Current dilemmas in the clinical use of heparin.

The more widespread use of heparin as an effective antithrombotic agent would be greatly facilitated by resolution of the following issues: Clarification of the biological differences between beef and hog heparin. The development of a standard heparin molecule. The modification of heparin for oral use. The availability of a standard in vitro assay of the drug's antithrombotic potential. A meaningful assay of plasma heparin together with a determination of whether or not heparin is present in human plasma in normal and diseased states. Establishment of criterions for determining the proper dosa

Content Validity of the Modified Functional Scale for the Assessment and Rating of Ataxia (f-SARA) Instrument in Spinocerebellar Ataxia.

The functional Scale for the Assessment and Rating of Ataxia (f-SARA) assesses Gait, Stance, Sitting, and Speech. It was developed as a potentially clinically meaningful measure of spinocerebellar ataxia (SCA) progression for clinical trial use. Here, we evaluated content validity of the f-SARA. Qualitative interviews were conducted among individuals with SCA1 (n = 1) and SCA3 (n = 6) and healthcare professionals (HCPs) with SCA expertise (USA, n = 5; Europe, n = 3). Interviews evaluated symptoms and signs of SCA and relevance of f-SARA concepts for SCA. HCP cognitive debriefing was conducted.

Establishing the Reliability and Limits of Meaningful Change of Lower Limb Strength and Power Measures during Seated Leg Press in Elite Soccer Players.

Measurement of lower limb strength, power and asymmetries of soccer players is important for monitoring physical development and injury risk. The aim of the present study was to establish the reliability and limits of meaningful change of single and double leg maximal strength, power and bilateral imbalance measures in elite soccer players using a pneumatic resistance based seated leg press. Thirteen participants undertook an incremental resistance leg press test on three separate testing days within a seven day period. Paired t-tests established no significant differences (p > 0.156) between

[Neurology of praxis disorder].

Based on the analysis of four types of basic praxis disorders, i.e. disturbance of single tool use (ideational apraxia), impairment of pantomiming ability to use a tool (ideomotor apraxia), inability to copy a non-meaningful finger pattern (limbkinetic apraxia), and inability to reproduce a series of hand movements (dynamic apraxia), a hypothesis on the neural organization of higher order motor production was proposed. It was stressed that these four types of praxis disorder should not be considered as different kinds of movement disorders at the same level. Rather they are examples of the mot

Adverse effects of marijuana: selected issues.

I have selected two areas of concern about cannabis that have emerged because new patterns of usage are perceptible over the past few years. In the case of pulmonary pathology there is good reason to suspect that long term use can result in inflammatory or neoplastic changes, particularly when the lung has a double burden of dealing with both tobacco and cannabis smoke. With reference to chronic substantial cannabis use and its impact on brain function, the definitive studies are yet to be done. If clinical impressions are meaningful, some people, especially young people, can sustain temporary

Endpoints for therapeutic interventions in faecal incontinence: small step or game changer.

Faecal incontinence (FI) is common and its pathophysiology and treatments continue to evolve. However, a standard measure(s) for assessing its clinical outcome has been elusive. Consequently, over 100 measures and scoring systems, each with intrinsic biases have been reported. These include adequate relief or global satisfaction, ≥50% reduction in episodes or days without FI, quality of life (QOL), FI severity scales and composite indices. Earlier scales relied on the frequency and type of solid, liquid or flatus incontinence and effects on life style whereas newer scales have incorporated urg

Evaluation of Clinical Meaningfulness of Fortasyn Connect in Terms of "Time Saved".

Assessment of meaningfulness in randomized clinical trials (RCTs) in Alzheimer's disease (AD) is challenging, particularly in early disease. Converting clinical outcomes to disease progression time allows assessment of treatment effects using a metric that is understandable and meaningful: time. We demonstrate time savings assessments using meta time component tests (TCTs) in the LipiDiDiet multinutrient RCT. Dietary patterns are important for dementia prevention, likely due to individual cumulative nutrient effects. LipiDiDiet used a multinutrient (Fortasyn Connect) formulation in patients wi

Bridging the gap between statistical significance and clinical relevance: A systematic review of minimum clinically important difference (MCID) thresholds of scales reported in movement disorders research.

