This study aimed to evaluate the applicability of the 10-minute submaximal treadmill test (T10 test), a self-paced test, in determining critical speed (CS) and predicting running performance. Specifically, we sought to identify the percentage of T10 velocity (vT10) that runners performed in official distance races, and to compare physiological and performance indicators between sexes. 60 recreational runners (n=34 males and n=26 females) underwent a maximum incremental test, the novel T10 test, and ran 1200-m and 2400-m on the track. Runners self-reported their best performance times. Generali

To determine whether sex differences in marathon pacing strategies and susceptibility to catastrophic deceleration ("hitting the wall") persist in a massive, high-performance dataset, and to quantify this risk across different performance levels. We analyzed 873,334 finishers from the Berlin Marathon (1999-2025). Pacing stability was quantified using the percentage slowdown in the second half relative to the first. "Hitting the wall" was operationally defined as a deceleration of ≥ 20%. A stratified analysis was performed to compare risks between sexes across five performance categories (Compe

Ultra-marathon running - where races are held in distance-limited (50 km, 50 miles, 100 km, 100 miles, etc.), time-limited (6 h, 12 h, 24 h, 48 h, 72 h, etc.), and multi-stage races - is gaining in popularity. However, we have no knowledge of where the fastest 48-hour runners originate and where the fastest 48-hour races are held. This study tried to determine the origin of the fastest 48-hour runners and the predictor factors associated with 48-hour ultra-marathon performance, such as age, gender, event country, country of origin and race course specific characteristics. Ultramarathon running elicits a profound inflammatory response, characterized by significant increases in interleukin-6 (IL-6) and C-reactive protein (CRP), with comparatively modest changes in tumor necrosis factor-alpha (TNF-α). We reviewed approximately 80 field studies of ultramarathon events (distances >42.2 km) that measured IL-6, CRP, and TNF-α before and after races. IL-6 typically spiked immediately post-race-often rising dozens or even thousands of times above baseline-then rapidly declined, usually returning to near baseline within 24-48 h. CRP, an acute-phase protein, exhibited a s

OBJECTIVE: To observe alterations in the microstructure and chemical composition of the Achilles tendon (AT) in amateur marathon runners before and after a race using synthetic magnetic resonance imaging (SyMRI).
METHODS: SyMRI examinations were performed on marathoners at three-time points: pre-race, 48 h post-race, and 2 months post-race. A control group also underwent SyMRI examinations. The AT was segmented into three regions: the muscle-tendon junction (MTJ), insertion (INS), and middle (MID) regions. T1 and proton density (PD) values were recorded in these regions. We compared T1 and PD

IntroductionUltramarathon races cover distances greater than 42.2 km. They are often held on mountain trails with significant challenges, such as inclement weather and rough terrain. Female participation in this sport is on the rise, but potential gynecologic health impacts, such as menstrual changes and pelvic floor dysfunction, are not well characterized.MethodsA convenience sample of female participants at New River Trail 50-km and Promise Land 50-km races were surveyed to determine the gynecologic health impacts of ultra running on female athletes as compared with the general female popula

INTRODUCTION: Previous studies described various adaptive neuroplastic brain changes associated with physical activity (PA). EEG studies focused mostly on effects during or shortly after short bouts of exercise. This is the first study to investigate the capability of EEG to display PA-induced long-lasting plasticity in runners compared to a sedentary control group.
METHODS: Thirty trained runners and 30 age- and sex-matched sedentary controls (SC) were included as a subpopulation of the ReCaP (Running effects on Cognition and Plasticity) study. PA was measured with the International Physical

PURPOSE: Hematopoietic stem cell (HSC) transplant is one of the curative methods for some patients with hematological malignancies. Granulocyte colony-stimulating factor (G-CSF) is the most common drug used to mobilize CD34+ cells, generally found in small numbers. Recent evidence showed that exercise causes transient mobilization in HSC. However, the type and intensity of exercise have not been fully revealed. We aimed to detect a significant increase in stem cell levels following 60 min of running at a personalized running pace.
MATERIALS/METHODS: Eighteen runners, 48.2 ± 1.9 years with

