CD30 expression is frequently decreased in relapsed classic Hodgkin lymphoma after anti-CD30 CAR T-cell therapy.

Chimeric antigen receptor (CAR) T-cells anti-CD30 is an innovative therapeutic option that has been used to treat cases of refractory/relapsed (R/R) classic Hodgkin lymphoma (CHL). Limited data are available regarding the CD30 expression status of patients who relapsed after this therapy. This is the first study to show decreased CD30 expression in R/R CHL in patients (n = 5) who underwent CAR T-cell therapy in our institution between 2018 and 2022. Although conventional immunohistochemical assays showed decreased CD30 expression in neoplastic cells in all cases (8/8) the tyramide amplificatio

[Management of mycosis fungoide : focus on brentuximab vedotin].

Recently, brentuximab vedotin (BV) (Adcetris®) obtained the reimbursement in Belgium for the treatment of the primary cutaneous NKT-cell lymphomas mycosis fungoides (MF), large cell anaplastic lymphoma and lymphomatoid papulosis type A. BV is a monoclonal antibody directed against the CD30 expressed on tumoral T cells. The inhibition of this pathway releases the process of apoptosis leading to the cell death of the tumoral cells. BV is reimbursed after the use of another systemic treatment without success and if the number of CD30 positive atypical T-cells is larger than 10 %. BV is administer

Antibody-Drug Conjugates: A Review on the Epitome of Targeted Anti- Cancer Therapy.

Cancer is one of the deadly diseases which is characterized by unchecked cell division or abnormal cell growth due to the incapability of cell cycle arrest. As the treatment for this is to kill the cancerous cells, the main challenge for scientists is to direct the cell killing to cancerous cells while leaving the normal cells unharmed. Antibody-Drug Conjugates (ADC) are one such targeted anti-cancer therapy. It is an effective drug delivery system that utilizes the targeting action of antibody along with cell death by potent cytotoxic agent, linked up with one another by a linker molecule and

Preparation and immunological characterization of β-lactoglobulin-amylose conjugate.

β-Lactoglobulin (BLG), a major allergen of cow's milk, was conjugated with the N-hydroxysuccinimide ester of the amylose-glycylglycine adduct (AG-ONSu) to reduce its immunogenicity, and the biochemical and immunological properties of the resulting conjugate (AG-BLG) were studied. The conjugate was prepared by modifying BLG with AG-ONSu, and was purified in a Sephadex G-100 column. The analytical data for AG-BLG indicated that 10.5 moles of AG-ONSu, with a mean molecular weight of 2,800, was covalently attached to the amino groups of the BLG molecule. Conjugation with AG-ONSu greatly decreased

Ultrasound-Triggered Site-Specific Chemistry Redefines Precision Oncology.

Ultrasound-triggered site-specific molecular chemistry has emerged as a transformative strategy for cancer therapy, yet precise spatiotemporal control of their activity remains a critical bottleneck. Endogenous stimulus-responsive strategies are hampered by tumor heterogeneity, while exogenous physical triggers provide superior controllability. This article proposes that ultrasound-triggered site-specific chemistry, termed "acoustic programming", offers a transformative solution. By integrating focused ultrasound deep-tissue penetration with molecular-level chemical control, acoustically progr

From Dual Targeting to Clinical Validation: A Translational Framework for Bispecific Antibody-Drug Conjugates.

Bispecific antibody-drug conjugates (bsADCs) are being developed to address limitations of conventional ADCs, including heterogeneous antigen expression, insufficient tumor-normal discrimination, and suboptimal intracellular trafficking. This review evaluates bsADCs from a preclinical-to-clinical translational perspective and defines the evidence needed to justify the added complexity of bispecific design. A structured search of PubMed/MEDLINE, Embase, Web of Science, Scopus, clinical-trial registries, and major oncology conference proceedings was conducted through 29 July 2026. Dual-antigen b

Antibody-Drug Conjugates in Breast Cancer: Therapeutic-Window Engineering, Tumor Heterogeneity, and Immunotherapy Combinations.

