Foundation models (FMs) and large language models (LLMs) are transforming cancer AI by integrating heterogeneous data sources, including medical imaging, electronic health records, and molecular profiles. By learning from large-scale, unstructured, and label-free inputs, these models may support diagnosis, biomarker discovery, prognostic assessment, treatment personalization, and workflow automation. In this narrative review, we propose the paradigm of "Leave No Data Behind" to describe the promise that broad oncology data integration may generate clinically meaningful outputs. We critically a

Accurate and convenient assessment of individual aging is crucial for identifying health risks and preventing aging-related diseases. Nonetheless, current aging proxies often face challenges such as methodological limitations, weak associations with adverse outcomes and limited generalizability. Here we propose a framework that leverages large language models (LLMs) to estimate individual overall and organ-specific aging using only health examination reports. We validated this approach across six population-based cohorts, encompassing over 10 million participants and demonstrated effectiveness

OBJECTIVE: Maintaining robust surveillance programs for abdominal aortic aneurysms (AAAs) is important, but these programs are expensive and labor-intensive, typically requiring manual data review by trained health care professionals. Studies have shown that natural language processing software can assist in these functions, but each task-specific algorithm requires human-directed training before use. Our objective was to evaluate the use of a large language model (LLM) to extract AAA-related data using generalized artificial intelligence, negating the need for task-based training.

BACKGROUND: Large language models (LLMs) such as ChatGPT are being explored for various medical applications, but their performance across languages is not established. Despite the promising results in English from previous studies, it is unclear how ChatGPT performs in other languages. We assessed the ability of ChatGPT-4 to answer common otolaryngology-related frequently asked questions in Chinese, Korean, and Spanish in this study.
METHODS: Eighteen clinical questions were selected from the American Academy of Otolaryngology-Head and Neck Surgery Foundation guidelines and ENT Health across

ABSTRACT: BACKGROUND: Large language models can return citations whose PubMed Identifier (PMID), digital object identifier (DOI), title, and journal appear plausible but identify different papers. Checking one valid identifier may therefore fail to detect an incorrect citation bundle.
ABSTRACT: OBJECTIVE: This study aimed to measure citation metadata incoherence in responses from five large language model snapshots to an internal medicine prompt benchmark.
ABSTRACT: METHODS: Five models answered 30 synthetic prompts each, producing 150 model-by-prompt responses. Twenty-six prompts per model

In this Viewpoint, we highlight the principal cybersecurity measures that should be implemented to facilitate safe and effective integration of large language models (LLMs) into health care and propose a conceptual, life cycle-based framework synthesizing evidence from security and clinical informatics literature. While LLMs offer significant potential for applications in clinical documentation, triage, and medical education, their deployment creates novel vulnerabilities that can compromise patient safety and data confidentiality. We argue that these vulnerabilities must be addressed across t

Mentorship in predictive large language models (LLMs) presents both opportunities and risks of inequity if these systems are not designed with care. Mentorship quietly shapes medical careers, yet opportunities for it remain unequal. LLMs can analyze student writing and may reveal potential that exams overlook. This predictive capacity could inform fairer mentorship and direct support to students who need it most, helping build a more equitable medical workforce.

OBJECTIVES: Large language models (LLMs) are increasingly studied for clinical decision support, but high-risk cardiology exposes persistent weaknesses in hallucination control, guideline adherence, and medication-safety reasoning. Heart failure with reduced ejection fraction (HFrEF) is a demanding test case because safe care requires structured guideline-directed therapy, comorbidity-aware monitoring, and reliable risk warnings.
METHODS: We developed a dynamic alignment framework using 1087 retrospective HFrEF cases from Affiliated Zhongshan Hospital of Dalian University. An open-source LLaM

BACKGROUND: Delayed diagnosis of rare orbital diseases is attributed to limited clinical awareness. Building on prior evidence of multimodal large language models (MLLMs) for detecting common ocular conditions, this study aims to evaluate whether integrating multimodal clinical data can enhance the diagnostic accuracy of MLLMs for rare orbital diseases.
METHODS: We conducted a multinational, multiracial, retrospective study. Two datasets were analyzed: Dataset 1, containing 6,786 single-eye photographs from China, was used to fine-tune a contrastive language-image pre-training (CLIP) for prel

Developments in large language models (LLMs) in the past 2 years have shifted the focus from text, image, and audio generation to LLMs capable of multistep reasoning (thinking). The development of LLMs is particularly important for medicine and health care, but the translation of these models has been limited by the black-box nature of previous LLMs. New reasoning-driven LLMs incorporate chain-of-thought prompting and reveal intermediate reasoning steps, offering transparency and traceability, potentially improving the clinical adoption and utility of LLMs. In this Viewpoint, we examine four e

Large language models (LLMs) are increasingly applied in medical education, question answering, and clinical reasoning, yet standardized datasets in non-English contexts remain limited. To address this gap, we present CNMLEQA, a benchmark dataset for evaluating LLMs on the Chinese National Medical Licensing Examination. The dataset integrates question-answer pairs from three sources, including PubMed, GitHub, and MedExamLLM. CNMLEQA comprises two subsets: CNMLEQA-10k (9,890 questions) and CNMLEQA-3k (2,949 questions), each consisting of multiple-choice questions with five options and one corre

OBJECTIVES: Past studies have shown that many clinical machine learning models have performance limitations due to imbalances in the training data. For voice data generation projects, the origin of the problem may lie in the recruiting methods used during data collection efforts. This study introduces a generative AI pipeline for "dataset decision support", recommending recruitment decisions based on high-dimensional insights.
METHODS: The publicly available GOSSIS-1-eICU dataset was filtered to create patient populations that were relevant to voice data generation projects. Lab results and v

Informed consent forms are essential for protecting patient rights, but are often difficult to understand, especially in non-English settings where formal language and long, context-dependent sentences hinder comprehension. This study evaluated whether large language models (LLMs) can simplify Chinese language surgical consent forms and whether clinician revision can further improve the output. Official forms from nine hospitals were used to create three versions of each document: the original, an LLM-simplified version, and a clinician-revised version. We assessed text structure, readability,

Psychotherapy process measures like the Experiencing Scale (EXP) offer valuable insight into clinical interactions but are time-intensive to code. Large language models (LLMs) like ChatGPT have the potential to streamline this process, but empirical validation is nascent. This exploratory study aimed to provide a proof-of-concept coding the EXP using ChatGPT with special attention to ethical considerations, limitations, and future directions. ChatGPT was used to code 79 psychotherapy transcripts drawn from the EXP manual. Multiple models of ChatGPT were tested using varied few-shot learning pr
