Inferior mastoidectomy-hypotympanic approach for surgical removal of glomus jugulare tumors: an anatomical and radiologic study emphasizing distances between critical structures.

The technique of inferior mastoidectomy-hypotympanic dissection, with preservation of the middle and inner ear structures, has been established for radical surgical removal of glomus tumors involving the skull base. In order to study the gross anatomic differences and correlate with the computed tomography (CT) scan, 20 human temporal bones were dissected and accurate measurements between vital structures were made. This study revealed a wide variation in distances between the neurovascular structures, whereas the distances between inner ear structures were not statistically different. This or

Single flap with three pedicles, bone paté and split-thickness skin graft for immediate mastoid obliteration after canal wall down mastoidectomy.

Our objective was to evaluate single flap with three pedicles, bone paté and split-thickness skin graft for mastoid cavity obliteration after canal wall down mastoidectomy done for chronic suppurative otitis media and its efficacy in producing a small and dry mastoid cavity. Over a period of 7 years (2003-2010), 100 consecutive procedures in 100 patients with chronic suppurative otitis media were performed at the Mansoura University Hospital (Egypt) with a minimum follow-up of 12 months (range 12-72 months). All patients had canal wall down mastoidectomy with simultaneous tympanoplasty. Anteri

Comparative Evaluation of Mastoid Cavity Obliteration by Vascularised Temporalis Myofascial Flap and Deep Temporal Fascial-Periosteal Flap in Canal Wall Down Mastoidectomy.

INTRODUCTION: A consensus is emerging amongst otologists that obliteration of the mastoid cavity that results after modified radical mastoidectomy is a sound option to prevent cavity related problems such as otorrhoea, infection, granulation tissue and hearing loss. A variety of techniques have been proposed to perform the obliteration. AIM: The present study aimed to compare the conventional method of mastoid cavity obliteration by vascularised temporalis myofascial flap with deep temporal fascial-periosteal flap in canal wall down mastoidectomy. MATERIALS AND METHODS: The prospective study

Efficacy of Tympanoplasty Without Mastoidectomy for Treating Chronic Otitis Media in Patients With Mastoid Cavity Opacification in Temporal Bone Computed Tomography Findings.

OBJECTIVES: Combined mastoidectomy is generally preferred to tympanoplasty alone when treating patients with chronic otitis media (COM), particularly when temporal bone computed tomography (TBCT) shows that the mastoid cavity contains opacification of soft tissue density. However, in cases with Eustachian tube dysfunction, a mastoid cavity volume may be a burden to its function. We hypothesized that tympanoplasty alone might be better than tympanoplasty combined with mastoidectomy because soft tissue in the mastoid cavity is a sequel to a protective physiological response. Thus, we explored th

Comparison of Postoperative Nausea and Vomiting Incidence between Remimazolam and Sevoflurane in Tympanoplasty with Mastoidectomy: A Single-Center, Double-Blind, Randomized Controlled Trial.

Background and Objectives: Postoperative nausea and vomiting (PONV) is a common adverse effect of general anesthesia, especially in middle ear surgery. Remimazolam is a newer benzodiazepine recently approved for use in general anesthesia. This study aimed to compare the incidence rate of PONV after tympanoplasty with mastoidectomy between using remimazolam and sevoflurane. Materials and Methods: This study included 80 patients undergoing elective tympanoplasty with mastoidectomy. The patients were randomly assigned to either the remimazolam or sevoflurane group. The primary outcome was the inc

Pseudo mastoid obliteration with conchal cartilage may be a safe alternative technique for cochlear implantation in canal wall down mastoidectomy with large meatoplasty.

OBJECTIVE: Safe cochlear implantation is challenging in patients with canal wall down mastoid cavities, and the presence of large meatoplasties increases the risk of external canal overclosure. This paper describes our results of obliteration of the mastoid cavity with conchal cartilage as an alternative procedure in cases of canal wall down mastoidectomy with very large meatoplasty. METHODS: The cases of seven patients with a canal wall down mastoidectomy cavity who underwent cochlear implantation were retrospectively reviewed. Post-operative complications were analysed. Efficacy and safety of intravenous tranexamic acid in microscopic modified radical mastoidectomy: a study protocol for a prospective, randomised, double-blind controlled trial.

INTRODUCTION: Radical mastoidectomy is a common procedure for chronic suppurative otitis media, typically performed under a microscope. The smooth operation is closely related to the clarity of the operative field. Our trial is designed to investigate whether the intravenous administration of tranexamic acid (TXA) can improve the clarity of the operative field, reduce the operative time, and increase surgeon satisfaction. METHODS AND ANALYSIS: This study is a prospective, randomised, double-blinded, controlled trial that aims to investigate the effects of TXA on patients with otitis media. Th

Comparison of Post-auricular Soft Tissue and Post-auricular Soft Tissue With Autologous Bone Pate in Obliteration of the Canal Wall Down Mastoidectomy Cavity.

Background The surgical management of chronic otitis media (COM) with squamous disease is canal wall down mastoidectomy (CWDM). Canal wall down procedures require the obliteration of the newly formed cavity to mitigate complications. Soft tissue flaps, including Rambo flap, Hong Kong flap, Palva flap, and inferior-based fascio-periosteal flap, as well as autologous bone pâté, have been the most successful and commonly used materials for obliteration over the past two decades. Although each flap has its advantages, the Palva flap is considered superior, primarily because of its ease of use and

To evaluate the results of mastoid obliteration and reconstruction of posterior meatal wall after canal wall down mastoidectomy using ready-to-use, self-setting hydroxyapatite bone cement.

OBJECTIVES: Despite a lot of scientific advancements in otology, canal wall-down mastoidectomy is still considered to be the standard of care in the management of extensive cholesteatoma. To avoid large cavity related problems, mastoid obliteration has been described in the literature for many decades. Controversy prevails among otologists regarding the materials used for obliteration. This study aimed to evaluate the results of mastoid obliteration and reconstruction of posterior meatal wall after mastoidectomy using ready-to-use, self-setting hydroxyapatite bone cement. Do patients with sclerotic mastoids require aeration to improve success of tympanoplasty?

CONCLUSION: We could not find any significant difference in the results for graft success rate and functional hearing results between the myringoplasty and tympano-mastoidectomy groups. So mastoidectomy may not be necessary for successful tympanic membrane reconstruction and hearing improvement. OBJECTIVE: To investigate the effect of aerating mastoidectomy on the surgical success rate of myringoplasty. METHODS: This was a retrospective study. Data were analyzed from 92 patients who underwent surgical repair of tympanic membrane perforations due to chronic suppurative otitis media (CSOM) wit
