AAV2retro Enters Axons of Passage and Extensively Transduces Corticospinal Neurons After Injection into Spinal White Matter.

BACKGROUND: Adult neurons in the central nervous system often fail to regenerate after spinal cord injury (SCI). Regenerative gene therapies could potentially promote corticospinal axon regeneration, restoration of motor circuitry, and functional improvement after SCI, but translational methods for targeted gene delivery to corticospinal neurons are needed. AAV2retro is an engineered variant of the adeno-associated virus 2 (AAV2) capsid that demonstrates greatly enhanced retrograde transduction of projection neurons. When injected into spinal gray matter, AAV2retro retrogradely transduces neur

Spinal cord injury in a 5 month old cat after single-shot lumbosacral epidural injection.

This report presents the anatomical and histopathological findings observed in a 5 month old Domestic Short Hair cat euthanized owing to long-lasting pelvic limb paralysis. Paralysis occurred after a lumbosacral epidural injection for perioperative pain management during femoral fracture repair. A lumbosacral epidural injection was performed according to established veterinary recommendations, with no immediate complications noted during surgery. On postoperative examinations, the cat had severe pelvic limb neurological deficits, which did not improve over the 6 week follow-up period. Necropsy

Injectable approaches to deliver innovative treatments for spinal cord: When regenerative medicine meets precision medicine.

Delivering therapeutic agents to the spinal cord is crucial for treating conditions such as Spinal Cord Injury (SCI), chronic pain and neurodegenerative diseases. Various administration routes, including systemic, epidural, intrathecal, subarachnoid, localized and intraparenchymal injections, offer distinct advantages and challenges when it comes to achieving effective therapeutic outcomes. The invasiveness, precision and ability to bypass biological barriers, such as the Blood-Spinal Cord Barrier (BSCB), differ between these methods. This literature review summarizes the latest research on th

Percutaneous Aspiration Injection and Re-aspiration as A Minimally Invasive Treatment for Spinal Cystic Echinococcosis: A Case Report.

Cystic echinococcosis is a parasitic disease with significant importance for public health in endemic regions. Spinal cystic echinococcosis, however, is a rare form that may lead to severe complications due to its localization. In this manuscript, we presented a 16-year-old male patient who admitted with abdominal and back edema for 2 months, evaluated with preliminary diagnoses of Pott's abscess and malignant mass, subsequently diagnosed with spinal cystic echinococcosis. It was concluded that cystic echinococcosis should be considered in differential diagnosis of large cystic masses and perc

CSF - injected contrast medium enhances post-traumatic spinal cord cysts. An experimental study in rats.

OBJECTIVE: Spinal cord injury (SCI) is still one of the most challenging problems in neurosurgical practice. One of the major obstacles to neural regeneration following trauma is the formation of glial scarring and post-traumatic cysts which acts against proper growth of axons through the site of injury. Cerebrospinal fluid (CSF) delivery of bioactive agents into cystic cavities could represent a promising therapeutic strategy. In the present study, we investigated specifically the dynamics of intradural delivery of contrast medium and its relocation into post-traumatic cysts in an experimenta

The utility of magnetic resonance imaging results in physician decision-making before initial lumbar spinal injection.

BACKGROUND CONTEXT: The need for advanced imaging before spinal intervention is an area of ongoing debate. Many studies have demonstrated the accuracy of magnetic resonance imaging (MRI) results in evaluating structural pathology in the lumbar spine, but few have addressed how frequently MRI findings change clinical management. A randomized controlled trial showed that viewing MRI results did not impact outcomes in patients with radiculopathy undergoing epidural steroid injection (ESI). The results suggested ESIs that correlated with both imaging and clinical findings experienced slightly more

Spinal Cord Injury by Direct Damage During CT-Guided C7 Transforaminal Epidural Steroid Injection.

Cervical transforaminal epidural steroid injection (TFESI) under the guidance of computed tomography (CT) can offer great anatomical resolution and precise needle placement in the axial plane. However, some complications, including blood pressure surge, allergic reactions, vasovagal syncope, and cerebral infarct, have been reported after CT-guided cervical TFESI. We report the first case of iatrogenic spinal cord injury during a CT-guided cervical TFESI. A 67-year-old woman, upon receiving TFESI on Lt. C7, experienced an electrical shock-like sensation throughout the body. The patient complain

Spinal Cord Infarction after Cervical Transforaminal Epidural Steroid Injection: Case Report and Literature Review.

INTRODUCTION: Transforaminal epidural steroid injection (TFESI) is a widely used nonsurgical procedure in the treatment of patients with radiculopathy. It is efficacious in relieving pain, but a number of complications are being reported. Recently, increasing frequency of major complications, such as spinal cord infarction and cerebral infarction, has been reported with the use of a particulate steroid within fluoroscopic-guided procedures. METHODS: We report a 49-year-old man with a history of chronic cervical radiculopathy, who experienced a devastating complication after TFESI. RESULTS: A

Acute Paraplegia After Lumbar Steroid Injection in Patients With Spinal Dural Arteriovenous Fistulas: Case Reports.

Spinal dural arteriovenous fistulas (SDAVFs) are the most common type of spinal vascular malformations. However, SDAVFs are still underdiagnosed entities because their clinical symptoms are usually non-specific, as they include low back pain or radiating pain to the limb. There have been several reports of acute paraplegia after lumbar epidural steroid injections in patients with SDAVFs. We present 4 patients with SDAVFs who received lumbar steroid injection. Among the 4 cases, acute paraplegia developed in 2 cases that received a larger volume of injectate than the other cases. Thus, we are s

Single-operator real-time ultrasound-guided spinal injection using SonixGPS™: a case series.

