Bilateral tri-arterial embolization for the treatment of epistaxis.

BACKGROUND: Intractable epistaxis is treated by ipsilateral trans-arterial embolization of the internal maxillary artery, but there is 13-26% recurrence of bleeding. Preemptive embolization of both internal maxillary arteries along with the ipsilateral facial artery could provide maximal protection against recurrent epistaxis. We report our experience with 8 patients treated with bilateral tri-arterial embolization. METHODS: We performed a retrospective review of the patients who were treated with bilateral internal maxillary artery and ipsilateral facial artery embolization from January 2005

Extended endoscopic endonasal approach to the pterygopalatine fossa: anatomic study.

AIM: Our aim in this study was to recognize the endoscopic anatomy of the pterygopalatine fossa (PPF) and the anatomic variations of the related neurovascular structures, to define the endoscopic endonasal approach to this region. METHODS: Six fresh adult cadavers were studied (N=6) by endoscopic endonasal approach. To reach the pterygopalatine fossa endonasally, we performed the middle meatus transpalatine approach in 2 cadavers, the middle meatus transnasal approach in 2 cadavers and the inferior turbinatectomy transnasal approach in 2 cadavers. The superior and posterior walls of the maxil

[Composite midfacial flap vascularized by the facial artery: a preliminary anatomic study].

INTRODUCTION: Extended facial tissue defects are difficult to reconstruct because of the anatomical and functional complexity of the area. Recently, composite facial allotransplantation was used for reconstruction. This could be performed because of specific facial blood supply and its facial and maxillary anastomoses. Could a composite naso-labio-mental flap rely on the sole blood supply of a facial artery anastomosis? We performed an anatomic study of a naso-labio-mental composite flap vascularized by the facial artery. MATERIAL AND METHOD: The study relied on arteriographies made on cadave

Characterization of the pterygomeningeal artery based on branching pattern and muscular distribution.

PURPOSE: The pterygomeningeal (accessory meningeal) artery arises from the middle meningeal or maxillary artery. Although there is the potential that this artery may be wounded by the surgery for the temporomandibular joint disorder, the current state of anatomical knowledge is insufficient. This study investigated the appearance and the branching pattern of this artery as a means to its characterization. METHODS: The pterygomeningeal artery was dissected in 14 cadavers and its branches and their distributions to the muscles inside the mandible were examined. RESULTS: The maxillary artery pa

Posterior epistaxis: systematic review on the effectiveness of surgical therapies.

OBJECTIVE: The optimal surgical treatment for patients with posterior epistaxis and failed conservative therapy is unknown. Therefore we planned a systematic review studying all available publications assessing the effect on bleeding recurrence and postoperative complications of ligation of the internal maxillary artery or the sphenopalatine artery. METHODS: We searched the electronic databases Medline, Medline In Process, and Cochrane Library. Data extraction was performed following standard methods. RESULTS: Twenty-eight studies could be included in the systematic review. All studies were

Acquired arteriovenous fistula associated with traumatic oroantral fistula: endovascular treatment.

BACKGROUND: Oroantral fistulas are pathologic connections between the oral cavity and the maxillary sinus. Arteriovenous fistulas are abnormal connections between an artery and a vein with no intervening capillary network. METHODS: We present an extremely rare case of barotrauma-related oroantral fistula with an associated arteriovenous fistula between the internal maxillary artery and the ophthalmic venous system. RESULTS: The patient developed an oroantral fistula from barometric pressure changes in the setting of sinusitis. After closure and revision of the oroantral fistula, he developed

Evaluation of the posterior superior alveolar artery canal by cone-beam computed tomography in a sample of the Egyptian population.

PURPOSE: This study was conducted to evaluate the accuracy of cone-beam computed tomography (CBCT) in detecting the posterior superior alveolar (PSA) artery canal in a sample of the Egyptian population. MATERIALS AND METHODS: CBCT images of 600 maxillary sinuses of patients were examined for the presence or absence of the PSA artery along the lateral wall of the maxillary sinus, and for the diameter and type of the canal in relation to age and sex. The distances from the canal to the alveolar crest and sinus floor were also measured. Each canal was assessed to determine whether it was bifid.

