INTRODUCTION: Epithelioid Hemangioendothelioma (EHE) is a rare vascular neoplasm. Common locations of EHE are the bone, soft tissue, liver, and lung, but the mediastinal location is extremely rare. Few cases of mediastinal EHE, invading the Superior Vena Cava (SVC) have been reported.
CASE PRESENTATION: We report a case of a 21-year-old man with EHE invading the SVC, which was incidentally detected on performing chest radiography. A contrast-enhanced chest Computed Tomography (CT) scan demonstrated a well-defined, oval mass located on the right side of the anterior mediastinum. The mass showe

OBJECTIVE: To evaluate the ability of 18FDG-PET in the identification of pulmonary mass and preoperative mediastinal lymph node staging of lung cancer.
METHODS: Thirty-four patients with pulmonary neoplasm were evaluated with 18FDG-PET and CT scan respectively. PET data were analyzed semiquantitatively by calculating standardized uptake value (SUV) and also by a visual method. PET and CT results were compared with pathological results.
RESULTS: When the visual method was used, the sensitivity and accuracy for detection of malignant mass of 18FDG-PET were 93% and 85% respectively, whereas tho

Pure mediastinal yolk sac tumor is an uncommon and aggressive malignant germ cell neoplasm that presents considerable diagnostic difficulties owing to its pronounced clinical and morphological variability. Mediastinal yolk sac tumors, in contrast to their gonadal equivalents, typically occur at later stages, are typically associated with mixed germ cell components, and have a diverse array of histologic patterns that may resemble both germ cell and somatic malignancies. Accurate identification of these types of cancer is essential since diagnostic misclassification may significantly impact tre

We conducted a five-year review of our use of TBNA during bronchoscopy in our clinical practice. Out of 1,630 bronchoscopic procedures, 633 patients (39 percent) had TBNA performed as part of the initial examination. Mediastinal nodes were sampled in 86 percent of the patients who had TBNA performed, and 127 (23 percent) of 547 of these aspirates were positive. Other areas of mucosal abnormality of the tracheobronchial tree were also sampled by TBNA under direct vision and showed malignant cells in 44 (25 percent) of 176 aspirates. Of 363 patients first diagnosed as having carcinoma of the lun

Fifty-seven thymomas, defined as neoplasms of the epithelial-reticular framework cells of the thymus, were assessed in respect to histologic type, inclusive of there ultrastructural aspects. The median age of the 57 patients was 40.4 years, with a range of 2 1/2--72 years. All neoplasms were located in the anterior mediastinum. The tumours in 40 cases were encapsulated and without invasion of adjacent tissue or implants (equal to non-invasive thymomas). The tumors in 17 cases were invasive of adjacent tissue, particularly mediastinal pleura, pericard and trachea. Six of 57 patients (equal to 1

Pulmonary tumors in children are rare. Nevertheless, there are besides malign primary neoplasms and metastases also some benign tumors that the radiologist should know. The identification of some tumors is difficult, since some of them may mimic pulmonary inflammation. The first diagnostic tool is chest radiography. After that, a CT with contrast medium should be performed, if possible a multislice-CT (MSCT). Identification of mediastinal structures is best with MRI.

The association of systemic sarcoidosis with malignant neoplasms has been reported by several publications although there is controversy as to whether sarcoidosis predisposes patients to certain types of tumors or whether it is the tumors that cause a sarcoid reaction. The relationship between endobronchial carcinoid tumor and sarcoidosis is uncommon. We report the case of a 43-year-old male who was hospitalized due to hemoptysis. Thoracic CT revealed a pulmonary mass in the right upper lobe (RUL) as well as hypermetabolic lymphadenopathies seen with PET-CT. The endobronchial biopsies were com

Castleman's disease is an atypical lymphoproliferative disorder of unknown etiology, which might be associated with various clinical conditions, including autoimmune diseases and malignant neoplasms. We report the case of a 72-year-old female patient who was referred to the thoracic surgery department of Getúlio Vargas University Hospital, in the city of Manaus, Brazil, for the resection of a posterior mediastinal tumor. Three months prior, the patient had been admitted to the ICU with signs of severe dyspnea, at which time she was diagnosed with myasthenia gravis. After the resection of the m

Lymphoblastic lymphoma (LBL) is a neoplasm of immature B cells committed to the B-(B-LBL) or T-cell lineage (T-LBL) that accounts for approximately 2% of all lymphomas. From a histopathological point of view, blasts may be encountered in tissue biopsy and/or bone marrow (BM). In tissue sections, LBL is generally characterized by a diffuse or, as in lymph nodes and less commonly, paracortical pattern. Although histological features are usually sufficient to distinguish lymphoblastic from mature B- or T-cell neoplasms, a differential diagnosis with blastoid variant of mantle cell lymphoma, Burki

