Prediction of Postoperative Complications Following Mandibular Fractures Using Machine Learning.

Postoperative complications in oral and maxillofacial surgery (OMFS), pose a significant challenge to patient recovery. Accurate prediction of such complications could enable earlier interventions and improve clinical outcomes. Advances in machine learning (ML) demonstrated potential in analysing complex data within healthcare, yet their application in OMFS remains limited. Here, the use of ML models, particularly random forest is explored, to predict postoperative complications for mandibular fractures, utilising demographic, clinical, and surgical data. With nested cross validation an accura

Surgical management of bilateral mandibular angle fractures: A systematic review.

Bilateral mandibular angle fractures (BMAF) are rare and complex maxillofacial injuries. This review describes the patient factors, procedural factors and post-procedural outcomes of BMAF management. PubMed and Scopus were searched for English papers from 1993 to 2023 using the strategy: (bilateral AND angle) AND (mandib* OR bmaf) AND fracture AND (management OR repair OR surgery). Four papers were eligible for inclusion, representing 198 cases. The mean age of patients was 25 years, with 94.9% of cases male. Interpersonal violence was the leading cause of injury (76.8%). The surgical management of triple mandibular fractures: A challenging task.

OBJECTIVE: Cisplatin is a chemotherapeutic agent with an ototoxic effect that is frequently used in head and neck cancers. There are studies in the literature conducted with various antioxidant substances to protect and/or prevent ototoxicity. This study aims to investigate whether platelet-rich plasma (PRP) has a protective and therapeutic effect on cisplatin ototoxicity. METHODS: A total of 40 Sprague Dawley albino rats were divided into six groups as control group (n=6), PRP-only group (n=6), cisplatin group (n=6), cisplatin + PRP group (n=6), PRP + cisplatin group (n=6), and donor group (

Preoperative Prophylactic Antibiotics in Mandibular Fractures and Surgical Site Infection.

BACKGROUND: Compared with other facial fractures, mandibular fractures have the highest rate of postsurgical site infection. There is strong evidence to suggest that postoperative antibiotics do not reduce rates of surgical site infections (SSIs) regardless of duration. However, there are conflicting data in the literature regarding the role of prophylactic preoperative antibiotics in reducing rates of SSIs. This study reviews the rate of infections in patients who underwent mandibular fracture repair who received a course of preoperative prophylactic antibiotics compared with those who receiv

Mini - Retromandibular access to sub condylar mandibular fractures - Our experience.

Condylar fractures alone accounts to about 25% to 40% of all the fractures of mandible. Management of condylar fractures has always been a controversy. Nowadays there has been more emphasis on open reduction of condylar fractures by the surgeons. The reasons could be the result of complications of closed reduction where the patient may not be able to masticate properly and deviation still present thereby the structural and functional loss forcing the surgeons' choice to open up. The anterior parotid approach has lesser risk of injury to parotid gland and also to facial nerve we attempted to use

Detection of mandibular fractures on panoramic radiographs using deep learning.

Mandibular fractures are among the most frequent facial traumas in oral and maxillofacial surgery, accounting for 57% of cases. An accurate diagnosis and appropriate treatment plan are vital in achieving optimal re-establishment of occlusion, function and facial aesthetics. This study aims to detect mandibular fractures on panoramic radiographs (PR) automatically. 1624 PR with fractures were manually annotated and labelled as a reference. A deep learning approach based on Faster R-CNN and Swin-Transformer was trained and validated on 1640 PR with and without fractures. Subsequently, the traine

A working paradigm for managing mandibular fractures under regional anesthesia.

OBJECTIVES: Isolated mandibular fractures contribute to approximately 45% of maxillofacial traumas. Improper management of mandibular fractures can cause myriad potential complications and can lead to serious functional and aesthetic sequelae. The objective of the study is to design a stepwise approach for managing isolated mandibular fractures using open reduction and internal fixation (ORIF) with regional anesthesia on outpatient basis. MATERIALS AND METHODS: Patients with isolated mandibular fractures presenting to the department of maxillofacial surgery were selected for ORIF under region

[Needle fractures during mandibular block: prevention and emergency care algorithm].

Cases of dental needle fracture during mandibular block were studied in the last 10 years. Despite dental anesthesia development, the cases of needle fracture occur regularly in the world practice. Some of these complications has iatrogenic nature while others may be attributed to independent reasons like patients' sharp movement and needle manufacturing defect. In most of the cases the needle migrates to surrounding anatomical spaces making operative removing a challenging task. Subsequent surgical removing of a foreign body has severe consequences to patient in the form of a large operating

Feasibility of purely endoscopic intramedullary fixation of mandibular condyle fractures.

The investigators of this study hypothesized that fractures of the mandibular condyle can be repaired using short-segment intramedullary implants and purely endoscopic surgical technique, using a basic science, human cadaver model in an academic center. Endoscopic instrumentation was used through a transoral mucosal incision to place intramedullary implants of 2 cm in length into osteotomized mandibular condyles. The surgical maneuvers that required to insert these implants, including condyle positioning, reaming, implant insertion, and seating of the mandibular ramus, are described herein. Pr

Reduction and fixation of mandibular condylar fractures with neck screw.

PURPOSE: The purpose of the present study is to describe a surgical technique for the treatment of condylar fractures using an intramedullary screw. MATERIALS AND METHODS: The treatment of these fractures remains very controversial. At the time of the choice of the surgical treatment, a wide range of accesses and of techniques of reduction and fixation is used, depending on the type of the fracture, the experience of the surgeon, and aesthetical issues. Among the difficulties posed by this type of fracture is the correct repositioning of the fractured condylar portion for which we have develo

Biomechanical analysis of fractures in the mandibular neck (collum mandibulae).

After treatment of fractures in the neck of the mandible by means of immobilization of the dentition, often more or less severe manifestations of malocclusion remain. It was hypothesized that this is caused by an altered articulation in the jaw joint on the affected side. Furthermore, it was hypothesized that an anteriorly displaced condyle, as observed frequently as a side effect of the treatment, is caused by pull of the lateral pterygoid muscle, despite maxillomandibular fixation. Intervention experiments were performed in silico to test these hypotheses. With a biomechanical model of the h

Background. Fractures of the condylar process of the mandible, whether isolated or coexisting with other injuries, are often difficult to manage. Treatment outcome in patients with mechanical injuries of the temporomandibular joints and fractures of mandibular condylar process, which is an element of this joint, depends mostly when and how treatment is initiated. The goal of our study was to assess functional abnormalities in patients following fractures of the condylar process of the mandible in relation to the treatment method used. Material and methods. We analyzed patient documentation and
