Merkel cell tumor: report of two cases and review of the literature.

The "Merkel cell carcinoma" is a rare tumor usually occuring in the sun-exposed skin. Its poor prognosis is explained by a high rate of metastasis (one third of the patients at the initial presentation) and a high post-operative recurrence rate. Descriptions of the imaging features of this type of tumor are rare. We present two cases of Merkel cell carcinoma localized to the head region. In the first patient, the lesion involved the soft tissues of the scalp with secondary invasion of the skull and meninges. The second patient had a metastasis to the parietal lobe.

Intracranial spread of Merkel cell carcinoma through intact skull.

We report an unusual case of Merkel cell carcinoma presenting as a frontal scalp mass with apparent invasion into underlying brain parenchyma through grossly intact calvaria. Despite wide local excision, craniectomy, intracranial tumor resection, and postoperative adjuvant irradiation, widespread systemic metastases resistant to chemotherapy developed, and the patient died 9 months after surgery. This case report confirms that Merkel cell carcinoma of the head and neck, already known to be an aggressive tumor, has the capacity for rapid intracranial extension. We propose that in this case, the

Merkel cell carcinoma in a malignant pleural effusion: case report.

BACKGROUND: Merkel cell (neuroendocrine) carcinoma is a small round blue cell malignant neoplasm that primarily presents in the skin. The diagnosis of Merkel cell carcinoma in a pleural fluid is challenging because of the morphological similarity to many other malignant neoplasms. Immunohistochemical stains can be essential to establish the diagnosis of Merkel cell carcinoma. CASE PRESENTATION: A 77 year-old woman presented with a mass in her right buttock thought clinically to be a boil or sebaceous cyst. Upon histopathologic review including immunohistochemical analysis, a diagnosis of Merke

Merkel cell carcinoma: a rare cause of hypervascular nasal tumor.

Cutaneous neuroendocrine carcinoma, first described in 1972, is an aggressive disease usually occurring in sun-exposed skin. Other sites have been described, however; such tumors occasionally occur within the nasal fossa. A high rate of metastasis (>30%) explains the poor prognosis. Descriptions of the imaging features of these tumors, mainly located in cutaneous region, are rare. We therefore present the imaging features of two cases of Merkel cell carcinoma involving the sinonasal region, suggestive of a hypervascular tumor.

Nuclear localization of E-cadherin expression in Merkel cell carcinoma.

CONTEXT: Cadherins are cell-cell adhesion proteins that act as tumor suppressor genes and have a critical role in cell sorting and tissue formation during organogenesis. The pattern of cadherin expression constitutes a useful diagnostic and prognostic tool in the evaluation of tumors and for determining the histogenesis of tumor cells. We have previously characterized the cell types of several tumors based on the expression of individual cadherins. OBJECTIVE: To investigate the expression of cadherins in Merkel cell carcinomas. DESIGN: Paraffin immunohistochemical analysis of the 3 best-stud

Postoperative hyponatremia in a patient with ACTH-producing Merkel cell carcinoma.

Merkel cell carcinoma is characterized by specific neuroendocrine features and the expression of several neuropeptides. We report a case of Merkel cell carcinoma with post-surgical hyponatremia in an 85-year-old Japanese woman. A tumor on the left cheek histopathologically showed the characteristics of Merkel cell carcinoma together with Bowen's disease. Although an increased level of ACTH was found both in the tumor and in the peripheral blood, the postoperative hyponatremia in our patient seems more likely to have been caused by the stress of the operation and indapamide, considering that th

[Merkel cell carcinoma. Utility of scintigraphy with 111In-DTPA-pentetreotide].

Merkel cell carcinoma (MCC) is an unusual malignant primary skin tumor, having a high incidence of local recurrent, and regional and distant metastasis. Due to its capacity to express somatostatin receptors, it can be detected in vivo with 111In-pentetreotide scintigraphy (Ostreoscan). We present a case of a MCC whose scintigraphy revealed regional metastases of a primary frontal cutaneous tumor that had been removed previously. The results verified a good correlation with clinical, radiological an histopathological findings.

