Degenerative changes in the intervertebral discs of the lumbar spine and their sequelae.

Careful pathological examination of lumbar spines removed at autopsy has shown that degenerative changes are present in the intervertebral discs of all subjects by middle age. The degenerative changes are more marked and occur at an earlier age when evidence of vertical or posterior disc prolapse is also present. Degenerative changes in the discs are always accompanied by osteophyte formation on the margins of the vertebral bodies and remodelling changes in the apophyseal joints. There is a direct relationship between the degree of disc degeneration, marginal osteophyte formation on vertebral

The role of sleep and circadian rhythms in intervertebral disc degeneration.

PURPOSE: Low back pain is a globally prevalent disabling condition, with intervertebral disc degeneration (IDD) as a key inducer. Sleep abnormalities and circadian rhythm disorders are widespread, but their bidirectional associations with IDD and underlying pathophysiological mechanisms remain unclear. This narrative review synthesizes existing studies to clarify the mutual regulatory relationship and corresponding intervention strategies. METHODS: A structured literature search was conducted across PubMed, Embase and Web of Science from database inception through November 2025. Peer-reviewed

The role of NOTCH signaling pathway in intervertebral disc degeneration.

Intervertebral disc degeneration (IDD) is a prevalent degenerative condition and is recognized as the primary cause of low back pain among the elderly population. IDD can be caused by a variety of factors, including aging, inflammatory stimulation, and oxidative stress. These factors collectively contribute to a reduction in nucleus pulposus (NP) cells and an excessive degradation of the extracellular matrix (ECM), thereby facilitating the progression of IDD. The NOTCH signaling pathway is a highly conserved pathway that conducts downstream signals through interactions between cells, thereby r

Neuroimmune Activation in a Goat Model of Intervertebral Disc Degeneration.

Intervertebral disc degeneration (IVDD) initiates a cascade of structural and biological changes that compromise mechanical function, often leading to chronic pain. While small animal models have provided insight into inflammatory and nociceptive mechanisms of IVDD, translational studies require large animal models that more closely replicate human spine anatomy and physiology. This study induced cervical disc degeneration via intradiscal chondroitinase ABC (ChABC) injection in a large animal model and evaluated the associated disc pathology and neuroinflammatory responses across IVDs and with

[Mechanism of extracellular vesicles in the repair of intervertebral disc degeneration].

Extracellular vesicles (EVs), defined as cell-secreted nanoscale vesicles that carry bioactive molecules, have emerged as a promising therapeutic strategy in tumor and tissue regeneration. Their potential in repairing intervertebral disc degeneration (IDD) through multidimensional regulatory mechanisms is a rapidly advancing field of research. This paper provided an overview of the mechanisms of EVs in IDD repair, thoroughly reviewed recent literature on EVs for IDD, domestically and internationally, and summarized their therapeutic mechanisms. In IDD repair, EVs could act through different me

Intervertebral disc impairments in a mouse model of Alzheimer's Disease.

Chronic low back pain, frequently associated with intervertebral disc (IVD) degeneration, is highly prevalent in individuals with Alzheimer's disease (AD), and the pain intensity is highly correlated with the degree of cognitive impairment. While the incidences of both afflictions increase dramatically in the elderly population, it is unknown whether AD exacerbates the health of the IVD. Utilizing one-year-old male and female 5xFAD mice that constitutively express human APP and PSEN1 transgenes with five AD-linked mutations, we measured the lumbar IVD's extracellular matrix composition, the th

An overview of mechanical microenvironment and mechanotransduction in intervertebral disc degeneration.

Cellular mechanotransduction, essential for many biological functions, involves the conversion of mechanical signals into biochemical signals related to cell activities and metabolism. Physical factors in the local cellular microenvironment include external mechanical forces, mechanical stimulation generated by the extracellular matrix and intercellular mechanical interactions mediated through cell-cell adhesions. Intervertebral disc degeneration (IDD) is a complex pathological process involving diverse etiological contributors, such as mechanical wear, oxidative damage and nutritional deficie

MicroRNA-targeting nanomedicines for the treatment of intervertebral disc degeneration.

Low back pain stands as a pervasive global health concern, afflicting almost 80% of adults at some point in their lives with nearly 40% attributable to intervertebral disc degeneration (IVDD). As only symptomatic relief can be offered to patients there is a dire need for innovative treatments.Given the accumulating evidence that multiple microRNAs (miRs) are dysregulated during IVDD, they could have a huge potential against this debilitating condition. The way miRs can profoundly modulate signaling pathways and influence several cellular processes at once is particularly exciting to tackle thi

Visualizing the bibliometrics of the inflammatory mechanisms in intervertebral disc degeneration.

