[Intrauterine Device Totally Embedded in the Bladder Wall:Report of One Case].

Intrauterine device(IUD)migrating to the bladder is rare, especially the migration far away from the uterus into the bladder wall.Due to no obvious clinical symptom in the early stage and being far away from the uterus, the IUD totally embedded in the bladder wall is prone to misdiagnosis and delay in treatment. We reported one case of such migration, aiming to improve the clinical management of the IUD totally embedded in the bladder wall.

Outcomes in the ultrasound follow-up of the Essure micro-insert: complications and proper placement.

OBJECTIVE: To review the use of three-dimensional ultrasound follow-up of the Essure micro-insert placement at three months for the identification of misplaced coils and complications. METHODS: We conducted a retrospective cohort study of reproductive age women requesting permanent sterilization in a tertiary care ambulatory women's clinic. Women who underwent placement of the Essure micro-insert were assessed for appropriate positioning of the Essure micro-insert coil using three-dimensional ultrasound as well as hysterosalpingography when indicated. RESULTS: A total of 610 women who had un

Postplacental intrauterine device insertion at a teaching hospital.

OBJECTIVE: To determine whether postplacental intrauterine device (IUD) insertion can be safely and effectively performed within a teaching program. STUDY DESIGN: This was a prospective cohort of 177 subjects planning vaginal delivery enrolled antenatally who desired postplacental IUD insertion of either the copper T380A IUD or levonorgestrel IUS. Insertions were performed primarily by resident physicians following a training session. Follow-up included a 4- to 8-week visit and telephone calls at 3 and 6 months. RESULTS: Ninety-nine subjects underwent successful postplacental IUD insertion o

Ureteric erosion and obstruction: A rare but dreaded complication of intrauterine contraceptive device.

Though intrauterine contraceptive device (IUCD) is a safe and most frequently used long-term contraceptive method, it has some complications. Uterine perforation and intra-abdominal migration have been reported often, but a retroperitoneal migration is exceptional. Here, we are reporting an IUCD which perforated the uterus and migrated to the retroperitoneum; impinging into and obstructing left ureter causing severe hydroureteronephrosis due to the development of the left lower ureteric stricture.

[Intrauterine device: about a rare complication and literature review].

The intrauterine device (IUD) is the most common contraceptive method used in the world. Transuterine migration is a rare complication, accounting for 1/350 - 1/10000 insertions in the literature. We report the case of a 40-year old patient, who had had an IUD insertion 12-year before, presenting with pelvic and right lower back pain associated with intermittent hematuria and burning during urination. Radiological assessment showed calcific deposits on intra bladder IUD. The patient underwent cystostomy, without any difficulty, allowing stone and IUD extraction. A urinary catheter was left in

[No visible strings during an IUD check-up].

BACKGROUND: When there are no strings visible during an IUD (intrauterine device) check-up and ultrasound does not show an IUD in the uterine cavity, expulsion is commonly assumed. If the patient has abdominal pain further investigation is advised. This case shows that this policy does not cover every eventuality. CASE DESCRIPTION: The patient is a 32-year-old gravida 2, para 2. One month after IUD insertion she developed abdominal pain. The strings of the IUD were not visible with a speculum and ultrasound revealed an empty uterine cavity. Over the next few years the abdominal pain decreased

[Hysteroscopy to relieve IUD-related symptoms].

A levonorgestrel-releasing intrauterine device (LNG-IUD) had been placed in 4 patients, aged 33, 27, 46 and 27 years. They subsequently experienced symptoms that were possibly related to the IUD, such as haemorrhagic discharge, pain and dyspareunia. Patients who have persistent symptoms after IUD placement need to be examined. If a malpositioned IUD is suspected, outpatient vaginoscopic hysteroscopy should be considered. Hysteroscopy can reveal the cause of the symptoms. Furthermore, an IUD that is incorrectly positioned can be repositioned during the same procedure. One advantage to this is t

A lost intrauterine device. Guess where we found it and how it happened?

