Chronic hypercapnia (CH) is a hallmark of respiratory diseases such as chronic obstructive pulmonary disease. In such patients, mechanical ventilation is often used to restore normal blood-gas homeostasis. However, little is known regarding physiological changes and neuroplasticity within physiological control networks after termination of CH. Utilizing our goat model of increased inspired CO2-induced CH, we determined whether termination of CH elicits time-dependent physiological and neurochemical changes within brain stem sites of physiological control. Thirty days of CH increased [Formula:

BACKGROUND: It is increasingly evident that perturbations to the diversity and composition of the gut microbiota have significant consequences for the regulation of integrative physiological systems. There is growing interest in the potential contribution of microbiota-gut-brain signalling to cardiorespiratory control in health and disease.
METHODS: In adult male rats, we sought to determine the cardiorespiratory effects of manipulation of the gut microbiota following a 4-week administration of a cocktail of antibiotics. We subsequently explored the effects of administration of faecal microbi

OBJECTIVES: Lung-protective ventilation is used to prevent further lung injury in patients on invasive mechanical ventilation. However, lung-protective ventilation can cause hypercapnia and hypercapnic acidosis. There are no large clinical studies evaluating the effects of hypercapnia and hypercapnic acidosis in patients requiring mechanical ventilation.
DESIGN: Multicenter, binational, retrospective study aimed to assess the impact of compensated hypercapnia and hypercapnic acidosis in patients receiving mechanical ventilation.
SETTINGS: Data were extracted from the Australian and New Zeala

BACKGROUND: The biological basis of uncued panic attacks is not yet understood. An important theory concerning the nature and cause of panic disorder is the 'suffocation false alarm theory'. This alarm is supposed to be over-sensitive in panic disorder patients and can be triggered by CO2. No neurobiological substrate has been identified for such an alarm. The present study investigates differences in brain activation in panic patients, healthy individuals and experienced divers in response to CO2, representing three groups with descending sensitivity to CO2.
METHOD: Brain activation was meas

BACKGROUND: Congenital diaphragmatic hernia (CDH) is a potentially lethal anomaly associated with pulmonary hypoplasia and persistent pulmonary hypertension. Permissive hypercapnia is a strategy designed to reduce lung injury from mechanical ventilation in infants. It has been shown to be a potentially superior method of ventilator management for patients with CDH. In 2001, the Divisions of Neonatology and Pediatric Surgery at the University of Virginia Children's Hospital established permissive hypercapnia as the management strategy for treatment of CDH. We hypothesized that permissive hyperc

At birth, the newborn fat-tailed dunnart relies on cutaneous gas exchange to meet metabolic demands, with continuous lung ventilation emerging several days later. We hypothesised that the delayed expression of lung ventilation (V(E)) in these animals is in part due to a low responsiveness of the respiratory control system to blood gas perturbations. To address this hypothesis, we assessed the ventilatory and metabolic response to hypoxia (10% O(2)) and hypercapnia (5% CO(2)) using closed-system respirometry from birth to 23 days postpartum (P). Neonatal fat-tailed dunnarts displayed no signifi

A reduction in diaphragm mobility has been identified in patients with chronic obstructive pulmonary disease (COPD) and has been associated with a decline in pulmonary function parameters. However, little information exists regarding the potential role of diaphragm mobility on hypercapnia in COPD. A new method of assessing the mobility of the diaphragm, using ultrasound, has recently been validated. The purpose of the present study was to investigate the relationship between diaphragm mobility and pulmonary function parameters, as well as that between arterial blood gas values and diaphragm mo

Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) are associated with impaired gas exchange, severe inflammation and alveolar damage including cell death. Patients with ALI or ARDS typically experience respiratory failure and thus require mechanical ventilation for support, which itself can aggravate lung injury. Recent developments in this field have revealed several therapeutic strategies that improve gas exchange, increase survival and minimize the deleterious effects of mechanical ventilation. Among those strategies is the reduction in tidal volume and allowing hyperca

Although recent models offer realistic descriptions of the human respiratory system, they do not fulfill all characteristics of a stable, comprehensive model, which would allow us to evaluate a variety of hypotheses on the control of breathing. None of the models offer completely realistic descriptions of the gaseous components of blood, and their description of delays associated with the propagation of changes in partial pressures of respiratory gases between the lungs and brain and tissue compartments have shortcomings. These deficiencies are of particular significance in an analysis of peri

Metabolic and ventilatory variables were measured in a large semifossorial marsupial, the hairy-nosed wombat (Lasiorhinus latifrons, 21.9 kg). In normoxia, the rate of oxygen consumption was 63% of that predicted for a similar-sized marsupial, and the level of ventilation (V(E)) was such that the convective requirement (V(E)/VO2) was similar to other mammals. Exposure to hypercapnia (5% CO(2)) evoked a hyperventilatory response (3.55 x normoxia) that was no different to that observed for epigeal (surface-dwelling) marsupials; the increase in V(E) was primarily achieved with an increase in tida

