Sync and swim: the impact of medication consolidation on adherence in Medicaid patients.

BACKGROUND: Medication nonadherence is associated with higher cost of care and poor outcomes. Medication refill consolidation (synchronization of refill dates for patients on multiple drugs) is an important component of regimen complexity. We presumed that Medicaid patients with a 30-day medication supply limit would have significant difficulty with refill consolidation. We evaluated regimen complexity and refill consolidation in relation to medication adherence in the Medicaid population. METHODS: A survey was administered to 50 Medicaid patients taking 2 or more daily medications in the out

Racial differences in antidepressant treatment preceding suicide in a Medicaid population.

OBJECTIVE: African Americans have less use of antidepressants than whites, suggesting racial differences in the diagnosis and treatment of mood disorders. This study assessed whether this racial disparity is present among patients with a very high risk of severe mood disorders--that is, those with completed suicides. METHODS: Completed suicides that occurred between 1986 and 2004 were identified by examining records of Tennessee Medicaid/TennCare enrollees aged 18 to 84 years who had no serious medical illness or chronic psychosis. Use of antidepressants in the 365 days before the suicide was

Physician and enrollee knowledge of Medicaid coverage for tobacco dependence treatments.

BACKGROUND: The 2000 Public Health Service Clinical Practice Guideline, Treating Tobacco Use and Dependence, recommends health insurance coverage for tobacco-dependence treatments proven effective in helping smokers to quit. Two states with comprehensive coverage for tobacco-dependence treatments in their Medicaid programs were selected to document awareness of coverage for tobacco-dependence treatments among primary care physicians who treat Medicaid enrollees and Medicaid-enrolled smokers. METHODS: In 2000, surveys were conducted among Medicaid smokers (n =400) and physicians (n =160) to do

Lipid-lowering agents and the risk of hip fracture in a Medicaid population.

CONTEXT: Three recent nested case-control studies conducted in automated databases suggest that users of 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors (statins) have a risk of hip and other osteoporotic fractures half that of non-users of any lipid-lowering drug. However, this comparison may be biased by unmeasured factors associated with treated hyperlipidemias. OBJECTIVE: To compare the risk of hip fracture among users of statins and other lipid-lowering agents, which is less susceptible to bias than the comparisons performed in the previous studies. DESIGN AND SETTING: Retros

A comparison of ambulatory Medicaid claims to medical records: a reliability assessment.

This study compares the documentation of ambulatory care visits and diagnoses in Medicaid paid claims and in medical records. Data were obtained from Maryland Medicaid's 1988 paid claims files for 2407 individuals who were continuously enrolled for the fiscal year, had at least one billed visit for one of six indicator conditions, and had received the majority of their care from one provider. The patients sampled were also stratified on the basis of the case-mix adjusted cost of their usual source of care. The medical records for these individuals as maintained by their usual source of care we

The urge to merge: linking vital statistics records and Medicaid claims.

This paper describes a procedure used to link Medicaid claims data to California vital statistics records for very low birthweight infants. The linkage involved about 53,000 infants born from 1980 to 1987 and 1.46 million claims for delivery/birth-related hospital admissions during the same period. Because the two data files did not share a unique identifier, record linkage required combining evidence across several linking variables: delivery hospital, delivery/birth date or hospitalization period, names, mother's age, and zip code. To combine the various pieces of evidence, we used record li

Minimizing H.R.1-related Medicaid coverage disruptions for high-risk patients.

Forthcoming H.R.1 Medicaid eligibility changes requiring documentation of work, school, or volunteer activities are projected to result in significant disenrollment. High-risk beneficiaries, including those experiencing homelessness, are more likely to experience barriers in navigating the eligibility process, such as qualifying for work requirement exemptions. We utilized an existing set of linked data across the medical safety net, state Medicaid agency, housing services providers, and justice agencies as a case study to understand the prevalence of select potential medical and social exempt

Projected Oral Health Outcomes and Costs Associated With Pediatric Medicaid Disenrollment.

IMPORTANCE: Medicaid provides comprehensive dental coverage to more than 37 million children in the US and plays a central role in reducing barriers to care and improving oral health outcomes. Under the 2025 One Big Beautiful Bill Act (OBBBA), many of these children may be at risk of losing their Medicaid coverage. OBJECTIVE: To estimate changes in oral health outcomes and health care costs associated with projected Medicaid coverage loss among US children. DESIGN, SETTING, AND PARTICIPANTS: A stochastic decision-analytic microsimulation model was developed to project changes in oral health

Medicaid Expansion and 1-Year Mortality Among Young Adults Initiating Dialysis.

IMPORTANCE: The Affordable Care Act (ACA) provided continued Medicaid coverage to US children transitioning into young adulthood, but whether expansion averted deaths among younger adults with kidney failure remains unclear. OBJECTIVE: To examine the association of Medicaid expansion with 1-year mortality among young adults with kidney failure initiating dialysis. DESIGN, SETTING, AND PARTICIPANTS: This cohort study using a quasi-experimental difference-in-differences design compared changes in outcomes before and after Medicaid expansion between patients aged 19 to 23 years (affected by exp

Stimulant Use Disorder Diagnoses in Adolescent and Young Adult Medicaid Enrollees.

IMPORTANCE: There has been a national increase in fatal and nonfatal overdoses involving stimulants, and 4.5 million US individuals meet criteria for stimulant use disorder (UD), with the highest prevalence in young adults. However, limited information exists on trends in diagnosed stimulant UD. OBJECTIVE: To estimate trends in the proportion of adolescent and young adult Medicaid enrollees diagnosed with a stimulant UD from 2001 to 2020. DESIGN, SETTING, AND PARTICIPANTS: A repeated cross-sectional study (2001-2020) was conducted using administrative health care claims data from Medicaid (p

Interventions To Automate Medicaid Renewals Reduce Procedural Denials And Increase Coverage.