Tumor-to-tumor metastasis is defined as when metastasis from a primary tumor (donor) grows in a different primary neoplasm (recipient). Due to the structure of the thymus and the low incidence rate, thymic epithelial neoplasm has been rarely described in the literature as a recipient for metastases. In this report,a patient with advanced prostatic cancer and under control after chemo/hormone therapy was directed to our thoracic surgery unit for an anterior mediastinal mass detected during the staging workup for prostate disease. In this case report, we describe an unusual presentation of gout, manifesting as a fluorodeoxyglucose-avid anterior mediastinal mass mimicking a malignant neoplasm on positron emission tomography/computed tomography. The unusual observation of tophaceous gout in the anterior mediastinum is of relevance to chest physicians and surgeons as well as to radiologists and pathologists evaluating patients with lesions in the mediastinum.

We present the case of a type AB thymoma in a 69-year-old man with previous coronary artery bypass grafting (CABG) in whom angiography revealed a left internal mammary artery graft supplying blood flow to a thymic neoplasm, which simultaneously occluded the graft. This required a redo sternotomy, lysis of pericardial adhesions, complete thymectomy, and redo one vessel off-pump CABG. This case seeks to sensitize physicians to the possibility of coronary adverse events in patients with a previous CABG in the setting of management of mediastinal neoplasms, and it presents the novel findings on ca

Resection and reconstruction of the superior vena cava (SVC) are required in a selected group of patients with anterior mediastinal tumors and lung neoplasms. We present the case of a 63-year-old woman who underwent invasive type B2 thymoma resection and a rare type of reconstruction of the superior vena cava using a patch of the left brachiocephalic vein (LBV). The various types of reconstruction of the superior vena cava are discussed.

Paraganglioma is a neuroendocrine tumor that originates from extraadrenal chromaffin cells. Primary thyroid paraganglioma is an extremely rare neoplasm. In this study, an exceptionally uncommon case of recurrent mediastinal malignant paraganglioma with primary origin from thyroid gland is presented. Median resternotomy, resection of left brachiocephalic vein, and extirpation of the mediastinal tumor were performed successfully. Commonly, it is preoperatively misdiagnosed and has unpredictable biological behavior. Incorrect diagnosis results in disastrous consequences for the patient, and conse

An incidental anterior-superior mediastinal rhabdomyoma is reported in a 68-year-old man who died of hypovolemic shock as a result of massive blood loss due to transection of aorta after being hit by a moving motor vehicle. This is the third reported case of mediastinal rhabdomyoma in the literature. The immunohistochemical evidence of expression of muscle-specific markers supports the rhabdomyomatous nature of this neoplasm, and electron microscopic demonstration of haphazardly arranged myofilaments with prominent Z bands, "jack-straws" in the mitochondria, and the absence of desmosomes is su

The causality of thymoma (TM) or thymic hyperplasia and various autoimmune diseases (AISs) remains poorly understood. To date, there have been no confirmed cases of pathologically complete regression of TM. We present a rare case of complete regression of an anterior mediastinal tumour secondary to the removal of metallic dental crowns as a treatment for metal allergy assessed to exacerbate pityriasis rubra pilaris, a rare cutaneous disease. After dental crown removal, the skin lesions improved, and the anterior mediastinal nodule (AMN) shrank rapidly through transient enlargement with a low d

Pleomorphic undifferentiated sarcoma (PUS) of the mediastinal thymus is a rare type of cancer. In the present case report, a 67-year-old female patient presenting a mediastinal mass for >1 year was assessed for clinical characteristics, histopathological, immunohistochemical expression and gene mutation using fluorescence in situ hybridization (FISH), and relevant literature was reviewed. Histological analysis revealed nodular changes of different sizes in the thymus, which consisted of a mixture of pleomorphic and spindle cells. The pleomorphic cells with distinct atypia were giant cells and

A Rock Alpine doe (Capra aegagrus hircus) was presented to the Colorado State University Veterinary Teaching Hospital because of scaling and ulceration over the withers, coronary bands, and dewclaws. The doe was euthanized because of poor prognosis associated with a radiographically identified cranial mediastinal mass, increased respiratory effort, and discomfort. Autopsy revealed a cranial mediastinal mass, and scaling-to-ulcerative lesions affecting the dorsum, ventrum, pinna, neck, teats, coronary bands, and dewclaws. Histologically, the mediastinal mass was an epithelial neoplasm with admi

The transmanubrial approach allows excellent unilateral exposure of the thoracic outlet. However, selected patients may require a bilateral cervicomediastinal exposure to completely resect the neoplasm. We report the use of a "double" transmanubrial approach for the resection of a giant mediastinal mass requiring bilateral vascular dissection and superior vena cava system resection and replacement.
