Gender, nutritional status and disability-free life expectancy among older people in Santiago, Chile.

BACKGROUND: This study was aimed to estimate life expectancy (LE), disability-free life expectancy (DFLE) and disabled life expectancy (DLE) among older adults from Santiago, Chile, and to determine the existence of differences by gender and by body mass index (BMI) categories in these indicators. METHODS: A sample of 1216 people aged 60 or more, from the Chilean cohort of the Study of Health, Ageing and Well-Being was recruited in 2000; two follow-up assessments were carried out in a 10-year period. Functional limitation was assessed through self-report of difficulties in activities of daily

Discontinuation of Preventive Medicines in Older People with Limited Life Expectancy: A Systematic Review.

BACKGROUND: In the presence of multimorbidity and limited life expectancy (LLE), the need for continued use of preventive medicines becomes uncertain as they may neither improve health nor confer continued health benefits. OBJECTIVE: Our objective was to systematically review the literature to examine the discontinuation of preventive medicines in older people with LLE. METHODS: A systematic literature search was conducted using the Ovid MEDLINE, Embase, Cumulative Index to Nursing and Allied Health Literature, and the Cochrane Central Register databases. Studies investigating discontinuatio

How much of the difference in life expectancy between Scottish cities does deprivation explain?

BACKGROUND: Glasgow's low life expectancy and high levels of deprivation are well documented. Studies comparing Glasgow to similarly deprived cities in England suggest an excess of deaths in Glasgow that cannot be accounted for by deprivation. Within Scotland comparisons are more equivocal suggesting deprivation could explain Glasgow's excess mortality. Few studies have used life expectancy, an intuitive measure that quantifies the between-city difference in years. This study aimed to use the most up-to-date data to compare Glasgow to other Scottish cities and to (i) evaluate whether deprivati

Optimal selection of patients for elective abdominal aortic aneurysm repair based on life expectancy.

OBJECTIVE: Elective abdominal aortic aneurysm (AAA) repair is beneficial when rupture is likely during a patient's expected lifetime. The purpose of this study was to identify predictors of long-term mortality after elective AAA repair for moderately sized AAAs (<6.5-cm diameter) to identify patients unlikely to benefit from surgery. METHODS: We analyzed 2367 elective infrarenal AAA (<6.5 cm) repairs across 21 centers in New England from 2003 to 2011. Our main outcome measure was 5-year life-table survival. Cox proportional hazards analysis was used to describe associations between patient ch

How to determine life expectancy change of air pollution mortality: a time series study.

BACKGROUND: Information on life expectancy (LE) change is of great concern for policy makers, as evidenced by discussions of the "harvesting" (or "mortality displacement") issue, i.e. how large an LE loss corresponds to the mortality results of time series (TS) studies. Whereas loss of LE attributable to chronic air pollution exposure can be determined from cohort studies, using life table methods, conventional TS studies have identified only deaths due to acute exposure, during the immediate past (typically the preceding one to five days), and they provide no information about the LE loss per

Cured tuberculoid patients have a greater life expectancy than cured lepromatous patients in Japan.

Leprosy patients lack specific cellular immunity against Mycobacterium leprae, but other immunological functions are thought to be preserved. However, in a leprosy sanatorium in South Japan between 1982 and 2000, we found that the average age at death of cured lepromatous leprosy patients was about 5 yrs younger than that of cured tuberculoid patients; [male/lepromatous, 76.0 +/- 10.0 yrs old vs. male/tuberculoid, 79.7 +/- 9.4 yrs old, p = 0.026], and [female/lepromatous, 78.0 +/- 10.5 vs. female/tuberculoid, 85.3 +/- 9.8, p = 0.0001]. This trend was also observed in autopsy records of two oth

Sex differential in life expectancy in Japan and Scotland: age and causes of death.

The contribution of age groups and causes of death to the sex difference in life expectancy (SDLE) at birth in Japan and Scotland was estimated for the period 1965-1990. The purpose was to determine the particular age groups and causes of death responsible for the opposite trend of SDLE in the two countries. SDLE has been widening and narrowing in Japan and Scotland, respectively. The availability of complete and reliable data for these two developed countries facilitated the study. A method of decomposing the total SDLE into age and cause of death components was employed. About 40-60% contrib

Life expectancy after hip arthroplasty. Case-control study of 1018 cases of primary arthrosis.

The long-term survival of 1018 total hip arthroplasty (THA) patients (237 McKee-Farrar, 449 Brunswik and 332 Lubinus patients) operated on for primary arthrosis was compared with pair-matched controls (i.e., patients operated on for other orthopedic indications) and with a normal population. The 10-year survival after surgery for the McKee-Farrar patients was 85 percent, for the Brunswik patients 82 percent, for the Lubinus patients 82 percent and for the orthopedic control patients 84 percent, respectively. The 10-year survival after 65 years of age for the THA patients was 78 percent and for

[Radical nature and life expectancy in the surgical treatment of esophageal and cardial carcinoma].