BACKGROUND: Minimum clinically important difference (MCID) is the smallest change in an outcome measure that is considered clinically meaningful. Using validated MCID thresholds for outcomes powers trials adequately to detect meaningful treatment effects, aids in their interpretation and guides development of new outcome measures. OBJECTIVES: To provide a comprehensive summary of MCID thresholds of various symptom severity scales reported in movement disorder. METHODS: We conducted systematic review of the literature and included studies of one or more movement disorders, and reporting MCID

High-Frequency Jet Ventilation in Neonatal and Pediatric Subjects: A Narrative Review.

High-frequency ventilation is commonly utilized with neonates and with children with severe respiratory failure. Both high-frequency oscillatory ventilation (HFOV) and high-frequency jet ventilation (HFJV) are used extensively in neonates. HFJV can also be used in older, larger children. The purpose of this narrative review is to discuss the physiologic principles behind HFJV, examine the evidence supporting its use in neonatal and pediatric ICUs, give meaningful guidance for clinical application, and highlight potential areas for future research.

Duration of clinically meaningful improvement in pain as a patient-centered endpoint in acute pain trials.

Current US Food and Drug Administration (FDA) guidance recommends the sum of pain intensity differences (SPID) as the primary efficacy endpoint for acute pain trials, which lacks validated thresholds for clinically meaningful change and requires imputation of pain scores after rescue medication use. We introduce the responder outcome over time (ROOT), defined as the proportion of study time a participant experiences clinically important improvement in pain intensity without recent rescue medication use or early discontinuation. The ROOT combines the clinical interpretability of a responder-bas

Impact of substance use disorder on gray matter volume in schizophrenia.

Substance use may confound the study of brain structure in schizophrenia. We used voxel-based morphometry (VBM) to examine whether differences in regional gray matter volumes exist between schizophrenia patients with (n = 92) and without (n = 66) clinically significant cannabis and/or alcohol use histories compared to 88 healthy control subjects. Relative to controls, patients with schizophrenia had reduced gray matter volume in the bilateral precentral gyrus, right medial frontal cortex, right visual cortex, right occipital pole, right thalamus, bilateral amygdala, and bilateral cerebellum re

Hyperhidrosis Disease Severity Measure-Axillary (HDSM-Ax): Evaluation of Measurement Performance.

BACKGROUND: Clinical trials of primary axillary hyperhidrosis (AHH) require rigorous measurement of AHH severity from the patient’s perspective. Previously, we reported conceptualization and item content development for the Hyperhidrosis Disease Severity Measure-Axillary (HDSM-Ax) scale. OBJECTIVE: To evaluate the psychometric performance and estimate clinically meaningful change scores for the HDSM-Ax in a Phase IIb clinical study of sofpironium bromide gel for AHH. A five-point change in Modified Mini-Mental State Examination was clinically meaningful in community-dwelling elderly people.

OBJECTIVE: To determine what change in the Modified Mini-Mental State Examination (3MS), a validated and widely used cognitive screening tool for which meaningful change scores have not been clearly characterized, should be considered meaningful. STUDY DESIGN AND SETTING: The 3MS was administered at baseline after 5 and 10 years, as part of the population-based Canadian Study of Health and Aging. We calculated Cohen's effect sizes to estimate detectable changes in 3MS screening scores over 5 and 10 years in this large and representative study sample. RESULTS: A total of 3,255 older adults wh

Development of the modified Daily Symptom Diary (mDSD): a patient-reported outcome measure of dysphagia for eosinophilic esophagitis.

OBJECTIVE: Eosinophilic esophagitis (EoE) is a progressive, chronic, Type 2 inflammatory and fibrostenotic disease causing dysphagia among other symptoms. Patient-reported outcome (PRO) measures are essential for the assessment of EoE symptoms. One such PRO measure is the Daily Symptom Diary (DSD), but its content validity has not been established. This study modified the DSD based on participants' feedback and assessed the content validity of the modified DSD (mDSD).
