Comparison of sternal, iliac, and humeral bone marrow aspiration in Beagle dogs.

BACKGROUND: Sternal bone marrow aspiration in dogs is not commonly performed as it is considered technically challenging in smaller dogs. However, the sternum is readily accessible and associated with less pain from aspiration compared with other sites. OBJECTIVES: The aim of the study was to investigate feasibility, ease, number of attempts, safety, and sample quality of sternal bone marrow aspirates in small dogs. METHODS: Bone marrow aspirates were obtained in a randomized order from 3 sites in 26 clinically healthy Beagles under general anesthesia. Samples were obtained from the sternum

Risks to the superior gluteal neurovascular bundle during percutaneous iliosacral screw insertion: an anatomical cadaver study.

BACKGROUND: Iliosacral screws are a popular technique used to treat complicated injuries of the pelvis. It is well recognized that this technique entails some potentially disabling complications, including damage to vessels and lumbosacral nerves. The recommended insertion site for iliosacral screws into the S1 body lies along the posterior ilium between the greater sciatic notch and the iliac crest. The anatomy and course of the superior gluteal nerve and vessels have been described along the outer aspect of the posterior ilium. Injury to the superior gluteal nerve and vessels has been report

Mandibular reconstruction using microvascular free flaps: a statistical analysis of 178 cases.

For this article, 178 consecutive cases of mandibular reconstruction using microvascular free flaps and performed from 1979 to 1997 were studied. The purpose of this report is to compare flap success rates, complications, and aesthetic and functional results. The ages of the 131 men and 47 women ranged from 13 to 85 years, with an average of 55 years. Donor sites included the rib (11 cases), radius (one case), ilium (36 cases), scapula (51 cases), fibula (34 cases), and soft-tissue flaps with implant (45 cases). Complications included total flap necrosis, partial flap necrosis, major fistula f

Multiple intraosseous cervical pneumatocysts: A case report of a rare incidental finding on cone-beam computed tomography.

This report presents a case of cervical pneumatocysts as an incidental finding on cone-beam computed tomography. Pneumatocysts are gas-containing lesions of unknown etiology. They usually present in the ilium or sacrum, adjacent to the sacroiliac joint. In the literature, 21 case reports have described cervical pneumatocysts. Cervical pneumatocysts should be differentiated from other lesions, such as osteomyelitis, osteonecrosis, and neoplasms, as well as post-traumatic and post-surgical cases. Computed tomography, cone-beam computed tomography, and magnetic resonance imaging are appropriate t

Pheochromocytoma presenting with remote bony recurrence twenty years after initial surgery: detection with 68Ga-DOTANOC PET/CT.

Pheochromocytomas are rare tumors which can be malignant in 10% of cases. We present the case of a 75-year-old woman who presented with headache and palpitation for 1 year. She had a past history of right adrenalectomy for pheochromocytoma 20 years back. In between, the patient was asymptomatic. Twenty-four-hour urinary vanillylmandelic acid was raised. Noncontrast CT and ultrasound of abdomen were unremarkable. The patient underwent 68Ga-DOTANOC PET/CT that showed metastasis to left ilium, which was confirmed on biopsy.

Implantation metastasis in a 13-year-old girl: a case report.

We report a case of a 13-year-old girl with an osteosarcoma of the right humerus, which had been diagnosed as an aneurysmal bone cyst at our institution. She underwent curettage and bone grafting of the lesion, which resulted in implantation metastasis of the tumour to the ilium. She died 15 months after presentation owing to pulmonary metastases. This report highlights the possibility of metastasis occurring by direct implantation to a graft donor site. We strongly recommend that a biopsy be performed in cases of presumed benign lesions before proceeding with the definitive surgery.

Multifocal osteosarcoma with extensive pleural metastatic disease.

