PURPOSE: To compare the outcomes of intravitreal melphalan (IVit-M) versus intravitreal topotecan (IVit-T) for vitreous seeds (VS) in retinoblastoma (RB).
DESIGN: Retrospective interventional study.
PARTICIPANTS: Patients of RB with VS receiving intravitreal chemotherapy (IVit-C) between December 2012 and December 2022, at a single quaternary ocular oncology referral center.
INTERVENTION: Intravitreal melphalan injection of 25 μg/0.1 mL in 25 eyes and IVit-T of 30 μg/0.15 mL in 39 eyes.
MAIN OUTCOME MEASURES: Resolution of VS, globe salvage, IVit-C-related complications.

OBJECTIVE: Multiple myeloma (MM) has a better survival outcome because of the development of drugs. However, equivalent outcomes cannot be expected from the same drug. Therefore, how the treatment schedule is managed is important. We analyzed VMP (bortezomib, melphalan, and prednisolone) data to determine an effective treatment strategy.

IMPORTANCE OF THE FIELD: Isolated limb infusion (ILI) is a simple, minimally invasive technique of delivering high concentrations of cytotoxic drugs to a diseased limb for achieving disease control in that limb. Long-term survival and outcome were determined for 80 patients with immunoglobulin light chain (AL) amyloidosis treated with high-dose melphalan and stem cell transplantation (HDM/SCT) more than 10 years ago. We conducted a phase II study of fludarabine, thiotepa, reduced dose melphalan conditioning for allogeneic transplantation using alternate donors. Thirty-eight patients with hematological malignancies were enrolled. Conditioning included fludarabine 160 mg/m2, thiotepa 10 mg/kg (dose adjusted to 5 mg/kg for age >60 years) and melphalan 100 mg/m2. BACKGROUND: Melphalan, an important component of hematopoietic stem cell transplantation (HSCT) preparative regimens, is associated with significant toxicity and large between patient variability in pharmacokinetics making it difficult to calculate the optimal dose for pediatric patients. Paperspray (PS) ionization generates gas phase analyte ions directly from a dried blood spot without the need for prior sample preparation or chromatography. With these advantages, a validated PS-MS/MS assay was developed and applied to the 'real-time' determination of melphalan pharmacokinetics (PK).
