Surgical landmarks for identification of the thoracodorsal pedicle during axillary dissection in carcinoma breast patients: a prospective observational study.

BACKGROUND: Axillary lymph node dissection (ALND) plays a pivotal role in breast cancer surgery, serving both staging and locoregional control. The thoracodorsal pedicle (TDP), comprising the thoracodorsal artery, vein, and nerve are critical structures encountered during ALND. Preserving the TDP is essential to minimize postoperative complications, such as impaired shoulder function and arm lymphedema, and to support successful reconstructive outcomes, particularly when utilizing the latissimus dorsi muscle for flap procedures. Accurate identification and careful preservation of the TDP espec

[The familial carcinoma burden and indications for subcutaneous mastectomy].

Histological examination of the surgical specimen was undertaken in 182 women (aged between 20 and 58 years, mean 44 years) after a subcutaneous mastectomy, which had been performed largely because the diagnosis of the breast lesion had been problematic. Among 46 patients, operated on exclusively because of either palpatory or radiological diagnostic problems, histological examination revealed occult carcinoma or precancerous lesion in two, while among 58 patients with combined palpatory and radiological diagnostic problems there were twelve with invasive or noninvasive carcinoma, and among se

[Drainage-free management of patients after radical surgery for breast cancer].

The paper deals with drainage-free management of disease and therapy after radical surgery for breast cancer. The data on the immediate results of surgery were assessed in 168 patients. Suture of a skin flap to the thoracic wall closed a gap in axillary area while ultrasound monitoring of the wound and aspiration of fluid made keeping track of postoperative recovery possible. Wound complications were registered in 51 patients (30.4%) serotoma--35 (20.8%).

[Axillary lymph node excision in breast cancer].

We report on 264 patients with operable breast cancer, treated between 1980 and 1984 at the First Clinic of Obstetrics and Gynecology in Vienna. The operation was in all cases a modified radical mastectomy. The patients were randomized in two groups. In one group of 140 patients, the lymphatic tissue of the axilla was marked by technetium-99 antimonsulfide and these patients were operated on a gammacamera. The other group of 124 patients was not marked. This unspecific imaging of lymph nodes enabled us to increase the average number of removed lymph nodes by 25% in the marked group. In this gr

Splenic Rupture Due to Metastasis of Breast Cancer: A Report of a Rare Case and Focused Literature Review.

The most common primary tumors that metastasize to the spleen include melanoma, lung, colorectal, ovarian, and breast cancers (BC). Nearly all patients with splenic metastases (SM) present with disseminated malignant disease rather than isolated involvement. We report the case of a 58-year-old female patient with BC who developed SM and subsequently died following splenic capsular rupture. The patient initially presented with biopsy-proven BC and underwent neoadjuvant therapy followed by right modified radical mastectomy. She later received adjuvant systemic therapy, radiotherapy, and tamoxife

Male occult triple-negative breast cancer.

Male breast cancer is a rare but important condition accounting for only 1% of breast cancer worldwide and less than 1% of all male malignancies. Occult male breast cancer is an extremely rare type of male breast cancer that manifests as axillary metastases without an identifiable breast lesion. We report a case of triple-negative occult male breast cancer who underwent modified radical mastectomy.

Hydronephrotic ectopic pelvic kidney simulates sacral metastasis from breast cancer.

A 57-year-old woman with a history of a right modified radical mastectomy after neoadjuvant chemotherapy for stage IIIB infiltrating ductal breast carcinoma presented for follow-up bone scan. Initial inspection was suspicious for sacral metastasis, but on closer review the absence of renal activity in the left flank was noted. Correlation with CT scan confirmed a hydronephrotic ectopic kidney overlying the sacrum.

Nipple- and areola-sparing mastectomy for the treatment of breast cancer.

BACKGROUND: The efficacy and safety of nipple-sparing mastectomy and areola-sparing mastectomy for the treatment of breast cancer are still questionable. It is estimated that the local recurrence rates following nipple-sparing mastectomy are very similar to breast-conserving surgery followed by radiotherapy. OBJECTIVES: To assess the efficacy and safety of nipple-sparing mastectomy and areola-sparing mastectomy for the treatment of ductal carcinoma in situ and invasive breast cancer in women. SEARCH METHODS: We searched the Cochrane Breast Cancer Group's Specialized Register, the Cochrane Ce

Evolution in the Management of Axillary Lymph Nodes in Breast Cancer.

