Emergency Department Pediatric Readiness and Short-term and Long-term Mortality Among Children Receiving Emergency Care.

IMPORTANCE: Emergency departments (EDs) with high pediatric readiness (coordination, personnel, quality improvement, safety, policies, and equipment) are associated with lower mortality among children with critical illness and those admitted to trauma centers, but the benefit among children with more diverse clinical conditions is unknown. OBJECTIVE: To evaluate the association between ED pediatric readiness, in-hospital mortality, and 1-year mortality among injured and medically ill children receiving emergency care in 11 states. DESIGN, SETTING, AND PARTICIPANTS: This is a retrospective co

Cultural competence in dementia care: Care workers and managers perspectives on physical and social environments in Long-Term Care.

This study explores how cultural identity can be preserved in dementia long-term care as resident populations become increasingly diverse. Using a deductive approach informed by the Person-Environment fit framework, the study examines care workers' and managers' experiences and strategies across physical and social environments. Findings show that indoor and outdoor settings, communication practices, activities, relationships, and cultural rituals all influence culturally responsive care. Competing priorities such as safety, limited resources, and varying cultural competence, shape the long-te

Effect Evaluation of the Long-Term Care Insurance (LTCI) System on the Health Care of the Elderly: A Review.

BACKGROUND: How to cope with the rapid growth of LTC (long-term care) needs for the old people without activities of daily living (ADL), which is also a serious hazard caused by public health emergencies such as COVID-2019 and SARS (2003), has become an urgent task in China, Germany, Japan, and other aging countries. As a response, the LTCI (long-term care insurance) system has been executed among European countries and piloted in 15 cities of China in 2016. Subsequently, the influence and dilemma of LTCI system have become a hot academic topic in the past 20 years. METHODS: The review was ca

Characteristics and unmet care needs of unbefriended residents in long-term care: a qualitative interview study.

Objectives: This study explored the impact of being 'unbefriended' for residents in Canadian long-term care (LTC) homes. Residents are 'unbefriended' if they lack decision-making capacity and family or friends to act as their legal representative. Research suggests that unbefriended individuals may have unmet needs and experience poor quality of care due to their limited social support. Our specific objectives were to identify resident characteristics, their unmet care needs, and implications for quality of care and quality of life. Methods: We conducted semi-structured interviews with 39 LTC s

Healthcare costs for the elderly in Japan: Analysis of medical care and long-term care claim records.

BACKGROUND: The population is aging rapidly in many developed countries. Such countries need to respond to the growing demand and expanding costs of healthcare (HC) for the elderly. Therefore, it is important to investigate the factors correlating such HC costs. In Japan, HC is composed of two sections, namely medical care (MC) and long-term care (LTC). While many studies have examined MC and LTC costs on their own, few studies have conducted comprehensive investigations of HC costs. The aim of this study is to examine the risk factors that influence HC costs for the elderly who enroll in the

The Association Between Characteristics of Care Environments and Apathy in Residents With Dementia in Long-term Care Facilities.

PURPOSE: Apathy is highly prevalent in dementia but often overlooked. Environment-based interventions have demonstrated positive impact on apathy, yet, influential environmental components are largely understudied. This study examined the relationship between care environments and apathy in long-term care residents with dementia. DESIGN AND METHODS: This study was exploratory and employed a descriptive and repeated observation design. A sample of 40 was selected from a parent study with 185 participants from 28 facilities. Three videos from each participant were coded to measure apathy and en

Professional oral health care prevents mouth-lung infection in long-term care homes: a systematic review.

BACKGROUND: Nursing home-acquired pneumonia (NHAP) is the leading cause of mortality among residents in long-term care (LTC) homes. Aspiration pneumonia (AP) is one cause of NHAP. Professional oral health care (POHC) and daily mouth care can be effective in decreasing AP risk. AIM: To identify, appraise, synthesize, analyze, and interpret results on the effectiveness of onsite POHC interventions/programs delivered to LTC home residents in reducing oral disease and NHAP. To summarize the findings and provide recommendations for clinical work and future research. METHODS: The PICO question add

Healthcare-Associated Adverse Events in Alternate Level of Care Patients Awaiting Long-Term Care in Hospital.