Antibody-drug conjugates (ADCs) have become a major therapeutic class in breast cancer, but their clinical performance cannot be predicted by antigen expression alone. This review uses therapeutic-window engineering as a unifying framework to connect ADC molecular architecture with tumor heterogeneity, clinical positioning, resistance, immune effects, and toxicity. We examine how target selection, antibody properties, linker stability, payload mechanism and membrane permeability, drug-to-antibody ratio (DAR), conjugation homogeneity, and intratumoral delivery jointly determine efficacy and saf

Monoclonal Antibodies and Derivatives: Therapeutic Tools for Cancer.

The production of murine monoclonal antibodies (mAbs) with defined specificity in 1975 marked the subsequent revolution of cancer therapy. mAbs have been essential to characterize the functional features of molecules involved in cancer cell growth and dissemination. The murine mAbs have been modified to create humanized antibodies and, subsequently, fully human antibodies for cancer therapy, thereby avoiding the side effects of xenogenic protein. The antibody-drug conjugates (ADCs) increased the antitumor effect of mAbs. We will analyze the functional features of ADCs that recognize the cluste

Engineering the subcutaneous delivery of antibody-drug conjugates.

Subcutaneous (SC) delivery has evolved beyond a convenience formulation to become an operational re-architecture of cancer care, with the potential to convert infusion-centered oncology into a distributed model without compromising exposure, safety, or product quality. Monoclonal antibodies have already completed this transition, establishing that the SC route delivers clinical equivalence while improving patient experience and system efficiency. Antibody-drug conjugates (ADCs) represent the next frontier, yet they remain almost exclusively intravenous (IV). This gap reflects not an incompatib

Alternating iterative exponential growth oligomers as a sequence-defined platform for hydrophilic antibody-drug conjugates.

Antibody-drug conjugates (ADCs) are a powerful therapeutic modality capable of tissue-specific delivery of highly potent cytotoxic warheads. One of the key challenges of ADCs is their hydrophobicity. Conjugation of typically hydrophobic cytotoxins to an antibody alters its physicochemical properties, resulting in decreased stability and poor pharmacokinetics. Herein, we report the synthesis of alternating iterative exponential growth (IEG) oligomers bearing orthogonal functionalities that serve as a versatile platform for installing diverse hydrophilic moieties and cytotoxic payloads. Incorpor

Emerging antibody-drug conjugates in metastatic castration-resistant prostate cancer.

Despite recent advances in therapies, metastatic castration-resistant prostate cancer (mCRPC) remains incurable with poor prognosis in which novel, more effective treatments are urgently needed. Antibody-drug conjugates (ADCs) are a novel class of agents comprised of a monoclonal antibody directed against a cell surface target of interest that delivers a cytotoxic payload through a cleavable linker. While they have achieved tremendous success in other genitourinary cancers such as urothelial cancer, ADCs have promising activity in mCRPC. In this review, we will provide a high-level overview of

Overcoming resistance to antibody-drug conjugates: mechanisms and emerging strategies.

BACKGROUND: Antibody-drug conjugates (ADCs) combine a tumor antigen-specific monoclonal antibody with a cytotoxic payload via a linker, enabling selective delivery of cytotoxic agents to cancer cells while minimizing damage to healthy tissues. This approach has revolutionized cancer treatment; however, despite these advantages, resistance to ADCs has emerged as a significant barrier to long-term efficacy. METHODS: In this review, we summarize and analyze molecular pathways implicated in ADC resistance across multiple cancer types, including triple-negative breast cancer, non-small cell lung c

Refractory Diabetic Ketoacidosis Associated with Enfortumab Vedotin: A Pharmacokinetic-Based Management Approach.

Enfortumab vedotin (EV), an antibody-drug conjugate approved for metastatic urothelial carcinoma, is increasingly recognized to cause severe hyperglycemia and diabetic ketoacidosis (DKA). However, optimal management of EV-associated refractory DKA remains undefined. We describe a 57-year-old man receiving EV and pembrolizumab who developed profound insulin-resistant DKA that persisted despite standard therapy. EV composed of a nectin-4-targeted monoclonal antibody linked to the cytotoxic agent monomethyl auristatin E (MMAE), which is the primary cause of systemic toxicity. Systemic elimination
