BACKGROUND: Patients with isolated lower extremity gunshot wounds are currently admitted for observation and often undergo angiography. We hypothesized that if such patients have a normal ankle-brachial index (ABI), they can be discharged safely from the emergency department without invasive imaging or admission.
STUDY DESIGN: We retrospectively reviewed the records of hemodynamically stable patients with isolated lower extremity gunshot wounds seen at our urban, university-based trauma center and who were discharged from the emergency department. Evaluation consisted of determining which pat

Peripheral arterial disease (PAD), a manifestation of systemic atherosclerosis, is a significant health problem. It manifests in lower extremities as intermittent claudication, limb ischemia, or gangrene and other locations as stroke, renal failure, or mesenteric ischemia. Fontaine and Rutherford classifications are the 2 commonly used classifications to stage the severity of PAD. The diagnostic tools include ankle-brachial index, a valuable tool in diagnosing lower extremity PAD, and a treadmill test. Other useful diagnostic tools include the San Diego Claudication Questionnaire to screen pat

BACKGROUND: This study was performed to evaluate when patients recover sufficiently to drive an automobile after operative repair of various other lower extremity fractures.
METHODS: A computerized driving simulator was developed and tested. Three groups of individuals were compared: (1) 12 healthy, volunteers tested once to establish normal mean values for variables tested (group I); (2) 22 patients with right-sided long bone lower extremity fractures (9 femur, 13 tibial shaft) tested at 6 weeks, 9 weeks, and 12 weeks after operative repair (group II); and 35 patients with right-sided articu

Patients presenting with >14 days lower extremity ischemia are usually considered as chronic; however, patients with subacute (>14 days and <3 months) and chronic (>3 months) occlusions often have organizing thrombus. This is not considered in current recommendations for the treatment of nonacute leg ischemia and, with the increased use of endovascular interventions, more complex lesions with long occlusions are treated percutaneously. Thrombolysis alone has been reported to successfully decrease this clot load in a significant proportion of patients with subacute occlusions, and percutaneous

OBJECTIVE: To determine the prevalence of uncommon ulcers, unrelated to venous or arterial etiology, in patients presenting to vascular clinics.
METHODS: This was a multicenter prospective study of consecutive patients presenting with lower extremity ulceration. The settings were university hospital outpatient centers and venous clinics. A total of 799 limbs in 710 patients with leg ulcers were evaluated. Patients with venous ulcer disease and with evidence of arterial disease with an ankle-brachial index less than 0.7 were excluded from the study. Out of 710 patients, 17 patients with a tota

Mirror movements usually seen in the Klippel-Feil syndrome are most commonly appreciated in the upper extremities. Lower extremity involvement is seen rarely and when observed, is found in conjunction with upper extremity mirror movements. We report what we believe to be the first case of mirror movements found exclusively in the lower extremities in a female patient presenting with tethered cord syndrome. Our hopes are that this report will help elucidate mechanisms involved with these anomalous movements, as currently there is no commonly accepted etiology.

A diverse set of conditions affect the lower extremities in athletes. Good history taking and knowledge of the anatomy of the affected areas can aid in accurate diagnosis. Most conditions can be treated conservatively with great success. Familiarity with rehabilitation strategies can assist physicians in working with physical therapists. New advances, especially in the therapy of plantar fasciitis, may aid physicians in more rapid treatment of conditions of overuse. Surgery can provide definitive relief from symptoms for athletes with whom conservative measures have failed.