We assessed the time course of changes in eupneic arterial PCO(2) (Pa(CO(2))) and the ventilatory response to hyperoxic rebreathing after removal of the carotid bodies (CBX) in awake female dogs. Elimination of the ventilatory response to bolus intravenous injections of NaCN was used to confirm CBX status on each day of data collection. Relative to eupneic control (Pa(CO(2)) = 40 +/- 3 Torr), all seven dogs hypoventilated after CBX, reaching a maximum Pa(CO(2)) of 53 +/- 6 Torr by day 3 post-CBX. There was no significant recovery of eupneic Pa(CO(2)) over the ensuing 18 days. Relative to contr

The purpose of this study was to determine factors increasing daytime PaCO2 or PaO2 in obstructive sleep apnoea syndrome patients (OSAS) with normal pulmonary function tests. Anthropometric, pulmonary function tests, arterial blood gases and sleep polygraphic data were analysed retrospectively in 218 OSAS patients (apnoea-hypopnoea index > 15 h(-1); 18 females, 55 +/- 11 years): 125 patients had abnormal pulmonary function tests, i.e. one or more flow or volume under 80% or above 120% of predictive value (group I) and 93 had normal pulmonary function tests (group II). Hypercapnia was defined a

OBJECTIVE: To test the hypothesis that chronic hypercapnia changes the composition of the respiratory muscle by continuous augmentation of ventilation.
MATERIALS AND METHODS: Eighteen male Wistar rats were housed in 10% CO(2) in air for 19 weeks, and their minute ventilation V(E) was measured every 6 weeks. The diaphragm, excited at 19 weeks of exposure, was classified as fiber type I, IIa, or IIb. Cross-sectional areas of individual fibers were measured. Fibers with a target-like appearance on reduced nicotinamide adenine dinucleotide-tetrazolium reductase (NADH-TR) stain also were counted.

This investigation determined the effects of sustained hypercapnia on cerebral blood flow (CBF; radiolabeled microspheres), cerebral metabolic rates for O2 and glucose (CMRO2 and CMRglc), and brain water content in conscious sheep instrumented with aortic, left ventricular, vena cava, and brain sagittal sinus catheters. PaCO2 was elevated from 38 +/- 3 to 53 +/- 3 (mean +/- SD) mm Hg and PaO2 from 109 +/- 7 to 131 +/- 4 mm Hg for 96 h in an environmental chamber. Hypercapnia did not alter sheep behavior, food and water intake, arterial pressures, core temperature, or brain lactate release. Tot

The impact of autonomic neuropathy (common in patients on haemodialysis) on ventilatory response to hypercapnia has been studied. We investigated cardiac reflex tests in 20 patients on chronic haemodialysis (8 patients were found with and 12 without neuropathy of the autonomic nervous system). Using the hyperoxic CO2-rebreathing method (according to Read), we tested the above-mentioned two groups of patients and compared them with 14 healthy control subjects. Accumulation of CO2 in blood with hyperoxic CO2 rebreathing stimulates central chemoreceptors, and therefore causes a progressive rise i

Dopamine (DA) and noradrenaline (NA) were measured in the rabbit carotid body (CB) in vitro bv HPLC-ED under the following experimental conditions: 1h superfusion in normoxic, hypoxic (10% O2 in N2) or hypercapnic (8% CO2, 20% O2, 72% N2) medium, 5h superfusion in normoxia or hypoxia. The contents of DA and NA were decreased by hypoxia and hypercapnia after 1 h and 5h indicating a possible DA and NA secretion. Under the same experimental conditions synthesis of DA and NA and catabolism of DA were studied with enzymatic inhibition of tyrosine hydroxylase and monoamine oxidase (MAO) respectively

The effect of acute hypoxia and hypercapnia on skeletal muscle relaxation rate was investigated in man. The contractile force of limb muscles (quadriceps femoris and adductor pollicis) was studied in normal subjects using electrical stimulation. The twitch transdiaphragmatic pressure was used to investigate the diaphragm. The maximum relaxation rate was measured from contractions of the non-fatigued quadriceps femoris, adductor pollicis and diaphragm. Severe hypoxia (mean transcutaneous oxygen tension 40 mmHg) did not alter the maximum relaxation rate from 20 Hz stimulated contractions of the

The electrical activity of the arytenoideus muscle, a vocal cord adductor, was measured in 14 normal adult humans during progressive isocapnic hypoxia and progressive hyperoxic hypercapnia. Electromyograms of the arytenoideus were obtained with intramuscular hooked-wire electrodes implanted by means of a fiber-optic nasopharyngoscope. Correct placement of the electrodes was confirmed by discharge patterns during voluntary maneuvers. In three of the subjects, respiratory-related arytenoideus activity was not present during quiet breathing or chemical stimulation. During quiet breathing in the 1
