Comparison between walking test and treadmill test for intermittent claudication associated with lumbar spinal canal stenosis.

PURPOSE: To clarify the priorities of the walking test and the treadmill test for intermittent claudication of lumbar canal stenosis. METHODS: The study population comprised 45 subjects, with a mean age of 72.6 years. An investigator walked with the subjects during the walking test or watched the subjects walking on the treadmill machine in the treadmill test. RESULTS: The pain scales became significantly worse after the walking test. Ten patients who were diagnosed as root symptom type or cauda equine symptoms were subsequently diagnosed as mixed type by the walking test. The numbers of pat

Insertion loads of the X STOP interspinous process distraction system designed to treat neurogenic intermittent claudication.

An interspinous process implant has been developed to treat patients suffering from neurogenic intermittent claudication secondary to lumbar spinal stenosis. As most patients who suffer from spinal stenosis are over the age of 50 and may have weaker bones, it is imperative to know how bone mineral density (BMD) correlates with lateral spinous process strength. The study was undertaken to characterize the lateral failure loads of the spinous process, correlate the failure loads to BMD, and compare the failure loads to the loads required to insert an interspinous process implant. Spinous process

Supervised exercise-based rehabilitation for people with intermittent claudication-Study protocol for a Danish implementation process (StRiDE).

INTRODUCTION: Intermittent claudication is a peripheral artery disease caused by arteriosclerosis. People with intermittent claudication experience leg cramping during walking, with relief of symptoms during rest. Evidence shows that by participating in supervised exercise therapy and smoking cessation programs, people with intermittent claudication can reduce those symptoms and improve health-related quality of life and maximal walking distance while minimizing the need for an operation. However, implementation of such health-promoting initiatives in clinical practice in Denmark and other cou

Extracorporeal shock wave therapy for peripheral arterial disease-related intermittent claudication: a systematic review and meta-analysis.

BACKGROUND: Intermittent claudication (IC) is a common manifestation of peripheral arterial disease. This study evaluates the efficacy of extracorporeal shock wave therapy (ESWT), a non-invasive treatment, for patients with IC. METHODS: A systematic review and meta-analysis of four major databases (PubMed, Embase, Cochrane Library, Web of Science) was conducted through March 2025. Eight studies met inclusion criteria for the analysis, which was performed using Review Manager 5.4 with evidence strength assessed by the GRADE system. RESULTS: The analysis included eight studies with a total of

Long-Term Outcomes of Invasive vs Noninvasive Treatment for Intermittent Claudication: A Systematic Review and Meta-Analysis.

BACKGROUND: Intermittent claudication (IC) is a hallmark symptom of peripheral arterial disease (PAD), causing pain and discomfort during physical activity caused by reduced blood flow to the lower extremities. The condition significantly impairs mobility and quality of life (QoL) in affected individuals. Treatment options for IC range from conservative approaches, including best medical therapy (BMT) and supervised exercise therapy (SET), to invasive interventions like angioplasty and open re-vascularization. AIM: This meta-analysis and systematic review seek to assess the long-term results

Walking training decreases cardiovascular load after walking to maximal intermittent claudication in patients with peripheral artery disease.

PURPOSE: This study investigated whether: 1) walking training (WT) changes cardiovascular load after walking until maximal intermittent claudication (IC) in patients with peripheral artery disease (PAD); and 2) acute and chronic cardiovascular responses to walking were related to each other. METHODS: A randomized, controlled, parallel-group design was employed. Thirty-two men with PAD and IC were randomly assigned to one of two groups: WT (n=16) or control (CO, n=16) twice a week for 12 weeks. The WT group completed 15, 2-min bouts of walking at moderate intensity interspersed with 2-min upri

Heel Raises and Calf Stretches Exercises Versus Medication Only in Ischemic Intermittent Claudication: A Randomized Controlled Trial.

