We report a 72-year-old female presenting with acute consciousness disturbance, ultimately diagnosed with Neuronal Intranuclear Inclusion Disease (NIID) masquerading as Hashimoto encephalopathy. Comprehensive evaluations revealed elevated antithyroid peroxidase antibodies, characteristic magnetic resonance imaging findings, and genetic confirmation of NIID. Corticosteroid treatment led to partial improvement, but persistent symptoms highlighted the complex presentation of NIID. This rare clinical presentation underscores the importance of thorough diagnostic approaches in complex neurological

Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) is an inflammatory neuropathy, characterized by a slowly progressive onset and symmetrical, sensorimotor involvement. Several diseases, nevertheless, may present similar features as CIDP. It is of paramount importance to recognize these mimics to yield an optimal therapeutic effect. Neuronal intranuclear inclusion disease (NIID) is a rare, progressive neurodegenerative disorder known for its diverse clinical manifestations. Since it may present as sporadic as well as inherited manner and may only involve the peripheral nerve syst

Neuronal intranuclear inclusion disease (NIID) is a rare and slowly progressing neurodegenerative disease characterized by the presence of eosinophilic intranuclear inclusions in the nervous system and multiple visceral organs. Sporadic NIID case was more frequently encountered than familial. In our study, we reported two adult-onset NIID patients from a family and described their clinical, imaging, and pathological features. The first patient was a 61-year-old man who only presented with non-specific headache and dizziness; however, Brain MRI with diffusion-weighted images (DWI) sequence show

Neuronal intranuclear inclusion disease (NIID) is a neurodegenerative disease presenting with various manifestations including dementia, weakness, transient impaired consciousness, encephalitis-like episodes and also epileptic seizures. However, the nature of epileptic seizures, focal or generalized onset, remains unclear. A man at age 76 was admitted to a local hospital due to febrile impaired consciousness lasting several days. During the hospital stay, a generalized convulsion occurred, and afterward he remained obtunded. He was transferred to our hospital for further treatment. One additio

Ferritinopathy (neuroferritinopathy) has recently been identified as an autosomal dominant, multisystem disease, mainly affecting the central nervous system. It is caused by mutations in exon 4 of the ferritin light chain gene on chromosome 19. Its fine structural hallmarks are granular nuclear inclusions in neurons, oligodendroglial and microglial cells with similar extracellular derivatives in the central nervous system, muscle, peripheral nerve, and skin. These pathognostic structures have previously been described in perivascular cells of muscle and nerve biopsy specimens in a case with an

Unverricht-Lundborg disease (EPM1A), also known as Baltic myoclonus, is the most common form of progressive myoclonic epilepsy. It is inherited as an autosomal recessive trait, due to mutations in the Cystatin-B gene promoter region. Although there is much work on rodent models of this disease, there is very little published neuropathology in patients with EPM1A. Here, we present the neuropathology of a patient with genetically confirmed EPM1A, who died at the age of 76. There was atrophy and gliosis affecting predominantly the cerebellum, frontotemporal cortex, hippocampus and thalamus. We ha

AIMS: Polyglutamine (polyQ) diseases are characterized by the expansion of a polymorphic glutamine sequence in disease-specific proteins and exhibit aggregation of these proteins. This is combated by the cellular protein quality control (PQC) system, consisting of chaperone-mediated refolding as well as proteasomal and lysosomal degradation pathways. Our recent study in the polyQ disease spinocerebellar ataxia type 3 (SCA3) suggested a distinct pattern of protein aggregation and PQC dysregulation.
METHODS: To corroborate these findings we have investigated immunohistochemically stained 5 μm s

Due to the high clinical heterogeneity of neuronal intranuclear inclusion disease (NIID), it is easy to misdiagnose this condition and is considered to be a rare progressive neurodegenerative disease. More evidence demonstrates that NIID involves not only the central nervous system but also multiple systems of the body and shows a variety of symptoms, which makes a clinical diagnosis of NIID more difficult. This review summarizes the clinical symptoms in different systems and demonstrates that NIID is a multiple-system intranuclear inclusion disease. In addition, the core triad symptoms in the

A 2-week-old emu chick (Dromaius novaehollandiae) of approximately 200 g body weight was presented for necropsy with a history of weakness, diarrhea, pallor of the head, and acute death. Hemorrhagic enteritis with mild hepatomegaly was noted on gross examination. Microscopic examination revealed necrohemorrhagic enteritis with intralesional intranuclear basophilic viral inclusion bodies in intestinal epithelial cells; splenic lymphoid necrosis and fibrin exudation; hepatocellular vacuolar change; and multiple clusters of small gram-negative bacilli in the liver, spleen, yolk sac, and intestine

A set of dizygotic twins, both born prematurely, developed herpes simplex encephalitis shortly after birth. The second twin had unilateral keratoconjunctivitis and bilateral endophthalmitis and subsequently died from disseminated herpes infection. Herpes simplex, type 2, was isolated from conjunctiva of both eyes, cerebrospinal fluid, nasopharynx and trachea. Histologic examination of the eyes reveaed bilateral endophthalmitis with a necrotizing retinitis. Intranuclear inclusion bodies were demonstrated by light microscopy in the retina, choroid and iris, and virus particles were demonstrated

