[Humeral head fractures following epileptic attacks. Case report on a rare but typical injury].

We report about two patients with posterior fracture-dislocation of the shoulder following convulsive seizures, one case unilateral, one case bilateral. The radiological findings are typical and similar to the cases described in literature: Impression or head splitting fracture of the ventro-medial part of the humeral head associated with posterior dislocation. The mechanism of injury, methods of treatment and late results are discussed.

False aneurysm of axillary artery following resection of a traumatized humeral head: case report.

A case of false aneurysm and damage to the neurovascular bundle as a late complication to excision of a shattered humeral head is described. The authors advocate smoothing of the end of the remaining diaphyseal part of the humerus and, further, that if a swelling in the operative area occurs postoperatively, arteriography should be performed.

Osteonecrosis of the Humeral Head Following Arthroscopic Repair of Rotator Cuff Tears: A Case Report.

INTRODUCTION: The osteonecrosis and the collapse of the humeral head may have many risk factors such as trauma, alcoholism, metabolic diseases, and corticosteroid therapy. Otherwise, it was described as a rare complication of shoulder arthroscopy in the past few years. CASE REPORT: We report the case of a 65-year-old right-handed woman who had a rotator cuff tear of the right shoulder. She underwent a double-row arthroscopic repair. Six months later she had an intense shoulder pain, with radiological and MRI signs of humeral head osteonecrosis. The patient had a reverse total shoulder arthrop

Hyaluronic acid injections for treatment of pediatric sickle cell avascular necrosis of the humeral head.

BACKGROUND: Avascular necrosis (AVN) can impact up to 50% of patients with sickle cell disease (SCD) and can result in significant pain, decline in physical function and decreased quality of life. While hyaluronic acid (HA) has been used in the adult population for shoulder osteoarthritic pain, we present the first published pediatric case of HA injections in the glenohumeral joint, used to improve function and pain control. CASE PRESENTATION: The patient is a 12-year-old woman with SCD, who suffered from chronic pain due to AVN of the humeral and femoral head. Despite engaging in a multidisc

Morphological and Apparent-Level Stiffness Variations Between Normal and Osteoarthritic Bone in the Humeral Head.

Osteoarthritis (OA) is characterized by morphological changes that alter bone structure and mechanical properties. This study compared bone morphometric parameters and apparent modulus between humeral heads excised from end-stage OA patients undergoing total shoulder arthroplasty (n = 28) and non-pathologic normal cadavers (n = 28). Morphometric parameters were determined in central cores, with regional variations compared in four medial to lateral regions. Linear regression compared apparent modulus, morphometric parameters, and age. Micro finite element models estimated trabecular apparent m

[Proximal humerus fractures: clinical assessment and functional outcome in patients with osteonecrosis of the humeral head].

INTRODUCTION: Fractures of the proximal humerus resulting in three or more fragments represent a high risk of necrosis. The former is the second most frequent site of necrosis, only after the femur, due to either traumatic or nontraumatic causes. The former are the most common, with an incidence rate of necrosis of 13-34% in fractures with more than four fragments. The purpose of this study is to assess the functional outcome of patients with a history of fracture of the proximal humerus and osteonecrosis treated conservatively. MATERIAL AND METHODS: This is a descriptive, retrospective, obse

The effect of morphometric relationship between the glenoid fossa and the humeral head on rotator cuff pathology.

OBJECTIVE: The aim of this study was to investigate the impact of the morphometric association between the glenoid fossa and the humeral head on rotator cuff pathology. METHODS: Shoulder MRI examinations performed for any cause in study centers between August 2008 and August 2009 were retrospectively evaluated. Shoulder MRI exams having rotator cuff pathology, such as trauma, degeneration, and acromion Type 2, 3 and 4 were excluded. The study included 62 shoulder exams with rotator cuff pathology having Type 1 acromion morphology and 60 shoulder exams without rotator cuff pathology (control g

[Femoral and humeral head osteonecrosis in a patient with hypofibrinolysis and hyperhomocysteinemia. A case report and a review of the literature].

Osteonecrosis is a disease characterized by the death of marrow and bone tissues. All bones may be affected, most commonly those of the hip, knee, shoulder, ankle as well as the small bones of the hands and feet. When the disease involves a weight-bearing joint there is a significant risk that subarticular fracture may develop leading to disabling arthrosis and requiring, therefore, arthroplasty surgery. Osteonecrosis typically affects patients in their third, fourth and fifth decades of life and is associated with many factors including other diseases and co-morbidities. Multifocal osteonecro

Abnormal modelling of the humeral head in the tricho-rhino-phalangeal syndrome: a new radiological observation.

The case of a 27-year-old female patient with tricho-rhino-phalangeal (TRP) type I syndrome is reported. The patient demonstrated the classical features of slowly growing hair, a bulbous nose and brachydactyly with swelling at the interphalangeal joints, but in addition showed some of the less common manifestations such as supernumerary teeth and prognathism. To the authors' knowledge this is the first reported case in which modelling abnormalities of the proximal humerus mirror those seen in the femoral head. This adds weight to the argument that the skeletal abnormalities in TRP are part of

A novel surgical procedure for osteonecrosis of the humeral head: reposition of the joint surface and bone engraftment.

