Efferent pathways in the reflex control of gastric emptying in rats.

Previous studies have established that acid, hypertonic, or protein-rich liquid test meals delay gastric emptying by reflex pathways involving the extrinsic innervation of the gut. To characterize the efferent pathways involved in these reflexes, we have studied the emptying of liquid test meals in control rats and in rats after celiac ganglionectomy, pyloroplasty, and treatment with guanethidine or 6-hydroxydopamine, and in rats with circulating vasoactive intestinal polypeptide (VIP) antibodies. The results suggest that the action of hypertonic solutions on gastric emptying requires an intac

[Experimental sclerosing cholangitis following intrabiliary injection of formol in the rat].

Our study was aimed at assessing the effect on the biliary epithelium of formol and hypertonic salt serum solutions used as parasiticide for hydatid cysts of the liver in rats. One hundred and forty six rats were administrated an intrabiliary injection of isotonic salt serum (control group), of 20% hypertonic salt serum and of 0.5% and 2% formol solutions. A histological study performed 3 months later did not show any abnormality in the rat control group (n = 11). In the hypertonic salt serum group (n = 14) and 0.5% formol group (n = 12), moderate lesions of biliary epithelium were observed. A

Transient Ca2+ mobilization caused by osmotic shock initiates salmonid fish sperm motility.

Salmonid fish sperm motility is known to be suppressed in millimolar concentrations of extracellular K(+), and dilution of K(+) upon spawning triggers cAMP-dependent signaling for motility initiation. In a previous study, however, we demonstrated that suspending sperm in a 10% glycerol solution and subsequent dilution into a low-osmotic solution induced motility independently of extracellular K(+) and cAMP. In the present study, we further investigated the glycerol-induced motility mechanism. We found that treatment with solutions consisting of organic or inorganic ions, as well as glycerol, i

Long term total parenteral nutrition through peripheral veins.

Most complications of total parenteral nutrition are directly related to the use of hypertonic glucose and central venous catheters. We describe a system in which hypertonic glucose is omitted from the total parenteral nutrition regimen and is replaced by Intralipid as the main source of calories. Protein hydrolysate and lipid solutions are infused simultaneously via peripheral veins. This method proved to be simple and efficient with no deleterious effects in 22 patients who were treated for periods of three to 14 weeks.

Use of hypertonic saline injection in trauma.

PURPOSE: The use of hypertonic saline injection in trauma patients is discussed. SUMMARY: Patients with hemorrhage, burns, and traumatic brain injury (TBI) may develop hypovolemic shock and require resuscitation. Compared with conventional isotonic crystalloids, hypertonic saline has several advantages, including hemodynamic, immune-modulating, and antiinflammatory effects, for use in trauma patients for resuscitation. In addition, hypertonic saline is also used in patients with TBI to reduce intracranial pressure (ICP). Overall, studies have not shown a difference in mortality or other clini

Influence of altered CSF solute composition on parotid salivary secretion in goats.

Infusions of hypertonic NaCl solution into the CSF of the lateral cerebral ventricle of the goat caused a marked reduction in parotid salivary flow concomitant with a rise in salivary [Na+]. Corresponding infusions of iso- or hypertonic glucose and glycerol solutions affected salivary secretion in the opposite direction. The possibility is discussed that a periventricular sodium-sensitive mechanism, which is of importance in the central control of fluid balance, also may participate in the regulation of parotid secretion in the goat. This interpretation of the results is to some extent obscure

Hypertonic versus near isotonic crystalloid for fluid resuscitation in critically ill patients.

BACKGROUND: Hypertonic solutions are considered to have a greater ability to expand blood volume and thus elevate blood pressure and can be administered as a small volume infusion over a short time period. On the other hand, the use of hypertonic solutions for volume replacement may also have important disadvantages. OBJECTIVES: To determine whether hypertonic crystalloid decreases mortality in patients with hypovolaemia. SEARCH STRATEGY: We searched MEDLINE, EMBASE, The Cochrane Controlled Trials Register and the specialised register of the Cochrane Injuries Group. We checked reference list

Intravenous Fluid Therapy Choice in Trauma Patients in the Intensive Care Unit: A Scoping Review.

BACKGROUND: In critically ill patients, intravenous fluid resuscitation is contentious. Although research has explored intravenous fluids for patients with sepsis or septic shock, evidence guiding fluid choices for trauma patients in intensive care units remains scarce. OBJECTIVE: To summarize current recommendations for intravenous fluid choices for resuscitation and their impact on outcomes in trauma patients in intensive care units. METHODS: The literature was appraised with a scoping review using the Joanna Briggs Institute framework. RESULTS: A search of databases (CINAHL Plus, MEDLINE

Hypertonic resuscitation after severe injury: is it of benefit?

