Osteoporosis treatment rate following hip fracture in a community hospital.

UNLABELLED: Treatment rates for osteoporosis after a major osteoporotic fracture are unacceptably low. We evaluate the effectiveness of an ortho-geriatric team (OGT) in initiating pharmacologic therapy for osteoporosis post-hip fracture. The OGT was able to achieve a higher treatment rate for patients post-hip fracture in comparison to usual care provided by the primary care hospitalist. Potential reasons for delaying or not proceeding with drug therapy include patient concern regarding potential rare side effects of antiresorptive therapy including osteonecrosis of the jaw and atypical femora

Heart Failure With Preserved Ejection Fraction: A Practical, Stepwise Guide to Evidence-Based Therapy in 2026.

Heart failure with preserved ejection fraction (HFpEF) represents a large and growing share of the overall heart failure population and remains clinically challenging to manage. HFpEF has historically lacked effective therapies and a clear treatment algorithm accessible to non-specialists. Recent years have brought new pharmacologic options and expanded regulatory approvals, along with combination-therapy modeling suggesting meaningful projected lifetime benefit, yet translation of this evidence into everyday practice remains slow, particularly outside cardiology. Existing reviews synthesizing

Thiamine for altered mental status: a high-value strategy beyond alcohol.

BACKGROUND: Altered mental status (AMS) prompts rapid diagnostic evaluation in hospitalized patients, yet a low-cost, reversible cause - thiamine deficiency - often receives inconsistent attention outside alcohol-associated contexts. Although classically linked to alcohol misuse, contemporary evidence links thiamine depletion to a broad range of medical, surgical, oncologic, obstetric, and critically ill populations. DISCUSSION: In these settings, AMS may be the earliest clinical manifestation, while standard diagnostic tools such as MRI and serum thiamine levels frequently lack sensitivity e

Acute Care of Patients with Moderate Respiratory Distress: Recommendations from an American College of Emergency Physicians Expert Panel.

INTRODUCTION: Patients with respiratory distress are frequently encountered in the emergency department (ED). Efforts to assess, initiate treatments, and stabilize these patients require a systematic and rapid response. Emergency physicians need a comprehensive and efficient approach for evaluating, treating, and managing patients presenting to the ED with moderate respiratory distress. METHODS: The American College of Emergency Physicians convened an expert panel of academic and community emergency physicians, critical care specialists, respiratory therapists, hospitalists, and pharmacists t

Bridging the Gap: From Early Recognition to Timely Referral for Cardiac Implantable Electronic Devices Infections.

An increasing number of patients are living with cardiac implantable electronic devices (CIEDs). When a patient with a CIED is hospitalized with concern for infection or positive blood cultures from any source, recognizing and managing a possible CIED infection is critical due to its significant morbidity and mortality risk. Internists are often the first to evaluate these patients and must consider the possibility of an underlying device infection. Gram-positive bacteremia carries a high risk of CIED infection, often necessitating cardiac imaging. In contrast, hematogenous seeding with gram-n

Case Report of Retarded Catatonia: Always Consider Catatonia as a Differential Diagnosis of Altered Mental Status.

BACKGROUND: Catatonia is a rare neuropsychiatric condition; it is estimated that around 10% of patients with mood disorders present signs and symptoms of catatonia. A catatonic syndrome is characterised by mutism, negativism, rigidity, and stupor. CASE REPORT: We report the case of a 59-year-old patient with a medical history of bipolar disorder who was admitted to the internal medicine service due to a seizure episode. During hospitalisation, the patient presented significant worsening of her clinical condition, showing marked symptoms of stupor and catatonia. Once the neurological and metab

Preoperative Management of Medications for Rheumatologic and HIV Diseases: Society for Perioperative Assessment and Quality Improvement (SPAQI) Consensus Statement.

Perioperative medical management is challenging because of the rising complexity of patients presenting for surgical procedures. A key part of preoperative optimization is appropriate management of long-term medications, yet guidelines and consensus statements for perioperative medication management are lacking. Available resources use recommendations derived from individual studies and do not include a multidisciplinary focus on formal consensus. The Society for Perioperative Assessment and Quality Improvement identified a lack of authoritative clinical guidance as an opportunity to use its m

Variations in Processes of Care and Outcomes for Hospitalized General Medicine Patients Treated by Female vs Male Physicians.

IMPORTANCE: Hospitalized medical patients cared for by female physicians may have decreased mortality rates compared with patients of male physicians. However, this association has yet to be assessed outside of the US, and little is known about factors that may explain this difference. OBJECTIVE: To determine whether mortality, other hospital outcomes, and processes of care differed between the patients cared for by female and male physicians. DESIGN SETTING AND PARTICIPANTS: This retrospective cross-sectional study included patients admitted to general medical wards at 7 hospitals in Ontari

Successful de-implementation of an ineffective practice: The fall of docusate.

BACKGROUND: Reducing wasteful practices optimizes value in medicine. Docusate lacks treatment efficacy yet is widely prescribed. This quality improvement project aimed to de-implement docusate in place of a new evidence-based order set. METHODS: This is an ambidirectional study of inpatient laxative orders from 2018 to 2022 at one institution. We stratified docusate data by service/unit to target prospective deimplementation initiatives. A new evidence-based constipation order set was embedded in Cerner. RESULTS: There were 701,732 docusate orders across 75 services on 68 units. Top docusat

Ovarian Hyperstimulation Syndrome: An Unusual Cause of Abdominal Pain Presenting to the Emergency Department.

