A t(5;16)(p15.32;q23.3) generating 16q23.3 --> qter duplication and 5p15.32 --> pter deletion in two siblings with mental retardation, dysmorphic features, and speech delay.

We report on two siblings (half brothers on the paternal side) with a syndrome consisting of delayed development, cardiac anomalies, chest deformity, hip rotation, metatarsus adductus, genital hypoplasia, dysmorphic face, depressed nasal bridge, mental retardation, and speech delay. All metaphases examined showed a normal karyotype in the patients, their father, and both mothers. High-resolution array CGH examination revealed a 16q (6 Mb) duplication dup(16)(16q23.3 --> 16qter) and a 5p (0.97 Mb) terminal deletion del(5)(p15.32 --> pter) in both affected boys but not their healthy siblings or

Hemangiosarcoma in a cow.

A 4-year-old cow initially examined because of hindlimb lameness had a waxing and waning course of illness for 31 weeks. Signs included lameness, swelling of the affected limb, and intermittent fever, and anorexia. Radiography and bacterial culturing suggested osteomyelitis. Terminally, there was peracute anemia and dyspnea. The final diagnosis was hemangiosarcoma of the metatarsus, with metastasis to the lungs.

Surgical approaches to certain long bones of the horse for application of tension band plates.

In the horse, surgical approaches for plating fractures of the ulna, radius, metacarpus, tibia, and metatarsus involved a single curvilinear skin incision and incision of the superficial fascia in a manner to prevent postoperative dehiscence. The tension side of the bones, as well as location of nerves, arteries, veins, and muscles, were the major determinants in developing these approaches for atraumatic surgery and short operative time.

[Operative treatment of hallux valgus with varus of the first metatarsal (author's transl)].

In cases of hallux valgus tendon transfer combined with wedge osteotomy of metatarsus I yields good results. Considering the experience of 36 operated feet in 25 patients the operation can be recommended especially for young patients. An exact technique helps to avoid complications such as hallux varus and delayed ossification of the osteotomy.

Hallux limitus and hallux rigidus. Clinical examination, radiographic findings, and natural history.

Numerous clinical features of hallux limitus/rigidus have been previously reported as isolated entities based on individual case review or myopic observations. Few attempts have been made to synthesize a comprehensive natural history which correlates the inter-relationship of these findings. Frequently unrecognized or overlooked subtle clinical findings, such as shoe-wear patterns, hyperkeratoses locations, and gait disturbances, precede significant radiographic changes or painful degenerative arthritis by months to years. Recognition of these subtle clinical features will aid in establishing

Complications of the Kalish bunionectomy.

The Kalish osteotomy has become a popular method for the correction of mild-to-moderate hallux abducto valgus deformities associated with metatarsus primus adductus. This paper reviews the common pitfalls associated with this procedure, including failure to observe the indications for the procedure, technical failures when performing the procedure, and the most frequent postoperative complications.

First metatarsophalangeal arthrodesis for hallux valgus.

Arthrodesis of the first metatarsophalangeal joint is a reliable operation in the treatment of selected cases of hallux valgus. It corrects deformity of hallux valgus and metatarsus primus varus, leading to good functional results with a low complication rate. It is a technique well suited to patients with hallux valgus associated with degenerative changes or severe deformity, and those for whom primary hallux valgus surgery has failed.

Modified Youngswick procedure for hallux limitus.

Multiple surgical procedures have been described for the correction of hallux limitus deformity. We describe a new modification of the Youngswick procedure for the surgical treatment of hallux limitus. Other procedures for hallux limitus correction are also discussed. This modified Youngswick procedure will provide a new approach to treating hallux limitus secondary to metatarsus primus elevatus when shortening of the first metatarsal is not indicated.

Prevalence of Hip Dysplasia and Associated Conditions in Children Treated for Idiopathic Early-onset Scoliosis-Don't Just Look at the Spine.