An 84-Year-Old Man with a History of Myeloma and Biphosphonate-Related Osteonecrosis of the Jaw Treated with Preoperative Vascular Embolization Before Partial Maxillectomy.

BACKGROUND Bisphosphonates and anti-receptor activator of nuclear factor kappa B antibodies are used to treat bone diseases associated with increased osteoclast activity, including myeloma. However, they can cause osteonecrosis of the jaw, known as medication-related osteonecrosis of the jaw. This report presents a case of a patient with a history of myeloma who required posterior maxilla resection for bisphosphonate-related osteonecrosis of the jaw, in which preoperative embolization prevented unexpected bleeding related to vascular injury and allowed for a safe procedure with minimal bleedin

Delayed presentation of pseudoaneurysm after Le Fort I osteotomy.

Although the osteotome is positioned in close vicinity to the maxillary artery and its branches during ptergomaxillary separation in a Le Fort I osteotomy, postoperative complications from vascular injuries are rare. The following report describes an unusual occurrence of a maxillary artery pseudoaneurysm following a Le Fort I and bilateral sagittal-split osteotomies for correction of mandibular and maxillary asymmetries in a patient with Goldenhar syndrome. This was recognized 8 months after the procedure when the patient developed acute facial swelling and required an emergent angiogram for

Intraosseous territory of the facial artery in the maxilla and anterior mandible: implications for allotransplantation.

AIM: The aim of this anatomical study was to define the intraosseous vascular territory of the facial artery. The clinical issue is whether ipsilateral facial artery anastomosis will guarantee blood supply to the ipsi- and contralateral mandibular symphyses and maxillae in allotransplantation. MATERIAL AND METHODS: Of 10 human cadaveric heads, the left facial artery was injected with a positive contrast agent. The maxillae and mandibular symphyses were investigated with cone-beam computed tomography (CBCT). RESULTS: Each ipsilateral maxilla and mandibular bone segment showed contrast medium

A Case of a Dentigerous Cyst in the Maxillary Sinus Treated Preoperatively With Vascular Embolization to Avoid Intraoperative Abnormal Bleeding.

Bone cysts involving the maxillary sinus are frequently observed, and controlling bleeding from the maxillary or posterior superior alveolar arteries is extremely difficult when the surgical site extends into the palatine fossa or the wing socket behind the maxillary sinus. In this report, we describe a case wherein preoperative endovascular arterial embolization prevented bleeding owing to an unexpected vascular injury that occurred during the removal of a dentigerous cyst from the maxillary sinus. This resulted in a safe operation with less intraoperative bleeding. Although this approach car

Transantral sphenopalatine artery ligation.

Posterior epistaxis from branches of the sphenopalatine artery can be rapidly and effectively controlled by a new ligation technique. The sphenopalatine artery or its branches are directly ligated as they exit the sphenopalatine foramen to enter the nose, completely avoiding the pterygomaxillary fossa. The vessels are exposed via a transantral approach, through the posterior portion of the medial antral wall. The mucoperiosteum of the lateral wall of the nose (medial antral wall) is preserved, elevated medially and posteriorly and used to tense the sphenopalatine vessels, bringing them into vi

Maxillary avascular necrosis after Le Fort I osteotomy: a systematic review of ischemic complications.

BACKGROUND: Maxillary avascular necrosis (AVN) after Le Fort I osteotomy is rare but potentially devastating. Although injury to the descending palatine artery has traditionally been regarded as a major cause, the relative contribution of individual vascular and surgical factors remains unclear. This systematic review aimed to synthesize the clinical evidence on maxillary AVN after orthognathic surgery and to evaluate whether postoperative ischemic complications are better explained by isolated arterial injury or cumulative vascular compromise. MAIN BODY: A systematic literature search of Pub