An eighteen year old female presented with multifocal synchronous osteosarcoma, with multiple skeletal foci of tumour in long bones, pelvis and spine. The radiographically dominant lesion was in the right ilium, and this was considered to be the primary site. There was an extensive osseous pleural plaque and there were also metastases to lungs, peritoneum, abdominal wall, adrenal gland and lymph nodes. A review of the literature reveals that multifocal osteosarcoma is an uncommon condition and is probably due to metastatic disease and not multiple primary lesions. Pleural metastatic disease in

[Radiation injuries of the bones during treatment of uterine cancer].

The data on 53 cases of bone injuries caused by radiation treatment for cancer of the uterus were analysed. The clinico-roentgenological signs of osteoradionecrosis (33), pathological fracture and dislocation (13), osteomyelitis (5) and radiation-induced sarcoma of bone (2) were considered. Lesions were found to occur most frequently in the upper flaring portion of the ilium and bones of the hip joints. Regular X-ray examinations of the pelvic bones are necessary for early detection of radiation injuries as well as conservative treatment to prevent such grave complications as pathologic fractu

Pelvic radiograph in skeletal dysplasias: An approach.

The bony pelvis is constituted by the ilium, ischium, pubis, and sacrum. The pelvic radiograph is an important component of the skeletal survey performed in suspected skeletal dysplasia. Most of the common skeletal dysplasias have either minor or major radiological abnormalities; hence, knowledge of the normal radiological appearance of bony pelvis is vital for recognizing the early signs of various skeletal dysplasias. This article discusses many common and some uncommon radiological findings on pelvic radiographs along with the specific dysplasia in which they are seen; common differential d

Successful ambulation by surgical conversion in a complex form of epigastric heteropagus.

A female neonate with a complex form of epigastric heteropagus and with three legs was referred to us. The left leg appeared normal, the central one was hypoplastic and resected 1 month after birth, and the right one projected in the cranial direction with knee flexion contracture. Three bones of the parasite were sandwiched between the right ilium and the ischiopubic bones of the autosite. At the age of 4, we performed the subtrochanteric femoral osteotomy with 90 degrees of varization and rotation, along with knee disarticulation. Ambulation was successfully achieved with the prosthesis with

False-positive Meckel's imaging and true-positive imaging of a gastrointestinal bleed and surgical lesion.

A 76-year-old woman presented after 1 day of melena and anemia. A Meckel's scan revealed an upper mid abdominal focus of activity that paralleled stomach activity. Later, the surgeon removed the corresponding lesion in the mid ilium. Pathology confirmed that the lesion was a carcinoid tumor, not ectopic gastric mucosa. The false-positive Meckel's scan had revealed the true source of gastrointestinal bleeding and an unsuspected solitary carcinoid tumor. This is the third published case of a carcinoid tumor discovered by a false-positive Meckel's scan.

The Spitzy shelf operation for the dysplastic hip. Retrospective 10 (5-25) year study of 124 cases.

Since 1961 we have used the Spitzy shelf operation combined with a vertical flap osteotomy of the outer cortex of the ilium for early arthrosis secondary to acetabular dysplasia, subluxation, and dislocation. In 124 hips, followed for 10 (5-25) years, clinical improvement was maintained in 67/85 of patients who were operated on under the age of 30 and in 22/39 of patients treated after 30. However, less than half of the hips in the younger group and only one out of ten in the older group were symptom-free.

[Chondrosarcoma of the vertebral column L5--S1].

A case of chondrosarcoma of the spine in a 15-year-old female patient has been described. The tumor was localized within the body of S1 and it partially involved the left wing of the ilium. The radiological condition corresponded with the so-called "ivory vertebra". The symptoms of compression of the cauda equina took the lead. Surgical decompression of the cauda equina was performed and an attempt of resection of the tumor was undertaken but it failed because of extension of the process. The further course of the disease, in spite of radiotherapy, was exceptionally malignant and within severa

Evaluation of the Interrater Reliability of Sonographic Measurements of Muscle Thickness of 38 Piriformis Muscles in 19 Patients with Piriformis Syndrome.