Intermittent claudication is a primary symptom of peripheral artery disease (PAD). a chronic progressive disease caused primarily by atherosclerosis. It is usually characterized by leg pain, aches, cramps, or fatigue when walking, which improves with rest. Physical therapy, including a supervised exercise program, is often recommended as the first treatment for sprains. This study aims to evaluate the short-term effects of incorporating heel raise and calf stretch exercises with standard medical therapy compared to medical therapy alone in managing intermittent claudication. From May 2022 to N

Objective Evaluation of Neurogenic Intermittent Claudication for Patients With Lumbar Spinal Stenosis Based on Plantar Pressure Analysis.

STUDY DESIGN: A cross-sectional study. OBJECTIVE: To quantify the severity of neurogenic intermittent claudication (NIC) for patients with lumbar spinal stenosis (LSS) based on the center of pressure trajectory. SUMMARY OF BACKGROUND DATA: NIC is one of the typical symptoms of LSS. So far, the severity level of NIC is mainly evaluated by the subjective description of patients, which might be biased by patients' background differences and thus lead to an ineffective diagnosis or inappropriate treatment for LSS. Therefore, it remains necessary to develop a reliable clinical technique for quant

Relationship Between Cauda Equina Conduction Time and Type of Neurogenic Intermittent Claudication due to Lumbar Spinal Stenosis.

INTRODUCTION: This study investigated whether the prolongation of the cauda equina conduction time (CECT) was related to the type of neurogenic intermittent claudication due to lumbar spinal stenosis. METHODS: In total, 149 patients who underwent surgery due to lumbar spinal stenosis with neurogenic intermittent claudication were classified into three groups as follows: cauda equina-type(n = 67), radicular-type(n = 29), and mixed-type(n = 53). Cauda equina conduction time was measured by placing disc electrodes on the abductor hallucis muscle, electrically stimulating the tibial nerve of the

Retrospective Cohort Study Analysing Response to Supervised Exercise Therapy and Subsequent Revascularization in Patients with Intermittent Claudication.

Background: All major international and national guidelines recommend supervised exercise therapy (SET) for intermittent claudication (IC) as a first line of treatment, with revascularisation options to be considered for those who do not respond. Revascularisation incurs complication risks and additional costs; therefore, the need to correctly identify individuals who potentially may progress to revascularisation following SET would be of benefit. This retrospective cohort study aimed to review responses and subsequent revascularisation for individuals with IC following completion of SET. Meth

Association of elective peripheral vascular intervention with outcomes among patients with peripheral arterial disease and intermittent claudication.

BACKGROUND: Peripheral vascular intervention (PVI) is increasingly used for the treatment of peripheral arterial disease (PAD) with intermittent claudication (IC). However, large, real-world comparative studies of the safety and effectiveness of PVI compared with no PVI are limited. We sought to compare the effectiveness and costs of elective PVI compared with no PVI among patients with PAD and IC. METHODS: We conducted a 1:1 propensity-matched retrospective cohort analysis of commercially insured and Medicare Advantage patients in OptumLabs Data Warehouse from January 1, 2016, to September 3

Markers of optimal medical therapy are associated with improved limb outcomes after elective revascularization for intermittent claudication.

BACKGROUND: Optimal medical therapy (OMT) is a modifiable factor that decreases mortality and cardiovascular events in patients with severe peripheral arterial disease. We hypothesized that preintervention OMT would be associated with improved 1-year reintervention and major adverse limb event (MALE) rates after elective endovascular revascularization for intermittent claudication (IC). METHODS: Using the Vascular Quality Initiative (2010-2020), we identified patients with IC undergoing elective endovascular, hybrid, and open surgical interventions. Preoperative antiplatelet, statin, and nons

Blood flow restricted walking in patients suffering from intermittent claudication: a case series feasibility and safety study.

UNLABELLED: To examine the feasibility and safety of blood flow restricted walking (BFR-W) in patients with intermittent claudication (IC). Moreover, to evaluate changes in objective performance-based and self-reported functioning following 12 weeks of BFR-W. MATERIALS AND METHODS: Sixteen patients with IC were recruited from two departments of vascular surgery. The BFR-W programme implied the application of a pneumatic cuff around the proximal part of the affected limb at 60% limb occlusion pressure in five intervals of 2 min, four times per week for 12 weeks. Feasibility was evaluated by ad