Neuroendocrine carcinoma of the breast with Merkel cell carcinoma-like features.

A case of neuroendocrine carcinoma of the breast with Merkel cell carcinoma-like features occurring in a 38-year-old female is described. A 2.5 x 2.5 x 1.3 cm tumor in the upper-outer quadrant of the right breast was ill-defined, yellowish white and rubbery. Microscopically, the tumor was characterized by a diffuse proliferation of non-cohesive round cells. The tumor cells had round nuclei with finely dispersed chromatin and inconspicuous nucleoli. The cytoplasm was generally scant, pale to eosinophilic, and argyrophilic. There were scattered foci of small nests or intraductal proliferations.

Fine-needle aspiration cytology of a metastatic Merkel-cell carcinoma.

The cytologic features of Merkel-cell carcinoma metastatic to inguinal lymph nodes are described. The primary tumor was in the buttocks. The cytologic smears from the inguinal lymph node were highly cellular, with small to medium-sized cells in a predominantly discohesive or single-cell pattern, as well as large clusters surrounding segments of capillary-sized blood vessels. The nuclei were uniform, round to oval, with delicate nuclear membranes. The chromatin pattern was fine and "powdery" with multiple small micronucleoli. The cytoplasm was very scanty. Intermediate filament "buttons" were a

Merkel cell tumor of the eyelid: a review and report of an unusual case.

We describe a case of a Merkel cell tumor of the eyelid that manifested as a recurrent chalazion. This tumor had the classic characteristics of a Merkel cell neoplasm by light microscopy. Electron microscopy showed dense-core neurosecretory granules and perinuclear microfilaments consistent with the diagnosis. Because a Merkel cell tumor may manifest as a chalazion, pathologic evaluation of all atypical chalazia is essential. Merkel cell tumors are malignant, and they must be treated aggressively to minimize recurrence or metastasis.

Eccrine and squamous differentiation in Merkel cell carcinoma. An immunohistochemical study.

Of the 42 Merkel cell carcinomas that we studied, two showed numerous tubular structures within sheets and nests of small cells. The small cells stained for both neuron-specific enolase and keratin. The keratin decorated a dot-like paranuclear structure. The ducts stained positively for carcinoembryonic antigen (CEA) and CF-1 (cystic fibrosis-1, a monoclonal antibody that only stains eccrine duct and acrosyringium). Electron microscopy performed on one case showed cytoplasmic dense-core neurosecretory granules and intercellular lumina lined by cells containing microvilli. These ultrastructural

Trabecular (Merkel cell) carcinoma arising in the wall of an epidermal cyst.

A trabecular (Merkel cell) carcinoma arising in the wall of an epidermal cyst in a 58-year-old white male metastasized, 17 months after local excision, to an inguinal lymph node. Ultrastructural studies showed polygonal tumor cells with typical dense-core granules. Although squamous-cell carcinoma and other malignant neoplasms have been reported rarely to have arisen in epidermal cysts, a literature search failed to find a precedent for the present case.

Reprogramming engineered autologous T cells to overcome resistance in patients with Merkel cell carcinoma.

Immune checkpoint inhibitors (ICIs) have transformed Merkel cell carcinoma (MCC) outcomes, but most patients with MCC develop resistance. We identified T cell receptor (TCR)MCC1, a highly avid, HLA-A*02:01-restricted TCR targeting the Merkel cell polyomavirus (MCPyV) oncoprotein large-T antigen15-23. Seven patients with ICI-refractory metastatic MCPyV+ MCC received TCRMCC1-transduced cells (TTCR-MCC1 cells) after lymphodepleting chemotherapy or HLA-enhancing interventions [radiation or interferon gamma-1b (Actimmune)], with concurrent ICIs (NCT03747484). TTCR-MCC1 cells trafficked to tumor sit

Ultrastructure of Merkel cells in the hard palate of the squirrel monkey (Saimiri sciureus).

