Head and Neck Schwannomas: A Surgical Challenge-A Series of 5 Cases.

BACKGROUND: Schwannomas, also known as neurilemmomas, are benign peripheral nerve sheath tumors. They originate from any nerve covered with schwann cell sheath. Schwannomas constitute 25-45% of tumors of the head and neck. About 4% of head and neck schwannomas present as a sinonasal schwannoma. Brachial plexus schwannoma constitute only about 5% of schwannomas. Cervical vagal schwannomas constitute about 2-5% of neurogenic tumors. METHODS: We present a case series of 5 patients of schwannomas, one arising from the maxillary branch of trigeminal nerve in the maxillary sinus, second arising fro

Extraoral anatomy in CBCT – a literature review. Part 3: Retromaxillary region.

In this review about extraoral anatomy as depicted by cone beam computed tomography, the retromaxillary region is discussed. A medium-sized (6 x 6 cm) or large (≥ 8 x 8 cm) field of view of the maxilla will inevitably depict the retromaxillary region that can be considered a «transition» zone between the viscerocranium and the neurocranium. Major structures of the region include the sphenoid bone and the pterygopalatine fossae. The sphenoid bone is a single but complex bone located between the maxilla and the brain. It is composed of a central body, bilateral greater and lesser wings, and pter

Extramedullary plasmacytoma masquerading as Tolosa-Hunt syndrome: a case report.

Painful ophthalmoplegia due to extramedullary plasmacytoma is a rare initial manifestation of multiple myeloma. The present report describes a 48-year-old man who suffered an acute onset of retro-orbital pain, left abducens palsy and left facial hypoesthesia. In addition, he exhibited an elevated erythrocyte sedimentation rate and partial responsiveness to corticosteroid treatment, all of which resemble the features of Tolosa-Hunt syndrome. Imaging studies revealed a multilobulated tumour invading the left sphenoid bone and sphenoid sinus, later confirmed as a plasmacytoma at pathology. Multip

Gross Anatomical, Radiographic and Doppler Sonographic Approach to the Infra-auricular Parotid Region in Donkey (Equus asinus).

The present study was performed on 12 heads of donkey's cadavers of both sexes for different anatomic techniques and on 20 live adult donkeys for ultrasonographic approaches of local anesthetic techniques. The aim was to achieve safe desensitization of the ear canal and tympanic membrane in addition to measuring different parameters of the structures occupying the infra-auricular parotid region. The internal auricular nerve was divided into two fine branches and constituting, at its origin, a characteristic V-shape with the caudal auricular nerve. The styloid process of auricular cartilage was

Modified Lateral Surgical Approach to the Pterygopalatine Fossa.

Oral cancer, especially that originates in the maxillary tuberosity, buccal mucosa, and maxillary alveolus, is apt to invade the pterygopalatine fossa, where an intraoral approach is nearly impossible. There are 2 main types of extraoral approach to the pterygopalatine fossa; the anterior approach and the lateral approach. Although the lateral approach has an advantage compared with the anterior approach in terms of cosmetics, after the operation numbness of the lower lip occurs due to the sacrifice of the mental nerve. In the conventional lateral approach, a vertical incision is made in the c

A Rare Case of Nasolacrimal Duct Partial Obstruction After Extraction of an Impacted Maxillary Canine.

The main complications of tooth extraction include jaw fracture, maxillary sinus damage, adjacent teeth damage, nerve damage, root fracture, infection, bleeding, and swelling. Despite the theoretical possibility that tooth extraction could result in nasolacrimal duct obstruction, to the best of our knowledge, no cases have yet been reported. In the present study, we describe a case in which the nasolacrimal duct was partially obstructed after extraction of an impacted maxillary canine, causing paranasal discomfort and lower eyelid swelling.

Ectopic Infra Orbital Nerves: Case Series of a Dangerous Normal Variant.

Normal variations in the paranasal sinus region are well documented in literature. We present five cases of a little known normal variant, which can have serious implications for the patient as well as the operating surgeon. An ectopic infra orbital nerve canal coursing through the maxillary sinus has rarely been described in imaging literature. This may sometimes be mistaken for a simple septum in the maxillary sinus and may cause serious complications during Functional Endoscopic sinus surgery (FESS) surgeries. We describe the imaging findings and present a brief review of the previous publi

Microsurgical anatomy of the infratemporal fossa.

