A patient with Waldenström's macroglobulinaemia presenting with numerous red or brownish-red, round, papulous lesions of the skin is reported. Skin biopsy revealed a massive infiltrate of immature lymphatic cells of the dermis down to the subcutaneous fat. Approximately 80% of these cells were positive for intracellular IgM and kappa light chains as shown by the immunoperoxidase method. This finding established the skin lesions as part of Waldenström's macroglobulinaemia.

We hypothesized that the presence of monoclonal free light chains (FLC) in the serum of patients with monoclonal gammopathy of undetermined significance (MGUS) is a marker of clonal evolution and a risk factor for progression. BACKGROUND: Light chain multiple myeloma (LCMM) is a malignant hematological disease characterized by bone marrow infiltration by tumor plasma cells and the secretion of monoclonal free light chains (κ or λ). A 59-year-old woman with goiter complained of nausea, vomiting and weight loss in April 2000. She underwent an endoscopic examination and was admitted to our hospital because gastric biopsy specimens revealed that she had diffuse large B-cell lymphoma. We report here the youngest known IgM myeloma patient to have presented with a mandibular mass. A 22-year-old Chinese man sought medical attention due to a mass over his right mandible that had been growing progressively for 6 months. A solitary osteolytic lesion in the right mandible was identified radiologically. The peripheral blood and bone marrow mononuclear cell immunophenotype was investigated in series of 41 patients with plasma cell dyscrasias (29 with multiple myeloma and 12 with monoclonal gammopathy of undetermined significance (MGUS) or solitary plasmacytoma). A well established case of von Recklinghausen's neurofibromatosis, in which a biclonal gammopathy developed, has been studied. One of the paraproteins was characterized as IgG kappa, the other as IgG lambda. This combination of anomalies has not been described previously in the literature.
