The role of cross-sectional geometry, curvature, and limb posture in maintaining equal safety factors: a computed tomography study.

The limb bones of an elephant are considered to experience similar peak locomotory stresses as a shrew. "Safety factors" are maintained across the entire range of body masses through a combination of robusticity of long bones, postural variation, and modification of gait. The relative contributions of these variables remain uncertain. To test the role of shape change, we undertook X-ray tomographic scans of the leg bones of 60 species of mammals and birds, and extracted geometric properties. The maximum resistible forces the bones could withstand before yield under compressive, bending, and to

Indications and long-term assessment of 10 cases of cross-leg free DCIA flaps.

Compound fractures of the lower limb with skin and bone loss are difficult repair problems, especially in multiple level injuries. With bone loss between 6 and 12 cms, the deep circumflex iliac artery (DCIA) flap is usually the flap of choice in reconstruction; the aim is to carry out microvascular anastomosis of the flap vessels to vessels on the damaged leg. However, this may be difficult or even impossible. Experience of 10 cross-leg DCIA flaps is outlined, with indications and surgical technique. The long-term bone healing is compared with 13 cases with anastomosis based on the same leg. T

Calcaneal measurement in estimation of stature of South African blacks.

Stature (height) is an important factor in establishing the identity of a person in the living as well as in the skeletonized state. When stature is estimated from the bones of the limbs, regression equations, which estimate the ratios of the lengths of bones to the height of the individual, are generated. The majority of bones that were used previously were the long bones. The calcaneus was used for estimating stature only in American whites and blacks (Holland [1995] Am. J. Phys. Anthropol. 96:315-320). The regression equations that he generated were found to be useful for stature estimation

Study of ectopic bone formation in experimental spinal cord injured rabbits.

There was no significant difference of incidence of ectopic bone formation between paraplegic rabbits and non-paraplegic rabbits under the same immobilization and passive movement of the posterior legs. This result suggests that ectopic bone may be produced by forcible passive movement of paralytic legs, as legs of paraplegic patients are in an immobilized condition because of the paralysis. Enchondral ossification was significantly observed histologically more frequently in paraplegic rabbits than in non-paraplegic rabbits. We suspect that the reason may be because the paralytic legs are poor

An unusual case of adamantinoma of long bone.

Adamantinoma of the long bones is an exceedingly rare and slow-growing tumor that affects the diaphysis of long bones, particularly the tibia. Based on the pattern of the epithelial cell component and the presence or absence of the osteofibrous dysplasia-like element, several histological variants have been described, such as (i) tubular (the most frequent), (ii) basaloid, (iii) squamous, (iv) spindle variant, (v) osteofibrous dysplasia -like variant, and (vi) Ewing's sarcoma - like adamantinoma (the least frequent). The diagnosis may be challenging since this tumor may be mistakenly interpret

Camurati-Engelmann disease type II: progressive diaphyseal dysplasia with striations of the bones.

We recently found mutations of the transforming growth factor beta 1 (TGF-beta1) gene (TGFB1) in 9 families, in which progressive diaphyseal dysplasia (Camurati-Engelmann disease) is segregating [Kinoshita et al., 2000: Nat Genetics 26:19-20]. During the study, we encountered two unrelated girls, aged 17 and 11 years, who had clinical manifestations of the disorder, such as marfanoid habitus, waddling gait, muscular weakness, intense leg pain, flexion contracture of the hip and knee joints, delayed sexual development, increased serum alkaline phosphatase levels, and increased erythrocyte sedim

Decreased tibial bone strength in postmenopausal women with aseptic loosening of cemented femoral implants measured by peripheral quantitative computed tomography (pQCT).

Aseptic loosening after total hip arthroplasty is related to bone loss of the operated leg. The aim of the present study was to investigate the effect of aseptic loosening on volumetric bone mineral density (vBMD) and bone geometry in the operated leg, in postmenopausal women with a loosened cemented femoral implant using peripheral quantitative computed tomography (pQCT). We matched 12 postmenopausal women with aseptic loosening of cemented femoral implant, with 12 women without aseptic loosening (control group) according to age, BMI, and years from operation. All patients underwent pQCT of b

Hemisoleus muscle flap in the reconstruction of exposed bones in the lower limb.

OBJECTIVES: To evaluate the efficacy of soleus muscle flap for covering complex defects of the middle and lower leg. The study also outlines functional gain after the reconstructive procedure, the donor site morbidity and the technical details of the operative procedure. METHOD: This prospective study consists of a total of 40 patients with middle and lower third defects with exposed bone present in the leg. In all cases, a proximally-based hemisoleus flap was used. All patients were between 15-65 years of age. Depending upon the position of the defect and ease of rotation, either the medial

The influence of gait stance on pedestrian lower limb injury risk.

The effect of pedestrian gait on lower limb kinematics and injuries has not been analyzed. The purpose of this paper was therefore to investigate the effect of pedestrian gait on kinematics and injury risk to the lower limbs using the Total Human Model for Safety adult male pedestrian model together with FE models of vehicle front structures. The modeling results indicate that the tibia and femur cortical bone von-Mises stress and the lateral knee bending angle of an adult pedestrian are strongly dependent on the gait stance when struck by both a sedan car and an SUV at 40km/h. The gait analys

Leg-arm length ratios correlate with severity of apical vertebral rotation in girls after school screening for adolescent idiopathic scoliosis (AIS): a dynamic pathomechanism in the initiation of the deformity?