The use of assisted reproductive technologies (ART), such as in vitro fertilization (IVF), is steadily increasing. Ovarian hyperstimulation syndrome (OHSS) is a rare but potentially serious complication arising from controlled ovarian hyperstimulation (COH), a critical component of ART. OHSS may vary in severity and usually causes abdominal pain, nausea, vomiting, diarrhea, and dyspnea, which are common complaints for patients presenting to the emergency department (ED). Preventive measures and early recognition by reproductive medicine specialists generally reduce the incidence of severe OHSS

Comparison of 2 Models of Care for Children With Medical Complexity Following Spinal Fusion.

We conducted a retrospective chart review of patients with neuromuscular scoliosis following spinal fusion surgery who were cared for post-operatively by either a hospitalist/orthopedics co-management team or a complex care clinic (CCC). Assignment to either treatment group was not random. To account for baseline differences between groups, we calculated propensity scores and used these as probability weights in generalized linear models. After matching, the CCC had a shorter length of stay (LOS, coefficient = -2.60; P = .04) without a significant difference in 30-day readmission rate (P = .62

Evaluation of concurrent benzodiazepine and opioid prescribing patterns with a focus on acute muscle spasms indication at hospital discharge.

BACKGROUND: In 2016, the CDC Guidelines for Prescribing Opioids in Chronic Pain recommended avoiding the co-prescribing of opioids and benzodiazepines when possible. This evaluation provides one institution's approach to improve safety for new, acute concurrent opioid and benzodiazepine prescribing. METHODS: Patients prescribed new, concurrent opioids and benzodiazepines upon discharge within one organization were identified. A chart review for the highest prescribing specialties was completed for a random sample of 100 patients to determine indications for prescribing, safety measures, and p

An Electronic Health Record-Integrated, Large Language Model-Powered Tool to Triage Surgical Patients.

IMPORTANCE: Surgical comanagement (SCM) is an evidence-based care model in which hospitalists jointly manage medically complex perioperative patients alongside surgical teams. Despite its clinical and financial value, effective use of SCM is limited by the need to manually identify eligible patients; large language models (LLMs) are increasingly prevalent in clinical workflows and could be useful in selecting patients for SCM. OBJECTIVE: To assess whether SCM eligibility triage can be automated. DESIGN, SETTING, AND PARTICIPANTS: This prospective, unblinded quality improvement study was cond

In-Hospital Management and Follow-Up Treatment of Venous Thromboembolism: Focus on New and Emerging Treatments.

Venous thromboembolism (VTE), encompassing pulmonary embolism (PE) and deep venous thrombosis (DVT), is a major cause of morbidity and mortality of particular relevance for intensivists and hospitalists. Acute VTE is usually managed with parenteral unfractionated heparin or low-molecular-weight heparin, followed by an oral vitamin K antagonist. Data are lacking for optimal treatment of less common occurrences, such as upper extremity DVT, and for approaches such as thrombolysis for PE associated with early signs of hemodynamic compromise or inferior vena cava filters when anticoagulation is co

Hospital Utilization in Children With Medical Complexity in a Cross-Setting Complex Care Program.

INTRODUCTION: Children with medical complexity (CMC) account for disproportionate pediatric hospital utilization. Complex care programs (CCP) may reduce utilization but are often resource intensive. We evaluated whether enrollment in a cross-setting CCP was associated with reduced hospitalizations and inpatient days. METHODS: We conducted a retrospective cohort study of CMC enrolled in a children's hospital CCP from 2015 to 2024. The CCP supports cross-setting care through a small team (pediatric hospitalist, nurse practitioner, and community health worker) without dedicated clinic space. Out

Timing of Surgery and Medical Optimization in Multisystem Trauma: A Systematic Review of Outcomes Based on Internal Medicine Co-management With Orthopedic, Neurosurgical, Vascular, Thoracic, and General Surgical Emergencies.

This systematic review explores the impact of internal medicine co-management on surgical outcomes and timing in adult patients across various surgical specialties, with an emphasis on trauma care. A comprehensive literature search identified five eligible studies comprising over 60,000 patients. Co-management models included hospitalist-led, advanced practice clinician-led, and multidisciplinary approaches. The outcomes consistently showed reductions in complications, hospital length of stay, rapid response team activations, and 30-day mortality. One study also highlighted indirect timing ben

An Overview of Interrupted Time Series as a Research Method.

Interrupted time series (ITS) study design is increasingly used to evaluate the impacts of guidelines, policy changes, and quality improvement efforts on clinical outcomes. Repeated observations of an outcome of interest over time are "interrupted" by an intervention, dividing the study into pre- and postintervention time periods. Regression analysis then compares the level change and slope of the 2 time periods to determine intervention effects. The benefits of this study design include (1) use of lower-cost, preexisting large data sets, (2) reduced susceptibility to confounding by individual

The intricate relationship of histoplasmosis and sarcoidosis: a case report.

INTRODUCTION: Histoplasmosis is an endemic mycosis with most cases of clinical illness reported in North and Central America. Rarely, patients develop progressive disseminated histoplasmosis with extrapulmonary manifestations. These infections are fatal if not appropriately treated. CASE PRESENTATION: We report a case of progressive disseminated histoplasmosis presenting with fever, progressive dyspnea, and pancytopenia in a 51-year-old Caucasian man who had been treated with chronic steroids for a diagnosis of sarcoidosis made 20 years previously. Attachments in public health for physicians.

Hospital-based specialist registrars in Nottingham are offered six month secondments in public health medicine. These attachments give clinicians valuable skills in public health and an opportunity to influence the development of local health service provision. Other skills gained are an understanding of the balance between health promotion and disease treatment; the management of limited resources; an appreciation of the effects of social deprivation; the chairing and preparing of committee meetings; and contact tracing of communicable diseases. We strongly recommend the experience gained fro