Burdensome Medicaid renewal processes are a known source of coverage loss among eligible people. In spring 2023, the pause in Medicaid disenrollment resulting from the COVID-19 public health emergency ended, and states began redetermining eligibility for more than ninety million beneficiaries. In the early months of these redeterminations, a large majority of those disenrolled lost coverage for procedural reasons, such as missing paperwork. We examined an effort by the federal government to work with four states to increase automated ex parte Medicaid renewals with the goal of reducing procedu

Children's Medicaid Enrollment Increased During First Year Of Consolidated Appropriations Act.

Under the Consolidated Appropriations Act, 2023, which required the provision of national twelve-month continuous Medicaid eligibility for children up to age nineteen, children's Medicaid enrollment increased. After implementation of the act, states without prior twelve-month continuous eligibility showed larger gains in children's enrollment than states with prior continuous eligibility. These gains occurred during the unwinding of COVID-19 pandemic-era protections.

Medicaid Managed Care Penetration and Mental Health Service Use Among Adults.

OBJECTIVE: To examine the association between Medicaid managed care (MMC) penetration and mental health (MH) service use among Medicaid-enrolled non-elderly adults, with a special focus on those with MH conditions. BACKGROUND: Medicaid covers over 9 million adults living with MH conditions, with many enrolled in MMC. Despite increases in MMC enrollment over the past decade, nationwide evidence of MMC's association with MH service use during this period is lacking. METHODS: Using 2015-2019 National Survey on Drug Use and Health data, we applied logistic and negative binomial regression models

Dental Care Utilization Among Medicaid-Enrolled Adults With Substance Use Disorder.

INTRODUCTION: Low-income Medicaid-enrolled adults are likely to have poor oral health and experience substance use disorders. Despite emerging evidence that comprehensive dental care can improve substance use disorder outcomes, there is a lack of evidence about the overlap of substance use disorder and dental care utilization. This study aims to fill that gap. METHODS: This cross-sectional study used 2019 data from the Transformed Medicaid Statistical Information System Analytic Files and included over 30 million nondually eligible Medicaid-enrolled adults aged 21-64 years. De-prescribing opioids among Medicaid patients with long-term opioid use.

BACKGROUND: Guidelines encourage deprescribing opioids for long-term opioid patients, especially those using opioids and benzodiazepines, z-drugs, or muscle relaxants ("other respiratory depressants"). OBJECTIVE: Were long-term opioid patients who were prescribed other respiratory depressants more likely to have deprescribing opioid trajectories? DESIGN: Cross-sectional retrospective study using pharmacy and professional claims from 2015 to 2019. Adjusted logistic regression models were stratified on low (<50 morphine milligram equivalents; MME) and high (>50 MME) starting opioid doses. We r

Emergency care use in early pregnancy among North Carolina Medicaid beneficiaries.

OBJECTIVE: We sought to describe characteristics of pregnant Medicaid beneficiaries seeking emergency care in early pregnancy and to identify risk factors, specifically social drivers of health (SDOH), for emergency care use. STUDY DESIGN: Linked Medicaid hospital claims, live birth records, and North Carolina Pregnancy Risk Screening Form data were used to identify risk factors for emergency care use among pregnancies resulting in a live birth between January 2014 and December 2019. Emergency care use included visits to the emergency department or obstetric triage unit that occurred in pregn

Postnatal Antiretroviral Prophylaxis and Perinatal HIV Infection in Medicaid-Enrolled Infants.

BACKGROUND: Advances in preventive strategies have reduced mother-to-child transmission of HIV. The US Department of Health and Human Services (HHS) guidelines on postnatal antiretroviral prophylaxis have evolved to reflect increasingly available evidence. We sought to provide data on postnatal prophylaxis in clinical practice. METHODS: We performed a retrospective study using the MarketScan Multi-State Medicaid Database (2009-2021). We identified antiretroviral use among infants and classified them into single-, double-, and triple-drug prophylactic, and therapeutic use. We also characterize

Commercial Versus Medicaid Insurance and Use of High-Priced Anticancer Treatments.

BACKGROUND: Because the markups on cancer drugs vary by payor, providers' financial incentive to use high-price drugs is differential according to each patient's insurance type. We evaluated the association between patient insurer (commercial vs Medicaid) and the use of high-priced cancer treatments. MATERIALS AND METHODS: We linked cancer registry, administrative claims, and demographic data for individuals diagnosed with cancer in North Carolina from 2004 to 2011, with either commercial or Medicaid insurance. We selected cancers with multiple FDA-approved, guideline-recommended chemotherapy

Chronic Opioid Use After Implementation of Oregon's Medicaid Back Pain Policy.

INTRODUCTION: In 2016, Oregon developed an innovative policy to improve care for Medicaid patients with back pain. The objective of this study was to identify the factors associated with dose reduction and discontinuation among Medicaid patients using chronic opioid therapy after implementation of this policy. METHODS: Using Medicaid administrative claims data, this was a retrospective cohort of patients on chronic stable opioid therapy between July and December 2016. Outcomes assessed were (1) 30% reduction in opioid dose and (2) an absolute discontinuation between January and December 2017.

Pain Management Treatments and Opioid Use Disorder Risk in Medicaid Patients.

INTRODUCTION: People with chronic pain are at increased risk of opioid misuse. Less is known about the unique risk conferred by each pain management treatment, as treatments are typically implemented together, confounding their independent effects. This study estimated the extent to which pain management treatments were associated with risk of opioid use disorder (OUD) for those with chronic pain, controlling for baseline demographic and clinical confounding variables and holding other pain management treatments at their observed levels. METHODS: Data were analyzed in 2024 from 2 chronic pain