BACKGROUND The piriformis muscle is a flat superficial muscle of the deep gluteal muscles that externally rotates the hip. Ultrasound is widely used to identify the piriformis muscle, especially for guidance of the needle during injections; however, its diagnostic use has recently gained popularity. The operator-dependent nature of ultrasound requires demonstration of reliability between operators. This study aimed to evaluate interrater reliability of sonographic measurements of muscle thickness of 38 piriformis muscles in 19 patients with piriformis syndrome. MATERIAL AND METHODS An ultrasou

Bilateral dual iliac screws in spinal deformity correction surgery.

BACKGROUND: Surgery for adult spinal deformity requires optimal patient-specific spino-pelvic-lower extremity alignment. Distal fixation in thoracolumbar spinal deformity surgery is crucial when arthrodesis to the sacrum is indicated. Although we had performed sacro-pelvic fixation with bilateral S1 and bilateral single iliac screws previously, iliac screw loosening and/or S1 screw loosening occurred frequently. So, the authors attempted to fuse spino-pelvic lesions with the dual iliac screws and S1 pedicle screws. METHODS: Twenty-seven consecutive adult spinal deformity patients underwent th

Mechanism matters: differences in injury patterns and outcomes between ballistic and non-ballistic iliac wing fractures.

PURPOSE: Iliac wing fractures are uncommon injuries that often result from high-energy or ballistic trauma. There are limited studies assessing injury profiles of ballistic and non-ballistic iliac wing fractures. The primary aim of this study was to evaluate injury-related characteristics and complications in patients who sustained ballistic versus non-ballistic iliac wing fractures, with the hypothesis that ballistic fractures would have higher rates of concomitant injuries and complications. METHODS: This study performed a retrospective multicenter review of patients presenting iliac wing f

Total Hip Arthroplasty for Treatment of Pseudoarthrosis of the Hip Following Slipped Capital Femoral Epiphysis: A Case Report.

CASE: A 22-year-old man with worsening hip pain was found to have chronic complete slippage of a previously pinned slipped capital femoral epiphysis (SCFE) and subsequent pseudoarthrosis between the femur and ilium. The deformity was successfully treated with total hip replacement. We report 13 months of postoperative follow-up with adequate restoration of leg length and symptomatic relief. CONCLUSION: This rare case demonstrates the importance of careful preoperative planning to ensure appropriate alignment of rotation of the hip and restoration of leg length in the setting of a chronic SCFE

The pelvic digit anomaly in a patient with multiple fractures: does it mimic the fracture?

Pelvic digit is a rare congenital anomaly where bone develops in the soft tissue adjacent to normal skeletal bone. Pelvic digits are most often associated with the ilium but may also pseudoarticulate with other pelvic bones or the abdominal wall. Its importance lies in its differentiation from acquired abnormalities due to trauma such as myositis ossificans and avulsion injuries of pelvis. In this article, we present a case of pelvic digit with multiple fractures. To avoid unnecessary investigation methods and treatment, this entity should be kept inmindwhen an atypical bone structure is noted

Iliac wing insufficiency fractures as unusual postoperative complication following total hip arthroplasty - a case report.

Insufficiency fractures present a significant problem in patients with osteoporosis. We report a case of bilateral iliac wing insufficiency fracture following low energy injury in an 87-year-old osteoporotic woman occurring 2 weeks after primary total hip arthroplasty. There are only a few reports of insufficiency fractures involving the ilium in the literature, and diagnosis has proven challenging, as radiographs are often negative at symptom onset. Magnetic resonance or radionuclide imaging is generally necessary for definitive diagnosis. This case highlights the importance of careful periop

The problem of stabilization after sacrectomy.

After sacrectomy, mobilization of the patient is only possible if a stable connection between the spine and pelvis can be obtained. We have developed an instrumentation to fix the pelvis to the spine. Two DHS screws connected to each other were implanted in the pelvis (one DHS screw into each ilium). An internal spine fixator, anchored in L3 and L4 through transpedicular Schanz screws, was attached to these DHS screws. Two patients were stabilized with this implant after sacrectomy. One patient was able to walk with crutches; the other patient was able to walk even without crutches.