The objective of this study is to clearly and precisely describe the topography and contents of the infratemporal fossa. Ten formalin-fixed, adult cadaveric specimens were studied. Twenty infratemporal fossa were dissected and examined using micro-operative techniques with magnifications of 3-40×. Information was obtained about the inter-relationships of the contents of the infratemporal fossa. The infratemporal fossa lies at the boundary of the head and neck, and the intracranial cavity. It is surrounded by the maxillary sinus anteriorly, the mandible laterally, the pterygoid process anterome

A Modified Pre-Auricular Approach in the Treatment of Orbital Zygomatic Maxillary Complex Fractures.

OBJECTIVE: This study aimed to evaluate the safety of a modified pre-aricular approach in the treatment of orbital zygomatic maxillary complex fractures. METHODS: This was a retrospective study of clinical data from 108 adults who were diagnosed with orbital zygomatic maxillary complex fractures and undergo open reduction and internal fixation through a modified pre-auricular approach. Exophthalmometry and limitation of extraocular muscle movement were determined using a Hertel exophthalmometer or synoptophore examination respectively. Primary complications were recorded, and postoperative fa

The nervous supply to the nasal cavity of the Bactrian camel (Camelus bactrianus).

The Bactrian camel is an important domestic animal in some of the desert and semi-desert areas of the world. However, there is no detailed report about the nervous supply to the nasal cavity of the Bactrian camel. In the present study, seven heads of adult Bactrian camels were collected and the nerve distribution in the nasal cavity was dissected grossly. The results demonstrated that the nerves supplying to the nasal cavity included the olfactory nerve, the ethmoidal nerve from the ophthalmic nerve, and the caudal nerve from the maxillary nerve. The general patterns of nervous distribution in

Microvascular Decompression versus Radiofrequency Ablation in Trigeminal Neuralgia of the Maxillary and Mandibular Divisions.

Background  Although medical treatment is the mainstay of therapy, in trigeminal neuralgia (TN), patients failing to respond to it make them candidates to ablative or nonablative procedures. Objective  The aim of this study was to compare the outcome of Microvascular decompression (MVD) and radiofrequency (RF) thermocoagulation in the management of TN affecting the mandibular and maxillary divisions. Materials and Methods  Retrospective analysis of the data of 40 patients suffering from intractable classical TN affecting the maxillary or mandibular divisions or both was carried out. Twenty pat

Blindness and total ophthalmoplegia. A complication of transantral ligation of the internal maxillary artery for epistaxis.

Unilateral blindness and total ophthalmoplegia developed on the same side on which transantral ligation of the internal maxillary artery was performed for persistent posterior epistaxis. Evaluation with arteriograms and phlebograms showed no compromise of retinal vessels or cavernous sinus thrombosis. At reexploration of the maxillary antrum, marked swelling of previously placed oxidized cellulose gauze was found. An experimental model, using the rabbit, was designed to determine if implantation of oxidized cellulose gauze adjacent to the optic nerve would produce blindness and ophthalmoplegia

Distributions of calcitonin gene-related peptide and substance P in the human maxillary sinus of Japanese cadavers.

BACKGROUND: Substance P (SP) and calcitonin gene-related peptide (CGRP) are released by the nociceptive sensory nerve and are involved in blood flow, pain and inflammation in the nasal mucosa. The purpose of this study was to assess the distribution of the SP and CGRP nerve fibres related to blood supply within human Schneiderian membrane of the maxillary sinus (MS). MATERIAL AND METHODS: In this study, the MS from Japanese cadavers was examined by whole-mount immunohistochemistry. Human male cadavers (ranging in age from 80 to 90 years) were used in this study. RESULTS: SP- and CGRP-positiv

Local anaesthetic efficacy provided by lidocaine or lidocaine-tramadol in dogs undergoing maxillary fourth premolar extraction.

Oral nerve blocks are an indispensable technique for pre-emptive pain management in various veterinary procedures, including tooth extractions. Local anaesthetic drugs are considered the safest and the most effective agents to inhibit oral pain. Recently, tramadol was used as an adjuvant to local anaesthetics to increase the duration of analgesia. The aim of this study was to compare the effect of maxillary nerve block induced by lidocaine or lidocaine-tramadol on perioperative pain in dogs undergoing upper fourth premolar extraction. A randomized blinded clinical study was conducted over five

An adolescent treated with rapid maxillary expansion presenting with strabismus: a case report.

INTRODUCTION: Few in vivo studies have investigated the effect of maxillary expansion on strabismus; however, some in vitro studies hypothesized that changes in the palatal width obtained with rapid maxillary expansion appliances could involve other bone structures that contain blood vessels and nerves conveying to the orbital cavity. The present case report seems to support that hypothesis, even if no analysis of pathogenetic mechanisms could be drawn. CASE PRESENTATION: We present the case of a 14-year-old Caucasian girl affected by strabismus and referred for the treatment of a class III m
