Therapists' Role in Patient Adherence to Internet-Based Cognitive Behavioral Therapy: Qualitative Study.

BACKGROUND: Internet-based cognitive behavioral therapies (iCBTs) are typically categorized into 2 types: therapist-assisted and self-guided. Both formats have accumulated substantial evidence supporting their cost-effectiveness and efficacy in treating a range of mental health conditions. However, therapist-assisted iCBTs tend to show lower dropout rates than self-guided versions. The relatively high dropout rates in self-guided programs suggest that some degree of therapist involvement may be necessary to improve engagement and treatment adherence. Yet, the specific reasons for therapist sup

A Freely Accessible, Anonymous Online Treatment for Social Anxiety: Protocol for a Partially Randomized Patient Preference Trial.

BACKGROUND: Many people with mental health problems do not receive the care they need. Digital mental health interventions have been shown to be effective in many trials and offer a promising way to reach more people in need. However, their uptake and use remain limited, in part due to concerns about data privacy. However, these concerns may not be equally significant for all users. OBJECTIVE: This trial aims to evaluate the efficacy of an anonymous online self-help tool for social anxiety that processes no personal data and has some disadvantages in terms of usability compared to an otherwis

Effectiveness of Internet-Based Personalized Normative Feedback Among Individuals Experiencing Problem Gambling: Randomized Controlled Trial.

This study aimed to examine whether fully automated and Internet-based personalized normative feedback (GAMCHECK) improves problem gambling symptoms and behaviors. We used a randomized parallel-group superiority design to examine whether GAMCHECK would be more effective than an assessment-only (AO) condition at 12-week follow-up. This study was conducted online using questionnaires and the LINE app. Participants were recruited through online surveys. All outcomes were assessed using Internet-based questionnaires. We used a linear mixed model to assess the effects of GAMCHECK on scores for the

Feasibility and efficacy of an online compassion-focused imagery intervention for veterinarian self-reassurance, self-criticism and perfectionism.

BACKGROUND: Veterinarians report high levels of psychological distress and self-criticism. However, there is minimal research investigating psychological interventions for veterinarians. Evidence suggests that compassion-focused therapy is effective at reducing distress in those with high self-criticism. This study aimed to investigate the feasibility and preliminary effectiveness of a 2-week online compassionate imagery intervention for veterinarians. METHODS: A one-group repeated measures design was used with 128 veterinarians. Participants completed measures of perfectionism, self-criticis

Examining the 4-D Model in Persons Enrolled in the Top DD Internet Intervention.

The 4-dimensional (4-D) model of trauma-related dissociation differentiates between dissociative experiences involving trauma-related altered states of consciousness and symptoms of distress that do not appear to involve alterations in normal waking consciousness across four phenomenological dimensions (i.e., our experience of time, thought, body, and emotions). The current study evaluated hypotheses associated with the 4-D model using analyses of variance and correlation analyses in individuals with a primary diagnosis of a trauma-related dissociative disorder who were participating in the TO

Exercise therapy in fibromyalgia patients: comparison of a web-based intervention with usual care.

OBJECTIVES: The aim of this study was to investigate the effectiveness of a web-based exercise intervention in improving the severity of symptoms in different health domains in patients with fibromyalgia (FM). METHODS: Consecutive FM patients insufficiently responding to drug treatment (defined as a score of >4 on a numerical rating scale of pain) were enrolled in a web-based intervention group (Fibro-Web) and underwent a 24-week daily home exercise programme. They were evaluated through the revised Fibromyalgia Impact Questionnaire (FIQR) and the Modified Fibromyalgia Assessment Status quest

Self-Help Treatment of Eating Disorders.

The authors provide an overview of the current state of research on self-help interventions for eating disorders. The efficacy of different forms of self-help interventions for bulimia nervosa, binge eating disorder, and other eating disorders at various stages of the care pathway (from prevention to relapse prevention) is described. Cost-effectiveness studies are also presented. Moderators of outcome, such as guidance and adherence, are discussed. Overall, the findings are promising and support the use of self-help interventions in the treatment of bulimic disorders, across the stages of the

Large Language Models as a New Tool for Therapists in Internet-Based Cognitive Behavioral Therapy: Blinded Clinician Rating Pilot Experiment.

BACKGROUND: Internet-based cognitive behavioral therapy (iCBT) is an effective and scalable alternative to face-to-face psychotherapy, but its reach is constrained by the time therapists spend reviewing patient input and manually drafting written responses. Studies suggest that large language models (LLMs) may be capable of generating high-quality therapeutic text, with the potential to support therapists in delivering treatment. Their suitability as therapist-support tools in structured iCBT, however, remains insufficiently studied. OBJECTIVE: This study aims to assess the quality of LLM-gen

Exploring engagement with a web-based self-directed psychoeducational program for advanced cancer patients and their caregivers (iFOCUS): A sub study analysis of the DIAdIC trial.

BACKGROUND: Supporting psychoeducational functioning for advanced cancer patients and informal caregivers (dyads) can be challenging. Self-managed digital interventions offer a possible cost-effective alternative but often face low engagement. AIM: To examine engagement with the iFOCUS intervention and to understand why it failed to improve the expected outcomes. DESIGN: iFOCUS, a four-session digital self-managed 12-week intervention for dyads, was tested in six European countries as part of a randomized controlled trial. However, it showed no effects on outcomes. Using process evaluation d

The Development and Refinement of a Web-Based Sexual Health Education Intervention for Pediatric Oncology Physicians and Advanced Practice Providers Caring for Adolescents and Young Adults With Cancer.