OBJECTIVE: Intervertebral disc degeneration (IVDD) constitutes a crucial pathological foundation for spinal degenerative diseases (SDD) and stands as a primary contributor to both low back pain (LBP) and disability. The progression of IVDD is linked to structural and functional alterations in tissues, where an imbalance in the inflammatory microenvironment can induce extracellular matrix (ECM) degradation, senescence, and apoptosis. This imbalance is a key pathomechanism in the disease's development, gaining considerable attention in recent years. This study aims to conduct a bibliometric anal

The influence of intervertebral disc overloading on nociceptor calcium flickering.

INTRODUCTION: Mechanical overloading can trigger a degenerative-like cascade in an organ culture of intervertebral disc (IVD). Whether the overloaded IVD can influence the activation of nociceptors (i.e., the damage sensing neurons) remains unknown. The study aims to investigate the influence of overloaded IVD conditioned medium (CM) on the activation of nociceptors. METHODS: In the static loading regime, force-controlled loading of 0.2 MPa for 20 h/day representing "long-term sitting and standing" was compared with a displacement-controlled loading maintaining original IVD height. In the dyn

The mechanisms and functions of IL-1β in intervertebral disc degeneration.

Intervertebral disc degeneration (IDD) is the leading cause of low back pain (LBP) and disability in the elderly, imposing significant public health and economic burden worldwide. Meanwhile, the pathological mechanisms of IDD remain complicated, and treatment strategy to reverse IDD is primarily due to the unclear specific mechanisms of IDD and the lack of particular effective targets. Interleukin-1β (IL-1β), one of the most important members of the IL-1 family, can induce solid pro-inflammatory activity by stimulating the secretion of various pro-inflammatory mediators and is considered the k

The mechanisms and functions of TNF-α in intervertebral disc degeneration.

Low back pain (LBP) is one of the most common health problems in people's lives, which brings a massive burden to clinicians, and the leading cause of LBP is intervertebral disc degeneration (IDD). IDD is mainly caused by factors such as aging, mechanical stress, and lack of nutrition. The pathological mechanism of IDD is very complex, involving inflammatory response, cell metabolism disorder, and so on. Unfortunately, in the current treatment of IDD, only relieving symptoms as the primary means of relieving a patient's pain cannot effectively inhibit or reverse the progression of IDD. Tumor n

The involvement of immune system in intervertebral disc herniation and degeneration.

Intervertebral disc (IVD) herniation and degeneration contributes significantly to low back pain (LBP), of which the molecular pathogenesis is not fully understood. Disc herniation may cause LBP and radicular pain, but not all LBP patients have disc herniation. Degenerated discs could be the source of pain, but not all degenerated discs are symptomatic. We previously found that disc degeneration and herniation accompanied by inflammation. We further found that anti-inflammatory molecules blocked immune responses, alleviated IVD degeneration and pain. Based on our recent findings and the work o

Recent Advances in Managing Spinal Intervertebral Discs Degeneration.

Low back pain (LBP) represents a frequent and debilitating condition affecting a large part of the global population and posing a worldwide health and economic burden. The major cause of LBP is intervertebral disc degeneration (IDD), a complex disease that can further aggravate and give rise to severe spine problems. As most of the current treatments for IDD either only alleviate the associated symptoms or expose patients to the risk of intraoperative and postoperative complications, there is a pressing need to develop better therapeutic strategies. In this respect, the present paper first des

The role of ageing and oxidative stress in intervertebral disc degeneration.

Intervertebral disc degeneration (IDD) is the primary cause of intervertebral disc (IVD) disease. With the increased ageing of society, an increasing number of patients are plagued by intervertebral disc disease. Ageing not only accelerates the decreased vitality and functional loss of intervertebral disc cells but also increases intracellular oxidative stress. Moreover, the speed of intervertebral disc ageing is also linked to high levels of reactive oxygen species (ROS) production. Not only is the production of ROS increased in ageing intervertebral disc cells, but antioxidant levels in dege

Biomarkers in the Degenerative Human Intervertebral Disc Tissue and Blood.

AbstractPatients with back pain comprise a large proportion of the outpatient practice among physiatrists. Diagnostic tools are limited to clinical history, physical examinations, and imaging. Nonsurgical treatments are largely empirical, encompassing medications, physical therapy, manual treatments, and interventional spinal procedures. A body of literature is emerging confirming elevated levels of biomarkers including inflammatory cytokines in patients with back pain and/or radiculopathy, largely because the protein assay sensitivity has increased. These biomarkers may serve as tools to assi