OBJECTIVE: To review the actual concepts concerning perioperative nerve injuries during surgical or anesthetic procedures.
MATERIAL AND METHODS: A summary of published medical literature from Medline search files and published reviews. CONFLICT IF INTEREST: We have not conflicts of interest.
RESULTS: Perioperative nerve injuries during surgical and anesthetic procedures of the lower extremities are the most common. Many possible etiologies have been proposed to explain perioperative nerve injury and include stretch, compression, and ischemia. Commonly this injuries are under recognition and

The aim of this study was to identify the incidence of lower extremity amputations (LEAs) in diabetics in Poland as background data for comparisons over time and with other communities. This was a cross-sectional study of the incidence of any non-traumatic and non-neoplastic LEAs in the population of Krakow region (1,239,703 inhabitants in its borders before 1.1.1999). It was calculated that for this population size it was sufficient to collect data for one year. The amputees had to be inhabitants of the Krakow region between 1.1.1996 and 31.12.1996. Data was collected from two sources: surgic

Onchocerciasis, or river blindness, is a parasitic infection caused by the filarial nematode, Onchocerca volvulus. It infects 18 million people worldwide, but is rarely seen in the United States. It is one of the leading causes of blindness in the developing world. Although onchocerciasis is also known as river blindness, it is not just a disease of the eyes, but rather a chronic multisystem disease. Clinically, onchocerciasis takes three forms: 1) eye disease; 2) subcutaneous nodules; and 3) a pruritic hypopigmented or hyperpigmented papular dermatitis. We present an 18-year-old African femal

A review of saphenectomy site complications following lower extremity revascularization was conducted. Leg incisions used for 133 consecutive infrainguinal bypass procedures were categorized by location. Patient and procedural risk factors were analyzed for risk of wound complications. Procedure, limb and patient outcome were reported via life table analysis. Incisional wound complications followed 32/133 procedures (24%), including 15 groin, eight saphenectomy, five distal and four vein/distal incisions. There were five grade I and three grade II saphenectomy complications. Only weight (body

Below-the-knee right leg hypoplasia and ipsilateral toe brachysyndactyly were observed in a 4-year-old female with an otherwise normal phenotype. Electromyographic and nerve conduction studies were normal. The Doppler evaluation was consistent with a 50% reduction in the blood supply from the femoral artery, suggesting vascular disruption as the pathogenic mechanism. Our observations support the hypothesis that a lower extremity counterpart of the Poland syndrome does exist and that the extent of limb involvement is dependent on the level of vascular disruption.

Disorders of the ankle and foot are common and may pose a significant clinical problem. Acute injuries of the ankle and foot are routinely evaluated by radiographs and, if inconclusive, by magnetic resonance imaging (MRI) or computed tomagraphy (CT). In subacute or chronic injuries, where prolonged pain is unexplained, bone scintigraphy may play a significant role. The bone scan may differentiate soft tissue from bone pathology, and, being a sensitive test, it may indicate the region that needs further specific radiologic examination. It may also indicate the clinical significance of a radiolo

BACKGROUND: Color-duplex ultrasound (CDU) surveillance of arterial bypass grafts has been validated, but the natural history of "failing" grafts remains poorly defined. Our purpose was to compare failing grafts having prophylactic revision with those that did not.
METHODS: Postoperative duplex surveillance was performed in an accredited vascular laboratory for all lower extremity bypass grafts performed at a single institution. Eighty-five infrainguinal grafts (57 vein, 21 polytetrafluoroethylene (PTFE), and 7 composite grafts) in 83 patients were identified as failing by accepted criteria. T

Patients with traumatic injury to the lower extremities are admitted to the hospital with multiple interrelated problems requiring an interdisciplinary team approach to meet their needs. This article will explore the assessment and formulation of nursing diagnoses, implementation of care, and patient outcomes achieved. It will describe the nurse's role as the link between the patient and the physician and other members of the health care team. The cooperative actions of the different health care teams that work to achieve the desired outcomes are explained. A case study will be presented to il

Mucormycosis is an uncommon, but important opportunistic fungal infection. The most commonly reported areas of infection are pulmonary, rhinocerebral and cutaneous. It has only rarely been reported to cause peripheral neurological symptoms. This report presents the case of a patient who presented with ascending paralysis due to pulmonary mucormycosis. The presentation is believed to be due to a lesion which abutted the vertebral column and caused infarction of the spinal cord.