BACKGROUND: Hip dysplasia, congenital muscular torticollis, plagiocephaly, and metatarsus adductus are known to be associated. The etiology of infantile idiopathic scoliosis and its association with the aforementioned conditions is unknown. This study reviews a series of infantile scoliosis patients to address this gap. METHODS: The medical records of all patients treated with casting for early-onset scoliosis (EOS) from 2001 to 2016 were retrospectively reviewed. Inclusion criteria were a diagnosis of idiopathic EOS and age below 4 years at the time of the first cast. Demographic information

Foot radiologic morphology as a predictor of pseudo-jones fractures: A retrospective case-control study.

BACKGROUND: Fifth metatarsal basis fractures are common injuries. Zone 1 fractures are also known as pseudo-jones fractures. This study aimed to elucidate the impact of foot radiologic morphology on pseudo-Jones fractures. METHODS: Patients with pseudo-Jones fractures formed the case group and individuals with ankle sprains but no fractures formed control group. Weight-bearing anteroposterior and lateral radiographs were evaluated for pre-determined measurements: metatarsus adductus angle (MAA), intermetatarsal angles (1-2, 4-5), calcaneal inclination (CI) angle, and fifth metatarsal base ang

Relationship between arthritis of the second and third tarsometatarsal joints and incongruity of the first tarsometatarsal joint in patients with hallux valgus.

OBJECTIVES: Hallux valgus is associated with tarsometatarsal arthritis; its pathophysiology remains unknown. Therefore, we aimed to elucidate the relationship between arthritis of the second and third tarsometatarsal joints and incongruity of the first tarsometatarsal joint in the sagittal plane. METHODS: Forty-three patients (64 feet) with hallux valgus who underwent surgery at University Hospital Kyoto Prefectural University of Medicine were included and divided into two groups: control (without second and third tarsometatarsal joint degeneration) and osteoarthritis (with second and third t

Intoeing--fact, fiction and opinion.

Intoeing in children is a common cause of parental concern. A normal neurologic examination and normal height and weight for age help the physician exclude most associated skeletal dysplasias, and neuromuscular or metabolic diseases. Three causes of intoeing affect otherwise normal newborns and infants. Metatarsus adductus is the diagnosis if a "C" shaped curve, rather than a straight border, is present on the lateral aspect of the foot. About 90 percent of cases resolve by one year of age. Internal tibial torsion, although a normal finding in the newborn, is usually a matter of concern at wal

[The evaluation and management of rotational deformity in cerebral palsy].

Rotational deformities are common lower extremity abnormalities in children with cerebral palsy, which include intoeing and outtoeing. Intoeing is caused by one of the three types of deformity: increased femoral anteversion, internal tibial torsion, and metatarsus varus, while out-toeing, the less common form, is caused by femoral retroversion and external tibial torsion. An accurate diagnosis should be made with careful physical and radiographic examination.

Painful os intermetatarseum.

Accessory ossicles are usually benign radiographic findings. However, they may become clinically significant if they fracture or cause soft-tissue irritation. An os intermetatarseum is an uncommon ossicle found between the bases of the first and second metatarsals. This article reviews some of the theories relating to the development of this uncommon ossicle and describes its role as a possible etiology of metatarsus primus varus.

Lapidus bunionectomy: arthrodesis of the first metatarsocunieform joint.

Fusion of the first metatarsocunieform (MC) joint allows for correction of the first metatarsal in three planes, including adduction, plantarflexion, and rotation. It also allows for decreased jamming of the great toe joint and increased medial column stability. As knowledge about other medial column procedures grows, fusion of the first MC joint will continue to grow in popularity. It is an excellent procedure for hallux limitus and also for metatarsus primus elevatus cases.

Distal metatarsal osteotomy for adolescent hallux valgus.

This is a review of 55 metatarsal osteotomies for hallux valgus and metatarsus primus varus in patients under 15 years of age. The results were analysed after the patients were reviewed clinically and radiologically. Complications of the osteotomies and the factors that caused them are discussed. Poor results, irrespective of the type of osteotomy, were due to excessive shortening of the first metatarsal and/or dorsal tilting of the metatarsal head. In both instances there was persistent metatarsalgia.