As individuals with breast cancer are presenting at earlier stages of disease and with greater biologic characterization, the surgical approaches have progressively de-escalated. In the mid-1900s, radical mastectomy was replaced with modified radical surgery and decreased utilization of level 3 nodal removal. By the early 2000s, sentinel lymph node surgery had replaced axillary dissection for early-stage disease and eventually those that underwent neoadjuvant therapy to downstage their tumors. In the 2020s, de-escalation of axillary surgery to subgroups that no longer require nodal evaluation

Synchronous vulvar and breast cancer.

Synchronous vulvar and breast cancer is rare. A 44-year-old women presented with a lesion in the right labia majora and right upper quadrant breast lump. After work-up, she underwent radical wide local vulvar excision and modified radical mastectomy with axillary lymph node dissection. The pathology of the vulva revealed moderately-differentiated squamous cell carcinoma and that of the breast infiltrating ductal carcinoma. Only two such cases have been previously reported: one was an elderly patient and the second a young patient with HIV infection. Our patient is a young and healthy woman mak

Adenoid cystic carcinoma of the breast with late recurrence and high-grade transformation.

Breast cancers have broad histological subtypes with varied molecular expression, which determines the management. Although ductal and lobular breast carcinomas constitute the significant 90% of these tumours, other subtypes constitute about 10% of breast cancers. Adenoid cystic carcinoma (AdCC) is a salivary gland-type tumour described in breast, constituting less than 1% of all breast carcinomas. These tumours have a favourable prognosis and are surgically managed by either lumpectomy or mastectomy. Solid-basaloid and AdCC with high-grade transformation are the sporadic subtypes of AdCC, in

Submuscular breast reconstruction--indications and techniques.

We review the care and treatment of 93 patients who underwent submuscular breast reconstruction in our service between 1975 and 1980. The indications and clinical situations in which submuscular reconstruction has been useful include: following simple mastectomy; following modified radical mastectomy; following subcutaneous mastectomy (immediate or delayed reconstruction); in standard augmentation mammaplasty in selected patients; and with unusual, unsatisfactory, or dangerous preexisting breast conditions. Unusual indications include unsatisfactory subcutaneous placement with capsule formatio

Refractory headache in a patient with breast cancer and carcinomatous meningitis unresponsive to analgesics: case report.

A 44-year old female presented with locally advanced breast cancer that had been treated with neoadjuvant chemotherapy followed by modified radical mastectomy and thereafter three cycles of paclitaxel. She presented with severe refractory headache that was unresponsive to oral analgesics including morphine. Both CT and MRI scans with contrast were normal, however her cerebrospinal fluid was positive for malignant cells. The patient's headache responded to whole brain radiotherapy.

Primary signet ring cell carcinoma of the breast: A case report and literature review.

Primary signet ring cell carcinoma (SRCC) of the breast is a rare and aggressive type of breast cancer characterized by increased intracellular mucin production. It has a high risk of metastasis and poor prognosis compared to other breast cancer types. We report a 56-year-old woman with primary SRCC of the breast who first presented with retraction on her left breast. Radiological examination revealed a mass that causes the retraction. The patient underwent left modified radical mastectomy, and pathology results showed a 70% signet ring cell pattern. Chemotherapy consists of an adriamycin-cycl

Radial Nerve Injury Caused by Compression Garment for Lymphedema: A Case Report.

Lymphedema is a condition characterized by localized fluid retention and tissue swelling caused by a compromised lymphatic system. To minimize fluid buildup and stimulate the flow of fluid through the lymphatic system, compression garments are usually applied to patients with lymphedema. There are few studies to report complications of compression garments to treat breast cancer-related lymphedema. To our knowledge, this is the first report of radial nerve compression neuropathy associated with wearing a compression garment to treat lymphedema. Level of Evidence: V.