PURPOSE: The SonixGPS™ is a novel needle tracking system that has recently been approved in Canada for ultrasound-guided needle interventions. It allows optimization of needle-beam alignment by providing a real-time display of current and predicted needle tip position. Currently, there is limited evidence on the effectiveness of this technique for performance of real-time spinal anesthesia. This case series reports performance of the SonixGPS system for real-time ultrasound-guided spinal anesthesia in elective patients scheduled for joint arthroplasty. METHODS: In this single-centre case seri

Chronic myelopathy due to a giant spinal arachnoid cyst: a complication of the intrathecal injection of phenol. Case report.

Acquired intradural arachnoid cystic lesions of the spine have been associated with trauma, hemorrhage, parasitic infections, and other insults that cause inflammation and subarachnoid adhesions. The authors describe the case of a previously healthy 36-year-old woman who presented with a chronic myelopathy due to the progressive development of a giant spinal arachnoid cyst that resulted after the intrathecal injection of phenol for the management of chronic upper extremity pain. Neurological examination, spinal computed tomography, and magnetic resonance imaging were used for diagnostic and fo

Distribution of morphine in brain regions, spinal cord and serum following intravenous injection to morphine tolerant rats.

In order to determine the possible contribution of altered distribution of morphine in the morphine tolerance process, the distribution of morphine was studied in brain regions and spinal cord, following its intravenous administration. Male Sprague-Dawley rats were made tolerant to morphine by implanting 6 morphine pellets, each containing 75 mg of morphine base, for 7 days. Seventy-two hours after the removal of the pellets, a time when serum morphine levels were negligible or absent and yet tolerance to the pharmacological effects of morphine was present, morphine (10 mg/kg, i.v.) was inject

Depressor and bradycardic effects induced by spinal subarachnoid injection of D-Ala2-D-Leu5-enkephalin in rats.

The effects of D-Ala2-D-Leu5-enkephalin (DADLE), a specific delta receptor agonist, on spinal control of cardiovascular function, were investigated by its intrathecal (i.th) injection into the spinal subarachnoid space at the T-9 level. In chloralose-anesthetized rats, DADLE (17.5, 35 and 70 nmol, i.th) caused dose-dependent hypotension and bradycardia. The mean maximal hypotension by 70 nmol of DADLE was -45 +/- 7 mmHg, with a bradycardia of -79 +/- 15 beats/min. These inhibitory cardiovascular effects were antagonized by the opiate antagonist naloxone (50 nmol, i.th.) given prior to DADLE. I

[Neurofilament hypertrophy induced in the rabbit spinal cord after intracisternal injection of aluminum chloride (author's transl)].

The cytological changes induced in the spinal cords of rabbits by intracisternal injections of aluminum chloride have been studied at the light- and electron-microscopic level for examining the progress of the 100 A filament hypertrophy. A moderate increase in the number of the 100 A filament was detected in perikarya of neurones in anterior, lateral, and dorsal horns 4--5 days after aluminum injection. Tangles of 100 A filament were seen in perikarya and proximal parts of dendrites in these neurones 1--2 weeks after the administration of aluminum. In this stage the tangles were detected as la

The Efficacy and Factors Associated With Epidural Injections in the Management of Lumbar Spinal Stenosis and in Terms of Delaying Laminectomy: A Retrospective Study.

BACKGROUND: Lumbar epidural injections have been studied as symptomatic treatments for lumbar spinal stenosis (LSS). However, results about their efficacy have been controversial, and data regarding their use is scarce. Our purpose in this article is to study the efficacy of epidural injections in the management of pain and disability in patients suffering from spinal stenosis, to study the factors which can affect their efficacy, and to discuss whether they could replace surgery or not. METHODS: A retrospective study between 2021 and 2022 took place in a Pain Clinic located in Notre-Dame des

The Efficacy and Safety of Intrasphincteric Botulinum Toxin-A Injections in Patients with Non-Spinal Cord Injury-Related Detrusor Sphincter Dyssynergia: A Retrospective Study.

Botulinum toxin-A (BoNT-A) injections into the external urethral sphincter are an established therapeutic procedure for reducing bladder outlet obstruction in patients with detrusor sphincter dyssynergia (DSD) due to spinal cord injury (SCI). Given the paucity of data on patients with DSD but without SCI, we aimed to assess the efficacy of intrasphincteric BoNT-A injections in this cohort. For this retrospective study, we screened all patients who underwent their first intrasphincteric BoNT-A injection at our institution between 2015 and 2021. The inclusion criteria were patients aged 18 years

Multiplicative interaction between narcotic agonisms expressed at spinal and supraspinal sites of antinociceptive action as revealed by concurrent intrathecal and intracerebroventricular injections of morphine.

In rats in which the fourth ventricular exits had been acutely occluded, morphine sulfate was injected concomitantly into the spinal subarachnoid space (0-10 micrograms/4 microliter) and into the third cerebral ventricle (0-50 micrograms/5 microliter). The combinations of intrathecal (i.t.) and intracerebroventricular (i.v.t.) dosages used were selected to yield particular ratios of supraspinal to spinal (SS:S) agonisms. Dose-response lines for both the tail-flick and hot plate responses were constructed for each SS:S ratio, with the abscissa representing i.v.t. morphine dosage. It was observe
