A prospective analysis of nosocomial wound infection after mastectomy.

We evaluated the postoperative course of all patients who had mastectomies from 1978 through 1982 at City of Hope National Medical Center (Duarte, Calif). The overall clean mastectomy wound infection rate was 24/294 (8.2%). The incidence of mastectomy wound infection varied with the method of biopsy and was 3.2% after needle aspiration and 9.5% after open biopsy. Mastectomy immediately after open biopsy ("one step") had an infection rate of 5.3%, whereas mastectomy at a subsequent procedure ("two step") had a rate of 12.4%. The maximal infection rate (23.0%) occurred following the two-step pro

Expected Reduction of The Nipple-Areolar Complex Over Time After Treatment of Gynecomastia with Ultrasound-Assisted Liposuction Mastectomy Compared to Subcutaneous Mastectomy Alone.

BACKGROUND: The enlarged nipple-areola-complex (NAC) is a characterizing aspect of gynecomastia. OBJECTIVE: The purpose of this study was to multidimensionally quantify the reduction of the NAC after a subcutaneous mastectomy (SCM) with or without ultrasound-assisted liposuction (UAL). MATERIALS AND METHODS: A retrospective assessment of patients who underwent SCM +/- UAL due to gynecomastia over a period of 11 years was conducted. The NAC diameters were measured before and after surgery. In addition, a survey (including the BREAST-Q) regarding patient-oriented outcome was performed. RESULT

Systemic Lupus Erythematosus in Complications of Mastectomies With or Without Reconstruction.

INTRODUCTION: Systemic lupus erythematosus (SLE) is a chronic autoimmune disease estimated to affect more than 200,000 patients in the United States. There is a scarcity of population-based studies of SLE as a risk factor for postoperative complications of mastectomies with or without reconstruction. METHODS: A retrospective cohort study was conducted using the TriNetX database. The exposure cohort comprised patients who underwent mastectomies with or without reconstruction with a prior diagnosis of SLE; the control cohort comprised those without. Propensity score matching (1:1) was performed

Analgesic efficacy of pregabalin in dogs undergoing mastectomy with ovariohysterectomy.

The aim of this study was to evaluate the analgesic efficacy of pregabalin in dogs diagnosed with mammary carcinoma undergoing mastectomy with ovariohysterectomy. In a randomized, blinded, clinical, placebo-controlled study, 24 dogs were assigned to receive either an oral pregabalin suspension (4 mg/kg/0.1 mL/kg, Pregabalin group, n = 12) or a placebo solution (0.1 mL/kg, Placebo group, n = 12), administered 60 minutes before and every 8 hours after surgery. The dogs were premedicated with intramuscular (IM) morphine (0.3 mg/kg). Anesthesia was induced with intravenous (IV) propofol at a suffi

Perioperative Pain Management for Mastectomy in Dogs: A Narrative Review.

Mammary tumours are the most common neoplasia in adult female dogs. Mastectomy leads to moderate to severe pain. Effective pain management is crucial in veterinary medicine. This review outlines analgesic techniques for managing perioperative pain in dogs undergoing mastectomy. A literature search on dog mastectomy analgesia was conducted from January 2001 to January 2025. Pre-emptive meloxicam reduces postoperative cardiovascular changes without affecting renal function. When combined with gabapentin, it lowers the need for rescue analgesic opioids, similar to robenacoxib. With regard to tram

The Impact of COVID-19 on "Reconstructive Burnout" Following Mastectomy.

BACKGROUND: Achieving successful breast reconstructive outcomes following mastectomy oftentimes coincides with the completion of a multistep surgical sequence. We hypothesize that the COVID-19 pandemic and subsequent postponement of elective procedures added an additional layer of complexity to this process, contributing to failure to achieve complete reconstruction. METHODS: From January 2018 to January 2022, a retrospective review of all included patients undergoing mastectomy at a single institution, Loyola University Medical Center, was conducted. Reconstructive burnout, a term coined by

The Use of Oxygen Therapy in Management of Mastectomy Flap Ischemia.

Oxygen plays a major role in wound healing by improving vessel growth and facilitating wound closure. Topical oxygen therapy has been widely accepted as a treatment for diabetic foot ulcers, but its benefit in compromised flaps and grafts in the field of plastic surgery has gained traction. It is possible to decrease flap necrosis and increase viability with topical oxygen therapy. We present a case of a 48-year-old woman who received topical oxygen therapy to treat postmastectomy areas of skin necrosis. As the patient underwent treatment using topical oxygen therapy to the mastectomy flaps, s

Are There Disparities in Breast Reconstruction After Contralateral Prophylactic Mastectomy?

INTRODUCTION: Despite national guidelines against contralateral prophylactic mastectomy (CPM) in low- to moderate-risk breast cancer, CPM use continues to rise. Breast reconstruction improves health-related quality of life and satisfaction among women undergoing mastectomy. Given the lack of data regarding factors associated with reconstruction after CPM and the known benefits of reconstruction, we sought to investigate whether disparities exist in receipt of reconstruction after CPM. METHODS: The 2004-2017 National Cancer Database was queried to identify women diagnosed with breast cancer wh

Prepectoral implant reconstruction in the setting of post-mastectomy radiation.

