can cause soft-tissue contracture, proximal migration of the femur, limb length discrepancy, and secondary degenerative changes. In chronic cases with joint destruction, total hip arthroplasty is often the preferred reconstructive option. However, reduction can be difficult and may require additional procedures, such as femoral shortening osteotomy. CASE REPORT: A 41-year-old male presented with right hip pain for 2 years and difficulty in walking for 18 months following a road traffic accident. Imaging showed neglected posteri

Clinical evaluation of a new orthosis, the 'walkabout', for restoration of functional standing and short distance mobility in spinal paralysed individuals.

The Walkabout orthosis is a relatively new device for assisted standing and mobility in spinal paralysed individuals. The design, with a medially-mounted single-axis hinge joint linking two knee-ankle-foot orthoses, is quite different to other currently available orthoses which have laterally positioned hip joints such as the Reciprocal Gait Orthosis or Hip Guidance Orthosis. Twenty-five spinal cord injured patients were fitted and trained with the Walkabout orthosis and followed up regularly for just under 2 years on average. Sixty percent of all the patients fitted have incorporated use of t

[Development of the S-form hip brace of Wakayama Medical College type for osteoarthritis of the hip].

In order to improve the therapeutic results of conservative treatment for coxarthrosis and to start functional training soon after surgery, the S-form hip brace of Wakayama Medical College type has been developed with due regard to the instability of the hip joint which is related to the origin of coxarthrosis and to biomechanics of the hip joint. Functional investigation was made by electromyography, etc. Even healthy people tend to develop gradual lack of stability at the hip joint with aging and the tendency is more pronounced in secondary coxarthrosis. This brace has been designed to reinf

Impact of the sagittal spinopelvic and coronal lower extremity alignments on clinical outcomes after medial unicompartmental knee arthroplasty.

INTRODUCTION: We evaluated whether the clinical outcomes, including postoperative knee range of motion (ROM), after unicompartmental knee arthroplasty (UKA) were associated with the sagittal spinopelvic parameters and coronal alignment of the full lower extremity. METHODS: Forty-two patients (50 knees: six men, seven knees; 36 women, 43 knees) who underwent medial UKA between April 2015 and December 2022 were included. Preoperative radiographic examinations of the index for sagittal spinopelvic alignment included the sagittal vertical axis (SVA), lumbar lordosis, sacral slope (SS), pelvic til

Why does pelvic deformity occur in experimental dislocation of the growing hip?

A three-dimensional pelvic deformity was observed when experimental dislocation of the hip was reproduced in Wistar rats using hormonal and biomechanical factors. Resection or surgical dislocation of the femoral head created its absence in the acetabular socket without pelvic deformity. Muscular contractures were observed in all animals. We conclude that progressive displacement of the femoral head could produce pelvic deformity and simultaneous alterations in the triradiate cartilage.

[Diastrophic dwarfism (author's transl)].

On the basis of their own patient the authors discuss clinical and radiological features of diastrophic dwarfism. In this rare disorder the infant is dwarfed, the limbs are shortened. It is associated with marked talipes equinovarus, limited movements and contractures of other joints. Dislocation in the hip or knee and development of kyphoscoliosis leads to furhter deformity. As aetiopathogenesis the authors consider the role of infection in the mother during her early pregnancy or teratogenic aspects of some drugs (tetracyclines, septrim).

A 5 year result of neglected posterior hip dislocation with unusual presentation of false acetabulum treat with skeletal traction followed by cemented total hip replacement: A case report.

INTRODUCTION AND IMPORTANCE: Neglected posterior hip dislocations in adults are rare, particularly when untreated for years. In developing nations, patients often rely on traditional bone setters, leading to delayed diagnosis and increased complications. Adult hip dislocations carry a higher risk of avascular necrosis and require complex treatments. This case illustrates a five-year delayed posterior hip dislocation with a false acetabulum, managed with skeletal traction and total hip replacement. This case report is written in compliance with the SCARE guideline. CASE PRESENTATION: A 64-year

Progress in the management of the paralytic hip in myelomeningocele.

1. The primary aim of management at the hip joint in the patient with myelomeningocele is the prevention and correction of deformity, usually a flexion deformity. 2. It is necessary to treat both flexion deformity and the causative muscle imbalance; correction of this imbalance will also prevent subsequent dislocation of the lip. 3. The psoas tendon is generally excised; transfer is reserved for a few children with excellent walking potential and a good prognosis from all viewpoints. 4. Flexion deformity is treated by a soft tissue release procedure (for children under the age of eight years a

Impact of flexion contracture severity on early outcomes of computer-navigated total knee arthroplasty: a retrospective cohort study.

OBJECTIVE: This study assessed the accuracy, safety, and early clinical outcomes of computer-navigated total knee arthroplasty (TKA) in the treatment of varying degrees of knee flexion contracture. METHODS: We conducted a retrospective cohort study of patients undergoing primary TKA by a single surgeon between January 2023 and June 2024. Patients were categorized into three groups according to preoperative knee flexion contracture severity: Group A (mild, ≤ 15°, 40 knees), Group B (moderate, 15°–30°, 35 knees), and Group C (severe, > 30°, 28 knees). Coronal plane varus deformity was assessed u

[Anesthetic management of a patient with Dyggve-Melchior-Clausen syndrome].

The Dyggve-Melchior-Clausen syndrome (DMCS) is a rare autosomal recessive skeletal dysplasia characterized by short-trunk dwarfism and mental retardation. A 49-year-old male with DMCS underwent resection arthroplasty for contracture of the right hip joint under general anesthesia using thiamylal, nitrous oxide, sevoflurane, and vecuronium. Although he was assumed to have difficult airway due to short neck, macroglossia, and disturbance of neck flexion, tracheal intubation was not difficult. No complications including malignant hyperthermia were observed during the 95 min of the operation.
