From Theory to Competence: A Conceptual Review of Humanized Care in Community Nursing in the Ecuadorian Context.

Community nursing in Latin America requires a shift from control-oriented biomedical models toward relational and culturally responsive approaches capable of addressing persistent structural inequalities and intercultural complexity. This review develops the Relate-Capacity-Comfort (RCC) framework. This integrated model synthesizes three foundational nursing theories: Jean Watson's Theory of Human Caring, Dorothea Orem's Self-Care Deficit Nursing Theory, and Katharine Kolcaba's Comfort Theory. The framework conceptualizes community nursing as a sequential process comprising relational engageme

Deep weakly-supervised breast tumor segmentation in ultrasound images with explicit anatomical constraints.

Breast tumor segmentation is an important step in the diagnostic procedure of physicians and computer-aided diagnosis systems. We propose a two-step deep learning framework for breast tumor segmentation in breast ultrasound (BUS) images which requires only a few manual labels. The first step is breast anatomy decomposition handled by a semi-supervised semantic segmentation technique. The input BUS image is decomposed into four breast anatomical structures, namely fat, mammary gland, muscle and thorax layers. Fat and mammary gland layers are used as constrained region to reduce the search space

A developmental framework for understanding the influence of sex and gender on health: Pediatric pain as an exemplar.

Sex differences are a robust finding in many areas of adult health, including cardiovascular disease, psychiatric disorders, and chronic pain. However, many sex differences are not consistently observed until after the onset of puberty. This has led to the hypothesis that hormones are primary contributors to sex differences in health outcomes, largely ignoring the relative contributions of early developmental influences, emerging psychosocial factors, gender, and the interaction between these variables. In this paper, we argue that a comprehensive understanding of sex and gender contributions

Psychiatric advance directives and consent to electroconvulsive therapy (ECT) in Australia: A legislative review and suggestions for the future.

Psychiatric Advance Directives (PADs) have been adopted in many jurisdictions around the world and in most Australian states and territories. They are seen as a less restrictive and patient-centered approach to the provision of mental health care. Electroconvulsive therapy (ECT) is a restricted treatment in most jurisdictions in Australia and across the world. This paper explores the history, regulation and use of ECT and PADs and the intersections between them. It provides an overview of the legislative framework in each Australian state and territory and explores some of the issues which hav

Building the Foundations for Trustworthy AI in Pediatrics.

Artificial intelligence (AI) adoption in health care is rapidly accelerating, with pediatrics positioned to benefit from improvements in quality, efficiency, and access to care. Recent frameworks have articulated ethical commitments for trustworthy AI in pediatrics, but less attention has been paid to the foundational infrastructure required to achieve them. In this article, we examine the challenges that emerge at the intersection of AI and pediatrics: unique challenges that AI poses for pediatric care, including nonintuitive errors, impacts on clinician cognition, and new infrastructure requ

Adapting Agriculture: The Case of European Policy Frameworks for Climate Resilience.

This study provides an in-depth analysis of key policies, reforms, and initiatives implemented at the European Union, national, and regional levels to address climate change impacts on agriculture. This systematic study mainly focuses on the evolution of European policy frameworks to enhance climate resilience in agriculture. The European Green Deal, the Nature Restoration Law and Common Agricultural Policy are key policy frameworks aimed at achieving climate neutrality by 2050. The EU has determined the application of non-uniform national mitigation targets to achieve a sectoral reduction in

An Organ-Guided Lightweight Multi-frame Integration Network for Real-Time Abdominal Lesion Detection in Ultrasound Video Streams.

OBJECTIVE: Abdominal ultrasound is widely used for the routine screening of hepatobiliary and renal diseases because it is safe, inexpensive and broadly accessible. In daily clinical practice, however, lesion detection remains highly operator-dependent, particularly when sonographers must identify subtle abnormalities across different abdominal organs under low-contrast, noisy and continuously changing views. Missed or delayed recognition may affect subsequent diagnostic assessment and patient management. Although deep learning has shown promise in ultrasound analysis, most existing methods fo

Telling the story of intersectional stigma in HIV-associated Kaposi's sarcoma in western Kenya: a convergent mixed-methods approach.

INTRODUCTION: The experience of stigma can be multifaceted for people with HIV and cancer. Kaposi's sarcoma (KS), one of the most common HIV-associated cancers in sub-Saharan Africa, often presents with visible skin lesions that may put people at risk for stigmatization. In this way, HIV-associated KS is unique, as people with KS can experience stigma associated with HIV, cancer, and skin disease simultaneously. The aim of this study is to characterize the intersectionality of HIV-related, cancer-related and skin disease-related stigma in people living with HIV and KS. METHODS: We used a conv

Bridging innovation and risk: the missing role of validated risk-stratification in IgE-mediated food allergy management.

Over the last decade, the prevalence of IgE-mediated food allergies has increased, alongside therapeutic innovations such as oral immunotherapy (OIT), omalizumab, combination approaches, intranasal epinephrine, and emerging biomarker-guided strategies. This mini review synthesizes evidence published between 2019 and 2025 and evaluates how these advances intersect with persistent safety and clinical decision-making challenges. Although these therapies demonstrate promising efficacy, validated risk-stratification tools capable of guiding individual treatment selection are lacking. The proposed c

Disability, sexuality, and 'cure' in Indra Sinha's Animal's people.

Indra Sinha's Animal's People (2007) narrates the story of nineteen-year-old Animal, a severely disabled survivor of a gas leak in Khaufpur, a fictionalized version of post-1984 Bhopal. The novel explores disability as a medical, physical, and social construct, with Animal's disfigurement shaping both his identity and Sinha's critique of power, marginalization, and industrial negligence in a poverty-stricken 'third-world' society. Rejecting victimhood and resisting neoliberal, Eurocentric models of disability tied to individuality and consumerism, Animal asserts a radical self-definition that

Framing 'Chemsexism': Chinese Gay Men Navigating Stigma and Labelling.