Fifty-four patients with carcinoma of the oesophagus and cardia were selected to have either a two-cavity radical operation or blunt oesophagectomy without thoracotomy. Squamous-cell carcinomas were randomized for postoperative radiotherapy or none. Neither the two-cavity radical operation nor additional radiotherapy prolonged life expectancy. The results cast doubt on the effectiveness of local radical or curative surgery for carcinoma of the oesophagus and cardia. The only objective in the choice of treatment must be to decrease operative mortality and to make the course of the disease toler

Changes in long-term life expectancy and years of life lost following the Great East Japan Earthquake in Fukushima Prefecture.

Prolonged evacuation after disasters increases the risks of diseases and challenges in healthcare access. This study aimed to analyze changes in life expectancy (LE) and years of life lost (YLL) in Fukushima Prefecture after the Great East Japan Earthquake. LE and YLL were calculated based on a dataset of 276,314 deaths in Fukushima Prefecture from January 2006 to December 2018 and were aggregated and compared in three time periods: 2006-2010, 2012-2015, and 2016-2018. LE and YLL were obtained using a life table method, focusing on four major causes of death: heart diseases, cerebrovascular di

Life-expectancy paradox and the selection of older patients with aortic stenosis for valve intervention.

The natural history of symptomatic severe aortic stenosis (AS) heralds a poor prognosis-a paradigm that rightly drives valve intervention. A widely voiced corollary holds that once patients exceed their nation's average life expectancy, aortic valve replacement offers no prognostic benefit. We argue that this corollary is statistically incoherent and potentially clinically harmful. Life expectancy at birth is a period average across a whole cohort, weighted by deaths at younger ages; it is not a ceiling on individual survival. Expectancy is conditional on survival already achieved: the longer

Obesity, insufficient physical activity and life expectancy across 172 countries: a longitudinal ecological panel study.

BACKGROUND: Obesity and insufficient physical activity are major global health risks, but their population-level associations with life expectancy may overlap because both evolve with economic and epidemiological transition. OBJECTIVE: To compare the strength and robustness of the associations of adult obesity prevalence and insufficient physical activity with life expectancy at birth in a longitudinal ecological panel of countries. MATERIAL AND METHODS: Annual country-level data for 2000-2020 were analysed. The full source panel contained 4,095 country-year observations from 195 countries,

Life expectancy and determinants of relative survival following surgical aortic valve replacement for aortic stenosis.

AIM: Surgical aortic valve replacement (SAVR) improves survival in severe, symptomatic aortic stenosis (AS), but whether it fully restores life expectancy is uncertain. This study aimed to evaluate long-term relative survival following SAVR compared with the general population and identify predictors of any residual survival discrepancies. METHOD: A total of 1287 patients (median age 71 (63-77) years, 32% female) undergoing SAVR for ≥moderate AS were identified from the Western Australian Heart Valves Study database. Flexible parametric models were used to estimate relative survival. RESULTS

Survival and life expectancy inequality by gender in Thai provinces: Trends from 2015 to 2023.

BACKGROUND: Gender disparities in survival and life expectancy are indicative of broader health and socio-economic inequalities. This study examines temporal trends and subnational variations in gendered survival outcomes across Thailand's 77 provinces between 2015 and 2023, emphasizing the impacts of the COVID-19 pandemic and environmental factors. For the first time, comprehensive provincial life tables have been produced for all Thai provinces for this period. METHODS: Using civil registration data, life tables were constructed, from which life expectancy at birth ([Formula: see text]), at

The Impact of a Menthol Cigarette Ban on Tobacco Use, Lung Cancer, and Life Expectancy.

INTRODUCTION: The Food and Drug Administration proposed a rule to ban the sale of menthol cigarettes, but recently withdrew the proposal. This study uses simulation modeling to estimate the impact of a national menthol cigarette ban on tobacco use, life expectancy, and lung cancer. METHODS: We used the Tobacco Use Individual-level Simulation and Tracking (TwIST) Model, a validated microsimulation model of tobacco use in the US adult population. The simulated population and model parameter estimates are primarily informed by data from the Population Assessment of Tobacco and Health Study. We u

Adiposity in Adults and Life Expectancy with and without Cardiovascular Disease. The Doetinchem Cohort Study.

BACKGROUND: While life expectancy (LE) in the Netherlands has increased over recent decades, it is not always accompanied by good health, particularly among individuals with adiposity. As adiposity is a major risk factor for cardiovascular disease (CVD), previous studies have explored its association with LE and CVD burden, but findings remain heterogeneous. OBJECTIVE: To assess the association between adiposity and LE in adults, with and without CVD. METHODS: We used data from the longitudinal Doetinchem Cohort Study (DCS), including 2,323 participants aged 50-70 years (49% women). Adiposit

Association of wearable-device-measured fragmented sedentary behavior with cardiovascular disease, mortality, and life expectancy.

OBJECTIVES: It remains unclear whether the degree of fragmentation modifies the adverse health outcomes associated with prolonged sedentary behavior. This study aims to investigate the associations of objectively measured sedentary patterns with incident cardiovascular disease (CVD), mortality, and life expectancy. METHODS: This prospective cohort study included 89 010 UK Biobank participants (median age 63.5 years, 56.5% women) who provided 3 to 7 valid days of wrist accelerometer data. Daily sedentary time and number of sedentary breaks were the metrics of interest. Four overall sedentary b