Equine Coital Exanthema (ECE) is a contagious venereal disease caused by Equid alphaherpesvirus 3 (EHV‑3). Although ECE is self-limiting, it causes significant economic losses to the global equine industry due to the temporary segregation of affected mares and stallions from breeding activities. This report describes the epidemiological, clinical, histological, and molecular findings from an outbreak of ECE in Uruguayan horses. Seven days following the introduction of a stallion, five mares developed extensive papular and vesicular dermatitis in the vaginal and perineal regions. The stallion e

Human herpesvirus type 1 (HHV-1) is widely dispersed among the human population. Although infection is often asymptomatic in humans, nonhuman primates develop a severe and often fatal infection. In August 2006, 13 black-tufted marmosets (Callithrix penincillata) from a group of 14 presented with clinical apathy, anorexia, and ataxia. Physical examination revealed conjunctivitis, erosive or ulcerative lesions on the skin, and swollen lymph nodes. Of the 14 animals captured, 10 died. Grossly, ulcers and erosions were observed on the skin of face, nasal planum, lips, and oral mucosa. Histological

The present case is the first description of a triple infection with canine distemper virus (CDV), canine adenovirus (CAV) type 2, and Mycoplasma cynos in a dog. The 5-month-old female Miniature Pinscher was euthanized because of dyspnea, croaking lung sounds, weight loss, and lymphopenia. Pathologic examination revealed a fibrinous necrotizing pneumonia with large amphophilic intranuclear and acidophilic intracytoplasmatic inclusion bodies in different lung cells. Immunohistochemically, CDV antigen was present in lung and many other organs. In situ hybridization for detection of CAV nucleic a

We present a case of acute lead poisoning in 10 calves. All calves died with few or no clinical signs prior to death. The clinical signs included neurologic and gastrointestinal symptoms but were of an unspecific nature. Several painted iron girders, stored on a field close to the farm, were determined as the source of the poisoning. Postmortem findings were minimal. Some animals presented acid-fast, intranuclear inclusion bodies in the renal tubules. A chemical analysis of some frozen parts of the liver and kidney revealed levels of lead as high as 12 ppm in the liver and 63 ppm in the kidney

Subacute sclerosing panencephalitis is a rare disorder of late childhood and early adolescence Affected patients usually show behavioural and intellectual disturbance and involuntary movements before dying in coma after about 12 months. At some stage most have characteristic electroencephalographic abnormalities. Pathologically, changes in the brain are those of subacute encephalitis with a variable gliosis of the white matter, and sometimes intranuclear inclusion bodies in neurones and glial cells. Recent studies in many patients have shown high levels of circulating anti-measles antibodies,

Opportunistic infections are the most common initial manifestations of AIDS and, in many instances, are first encountered in surgical specimens. Pneumocystis carinii pneumonitis is by far the most frequent infection seen in biopsy specimens of AIDS patients. Most pathologists are familiar with its histopathologic presentations from previous experience. By contrast, many other opportunistic infections are either new or present clinically and pathologically in unfamiliar ways. Cytomegalovirus affects primarily the alimentary tract and lung. Colitis is the most common presentation. Penetrating ul

BACKGROUND: Neuronal intranuclear inclusion disease (NIID) is a rare, progressive multisystem disorder most commonly associated with GGC repeat expansion in the NOTCH2NLC gene. Parkinsonism can be an initial presentation and may be misdiagnosed as idiopathic Parkinson's disease, particularly when prominent non-motor features are present. While many cases are levodopa-responsive, diagnosis is challenging when prominent non-motor features and drug intolerance are present.
CASE PRESENTATION: We report a case of a 70-year-old woman of Han Chinese who developed atypical parkinsonism, severe cognit

The aberrant GGC repeat expansion in the 5'-untranslated region of the NOTCH2NLC gene causes neuronal intranuclear inclusion disease (NIID), a progressive neurodegenerative disorder. The clinical features of NIID are highly variable and include cognitive dysfunction, peripheral neuropathy, and episodic neurogenic symptoms. The pathogenesis of episodic symptoms in NIIDs remains unknown, and histopathological studies are limited. Here, we report an autopsy case of NIID in a 32-year-old Japanese female who developed severe episodic symptoms, including hemiplegic migraine, seizures, and impaired c

OBJECTIVE: Episodic encephalopathy is an important yet underexplored manifestation of NOTCH2NLC-related neuronal intranuclear inclusion disease (NIID). Current studies are limited by small sample sizes and a lack of long-term follow-up, hindering a comprehensive understanding of its clinical and radiological features, which this study aims to address.
METHODS: Patients diagnosed as NOTCH2NLC-related NIID were evaluated retrospectively and categorized into episodic encephalopathy-positive group (NIID-EP) and episodic encephalopathy-negative group (NIID-EN) depending on the presence of persiste