Perforation of the uterus by an intrauterine device (IUD) is a serious complication occurring at or following 1/350 to 1/2,500 insertions. It is more common among women with 'lost' IUDs. If a woman presents with pelvic pain and a history of a 'lost' IUD, X-rays of the abdomen and of the pelvis should be ordered. We report on a 'lost' IUD that had been inserted 12 years previously. It was found in the lower anterior abdominal wall. Most probably uterine perforation had happened during a dilatation and curettage (D & C) attempted for removal of the device. Thereafter the IUD must have migrated t

Uterine perforation by copper intrauterine device.

Sixteen cases with uterine perforation by a copper intrauterine device (IUD) are presented. In 13 cases the IUD had been inserted within 5 months following delivery, and in 6 cases the insertion had been painful. Missing string was the first sign in most cases. Laparatomy was performed in all cases to remove the IUD. The IUD was adherent to omentum or sigmoid in 10 cases. There is an increased risk of uterine perforation if the IUD is inserted postpartum during lactation and involution of the uterus. Therefore a painful IUD insertion and a missing string demand investigation of a partial or co

Computed tomographic confirmation of distally placed Essure* microinsert.

OBJECTIVE: To highlight the use of CT scan to confirm proper placement of the Essure microinsert tubal occlusion device. DESIGN: Case report. SETTING: Community-based, outpatient setting. PATIENT(S): A 37-year-old woman (G2P2002) who desired permanent sterilization. INTERVENTION(S): The patient underwent hysteroscopic placement of Essure microinserts, which was complicated by inability to confirm intraluminal placement of the left implant by hysteroscopic visualization. A CT scan performed that day confirmed intraluminal placement and ruled out intraperitoneal placement of the insert. MAI

Migration of levonorgestrel IUS in a patient with complex medical problems: what should be done?

Patients with complex medical problems should be counselled about the need for highly effective contraception. As failure resulting in pregnancy, could cause significant morbidity and mortality. The LNG-IUS has gained great popularity and generally has a low side effect profile; however, perforation of the uterus and migration of the device is a potentially serious complication known to be associated with its use. The current accepted management is removal of the device from the abdominal cavity in order to prevent further morbidity. However this is not always a simple matter in patients who h

Laparoscopic removal of an intraperitoneal translocated intrauterine contraceptive device.

The use of an intrauterine contraceptive device (IUD) is often accompanied by various complications, the perforation of the uterus constituting the most dangerous. Here we report the case of a patient who complained of abdominal pains. She had had an IUD inserted 15 months previously. Three months later, as she could no longer see the IUD strings at the external os of the cervix, she underwent pelvic ultrasonography, which did not show the IUD in the uterine cavity. A diagnosis of expulsion of the IUD was made. A few months later, the patient accidentally became pregnant, and decided to have a

[Diagnosis and treatment of an occult intrauterine device].

Between February 1978 and December 1985 53 women with lost intrauterine devices (IUD) were treated. To exclude unrecognized expulsion and to locate the device sonography was primarily employed. In 90.6% of cases a correct diagnosis was established. Only in 9.4% of cases a plain X-ray of the abdomen was necessary because sonography was not diagnostic. In 47 patients (88.7%) the IUD could be located by hysteroscopy and was removed. Using biopsy forceps or curette. Of the remaining 6 cases there were 4 with perforation and intraabdominal dislocation, 1 spontaneous expulsion in the interval betwee

Combined Management of a Rare Case of an Incarcerated Intrauterine Device: Case Report and Review of the Literature.

BACKGROUND: Hormonal intrauterine systems are widely used for long-acting reversible contraception and for managing gynecological conditions such as heavy menstrual bleeding and endometrial hyperplasia. Although rare, complications such as uterine embedment and perforation can occur, potentially leading to device malfunction or injury displacement. This is the first reported case describing the removal of a fully incarcerated hormonal intrauterine system using a combined hysteroscopic and laparoscopic approach. Our findings suggest that this technique may offer a safe and effective alternative
