Rectal anastomosis and hyperthermic intraperitoneal chemotherapy: Should we avoid diverting loop ileostomy?

INTRODUCTION: Literature on rectal anastomosis and diverting ileostomy in patients treated with hyperthermic intraperitoneal chemotherapy (HIPEC) is limited. This study assesses the safety of rectal anastomoses during cytoreductive surgery (CRS) and HIPEC, with and without fecal diversion, and its morbidity when performed. MATERIALS AND METHODS: From January 2012 to January 2020, patients with peritoneal metastases who underwent CRS and HIPEC that required a rectal anastomosis were included in this single-hospital retrospective chart review. RESULTS: 84 patients were included, of which 29 ha

Complete Recovery of Immune Checkpoint Inhibitor-induced Colitis by Diverting Loop Ileostomy.

Checkpoint inhibitor-induced side effects such as diarrhea and colitis occur in up to 30% of patients. We present a case of recurrent episodes of checkpoint inhibitor-induced colitis and subsequent Fournier gangrene that resolved after ileostomy formation. Once the Fournier gangrene and colitis had resolved, the ileostomy was reversed. However, within only 4 days, another serious flare-up of colitis occurred, necessitating emergent re-formation of the ileostomy. Expertise in the management of side effects of immune checkpoint inhibitor therapy is currently limited. Although most side effects a

Laparoscopic diverting loop ileostomy for spontaneous colon perforation in advanced ovarian cancer.

OBJECTIVE: Neoadjuvant chemotherapy for advanced ovarian cancer is associated with reduced morbidity in the elderly (Meyer et al., 2018). Spontaneous colonic perforation often leads to multisystem organ failure and death (Carter and Durfee, 2007; Rose and Piver, 1995). METHODS: A 76-year old woman with stage IIIC disease initiated carboplatin AUC 5 and paclitaxel 175 mg/m2 with unanticipated development of profound neutropenia. She clinically deteriorated by day nine and CT scan revealed a large volume of free air. Emergent surgery was performed. RESULTS: Diagnostic laparoscopy confirmed the

Magnetic Compression Anastomosis (Magnamosis) for Functional Undiversion of Ileostomy in Pediatric Patients.

INTRODUCTION: Magnamosis forms a compression anastomosis using self-aligning magnetic Harrison rings. The device has been approved by the Food and Drug Administration for first-in-human testing and has been applied in adults for intestinal anastomosis during urologic reconstructions. We now report the first cases of magnamosis to functionally undivert the fecal stream from a previously created loop ileostomy in pediatric patients. MATERIALS AND METHODS: Case 1: A 4-year-old male underwent a diverting loop ileostomy for malignant bowel obstruction. The obstruction gradually resolved with chemo

Stimulation of the efferent limb before ileostomy closure: a randomized clinical trial.

BACKGROUND: Postoperative ileus is the most common complication after ileostomy closure with an increase in morbidity, hospital stay, and health care costs. OBJECTIVE: The aim of this study is to assess the utility of a new technique for reducing postoperative ileus after protective ileostomy closure. DESIGN: This is a prospective randomized study registered at ClinicalTrials.gov (NCT01881594). Patients were randomly assigned to undergo either stimulation through the efferent limb of the ileostomy before surgery or nonstimulation before surgery. SETTING: This study was conducted at the Depa

T-tube ileostomy for intestinal perforation in extremely low birth weight neonates.

To evaluate the results of use of T-tube ileostomy in selected cases of intestinal perforation in extremely low birth weight (ELBW) neonates. The records of 288 ELBW neonates treated at author's institution, from 1998 to 2003 were retrospectively reviewed to identify neonates operated for intestinal perforation with T-tube placement. T-tube was inserted into the bowel through the site of perforation or proximally to the perforated gut via separate stab incision. T-tubes were used in five ELBW neonates (BW 600-900 g, gestational age 25-27 weeks) with intestinal perforation, in four of them at t

Management of ketogenic diet therapy in patients with ileostomy: A report of 2 cases.

This case series explores the feasibility and clinical considerations of implementing the ketogenic diet therapy (KDT) for epilepsy management in patients with ileostomies. Two pediatric patients with medication-resistant epilepsy and ileostomies continued KDT under clinical supervision. The first patient had an ileostomy placed while on KDT. Fluids and electrolytes were increased initially, although sodium (Na) levels remained within a normal range. The second patient had an ileostomy before starting KDT. Hyponatremia and acidosis occurred initially. With routine laboratory monitoring, and el

From Surgery to Strength: The Case for Mental Health Integration in Ileostomy Care.

Источник пометил публикацию как отозванную

Ileostomy, a surgical intervention required for conditions such as inflammatory bowel disease (IBD), colorectal cancer, trauma, and sepsis, can be a transformative intervention and imposes substantial physical and psychological challenges on patients. While the physical aspects of ileostomy care are well addressed, the psychological burden-marked by anxiety, depression, and diminished quality of life-remains underrecognized in clinical practice. This perspective article underscores the critical role of mental health in ileostomy recovery, highlighting variations in psychological distress and r

Laparoscopic intracorporeal anastomosis is the future standard for ileostomy reversal in Crohn's disease.