INTRODUCTION: A growing number of Canadian older adults are designated alternate level of care (ALC) and await placement into long-term care (LTC) while admitted to hospital. This creates infrastructural challenges by using resources allocated for acute care during disproportionately long hospital stays. For ALC patients, hospital environments maladapted to their needs impart risk of healthcare-associated adverse events. METHODS: In this retrospective descriptive study, we examined healthcare-associated adverse events in 156 ALC patients, 65 years old and older, awaiting long-term care while

Short-term and long-term success of electrical cardioversion in atrial fibrillation in managed care system.

BACKGROUND: Initial success of electrical cardioversion (ECV) of atrial fibrillation (AF) has been reported in several studies as 50%-90%, of which only 50% patients remain in sinus rhythm (SR) at the end of one year. We conducted this study to see if outcomes of other trials are applicable in managed care setting. METHODS: We conducted a retrospective study in 370 consecutive patients who underwent ECV for AF. They were reviewed for initial outcome of ECV and recurrence of AF after a successful ECV, with and without prophylactic antiarrhythmic drugs. RESULTS: Initial success of ECV for AF w

A community-academic partnership to improve palliative care in post-acute and long-term care settings.

Palliative care in post-acute and long-term care (PALTC) settings is often inadequate due to limited access to trained clinicians, leaving seriously ill residents with unmet needs. We established a partnership between a national PALTC organization and an academic school of nursing to build the infrastructure for improving palliative care capacity. This paper describes the formation of a partnership and the initial efforts to lay the groundwork to improve palliative care delivery in PALTC settings. Guided by community-engaged research principles and an adapted comparative effectiveness research

Market Care and Custody: A Health Policy Analysis of Incarceration and Long-Term Care Systems in the U.S.

The United States has one of the world's largest criminal justice systems, with nearly 5.5 million people under correctional supervision and almost 2 million incarcerated. This scale of confinement, coupled with the rise of privatization across correctional and related services, reflects a broader neoliberal trend in public governance. This paper explores the consequences of privatization and marketization of the U.S. criminal justice system-particularly the proliferation of private prisons and immigrant detention centers-and draws parallels to the consequences of privatization of health and s

Developing a Frailty Care Pathway for Older Adults in Long-Term Care: A Modified Delphi Process.

OBJECTIVE: We developed a clinical care pathway for the detection and management of frailty for older adults living in long-term care (LTC) homes. METHODS: We utilized a modified Delphi with residents of LTC homes experiencing frailty, their caregivers, and care providers. The pathway was created using existing literature and input from key LTC experts. FINDINGS: Fifty-two panelists completed round one of the Delphi, and 55.8% of these respondents completed round two. Both rounds had high agreement and ratings. We added six new statements following analysis of round two, and 15 statements we

Understanding the transition to long-term care: Perceptions of family care partners of older adult Veterans.

INTRODUCTION: Veterans and their family care partners represent a unique demographic with specific needs that may differ from those of the general population. The transition to a long-term-care home (LTCH) has the potential to profoundly affect older adult Veterans and their care partners, yet research on how this move affects them is limited. This study explored the experiences and needs of older adult Veterans and their family care partners transitioning to a LTCH from the perspective of the care partner. METHODS: A qualitative descriptive design was used for this project. One-time semi-str

Shared care across the interface between primary and specialty care in management of long term conditions.

BACKGROUND: Shared care has been used in the management of many chronic conditions with the assumption that it delivers better care than primary or specialty care alone; however, little is known about the effectiveness of shared care. OBJECTIVES: To determine the effectiveness of shared care health service interventions designed to improve the management of chronic disease across the primary/specialty care interface. This is an update of a previously published review.Secondary questions include the following:1. Which shared care interventions or portions of shared care interventions are most

Improving the care of people with long-term conditions in primary care: protocol for the ENHANCE pilot trial.

BACKGROUND: Long-term conditions (LTCs) are important determinants of quality of life and healthcare expenditure worldwide. Whilst multimorbidity is increasingly the norm in primary care, clinical guidelines and the delivery of care remain focused on single diseases, resulting in poorer clinical outcomes. Osteoarthritis, and anxiety and/or depression frequently co-occur with other LTCs, yet are seldom prioritized by the patient or clinician, resulting in higher levels of disability, poorer prognosis, and increased healthcare costs. OBJECTIVE: To examine the feasibility and acceptability of an
