A 33-year-old woman had keratoconus and retinal cone dystrophy, an association that has not been reported previously. The family history suggested a recessive inheritance pattern. Genetic probes for these conditions are needed to determine whether the coincidental occurrence of keratoconus and cone dystrophy is due to a genetic relationship or was a chance event.

PURPOSE: To determine the effect of bilateral surgery on the rejection-free survival of penetrating keratoplasties (PK) performed for keratoconus.
METHODS: The records of 587 patients with a PK for keratoconus who attended a single center over a 7-year period were reviewed. Details on 400 patients were sufficiently complete to permit a multivariate analysis of the factors suspected to influence graft survival. In 165 of these patients with bilateral grafts, the effect of a contralateral PK on graft survival was evaluated using actuarial methods.
RESULTS: In first grafts for all patients, the

BACKGROUND: We utilized videokeratography to determine the frequency of keratoconus suspects in a refractive surgery population, and compared the results of refractive keratotomy in these patients to those of patients with normal corneal topography.
METHODS: The records of 67 consecutive patients (120 eyes) undergoing refractive keratotomy were reviewed retrospectively. Quantitative measurements of relative inferior corneal steepening (inferior-superior value, I-S value) were obtained from preoperative videokeratography.
RESULTS: Three of 120 eyes (3 of 67 patients, 4.5%) were identified as

BACKGROUND: The Mulvihill-Smith syndrome consists of a large number of stigmata. A few of them are also seen in the LEOPARD syndrome. In both syndromes keratoconus has not yet been reported.
PATIENT: We present a twenty-year-old male patient suffering from keratoconus in connection with a Mulvihill-Smith syndrome. A keratoplasty à chaud was done after perforation of the corneal ulcer.
CONCLUSION: As a major complication corneal graft rejection is known after keratoplasty à chaud. However, in our patient immunological graft failure may be prevented by the T-cell defect.

A 34-year-old patient presents bilateral keratoconus with osteo-condensing and osteolysis at the level of long bones, oligophrenia, perception deafness, muscular atrophy, anemia, arterial hypertension--typical manifestation of Albers-Schönberg malady. Osteopetrosis is manifested through an important condensing of the cortical and of bones' spongeous traveas, with medullary channel disappearance and particular bone fragility, which made the affection to be named as "marble bones' illness". The affection is caused by a congenital alteration of the mesenchyma, with excessive deposit of calcium at

Proteoglycan molecules in keratoconus corneas were studied by immunohistochemical and electron microscopic histochemical methods. Compared with normal human control subjects, the staining intensity with monoclonal antibody 9-A-2 was enhanced in the stroma of scarred keratoconus corneas, whereas the intensity with antibody J-19 was reduced. The 9-A-2 experiment showed an increased immunoreactivity of dermatan sulfate proteoglycan epitopes, and the J-19 experiment indicated a decreased immunoreactivity of sulfated keratan sulfate epitopes. Uronic acid analyses were consistent with the 9-A-2 data

Two-dimensional electrophoretic maps of extracts from eleven normal and eleven keratoconus corneas were compared. Of the eleven corneas analyzed, eight were pooled and the remaining three were analyzed individually. Several differences were demonstrated between electrophoretic patterns of normal and keratoconus corneas. In keratoconus corneas, 1) two abnormal components (MW 54kD and 26kD) were observed; 2) three normal corneal components (MW 12kD, 14kD, and 39kD) were present in significantly higher amounts; and 3) three normal corneal proteins (MW 66kD, 55kD, and 13kD) were present in reduced

PURPOSE: Although the influence of flat-fitting contact lenses on corneal scarring in keratoconus is frequently debated, the current standard of care with regard to the apical fitting relationship in keratoconus remains undocumented.
METHODS: Patients were examined at baseline in the Collaborative Longitudinal Evaluation of Keratoconus (CLEK) Study (N = 1209). Patients wearing a rigid contact lens in one or both eyes (N = 808) had their habitual rigid contact lenses analyzed, and the fluorescein patterns and base curves were compared to the first definite apical clearance lens (FDACL). This review presents literature data indicating that tear fluid composition in keratoconus reflects the course of metabolic processes in this disease. In keratoconus, tear fluid shows changes in the activity of lysyl oxidase, prolidase, and heparanase, which participate in corneal structure remodeling. Tear fluid also shows enhanced free radical reactions, reduced intensity of energy processes, and disturbances in the protease-inhibitor balance and cytokine status. Despite the absence of visible signs of inflammation, tear fluid changes confirm the role of inflammation in the pathogenesis of k

Keratoconus (KC) is a progressive non-inflammatory disorder characterized by significant changes in the corneal structure, leading to severe vision loss. Risk factors include eye rubbing, a positive family history, and allergic reactions. There is growing evidence suggesting that sex hormones may influence the development and progression of KC, but the exact mechanisms and extent of their impact remain unclear and controversial. This review aims to examine the current literature on the association between KC and sex hormones and to evaluate the potential of these hormones as clinical markers f

INTRODUCTION: Keratoconus (KC) is a progressive corneal disorder characterized by thinning and conical protrusion, leading to visual impairment. Whereas traditionally viewed as non-inflammatory, recent evidence highlights the role of chronic inflammation, oxidative stress, and genetic susceptibility in its pathogenesis.
METHODS: A literature review was conducted using PubMed, Embase, and Scopus databases focusing on keratoconus etiology and risk factors.
RESULTS: The most significant factors include eye rubbing, male sex, time spent using computers after work or school, and exposure to dust

Keratoconus (KC) is a progressive, multifactorial corneal ectatic disorder characterized by localized stromal thinning and irregular astigmatism, with incidence and prevalence varying markedly among populations. These differences are influenced by environmental exposures, behavioral factors, and genetic predisposition. A positive family history is a well-established high-risk factor, and KC has also been documented in association with syndromic disorders such as Down syndrome, connective tissue disorders, and certain metabolic diseases. Over the past decades, numerous candidate genes have been

PURPOSE: To report a pediatric patient with bilateral temporal keratoconus.
OBSERVATIONS: A 14-year-old male presented with a two-year history of progressively worsening visual acuity in both eyes and suspicion for undiagnosed amblyopia in the right eye. Retinoscopy revealed a scissoring reflex in both eyes and corneal topography demonstrated high keratometry values (Kmax 57.9 D and 46.1 D in the right and left eyes, respectively), with relative temporal steepening approximately coinciding with the thinnest pachymetry in both eyes. Corneal cross-linking was recommended.