In this editorial, we comment on an article by Liu et al. We focus specifically on the novel use of laparoscopic intracorporeal anastomosis (LIA) compared with open ileostomy reversal in patients with Crohn's disease (CD), which is an innovative approach for improving postoperative outcomes and accelerating recovery in ileostomy reversal procedures. This study also highlights the limitations of the conventional open surgical technique, which is associated with higher rates of feeding intolerance, prolonged hospital stays, and delayed initiation of enteral nutrition. The chronic and recurrent n

Chronic Diseases Related to Diet and/or Nutrition in Patients With an Ileostomy - A Scoping Review.

AIM: Ileostomy formation alters bowel function and is associated with changes that could lead to the development of chronic diseases. The literature on these diseases has not previously been summarised and specific guidelines on their management are absent. This scoping review aimed to explore the extent and type of literature examining such diseases. METHODS: MEDLINE, Embase, CINAHL, AMED, and Web of Science were searched from database inception to 28 April 2025. Studies on chronic diseases (kidney, bone, metabolic disease, and anaemia) related to diet and/or nutrition in adults with any typ

Is Early Reversal of Defunctioning Ileostomy a Shorter, Easier and Less Expensive Operation?

BACKGROUND: A defunctioning loop ileostomy mitigates the consequences of anastomotic leak from low rectal anastomosis but it is associated with significant morbidity. In this study, the outcome of early reversal of defunctioning ileostomy during the same admission with the primary operation was assessed. METHODS: This randomized study was carried out at York Teaching Hospital during the period 2003-2007. All patients with defunctioning ileostomy were considered for an early second operation if they had an uneventful recovery and were in good general condition. Patients on steroids, at high ca

Protective ileostomy in low anterior resection for rectal cancer -- can it be avoided?

The necessity of a protective ileostomy in patients subjected to low anterior resection for rectal cancer has been discussed controversially. This prospective observational study was carried out to see the outcome of low anterior resection without a covering ileostomy. Forty patients underwent low anterior resection for primary rectal carcinoma in mid and distal rectum without any covering ileostomy from January 2007 and June 2010 in the department of Surgery of Bangabandhu Sheikh Mujib Medical University and two other private hospitals in Dhaka city. The primary objective of the study was to

Changes in glucocorticoid and mineralocorticoid hormone levels due to compensation for ileostomy losses.

BACKGROUND: The Ileo-Pouch-Anal-Anastomosis (IPAA) is the standard restorative procedure for Ulcerative Colitis and Familial Adenomatous Polyposis (FAP). IPAA may lead to considerable losses of fluids, especially in association with a protective loop ileostomy. AIM: The aim of this study was to investigate adrenal mechanisms in the regulation of volume homeostasis immediately after IPAA and protective ileostomy. METHODS: For that purpose, 20 patients out of our patient population with elective IPAA with ileostomy participated in this study between 1993 and 1997. In all patients, routine labo

Retrograde colostomy and ileostomy enemas in neonates and infants: a simple combination of techniques.

A combination of previously known techniques for retrograde opacification of ileostomies and colostomies has been used successfully in neonates and infants. This combination of techniques used on very small stomas allows easy retrograde opacification of bowel. The technique uses a small feeding tube, a larger Foley catheter with an inflatable balloon, and a Lucite compression device, the construction and use of which are discussed here.

Ileostomy of the distal end of the bypassed intestine in a patient with jejunoileal bypass for obesity.

Ileostomy of the distal end of the bypassed segment of small intestine was done twenty-three months after a 28 to 20 cm (12 to 8 inch) end-to-end jejunoileal bypass for obesity (Scott operation) in a forty-eight year old white female, thus creating a Thiry fistula. Weight prior to jejunoileal bypass was 130 kg (287 pounds). Before ileostomy it had stabilized at 80.3 kg (177 pounds). Indications for ileostomy were three episodes of blind loop syndrome and three episodes of severe bleeding from the ileotransverse colostomy anastomotic site. Culture of the bypassed segment at laparotomy revealed

A predictive tool for postoperative ileus after ileostomy closure: Model development and validation.

OBJECITVE: Postoperative ileus (POI) is a significant complication after ileostomy closure, which results in recurrent vomiting, dehydration, delay in starting enteral feeding, and even anastomotic breakdown. We aimed to develop a prediction model for POI occurrence after ileostomy closure. MATERIAL AND METHODS: One hundred consecutive patients undergoing ileostomy closure were studied prospectively and data of various demographic and clinical variables were recorded in a predesigned proforma. The final prediction model was developed using logistic regression and internally validated in the n