A novel surgical procedure was performed on a 33-year-old woman with idiopathic osteonecrosis of the head of the left humerus. The operation involved repositioning of the joint cartilage and bone engraftment through her humeral head from under the greater tuberosity with shoulder arthroscopy. The patient wore an abduction brace for 8 weeks after the operation to hold the joint surface in its new position. This surgical procedure resulted in considerable improvement of the functional status of the shoulder by relieving pain and increasing range-of-motion. A preoperative radiograph showed stage

Delayed complications of treated reduced recurrent anterior dislocations of the humeral head in young adults.

Patients with recurrent anterior dislocations of the humeral head are advised to have the disability corrected by open surgical intervention. Painful and restricted motion after such a procedure is not always due to recurrence of the dislocation; it may be a consequence of changes in the articular surfaces and soft tissues of the joint-i.e., osteoarthritis. We observed that osteoarthritis was present in instances of recurrent anterior dislocations in both operative and nonoperative cases, but to a lesser degree in some of the operative cases.

Effects of adipose allograft matrix on viability of humeral head cartilage and rotator cuff tendon.

BACKGROUND: Rotator cuff repairs may fail because of compromised blood supply, suture anchor pullout, or poor fixation to bone. To augment the repairs and promote healing of the tears, orthobiologics, such a platelet-rich plasma (PRP), and biologic scaffolds have been applied with mixed results. Adipose allograft matrix (AAM), which recruits native cells to damaged tissues, may also be a potential treatment for rotator cuff tears. METHODS: To assess the potential use of AAM on rotator cuff tears, humeral head cartilage and subscapularis tendon were collected from patients undergoing reverse s

Osteonecrosis of the humeral head: trends in surgical management of a nationwide sample from 2010 to 2021.

BACKGROUND: The humeral head is the second most common site for osteonecrosis (ON) but its epidemiology is poorly described. This study aimed to better understand its treatment in the United States by 1) evaluating total operative procedures with rates normalized to the annual surgical volume; 2) determining trends of nonjoint preserving (shoulder arthroplasty) vs. joint preserving procedures; and 3) quantifying rates of operative techniques in different aged cohorts (<50 vs. ≥50 year old). METHODS: A nationwide database was queried for patients with humeral ON who underwent surgery between J

The arthroscopic double-button Latarjet does not modify the static posterior translation of the humeral head.

INTRODUCTION: The Latarjet procedure treats anterior instability of the shoulder and is based on a triple anterior lock, where the conjoint tendon straps the lower third of the subscapularis muscle. Excessive posterior translation of the humeral head is a known risk factor for shoulder osteoarthritis. No in vivo study has investigated the effect of the bone block on the posterior static translation of the humeral epiphysis. The purpose of this study was to evaluate the effect of the bone block on the static posterior translation of the humeral head. The hypothesis was that this procedure incre

The "BioHumi" Humeral Head Osteochondral Allograft Transplantation for Reverse Hill-Sachs Lesion of the Shoulder.

Osteochondral defects of the shoulder due to posterior instability are less frequent than those caused by anterior instability. Although uncommon, locked posterior dislocations can create sizable osteochondral lesions of the anterior humeral head known as reverse Hill-Sachs lesions. Treatment of these defects to restore the congruent contour of the glenohumeral joint is essential to reduce recurrence of instability and prevent long-term sequelae of arthritis. Historically, nonanatomic options, such as transposition of the subscapularis tendon or lesser tuberosity into the defect and humeral ro

Reconstruction or replacement? A challenging question in surgical treatment of complex humeral head fractures in the elderly.

INTRODUCTION: Surgical treatment of complex humeral head fractures in the elderly is challenging due to osteoporotic bone, comorbidities and reduced compliance. The treatment strategy (reconstruction versus replacement) should allow for a functional aftercare and result in a high patient satisfaction. Major complications leading to surgical revision are crucial and should be avoided. The purpose of this study was to analyse the major complication rate leading to surgical revision and the patient-based outcome in complex humeral head fractures of the elderly population treated either using lock

Wear behaviour of polyethylene glenoid inserts against PyroCarbon humeral heads in shoulder arthroplasties.

The generation of polyethylene wear debris, and the subsequent tissue reaction to such debris is considered to be a limitation in the long-term survival of shoulder arthroplasties. The purpose of this study was to investigate, for the first time, the wear of a novel PyroCarbon-on-Polyethylene (PyCoP) shoulder arthroplasty system. A 5 million cycle wear test was performed on PyroCarbon humeral heads, which were articulated against commercially available polyethylene glenoid insert components to form an anatomic total shoulder arthroplasty (aTSA). A "Repeat-motion-load" physiological combined cy

Arthroscopic Osteochondral Autograft Transfer for Juvenile Osteochondritis Dissecans of the Humeral Head: A Case Report.

CASE: Osteochondritis dissecans (OCD) rarely affects the humeral head. We describe a 14-year-old adolescent who, despite conservative treatment, had persistent pain in the left shoulder as well as limited function. Radiographs and a computed tomography (CT) arthrogram revealed an osteochondral defect of the humeral head. He was managed with an arthroscopic osteochondral autograft transfer from the knee, which provided a satisfactory outcome. CONCLUSION: Arthroscopic osteochondral autograft transfer is an effective option for the treatment of OCD of the humeral head.