The distribution and ultrastructure of Merkel cells in the hard palate was investigated in the squirrel monkey (Saimiri sciureus) after fixation by vascular perfusion. Merkel cells were clustered at the base of the epithelial rete pegs of the hard palate. They were characterized by concentrations of dense-cored granules and closely associated intraepithelial nerve endings. Numerous spiny processes extending from the Merkel cell were intercalated with adjacent keratinocytes, probably serving to detect and amplify movement of adjacent cells. These spiny processes contained a rigid core of parall

Clinicopathologic and Molecular Characteristics of Merkel Cell Polyomavirus Positive T-Cell Lymphoproliferative Disorders.

Previous work described 3 cases of CD4+ T-cell lymphomas harboring Merkel cell polyomavirus (MCPyV), as demonstrated by in situ hybridization (ISH), next generation sequencing, and polymerase chain reaction. Two of these cases were cutaneous T-cell lymphomas compatible with mycosis fungoides in postcardiac transplant patients, while the third was a peripheral T-cell lymphoma, not otherwise specified (PTCL-NOS), in a patient without known immunosuppression. In this study, we assess an expanded cohort of T-cell neoplasms from 4 institutions for expression of MCPyV by ISH. This cohort includes 9

B-cell lymphoma-extra large is a promising drug target in Merkel cell carcinoma.

BACKGROUND: Merkel cell carcinoma (MCC) is an aggressive skin tumour with neuroendocrine differentiation. Immunotherapies are effective in the treatment of patients with advanced-stage MCC, but for patients whose tumours cannot be controlled by the immune system, alternative approaches are urgently needed. OBJECTIVES: To identify overexpressed oncogenes as potential drug targets for MCC. METHODS: NanoString platform, digital droplet polymerase chain reaction (ddPCR) and fluorescence in situ hybridization (FISH) assays were used to determine copy number variations (CNVs); BCL2L1 and PARP1 mRN

Oral hairy leukoplakia with ultrastructural evidence of Merkel-like cells in human tongue epithelium.

The purpose of this investigation was to examine cells in the stratum basale and subjacent lamina propria of oral hairy leukoplakia (OHL) specimens for previously unreported morphological changes. Tongue biopsy specimens were obtained from ten HIV-positive and five healthy male patients and examined by light (LM) and transmission electron microscopy (TEM). Observations made during the investigation revealed the following: (1) LM comparisons of control and OHL specimens indicated that clear cells in the stratum basale ranged from 3 to 7 per 100 basal epithelial cells in healthy tongue mucosa vs

An immunocytochemical study of the carbonic anhydrase I isoenzyme in human oral Merkel cells.

Merkel cells in human buccal mucosa and hard palate possess the carbonic anhydrase I isoenzyme (CAI). CAI colocalized immunocytochemically with a range of Merkel cell cytokeratins, namely CK 7, 8, 18, 19 and 20. No other cells in the oral epithelium were immunoreactive for the CAI antibody. The presence of the enzyme may be related to the function of sensory receptors that produce a sustained response to a maintained mechanical stimulus.

Probable mechanisms of loss of Merkel cells in completely depigmented skin of stable vitiligo.

Vitiligo is frequently associated with segmental involvement. It spares paralyzed limbs in transverse myelitis. There is spontaneous repigmentation of vitiligo lesional skin (VL) in diabetic neuropathy. Increased neuropeptide in VL and adjacent normal skin of vitiligo (ANS) along with thickened nerve fibers showing ultrastructural abnormalities all indicate a neural pathogenesis (1). Loss of Merkel cells has been observed in early vitiligo lesional skin (2). Recently, catecholamines have been found to play a major role in initiating the cascade of events leading to loss of melanocytes (1, 3, 4