There is increasing support for the view that the unique human bipedalism and the erect posture are prerequisites for the pathogenesis of adolescent idiopathic scoliosis (AIS). How human bipedalism may contribute to the pathogenesis of AIS is not clear. In normal humans, axial rotations and counter-rotations of the trunk are carried out frequently and forcibly in activities that are not performed by quadrupeds. Some workers have analysed gait in AIS subjects, others have studied torsions in lower limb bones, but there are only two reports on leg-arm ratios in relation to AIS. In this paper, le

Skin behavior during leg lengthening in patients with achondroplasia and hypochondroplasia: a short-term observation during leg lengthening.

BACKGROUND: Although distraction osteogenesis is a recently established method of limb lengthening, it is not well known how soft tissues behave during bone lengthening. Because skin is tissue that can be easily and directly observed, we analyzed skin extension during limb lengthening to study the condition of soft tissues. METHOD: We examined three achondroplastic and two hypochondroplastic patients who underwent bilateral tibial lengthening with unilateral bone lengtheners. To analyze skin extension during leg lengthening, the bilateral skin surface was marked in a latticework pattern, star

Fractures of the talus.

The talus is the most proximal bone of the hindfoot that couples the foot to the leg. It is the second most common fracture of the tarsal bones, second in frequency to the calcaneous. However, overall injuries to the talus are relatively rare, and most surgeons have little experience in managing them. This article discusses fractures of the talus, including injuries to the talar neck, body, head, and processes. Although subtalar dislocations and osteochondral injuries are important topics, they are not addressed in this article.

The management of dog bites and dog bite infections to the hand.

Dog bites account for approximately one of 200 emergency room visits. The majority of bites are from dogs known to the victim. Due to the numerous small compartments and the thin covering of soft tissue over the bones and joints, bite wounds to the hand are more likely to become infected that are bites to the arm, leg, or face. Improper management can result in significant morbidity and prolonged treatment. This article discusses the microbiology of dog bite wounds, antibiotic selection, and proper wound management.

The influence of multifunctional platforms and heat stress on broiler chicken bone health and lameness.

The skeletal health of broiler chickens is a key concern due to their rapid growth rates, which often lead to leg weakness, impacting both animal welfare and production efficiency. Heat stress further compromises bone development by disrupting bone metabolism. Environmental enrichment may increase broiler activity and thereby improve skeletal health. In this study, we evaluated the effects of a multifunctional platform (providing local cooling, shelter, and manure separation) on leg bone development under thermoneutral and heat stress conditions. Three production cycles were conducted using 56

Is differential diagnosis attainable in disarticulated pathological bone remains? A case-study from a late 19th/early 20th century necropolis from Juncal (Porto de Mós, Portugal).

Differential diagnosis is a fundamental step in every palaeopathological study. It is a challenging exercise since many intrinsic and extrinsic factors may negatively impact the accurate interpretation of bone changes in human skeletal remains. Among these, the completeness and preservation of skeletal elements plays a significant role. This study aims to explore the limits of differential diagnosis in the analysis of disarticulated, fragmented bones. The sample consists of twelve adult bone fragments with noticeable changes. The remains were identified in a dis-articulated skeletal assemblage

Biomechanical evaluation of two commonly used external fixators in the treatment of open subtalar dislocation--a finite element analysis.

Subtalar dislocation is a rare injury caused by high-energy trauma. Current treatment strategies include leg casts, internal fixation and external fixation. Among these, external fixators are the most commonly used as this method is believed to provide better stabilization. However, the biomechanical stability provided by these fixators has not been demonstrated. This biomechanical study compares two commonly used external fixators, i.e. Mitkovic and Delta. CT imaging data were used to reconstruct three-dimensional models of the tibia, fibula, talus, calcaneus, navicular, cuboid, three cuneifo

Deflazacort treatment in progressive diaphyseal dysplasia (Camurati-Engelmann disease).

Progressive diaphyseal dysplasia (PDD), a rare disorder of bones, in recent years has been accepted as a systemic disease within the spectrum of connective tissue disorders associated with immunological abnormalities. Steroids have been used in the treatment of PDD with variable success. In this report PDD is described in a 5-year-old boy who presented with leg pain, fatigue, headache and anorexia with an onset in infancy. Physical examination revealed a waddling gait, thorax deformity and thickening in the upper extremities. The diagnosis was made by radiologic demonstration of cortical thick

Bone mineral content of the lumbar spine and lower extremities years after spinal cord lesion.

Bone mineral content (BMC) was measured by dual photon absorptiometry in the lumbar spine, femoral neck and shaft, and proximal tibia in 26 individuals with spinal cord lesions sustained 2 to 25 years previously. In average BMC of the lumbar spine was within the range of normal values. BMC of the femoral neck and shaft was in average 25% lower than the normal values, and for the proximal tibia more than 50% lower than the normal value. Participants with cervical lesions had lower BMC values in the femoral bones than those with thoracic lesions. Neither presence of spasticity nor daily use of l

Treatment Failure and Leg Amputation Among Patients With Foot Osteomyelitis.

We sought to identify factors associated with treatment failure and leg amputations among those patients who presented with foot osteomyelitis. Characteristics, treatments, and outcomes for all patients treated for probable or definite foot osteomyelitis (per consensus definition) between January 2011 and March 2015 were reviewed. Multivariate Cox regression models were used to identify risk factors for treatment failure (unanticipated resection of additional bone or leg amputation) and of leg amputation alone. A total of 184 episodes of foot osteomyelitis met inclusion criteria. Treatment fai