In the gay community, chemsex intersects with multiple stigmas that exacerbate marginalization and create layered disadvantages, particularly in China. Despite this, there is limited understanding among researchers and care practitioners about how individuals counter-narrate and manage stigmatizing stress in contexts involving multiple stigmas. This study addresses this gap by using online semi-structured interviews, drawing on a process-oriented understanding of addiction stigma to explore how Chinese gay men construct empirical counter-narratives. These narratives aim to reposition themselve

Spitzoid melanoma with histopathological features of ALK gene rearrangement exhibiting ALK copy number gain: a novel mechanism of ALK activation in spitzoid neoplasia.

Spitzoid neoplasms pose diagnostic difficulties because their morphology is not consistently predictive of their biological potential. Recent advances in the molecular characterization of these tumours provides a framework by which they can now begin to be categorized. In particular, spitzoid lesions with ALK rearrangement have been specifically associated with a characteristic plexiform growth pattern of intersecting fascicles of amelanotic spindled melanocytes. We report the case of an 87-year-old man with a 3-cm nodule on his mid-upper back comprised of an intradermal proliferation of fusif

An intersectionality-based policy analysis of COVID-19 measures and women informal food vendors in Kisumu, Kenya.

INTRODUCTION: Women working as informal food vendors in Kisumu faced precarious work conditions that worsened during the COVID-19 pandemic. Despite playing a vital role in urban household food security, they were excluded from policy decisions, which were largely confined to technical experts and national government officials. METHODS: Using the Intersectionality-Based Policy Analysis (IBPA) framework, this study examined the engagement of women informal food vendors and the disparities in COVID-19 control policies. Between July 10, 2024, and October 15, 2024, we conducted a qualitative study

User perceptions of affordability of methadone maintenance treatment in Kenya: A mixed methods analysis.

BACKGROUND: Methadone Maintenance Treatment (MMT) is a global evidence-based treatment intervention combining pharmacological and psychological approaches for the management of opioid use disorder. MMT in Sub-Saharan Africa currently faces significant sustainability challenges amid donor funding transitions. This study examined user perceptions of affordability for MMT services in Kenya using an adapted socio-ecological framework to inform sustainable financing models. METHODS: An explanatory mixed-methods design was employed with 44 MMT participants interviewed at an urban facility in Nairob

Diabetes and Foot Health Among South Asian People Seeking Asylum in the United Kingdom: A Theory-Informed Scoping Review.

BACKGROUND: Globally, it is estimated that, as of June 2025, 8.42 million people were seeking asylum due to persecution and political instability. Those seeking protection in other countries can present with undiagnosed complex health needs, including type 2 diabetes mellitus. Yet diabetes related foot health and footwear remain poorly understood in this population. This scoping review mapped evidence on diabetes, foot health and footwear among adults seeking asylum in high-income countries, with attention to postmigration factors and diabetes-related stigma. METHODS: Arksey and O'Malley's sc

Development of the W-PREV Model: Integrating HIV/STBBI Prevention and Women's Sexual and Reproductive Healthcare Using an Intersectional Women-Centered Approach.

IntroductionWomen, both cisgender and transgender, experience persistent inequities in sexual and reproductive health (SRH) due to sexism, transphobia, racism, and systemic discrimination. In Canada and globally, these intersecting inequities are exacerbated by limited, biomedical, and colonial models of care that overlook women's needs, leaving them underserved in HIV and sexually transmitted and blood-borne infection (STBBI) prevention. Our study describes the development of the Women-Centred Prevention (W-PREV) Model, designed to integrate HIV/STBBI prevention and SRH through an intersectio

Navigating intersectional stigma and post-traumatic growth among heterosexually married men who have sex with men living with HIV in China.

BACKGROUND: Heterosexually married men who have sex with men living with HIV (MSMW-HIV+) in China occupy a uniquely marginalized position shaped by intersecting stigmas related to HIV, same-sex behavior, and marital norms. Existing research has largely emphasized risk, pathology, and moral failure, with limited attention to how individuals navigate, reinterpret, and potentially transform these experiences under structural constraint. Drawing on post-traumatic growth (PTG) theory and an intersectionality framework, this study examined how growth and adaptation unfold within a collectivist socio

Prevalence of psychiatric disorders at the intersection of race and sexual orientation: Results from the National Epidemiologic Survey of Alcohol and Related Conditions-III.

OBJECTIVE: Sexual minority individuals display elevated rates of psychiatric and substance use disorders compared with heterosexuals. Racial/ethnic minority individuals report lower prevalence of disorders compared with White individuals. Research on sexual minority mental health often neglects research on racial/ethnic minority mental health and vice versa. Therefore, at the intersection of sexual and racial/ethnic minority status, the prevalence of disorders remains unclear. METHOD: In a nationally representative sample (N = 36,309), we compared disorder prevalence between sexual minorities

Intersecting Generational, Gender, and Racial Disparities in Intimate Partner Physical and Sexual Violence: A Multivariate Analysis of Victimization and Perpetration.

Research on demographic disparities in intimate partner violence (IPV) has yielded mixed findings. While age, gender, and race are recognized risk factors, their intersection across generational cohorts remains underexplored. This study addressed this gap by examining how gender, race/ethnicity, and generation jointly influence physical and sexual IPV victimization and perpetration. Using an intersectional framework, a three-way multivariate analysis of variance (MANOVA) was conducted on data from 467 US adults in intimate relationships, spanning Generation Z, Millennials, Generation X, and Ba