There is a wealth of preclinical data suggesting potential benefit from the administration of hypertonic solutions after severe injury with hypovolemic shock, including improved tissue perfusion, improved flow through the microcirculation, and modulation of the inflammatory response, which may mitigate subsequent organ failure. However, despite these potential advantages, clinical trials of hypertonic resuscitation early after injury have failed to demonstrate significant benefit for resuscitation of hemorrhagic shock, and although there is no difference in overall mortality, there appears to

Intravenous hypertonic dextrose in conservatively managed adhesive small bowel obstruction: An exploratory observational study.

Adhesive small bowel obstruction (ASBO) is commonly managed conservatively, with adjuncts such as CT scan with per-oral water-soluble contrast, which are diagnostic and in some cases therapeutic. However, these may not be consistently available in low-resource settings. Hypertonic solutions have been shown to influence intestinal oedema and function, but the role of intravenous hypertonic dextrose is unclear. We describe an observational experience of adjunctive 25% dextrose (40 mL daily for 3 days) in selected patients with ASBO managed non-operatively at a tertiary centre in central India.

[Is there any place for hypertonic saline for fluid resuscitation in septic shock].

Fluid loading is the first step, necessary to care for severe sepsis. Two main classes of solutions are currently available: crystalloids and colloids. The concept of small volume resuscitation with hypertonic saline has emerged these last years in the care of traumatic haemorrhagic shock. The main benefits are the restoration of intravascular volume, improvement of cardiac output and improvement of regional circulations. Many experiments highlight modulation of immune and inflammatory cascades. We report the mechanisms of action of hypertonic saline based on experimental human and animal stud

Comparison of isotonic and hypertonic fluids in resuscitation from hypovolemic shock.

Hypertonic electrolyte solutions provide effective fluids for resuscitation of burn patients and concurrent replacement of hemorrhage. Infusion of mannitol has also been advocated as a means of increasing cardiac output in acutely ill patients. Pigs which were bled to one-third of their blood volume were used in this random study of resuscitation with Ringer's lactate solution (RL), hypertonic saline solution (HSL) and RL with mannitol added (HMR) to give the same osmolality as HSL. Smaller volumes of the hyposmolar solution than of RL restored blood pressure and cardiac output. RL and HSL res

Both inhaled histamine and hypertonic saline increase airway reactivity in non-sensitised rabbits.

BACKGROUND: Asthmatics react with bronchoconstriction upon a variety of stimuli, i.e. exercise and hypertonic aerosol challenge. We have previously shown that hyperventilation with dry gas in a rabbit model resulted in a change of the ion content of the tracheal wall. This was followed by a hyperreactive response to histamine. OBJECTIVE: We hypothesised that nebulisation with 3.6% hypertonic saline will be accompanied by a hyperreactive response to histamine in a rabbit model. METHODS: Anaesthetised rabbits were given histamine after nebulisation with hypertonic saline. In addition, repeat n

Duodenal resistance to the emptying of various solutions from the isolated cat stomach.

Experiments were conducted to determine to what extent the duodenum resists the emptying of various solutions from the isolated stomach. Cat stomachs were maintained in Krebs solution in vitro, and filled with 100 ml isotonic saline solution, isotonic glucose, hypertonic saline solution, hypertonic glucose, or dilute acid. Gastric pressure was constantly monitored; gastric output was collected constantly by a cannula in the proximal or distal duodenum. For all solutions, cumulative gastric output at all times was considerably less with the distal than the proximal cannula position. This differ

Characteristics of fluids used for intravascular volume replacement.

In this review, the relative merits of the different fluid solutions used for fluid replacement will be discussed. Differences in chemical composition may have important implications on volume expansion capacities but also on organ function. Among crystalloids, differences in electrolyte composition, and in particular chloride load, alter acid base status but may also affect other aspects of organ function. Large observational cohorts suggest that chloride load may be associated with a poor outcome. Colloids provide greater expansion capacities than crystalloids. When colloids are indicated, a

[Therapy of hypertonic dehydration].

Hypertonic dehydration developed in 30 patients (age ranged from 17 days to 28 months) with acute gastroenteritis. Rehydration was achieved with i.v. solutions, early potassium replacement, and especially with oral solutions (WHO-solution: 90 mval/l Na+ content). The average amount of sodium and fluids was 23.2 mval/kg/d and 270 ml/kg/d respectively to cause an average drop in sodium of 0.6 mval/h. There were no CNS-complications or deaths.
