Repair of Descemet's membrane detachment following inadvertent intrastromal viscoelastic injection for hypotony: anterior and posterior approaches.

Descemet's membrane (DM) detachment is a significant, if rare, risk of intracameral injection of viscoelastic to treat hypotony after glaucoma filtration surgery. We describe two cases of DM detachment following inadvertent injection of viscoelastic into the posterior stroma and the techniques used for their repair. In both cases, conventional air tamponade failed to resolve the detachment, and further surgical intervention was required. One underwent anterior drainage through venting incisions performed in the operating theatre, while the other had posterior drainage through the DM performed

Case Report: Burkholderia pseudomallei-Caused Sclerokeratitis.

This report is on a 45-year-old male patient without underlying disease who presented with a Burkholderia pseudomallei-caused keratoscleritis in his right eye. Slit-lamp examination revealed multiple, indistinct corneal infiltrations with subconjunctival/scleral abscesses. Corneal tissue culture was positive for B. pseudomallei and confirmed by mass spectrometry. The patient was treated with fortified ceftazidime, fortified gentamicin eyedrops, and intravenous ceftazidime injection. Penetrating keratoplasty, including intracameral ceftazidime injections, was undertaken due to corneal lesion wo

Management of Fusarium keratitis and its associated fungal iris nodule with intracameral voriconazole and amphotericin B.

A retrospective case report on the management of a 56-year-old man who was diagnosed with Fusarium keratitis with an associated iris fungal nodule. He presented with persistent right ocular pain, redness, photophobia and reduced vision following an injury six weeks earlier. Conservative management with topical antibiotics and intracameral injections was successfully used to treat a rare case of Fusarium keratitis with an associated fungal iris nodule. The patient had complete resolution of his vision. Therefore, conservative management of the fungal nodule is a successful therapy without the r

Posterior pathway for aqueous humor drainage in the dog.

A posterior route for the drainage of aqueous humor was determined, using latex spheres as tracers. Continuous intracameral injections were made on live Beagles for periods up to 1 hour. Intraocular pressure was maintained between 15 and 30 mm of Hg during the infusion process. Tissue macrophages ingested many of the spheres, and their ingestion served to fix the position of the markers as they passively followed aqueous drainage routes. Thin sections were prepared from tissues that reportedly constituted the uveoscleral route. The electron microscope was used to visualize the latex spheres wi

Pseudophakic Penicillium endophthalmitis.

A healthy 76-year-old woman complained of redness and a painful sensation in her right eye after cataract surgery 5 months previously. The eye was treated with topical corticosteroid and ciprofloxacin, and a soft exudative mass on the nasal lower iris was noted later. Fungal endophthalmitis was suspected and a repeat aqueous culture grew Penicillium spp. After treatment with topical natamycin and an intracameral injection of amphotericin B, the endophthalmitis was cured. We describe the disease course and management and discuss topical corticosteroids as a possible risk factor for increasing t

Tetanus prophylaxis following ocular injuries.

The administration of prophylaxis against tetanus following a corneal abrasion is routinely performed in many acute care facilities, despite a lack of support in the literature for its necessity. The risk of developing clinical tetanus from three different types of injuries to the eye was evaluated in an animal model. Clinical tetanus was induced in unimmunized mice by injecting Clostridium tetani organisms or toxin into the anterior chamber. Immunized mice injected intracamerally did not develop signs of tetanus. Tetanus was not induced by topical inoculation of either live organisms or toxin

Update on the prevention and treatment of endophthalmitis.

Endophthalmitis remains a rare but serious cause of visual loss. Over time, changes have been noted in endophthalmitis in terms of predominant causes, infecting organisms, and antibiotic susceptibilities. There is controversy regarding the use of intracameral prophylactic antimicrobials during cataract surgery. Alternatively, there appears to be increasing evidence against using routine topical antibiotics for intravitreal injections. There are also increasing reports of multidrug-resistant organisms causing endophthalmitis, but the combination of vancomycin and ceftazidime appears effective f

High Adhesion Superoleophobic Oil Microdroplets to Create Ocular Hypertension In Vivo.

Ocular hypertension is commonly linked with glaucomatous neurodegeneration. To investigate the mechanisms of glaucoma, in vivo models typically induce intraocular pressure elevation by obstructing aqueous humor flow in the trabecular meshwork. However, there is no satisfying in vivo model to maintain stable ocular hypertension and replicate neurodegenerative features accurately. Here, we introduce an injectable oil microdroplet model that achieves sustained IOP elevation and a 44.3% loss of retinal ganglion cells within four weeks in mice. The oil microdroplets exhibit an underwater "superoleo

Antivascular endothelial growth factors in anterior segment diseases.

Proangiogenic growth factors, mainly VEGF (vascular endothelial growth factor) play a significant role in anterior segment diseases, characterized by neovascularization. Newly grown vessels in the cornea can lead to an impairment of transparency and visual acuity. Neovascularization of the iris (rubeosis iridis) and the anterior chamber angle are caused by ischemic retinopathies, usually leading to neovascular glaucoma with serious loss of vision. A pterygium is characterized, amongst others, by fibrovascular proliferation and may have vision threatening consequences if left untreated. Several