This study examined gastrointestinal temperature (TGI) during and body mass (BM) changes pre/post the Western States Endurance Run 100 (WSER), a 100-mile ultramarathon in hot conditions. Thirty-nine participants ingested a telemetric pill 30-90 min before the WSER start, and BM was measured pre- and post-race. Environmental conditions were recorded every 30 min at 15 locations along the course. In-race TGI data were obtained from 23 finishers (54%; 7 females; 42 ± 11 years), including 2 podium finishers and an additional top-10 finisher. Participants finished in 14-18 h (n = 4), 19-25 h (n = 6

AIM: Regular exercise is beneficial, but more exercise may not always benefit cardiovascular health (extreme exercise hypothesis). This is concerning, because ultramarathon participation is growing, but previous work on cardiovascular health among ultramarathon runners is equivocal. Prior work has not examined this population's age-related differences in prognostic cardiovascular health metrics.
PURPOSE: Measure brachial and central BP and arterial stiffness (carotid-to-femoral pulse wave velocity [cfPWV]) among ultramarathon runners.
METHODS: We measured supine BP and cfPWV (SphygmoCor-XCEL

Footwear has the potential to reduce soft-tissue vibrations (STV) but responses are highly subject-specific. Recent evidence shows that compressive garments minimizing STV have a beneficial effect on neuromuscular (NM) fatigue. The aim was to determine whether an individualized midsole hardness can minimize STV and NM fatigue during a half marathon. Twenty experienced runners were recruited for three visits: a familiarization session including the identification of midsole minimizing and maximizing STV amplitude (MIN and MAX, respectively), and two half marathon sessions at 95% of speed at the

BACKGROUND: Improved treatment options of HIV have resulted in regular physical activities of many HIV-infected patients. However, data on effects of sports in HIV-patients are scarce.
METHODS: 21 HIV-infected persons were monitored prospectively while preparing for a marathon run. Multiple parameters with regard to immunology, quality of life and metabolism were measured at 4 time points (at baseline 1 year before the marathon run, 3 and 6 months after beginning of training, and immediately before marathon).
RESULTS: 13 out of 21 participants completed the marathon (12 male, 1 female; media

OBJECTIVE: This study examined the accuracy of smartwatches in predicting running performance.
METHODS: A total of 154 amateur runners (123 males and 31 females) were recruited. After wearing the HUAWEI WATCH GT Runner for a minimum of six weeks, the runners' actual completion times for 5 km, 10 km, and half marathon distances were measured, resulting in 288 test instances. The predicted completion times for the same distances displayed on the watch on the test day were recorded simultaneously.
RESULTS: The actual and predicted performances for the 5, 10, and 21.1 km distances were highly co

Background: Previous research has identified differences in marathon times and pacing between men and women by age and distance. Purpose: The current study adjusted the differences in times by accounting for differences in age distributions between men and women and assessed gaps in times and pacing by place ranking. Data Collection and/or Analysis: Analyses are based on 2033 men and 1450 women who completed the 2022 St. George Marathon. Results: The average marathon time (hours, minutes, seconds) was 4:17:03 (0:09:49 per mile) for men and 4:32:45 (0:10:25 per mile) for women. The faster time

This study evaluated whether sustained participation in multi-marathon running is associated with elevated aerobic capacity, more gradual age-associated differences in VO2max, and reduced estimated all-cause mortality risk. A global cross-sectional online survey collected self-reported data from 340 multi-marathoners (mean age = 52.2 years; 54.1% men) representing 24 countries, with an average of 121 lifetime marathon completions. VO2max was estimated using wearable fitness device data and analysed by sex and age group. Estimates were compared against normative VO2max trajectories from the FRI

PURPOSE: Calf muscles play an important role in marathon race, and the incidence of injury is high in this process. This study prospectively quantified diffusion tensor metrics, muscle fat fraction (MFF) and cross-sectional area (CSA) of calf muscles induced by endurance exercise in amateur marathoners, and the potential mechanisms underlying the changes in these parameters were analyzed.
METHOD: In this prospective study, 35 marathoners (27 males, 8 females; mean age (standard deviation, SD), 38.92 (4.83) years) and 26 controls (18 males, 8 females; mean age (SD), 38.35 (6.75) years) underwe

Although patients with heart disease have successfully completed marathon runs, the immediate cardiac effects of similar and greater distance endurance exercise competition are unknown. Two such cases are presented, demonstrating that vigorous exercise and extreme levels of fitness are not precluded in the cardiac patient.