PURPOSE: Adolescent and young adult (AYA) oncology patients demonstrate an expressed need for improved sexual health (SH) communication with oncology clinicians, yet these conversations rarely take place, with clinicians identifying lack of education as a key barrier. This work describes the development and revision of a suite of educational modules aiming to improve clinician SH communication. METHODS: Literature review, data collected from prior research by the study team, and expert input from the Children's Oncology Group AYA Sexual Health Task Force were used to develop a series of inter

Pilot Randomised Trial of a Brief Online Personalised Feedback Intervention for the UK Context Designed To Prevent, Reduce, and Address Gambling Harm.

Participation in online gambling is growing and the risk of experiencing harms is also increasing. Brief personalised feedback interventions have been shown to prevent, reduce and address gambling harm and this randomised controlled trial tested the effectiveness of a version customised for the UK. A sample of 1586 online gambling participants with moderate or problem gambling were rapidly recruited from an existing Internet panel of UK residents. Participants were randomised to a no intervention control group or received the self-directed, online intervention which included normative feedback

Credibility and expectations: Important factors for understanding clinical response, treatment completion, and dropout in internet-delivered psychological interventions.

OBJECTIVE: Several large studies have identified perceptions of treatment credibility and treatment outcome expectations as potential moderators of treatment response in internet-delivered psychological treatment. However, no studies have specifically focused on these two constructs and their potential roles in the moderation of treatment-related change, treatment completion, or study dropout. METHOD: Adult samples were derived from four large randomized controlled trials examining internet-delivered treatment for symptoms of depression (n = 445), generalized anxiety (n = 454), social anxiety

A pilot feasibility and acceptability trial of an internet indicated prevention program for perfectionism to reduce eating disorder symptoms in adolescents.

PURPOSE: Perfectionism is a transdiagnostic risk factor for eating disorders. Treating perfectionism can reduce symptoms of eating disorders. No research has examined an indicated prevention trial using internet-based Cognitive-Behavioural Therapy for Perfectionism (ICBT-P) in adolescent girls at elevated risk for eating disorders. Our aim was to conduct a preliminary feasibility trial using a co-designed ICBT-P intervention. It was hypothesised that a higher proportion of participants in the ICBT-P condition would achieve reliable and clinically significant change on perfectionism, eating dis

Managing chronic pain after breast cancer treatments: are web-based interventions the future?

PURPOSE OF THE REVIEW: Chronic post-treatment pain in breast cancer affects a high proportion of patients. Symptom burden and financial costs are increasingly impacting patients and healthcare systems because of improved treatments and survival rates. Supporting long-term breast cancer symptoms using novel methodology has been examined, yet few have explored the opportunity to utilise these interventions for prevention. This review aims to explore the need for, range of, and effectiveness of such interventions. RECENT FINDINGS: Three papers describe risk factors for chronic pain, with six rec

Goal setting and achievement for walking: A series of N-of-1 digital interventions.

OBJECTIVE: Despite evidence that goal setting is valuable for physical activity promotion, recent studies highlighted a potential oversimplification in the application of this behavior change technique. While more difficult performance goals might trigger higher physical activity levels, higher performance goals might concurrently be more difficult to achieve, which could reduce long-term motivation. This study examined (a) the association between performance goal difficulty and physical activity and (b) the association between performance goal difficulty and goal achievement. METHOD: This st

Effectiveness of Guided Internet-Based Cognitive Behavioral Therapy vs Face-to-Face Clinical Care for Treatment of Tinnitus: A Randomized Clinical Trial.

IMPORTANCE: Accessible clinical care is not always available to individuals with distressing tinnitus. Internet-based cognitive behavioral therapy has the potential to increase access to evidence-based services that manage tinnitus. Research comparing the effectiveness of this internet-based intervention with face-to-face care is required. OBJECTIVE: To evaluate whether an internet-based cognitive behavioral therapy intervention is at least as effective as established individualized face-to-face clinical care in reducing tinnitus distress and tinnitus-related difficulties. Internet-based treatment for panic disorder.

Increasingly, the internet is being used as a source of mental health information as well as structured treatment programs for people with mental health concerns or problems. The present review focuses mainly on research conducted at the University of Ballarat in Australia, and Uppsala University in Sweden, on internet-based treatment for panic disorder, one of the more common and disabling anxiety disorders. Our studies suggest that internet-based cognitive behavioural therapy is an effective intervention for panic disorder, and that it is more effective than some other self-help therapies. W

The IDEA3 pilot trial: feasibility, acceptability, and preliminary efficacy of an online adaptation of an evidence-based intervention for campus sexual assault prevention.

BACKGROUND: Enhanced Assess, Acknowledge, Act (EAAA) is the only intervention shown in a randomized controlled trial to reduce sexual violence victimization against undergraduate women by 50%. EAAA is an in-person, small-group intervention led by two trained peer facilitators over four three-hour sessions. Implementation cost and train-the-trainer complexities have led to limited uptake with relatively few institutions implementing the intervention, despite its efficacy. Adapting EAAA for online delivery would enable use of a centralized facilitator pool, dramatically cutting cost and complexi

The effect of a Web-Based BF education program for adolescent pregnant women in the third trimester on BF practices.

BACKGROUND: It is crucial to prepare adolescent mothers-who represent a vulnerable group where the importance of breastfeeding is increasingly emphasized-for the BF experience. A method on the rise or increasingly popular for promoting breastfeeding is through web-based education interventions. The aim of this study was to evaluate the effect of a web-based breastfeeding education program, developed by the researchers, on breastfeeding outcomes among adolescent pregnant women in the postpartum period. METHODS: This randomized controlled study involved a sample of 100 pregnant adolescents, age