Prepectoral breast reconstruction after mastectomy is a more commonly performed technique in recent years due to its numerous advantages over subpectoral breast reconstruction. This study reviews the current state of clinical outcomes for patients undergoing postmastectomy radiation therapy (PMRT) after prepectoral breast reconstruction. A comprehensive search of the literature was performed using the Preferred Reporting Items for Systematic Review and Meta-Analyses guidelines to identify all relevant studies. Outcome measures included demographics, mean follow-up, and complication measures. T

Regret and Quality of Life After Mastectomy With or Without Reconstruction.

BACKGROUND: Mastectomy represents a deep burden for women with breast cancer. Very little is known about the psychological consequences over time and the quality of life (QoL) of women so treated, with or without breast reconstruction (BR). PATIENTS AND METHODS: A total of 709 patients underwent mastectomy with or without BR between 2002 and 2012 at one institution. Among 468 surviving patients, a 60-query QoL questionnaire on personal issues including some European Organization for the Research and Treatment of Cancer Quality of Life Questionnaire items was presented either by email, letter,

A Technique of Endoscopic Nipple-Sparing Mastectomy for Breast Cancer.

BACKGROUND AND OBJECTIVES: Nipple-sparing mastectomy (NSM) is a widely accepted surgical technique for patients with early breast cancer. The technique improves cosmetic outcomes, but a decrease in nipple sensitivity has been observed with NSM because of the incision into the nipple-areola complex (NAC). Endoscopic nipple-sparing mastectomy with skin lifting system (ENSMSLS) removes all breast tissue through the axillary incision used for the sentinel lymph node biopsy, to avoid incision around the NAC area. With only one incision, NAC sensitivity is less likely to be affected by this techniqu

Nipple- and areola-sparing mastectomy for the treatment of breast cancer.

BACKGROUND: The efficacy and safety of nipple-sparing mastectomy and areola-sparing mastectomy for the treatment of breast cancer are still questionable. It is estimated that the local recurrence rates following nipple-sparing mastectomy are very similar to breast-conserving surgery followed by radiotherapy. OBJECTIVES: To assess the efficacy and safety of nipple-sparing mastectomy and areola-sparing mastectomy for the treatment of ductal carcinoma in situ and invasive breast cancer in women. SEARCH METHODS: We searched the Cochrane Breast Cancer Group's Specialized Register, the Cochrane Ce

Lifetime Costs of Prophylactic Mastectomies and Reconstruction versus Surveillance.

BACKGROUND: The past decade has seen an increasing prevalence of prophylactic mastectomy with decreasing ages of patients treated for breast cancer. Data are limited on the fiscal impacts of contralateral prophylactic mastectomy trends, and no study has compared bilateral prophylactic mastectomy with reconstruction to surveillance in high-risk patients. METHODS: Lifetime third-party payer costs over 30 years were estimated with 2013 Medicare reimbursement rates. Costs were estimated for patients choosing contralateral or bilateral prophylactic mastectomy versus surveillance, with immediate re

Predictors of contralateral prophylactic mastectomy and the impact on breast reconstruction.

BACKGROUND: Contralateral prophylactic mastectomy (CPM) is being performed with increased frequency. Predictors of CPM and their impact on breast reconstruction are examined. METHODS: A retrospective review of a dually trained oncologic and plastic surgeon's experience with patients undergoing total mastectomy from 2002 to 2012 was performed. Patients who underwent bilateral therapeutic mastectomies or who had previous contralateral mastectomy were excluded from this series. RESULTS: Four hundred forty-six patients were treated with total mastectomy and 174 (39%) underwent CPM. The incidence

Necrotizing fasciitis of the breast: a case managed without mastectomy.

OBJECTIVE: To report a rare presentation of necrotizing fasciitis (NF) in the breast and its management. CLINICAL PRESENTATION AND INTERVENTION: A 61-year-old non-diabetic lady presented with a painful swollen right breast and yellowish discharge associated with fever for the last few days. Based on clinical examination and haematological parameters, a provisional diagnosis of breast abscess was made that later proved to be a case of NF. She was managed conservatively with repeated debridement followed by split-skin grafting with preservation of the breast. CONCLUSION: This case showed that

[External periareolar incision for subdermal mastectomy in men with gynecomastia].

BACKGROUND: Gynecomastia describes a benign increase of the mammary gland in men. When medical treatment fails, symptoms and psychological alterations persist. Subdermal mastectomy is the definitive treatment and can be achieved by different incisions, each with potential complications. We undertook this study to present clinical characteristics of 11 patients with gynecomastia and the results obtained with subdermal mastectomy by means of external periareolar incision. METHODS: A descriptive cohort study in male patients with gynecomastia was carried out in a third-level medical care hospita
